To investigate the prevalence, causes and impact of unilateral visual impairment (UVI) on visual function in the elderly in 'home for the aged' in Hyderabad, India.Participants aged ≥60 years were recruited from 41 'homes for the aged'. All participants had complete eye examinations including visual acuity assessment, refraction, slit-lamp and fundus examination. Unilateral visual impairment (UVI) was defined as presenting VA worse than 6/18 in one eye and presenting VA 6/18 or better in the other eye. Indian Vision Function Questionnaire (INDVFQ) was used for assessing visual functioning.Of the total 1,513 elderly participants enumerated, 1,182 (78.1%) were examined. After excluding 356 participants with VI in the better eye, data were analysed for the remaining 826 participants. The mean age (standard deviation) of these participants was 74.4 ± 8.4 years; 525 (63.6%) were women, and 111 (13.4%) had no schooling. The prevalence of unilateral VI was 38.1% (95% CI: 34.8-41.5; n = 315). Cataract (37.5%; n = 118) was the leading cause of UVI followed by Uncorrected Refractive Error (22.2%; n = 70) and posterior capsular opacification (18.4%; n = 58). The overall INDVFQ score was higher among those with UVI than those without UVI (37.7 versus 34.5; p .01) suggestive of poor visual functioning.UVI was common and largely due to avoidable causes among the elderly in residential care with an adverse impact on visual functioning. Screening for vision loss in 'homes for the aged' and the provision of appropriate services should become a routine practice to achieve the goal of healthy aging in India.

To report the barriers for seeking eye care among the elderly population aged ≥60 years with avoidable vision impairment (VI) in the South Indian state of Telangana.A total of 3640 participants aged ≥60 years were recruited using cluster-random sampling. Demographic information, including presenting visual acuity, was measured using the standard Rapid Assessment of Visual Impairment (RAVI) protocol. "Avoidable VI" was considered if the VI was due to cataract or uncorrected refractive error (URE). A detailed interview was conducted using a validated questionnaire to report the barriers for not seeking eye care. Data were analyzed using the Stata statistical software version 14.Prevalence of avoidable VI was 30.2% (95% CI: 28.02-31.06; n = 1102). Among those who noticed decreased vision (n = 1074), only 392 participants (36.4%) reported that they felt the need for seeking eye care. The major barriers for not seeking eye care were: cannot afford the consultation fee and services (42.0%) and no escort (25.7%). Overall, the personal barriers (57.9%) were the major reason for not seeking care, followed by economic barriers (42.0%). No significant difference was reported in barriers between the participants with unilateral and bilateral VI (0.05).Overall, among the elderly people, personal and economic barriers were the major reason for not seeking eye care. Health care providers and policymakers should focus on newer models of eye care delivery to ensure better accessibility and uptake of care by the elderly people.

This study was conducted to report on the pattern of spectacles use and compliance among the elderly (aged ≥60 years) in homes for the aged in Hyderabad region in Telangana State, India.Participants were recruited from 41 homes for the aged centres for comprehensive eye health assessments. A questionnaire was used to collect information on current and past use of spectacles, type of spectacles, spectacles provider and amount paid for the spectacles. For those that reported using spectacles in the past, information was collected on the reasons for their discontinuation. Compliance with spectacles use was assessed after eight months of provision of the spectacles.A total of 1182/1513 participants were examined from 41 homes for the aged in Hyderabad, India. The mean age of the participants examined was 75 years (standard deviation (SD): 8.8 years; range: 60-108 years); 764 (64.6%) of them were women and 240 participants (20.3%) had no formal education. The prevalence of spectacles use was 69.9% (95% confidence interval [CI]: 67.1-72.4; n = 825). Bifocals were the most commonly used type of spectacles (86.7%) followed by single vision glasses for distance vision (7.4%). Private eye clinics were the largest service provider (85.5%) followed by local optical outlets (6.9%) and other service providers (7.7%). The prevalence of spectacles compliance was 81.5% (211/259).Use of spectacles and compliance are high among the elderly living in residential care homes in the Hyderabad region. Spectacles use can be further improved by periodic eye assessments along the lines similar to school eye programs, which can immensely benefit this vulnerable, aged population.

