Background: Dupuytren disease is a common fibroproliferative disease that affects the palmar fascia of the hands. Currently, there is limited consensus regarding the optimal therapy for this condition, with treatment decisions based largely on surgeon preference. Therefore, the aim of this study was to determine which treatments are the most effective for Dupuytren disease. Method: A systematic review and network meta-analyses were conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines. Medline, EMBASE, and Web of Science were searched for randomized trials comparing treatments for Dupuytren disease in adults. Eligible treatments included open limited fasciectomy, collagenase injection, and percutaneous needle fasciotomy. Study selection, data extraction, and quality appraisal were performed in duplicate. The methodological quality was evaluated with the Cochrane risk-of-bias critical appraisal tool. Results: Eleven randomized clinical trials were included in this study. At short-term (1-12 weeks) and long-term (2-5 years) time points, fasciectomy improved contracture release more than collagenase and needle fasciotomy as inferred by a lower total passive extension deficit. However, there was no difference between the groups regarding the best possible outcome at any time point. Fasciectomy was also superior in terms of recurrence and patient satisfaction compared with collagenase and needle fasciotomy, but only at later time points. There was no difference in skin damage–related and nerve damage–related complications following fasciectomy compared with other modalities. Risk of bias was generally moderate. Conclusions: Fasciectomy provides superior long-term advantages in terms of patient outcomes when compared with collagenase and needle fasciotomy. Background: Sensorineural hearing loss (SNHL) may occur following cardiac surgery. Although preventing post‐operative complications is vitally important in cardiac surgery, there are few guidelines regarding this issue. This review aimed to characterize SNHL after cardiac surgery. Method: This systematic review was registered on PROSPERO and conducted in accordance with PRISMA guidelines. A systematic search of the PubMed, Embase and Cochrane Library were conducted from inception. Eligibility determination, data extraction and methodological quality analysis were conducted in duplicate. Results: There were 23 studies included in the review. In the adult population, there were six cohort studies, which included 36 cases of hearing loss in a total of 7135 patients (5.05 cases per 1000 operations). In seven cohort studies including paediatric patients, there were 88 cases of hearing loss in a total of 1342 operations. The majority of cases of hearing loss were mild in the adult population (56.6%). In the paediatric population 59.2% of hearing loss cases had moderate or worse hearing loss. The hearing loss most often affected the higher frequencies, over 6000 Hz. There have been studies indicating an association between hearing loss and extracorporeal circulation, but cases have also occurred without this intervention. Conclusion: SNHL is a rare but potentially serious complication after cardiac surgery. This hearing loss affects both paediatric and adult populations and may have significant long‐term impacts. Further research is required, particularly with respect to the consideration of screening for SNHL in children after cardiac surgery. Purpose: Nausea is a common and unpleasant sensation for which current therapies such as serotonin (5-HT3) antagonists are often ineffective, while also conferring a risk of potential adverse events. Isopropyl alcohol (IPA) has been proposed as a treatment for nausea. We aimed to compare IPA with 5-HT3 antagonists for the treatment of nausea across all clinical settings. Methods: MEDLINE, EMBASE, PubMed, CENTRAL and CINAHL were searched from inception to 17 July 2023 for randomised controlled trials (RCTs) comparing inhaled IPA and a 5-HT3 antagonist for treatment of nausea. Severity and duration of nausea, rescue antiemetic use, adverse events and patient satisfaction were the outcomes sought. Risk of bias (RoB) was assessed using Cochrane RoB 2. Random-effects model was used for meta-analysis. Combination of meta-analyses and narrative review was used to synthesise findings. The evidence was appraised using GRADE. Results: From 1242 records, 4 RCTs were included with 382 participants. Participants receiving IPA had a significantly lower mean time to 50% reduction in nausea (MD − 20.06; 95% CI − 26.26, − 13.85). Nausea score reduction at 30 min was significantly greater in the IPA group (MD 21.47; 95% CI 15.47, 27.47). IPA led to significantly reduced requirement for rescue antiemetics (OR 0.60; 95% CI 0.37, 0.95; p = 0.03). IPA led to no significant difference in patient satisfaction when compared with a 5-HT3 antagonist. The overall GRADE assessment of evidence quality ranged from very low to low. Conclusion: IPA may provide rapid, effective relief of nausea when compared with 5-HT3 antagonists. [ABSTRACT FROM AUTHOR]

The article addresses the issue of long waiting times and limited access to colonoscopy services for rural patients in Australia. It proposes a novel tool and pathway, called the Whyalla Direct Access Tool, which is a simple questionnaire aiming to expedite rural colonoscopy referrals when it is clearly indicated. Topics include direct access to improve national bowel cancer screening, quality assurance, and streamlining access to colonoscopy services.

This paper applies the market map tool to the Nepalese biomass improved cookstove (ICS) sector highlighting existing weaknesses in government policy and biomass cookstove market chains to provide recommendations to better address the social, economic, and cultural needs of users. This addresses the problem of low adoption rates of biomass ICS in Nepal. Our research objectives set out to explore the effectiveness of market maps designed for East Africa's ICS sector (Stevens et al. [2019]. "Market Mapping for Improved Cookstoves: Barriers and Opportunities in East Africa." Development in Practice) in Nepal, co-develop a revised market map for Nepal's biomass ICS sector, conduct a parallel process for institutional-scale biomass ICS and draw on the co-produced market map to inform policy and regulatory frameworks relating to biomass-fuelled ICS. The methodological approach involved reviewing cookstove-related policy documents and regulatory frameworks, undertaking 31 semi-structured interviews, analysing findings from an Institutional Top-Loading Down-Draft (TLUD) Natural Draft Gasifier Pilot study and co-developing the final market map in collaboration with key ICS stakeholders. The results indicate that although government policy actively promotes biomass ICS, this often results in cookstove "stacking" rather than the sustained and exclusive use of clean cooking solutions necessary to promote health benefits. Attention is also focused on the underdeveloped nature of the institutional cookstove market. Our conclusions highlight the usefulness of market maps with a monitoring and evaluation element for identifying barriers to clean cooking uptake and facilitating product improvement by integrating end-user feedback.
