INTRODUCTION The valid estimation of long-term dietary intake in large-scale cohort studies represents a methodological challenge. Food frequency questionnaires (FFQs) have long been the instrument of choice. However, FFQs measure [...], BACKGROUND/OBJECTIVES: The validity of dietary assessment in large-scale cohort studies has been questioned. Combining data sources for the estimation of usual intake in a blended approach may enhance the validity of dietary measurement. Our objective was to develop a web-based 24-h food list for Germany to identify foods consumed during the previous 24 h and to evaluate the performance of the new questionnaire in a feasibility study. SUBJECTS/METHODS: Available data from the German National Nutrition Survey II were used to develop a finite list of food items. A total of 508 individuals were invited to fill in the 24-h food list via the Internet up to three times during a 3-6-month time period. In addition, participants were asked to evaluate the questionnaire using a brief online evaluation form. RESULTS: In total, 246 food items were identified for the 24-h food list, reflecting > 75% variation in intake of 27 nutrients and four major food groups. Among the individuals invited, 64% participated in the feasibility study. Of these, 100%, 85% and 68% of participants completed the 24-h food list one, two or three times, respectively. The average time needed to complete the questionnaire was 9 min, and its acceptability by participants was rated as high. CONCLUSIONS: The 24-h food list represents a promising new dietary assessment tool that can be used as part of a blended approach combining multiple data sources for valid estimation of usual dietary intake in large-scale cohort studies. Abstract

Das Memorandum "Kirche 2011: Ein notwendiger Aufbruch" ist ein Aufruf, den deutschsprachige katholische Theologieprofessor:innen im Februar 2011 veröffentlicht bzw. mitgetragen haben. Im Jahr zuvor war das Ausmaß des sexuellen Missbrauchs an Kindern und Jugendlichen durch Priester und Ordensleute in deutschen Diözesen öffentlich geworden. Das Memorandum richtete sich in dieser Situation „an alle, die es noch nicht aufgegeben haben, auf einen Neuanfang in der Kirche zu hoffen und sich dafür einzusetzen." Es forderte einen „Mut zur Selbstkritik und zur Annahme kritischer Impulse – auch von außen", und unterstrich, dass die „Freiheitsbotschaft des Evangeliums ... den Maßstab für eine glaubwürdige Kirche, für ihr Handeln und ihre Sozialgestalt" bildet. Abschließend benannte das Memorandum sechs Bereiche bzw. Handlungsfelder, in denen ein offener Dialog über notwendige Reformen geführt werden müsse.

Abstract

Background: Joint data analysis from multiple nutrition studies may improve the ability to answer complex questions regarding the role of nutritional status and diet in health and disease. Objective: The objective was to identify nutritional observational studies from partners participating in the European Nutritional Phenotype Assessment and Data Sharing Initiative (ENPADASI) Consortium, as well as minimal requirements for joint data analysis. Methods: A predefined template containing information on study design, exposure measurements (dietary intake, alcohol and tobacco consumption, physical activity, sedentary behavior, anthropometric measures, and sociodemographic and health status), main health-related outcomes, and laboratory measurements (traditional and omics biomarkers) was developed and circulated to those European research groups participating in the ENPADASI under the strategic research area of 'diet-related chronic diseases.' Information about raw data disposition and metadata sharing was requested. A set of minimal requirements was abstracted from the gathered information. Results: Studies (12 cohort, 12 cross-sectional, and 2 case-control) were identified. Two studies recruited children only and the rest recruited adults. All studies included dietary intake data. Twenty studies collected blood samples. Data on traditional biomarkers were available for 20 studies, of which 17 measured lipoproteins, glucose, and insulin and 13 measured inflammatory biomarkers. Metabolomics, proteomics, and genomics or transcriptomics data were available in 5, 3, and 12 studies, respectively. Abstract

There is a need for harmonized public health surveillance systems to monitor regional variations and temporal trends of health behaviours and health outcomes and to align policies, action plans and recommendations in terms of healthy diet and physical (in)activity within Europe. We provide an inventory of currently existing surveillance systems assessing diet, physical activity, and sedentary behaviours in Europe as a tool to assist in the identification of gaps and needs and to contribute to the roadmap for an integrated pan-European surveillance system.

Abstract

BACKGROUND AND OBJECTIVE Endoscope-guided fine-needle biopsy of mediastinal space-occupying lesions is a recently introduced method of low invasiveness. This is a report of the authors' experience. PATIENTS AND METHODS Between Nov. 1996 and April 1999 endoscope-guided mediastinal biopsies (Pentax FG 32 UA) were performed in 31 patients (eight women, 23 men; aged 27-80 years). The space-occupying lesion had to be less than 1 cm from the oesophagus and not more easily approachable in other ways. RESULTS The method was successful in 23 of the 31 patients (74%). In ten of the 31 patients (32%) a malignant lesion was histologically proven. In six patients (19%) sarcoidosis was revealed. Of the eight patients with inadequate biopsy material a malignancy was ultimately diagnosed in three, sarcoidosis in one (in three by mediastinoscopy, in one by laparoscopy). Thus the sensitivity of diagnosing a malignancy was 77%, with a specificity of 100%. Follow-up examinations in the remaining four patients excluded a malignancy. CONCLUSION Endoscope-guided fine-needle biopsy of mediastinal space-occupying lesions is a technically simple method causing little stress to the patient that can frequently elucidate the lesion's benignity or malignancy. Invasive procedures, such as mediastinoscopy, may thus be avoided in some patients.

Abstract

The innervation of axillary lymph nodes from 20 juvenile rats (aged < 6 wk) was compared with that of 20 old rats (aged > 2 y). One half of each group was investigated by light microscopy on silver-impregnated paraffin sections, the other half by electron microscopy. The lymph nodes of the old animals were larger than those of juvenile animals, as derived from the volumes analysed by light microscopy. By both light and electron microscopy, the nerves were found to be confined almost exclusively to the medulla and were frequently associated with groups of plasma cells. Nerves identified as cortical in location were consistently found only in cortical areas adjacent to the medulla. In the old animals the nerves were thicker and more prominent in the light microscopic preparations. No such differences were noted at the ultrastructural level. Myelinated nerves were found in all lymph nodes, except in a single old animal. Quantitative analysis of the nerves revealed a significant increase in the density of innervation of the medulla in the old animals both by light and electron microscopy. By light microscopy there also appeared to be a significant increase in the innervation density of the cortex, but these nerves were considered to be mislocalized because of difficulty in precise localisation at this level of magnification. On light microscopy, a significant increase in the incidence of nerve branching was found in the nodes of the old animals.

The development of the first random stream, high capacity, large particle radiometric sorter designed to process low-grade ore in the 80 mm to 300 mm size fraction is the first progress in the field since the 1970s. The electronics and detection systems are described. The sorter was designed for Rossing Uranium, Namibia; a feasibility study found that radiometric sorting lowers the unit cost of production, making it cost-effective at constant production level, while increasing production levels provides a significant benefit. A net positive environmental impact was found.
