Abstract

Investigation of parameters leading to tumor formation should include simultaneous assessment of pro- and anti-oxidation, since oxidative stress is associated with malignant diseases. Low antioxidant status and high lipid peroxidation induce pro-oxidation while high vitamin intake and high vitamin concentration in blood protect against it. Colon cancer patients show low levels of polyunsaturated fatty acids and prostate cancer patients have elevated levels.

35. Ambulatory blood pressure: associations with coronary heart disease in the aged Finnish population.

Abstract

BACKGROUND AND AIMS: Ambulatory blood pressure (ABP) has been shown to be a better predictor of cardiovascular events than clinical blood pressure (BP) in middle-aged and older populations. This study studied the association of various components of ABP (daytime, night-time, 24-hour ABP) in the presence of coronary heart disease (CHD) in an older Finnish population. METHODS: This cross-sectional, observational, population-based study was carried out in The Lieto Health Centre, Finland, in 1998-99. The study population consisted of 502 subjects (237 men, 265 women) aged 64-87 years. ABP measurements for 24 hours, daytime (awake) and night-time (asleep), were made. Resting electrocardiograms (ECG) were recorded. A person was considered to have CHD if at least one of the following criteria was met: (I) history of coronary by-pass surgery or coronary angioplasty, (II) diagnosis of CHD in previous medical records, (III) ischemia-related changes on ECG. RESULTS: CHD increased by 30% with a 10 mmHg increase in night-time systolic blood pressure (SBP) (OR 1.30, 95% Cl 1.15-1.47). When ambulatory 24-hour and daytime SBP values were each entered separately into the multivariate model, 24-hour SBP, but not daytime SBP, was associated with CHD. CONCLUSIONS: The most important information gained from 24-h BP monitoring in subjects with CHD is night-time BP. Night-time BP may provide new information about the CHD risk which is not identified in common clinical diagnoses of hypertension. 40. Mucosal biotransformation rates in the small intestine of children.

Abstract

Biotransformation of ingested xenobiotics is known to take place in the gastrointestinal mucosa of laboratory animals and adult humans as well as in the liver. We studied the activities of aryl hydrocarbon hydroxylase, epoxide hydrolase, and glutathione peroxidase in 242 peroral small intestinal biopsy samples of children aged eight months to 18 years: 201 with normal histology, 21 with partial villous atrophy, and 20 with severe villous atrophy. All these enzymes were detectable even in the youngest children. The aryl hydrocarbon hydroxylase activity was age dependent, while the other measured enzyme activities were not related to the age of the patients. The aryl hydrocarbon hydroxylase activity was not related to the mucosal histology, but the epoxide hydrolase and glutathione peroxidase activities were diminished in samples with severe villous atrophy as compared with normal mucosa. This suggests that small intestinal mucosa with villous atrophy may produce oxidated, reactive metabolites, but further metabolism into detoxication products is decreased. This may expose persons with mucosal atrophy to possible harmful effects of environmental xenobiotics entering the body even at low doses. [ABSTRACT FROM PUBLISHER]

42. Radiographic pulmonary changes of gastro-oesophageal reflux disease in elderly patients.

Abstract

The aim of this study was to investigate the association of gastro-oesophageal reflux disease (GORD) with radiographic pulmonary changes. One hundred and thirty-seven patients aged over 60 years referred for endoscopy for abdominal symptoms were included in the study. In patients with any suspicion of GORD (n = 95), 24-hour oesophageal pH monitoring was carried out. All the patients were interviewed before the examinations. Chest radiography was performed in all patients. Odds ratio of respiratory symptoms was 8.7 (95% confidence interval 3.4-22.4) in patients with total reflux time of more than 10% compared with those who had total reflux time of less than 10%. In chest radiography, bilateral pleural adhesions and thickenings and bilateral parenchymal scars were significantly more common in patients with, than in those without, regurgitation or large hiatal openings. The occurrence of bilateral scars and pleural thickenings increased with the extent of gastro-oesophageal reflux in pH monitoring. Odds ratios of bilateral pleural thickening and pulmonary scars were 3.1 (95% confidence interval 0.8-11.5) and 5.8 (1.1-29.6) in patients with total reflux time over 10%, compared with patients who had normal total reflux time. The findings indicate that respiratory involvement is a characteristic feature of GORD in elderly patients. [ABSTRACT FROM AUTHOR]

47. Fatty acid composition of phospholipids and neutral lipids and lipid peroxidation in human breast cancer and lipoma tissue.

Abstract

Since unsaturated fatty acids have been shown to control cell division experimentally the fatty acid composition in human mammary cancers as compared with the healthy surrounding tissue was studied. Both phospholipid and neutral lipid fractions were extracted for study and further lipid per-oxidation expressed as thiobarbituric add reactive substances was measured. For a comparison a benign adipose tissue tumor, lipoma, was studied. There was a 3.6-fold increase in phospholipid contents of cancer as compared with a reference tissue. No such differences were observed between lipoma and surrounding adipose tissue. In the phospholipid fatty acids of mammary cancers there was a marked increase in the relative amounts of unsaturated fatty acids, especially arachidonic acid, while saturated fatty acids were decreased, as compared with a healthy reference breast tissue or lipomas. The data suggest that the fatty acids might have a role in the development of breast cancer, possibly related to the formation of reactive metabolites of unsaturated fatty acids in the initiation of cancer or to their promotional effects. Abstract

In light static exercise the heart rate and blood pressure increase much more than during dynamic exercise at the same oxygen uptake level. Heavy static exercise is characterized by a failure of the local blood flow to adjust to the oxygen demands of the exercising muscles. Respiratory and circulatory responses are dominated by an incompetence to obtain steady-state conditions, and thus the worktime is short. After the cessation of heavy static exercise a sudden compensatory increase occurs in cardiac output and oxygen uptake. Due to the higher increase in blood pressure, even light static exercise causes much higher strain on the heart than an equivalent amount of dynamic exercise. The heart responds to the increased afterload by increasing contractility and heart rate and thus improves cardiac output. In persons with a poor cardiac reserve a rise in the left ventricular end-diastolic pressure is seen, along with a fall in the stroke work index in response to the increased afterload caused by static exercise. It is possible that a discrepancy exists between work capacity during tasks demanding also isometric muscle work and a dynamic exercise test performance. The decreased cardiac reserve may first appear after the great increase in afterload, even in relatively light static work.
