3. Feasibility, Safety and Timing of Secondary Percutaneous Coronary Intervention via Distal Transradial Artery Approach

Abstract

Background Percutaneous coronary intervention (PCI) via distal transradial artery approach (dTRA) is effective in reducing the incidence of radial artery occlusion, however, there are no reports on the feasibility, safety and timing of secondary PCI via dTRA domestically. Objective To explore the feasibility, safety and timing of secondary PCI via dTRA. Methods A total of 70 patients who were to undergo secondary PCI via dTRA in Fuwai Hospital, CAMS&PUMC from July 2021 to July 2022 were consecutively included and divided into ≤30 d group (n=33) and >30 d group (n=37) according to the time interval since the last PCI via dTRA. The general clinical data, operation-related indicators, coronary artery lesions and radial artery inner diameters at 5, 10 and 15 cm proximal to the radial styloid process before and 24 h after PCI were compared between the two groups. Results The dTRA puncture and catheterization were successfully conducted in 69 of 70 patients, with a success rate of 98.6% (69/70) . Second PCI via dTRA was successfully completed in 66 of 67 patients assessed for secondary PCI, with a success rate of 98.5% (66/67) . There were significantly difference in age, platelet count, incidence of diabetes, number of lesioned vessels and ACC/AHA coronary artery lesions typing (P30 d group before and 24 h after PCI when comparing between the two groups, respectively (P30 d group (P30 days from the first dTRA PCI) , and the timing of secondary PCI via dTRA should be determined according to the patient's condition and puncture site.

30. Prolonging Dual Antiplatelet Therapy Improves Long-term Prognosis in Diabetic Patients Undergoing Complex Percutaneous Coronary Intervention with Drug-eluting Stents

Abstract

Background The optimal dual antiplatelet therapy (DAPT) duration after percutaneous coronary intervention (PCI) remains controversial, especially for patients at high risk of ischemic events. Diabetes mellitus (DM) and complex coronary interventions have been considered as high risk factors for ischemic events. We aim to investigate if prolonging DAPT duration in diabetic patients after drug-eluting stent (DES) implantation can reduce long-term adverse events.Methods This study is an observational cohort study carried out in the largest cardiovascular center in China. 10,724 patients who underwent PCI from January, 2013 to December, 2013 were consecutively enrolled. The inclusion criteria included: documented history of DM; underwent DES implantation; free of adverse events within 1 year after operation; and had at least one high thrombotic risk technical aspects. These patients were divided into three groups according to DAPT duration: Standard group (11 ≤ DAPT ≤ 13 months) and prolonged group (13 < DAPT ≤ 24 months; DAPT > 24 months). Baseline characteristics and 5-year clinical outcomes were collected and analyzed. Results A total of 2,403 diabetic patients with complex PCI were included in the current analysis (Standard DAPT n=689; Prolonged DAPT n=1714). Baseline and angiographic characteristics, and complexity of PCI were comparable among patients with different DAPT durations. Patients in 1324 months was related with lower risk of all-cause death (HR: 0.681, 95%CI: 0.493-0.942) and cardiac death (HR: 0.620, 95%CI: 0.403-0.952). Prolonging DAPT duration did not increase the risk of major bleeding (1324 months: HR: 0.967, 95%CI: 0.682-1.371).Conclusions For diabetic patients undergoing complex PCI, prolonging DAPT duration might improve long-term prognosis by reducing the risk of adverse ischemia events and without increasing the risk of bleeding.

Abstract

Local information has significant contributions to visual sentiment analysis (VSA). Recent studies about local region discovery need manually annotate region location. Affective local information learning and automatic discovery of sentiment-specific region are still the challenges in VSA. In this paper, we propose an end-to-end VSA method for weakly supervised sentiment-specific region discovery. Our method contains two branches: an automatic sentiment-specific region discovery branch and a sentiment analysis branch. In the sentiment-specific region discovery branch, a region proposal network with multiple convolution kernels is proposed to generate candidate affective regions. Then, we design the multiple instance learning (MIL) loss to remove redundant and noisy candidate regions. Finally, the sentiment analysis branch integrates both holistic and localized information obtained in the first branch by feature map coupling for final sentiment classification. Our method automatically discovers sentiment-specific regions by the constraint of MIL loss function without object-level labels. Quantitative and qualitative evaluations on four benchmark affective datasets demonstrate that our proposed method outperforms the state-of-the-art methods. [ABSTRACT FROM AUTHOR]