Background: Aim of the study was to assess the awareness, knowledge and willingness to donate eyes among general public in southern Indian states of Andhra Pradesh and Telangana.Methods: A cross sectional population-based survey used a semi structured questionnaire on awareness, knowledge and willingness for eye donation. Stratified multistage cluster random sampling method was used with a sample size of 867 adults >16 years based on, estimated awareness of cataract 70%, assuming an expected rate of 85%, design effect of 1.5, margin of error 4% with 95% confidence interval with 25% from urban areas and 75% from rural areas. Having heard of eye donation was defined as awareness and having knowledge of the ideal time to donate the eyes was considered as knowledge. A pilot study was conducted to validate the questions used in the main study. Statistical package SPSS (version 19) was used for analysis to calculate logistic regression and odds ratios for gender, age, education and urban-rural areas.Results: A total of 782/867 (90.1%) subjects participated in the survey with females 47.4%. 603 subjects (77.1%) were aware of eye donation. 301 subjects (38.5%) were willing to donate eyes. The 297 subjects (49.3%) were aware of the ideal time for donation after death.Conclusions: Awareness of eye donation was high among the general public but the willingness to donate eyes was poor. There is a need to identify barriers for eye donation to increase the voluntary donation of the eyes to combat corneal blindness.

Background: To assess the awareness, knowledge about refractive errors and strabismus among the general public in southern Indian states of Andhra Pradesh and Telangana.Methods: A cross sectional population-based survey used a semi structured questionnaire on awareness, knowledge on refractive errors and strabismus done as part of knowledge, attitude and practices study (KAP). Stratified multistage cluster random sampling method was used with a sample size of 867 adults- ≥16 years. Having heard of refractive error and strabismus was defined as awareness and having knowledge of the type of error for which spectacles were worn, was considered as knowledge. A pilot study was conducted to validate the questions used in the main study. Statistical package SPSS (version 19) was used for analysis to calculate logistic regression and odds ratios for gender, age, education and urban-rural areas.Results: A total of 782/867 (90.1%) subjects participated in the survey with females 47.4%. 581 subjects (74.3%) were aware of refractive error. 690 subjects (88.2%) were aware of squint. With multiple logistic regression about awareness of refractive error to various variables, subjects who were educated 11th class to degree had a higher awareness (OR: 2.40; CI: 1.25-4.60). With multiple logistic regression about awareness of squint to various variables, females had a higher awareness of squint (OR: 1.98; CI: 1.19-3.31).Conclusions: Awareness of squint and refractive error was high among the general public, but the knowledge of it was limited.

PubMed: 28779882, Background Global and regional prevalence estimates for blindness and vision impairment are important for the development of public health policies. We aimed to provide global estimates, trends, and projections of global blindness and vision impairment. Methods We did a systematic review and meta-analysis of population-based datasets relevant to global vision impairment and blindness that were published between 1980 and 2015. We fitted hierarchical models to estimate the prevalence (by age, country, and sex), in 2015, of mild visual impairment (presenting visual acuity worse than 6/12 to 6/18 inclusive), moderate to severe visual impairment (presenting visual acuity worse than 6/18 to 3/60 inclusive), blindness (presenting visual acuity worse than 3/60), and functional presbyopia (defined as presenting near vision worse than N6 or N8 at 40 cm when best-corrected distance visual acuity was better than 6/12). Findings Globally, of the 7·33 billion people alive in 2015, an estimated 36·0 million (80% uncertainty interval [UI] 12·9–65·4) were blind (crude prevalence 0·48%; 80% UI 0·17–0·87; 56% female), 216·6 million (80% UI 98·5–359·1) people had moderate to severe visual impairment (2·95%, 80% UI 1·34–4·89; 55% female), and 188·5 million (80% UI 64·5–350·2) had mild visual impairment (2·57%, 80% UI 0·88–4·77; 54% female). Functional presbyopia affected an estimated 1094·7 million (80% UI 581·1–1686·5) people aged 35 years and older, with 666·7 million (80% UI 364·9–997·6) being aged 50 years or older. The estimated number of blind people increased by 17·6%, from 30·6 million (80% UI 9·9–57·3) in 1990 to 36·0 million (80% UI 12·9–65·4) in 2015. This change was attributable to three factors, namely an increase because of population growth (38·4%), population ageing after accounting for population growth (34·6%), and reduction in age-specific prevalence (?36·7%). The number of people with moderate and severe visual impairment also increased, from 159·9 million (80% UI 68·3–270·0) in 1990 to 216·6 million (80% UI 98·5–359·1) in 2015. Interpretation There is an ongoing reduction in the age-standardised prevalence of blindness and visual impairment, yet the growth and ageing of the world's population is causing a substantial increase in number of people affected. These observations, plus a very large contribution from uncorrected presbyopia, highlight the need to scale up vision impairment alleviation efforts at all levels. The objective of this study was to characterize the causes of ocular trauma and determine the risk factors for infection and vision loss following ocular trauma in the Solomon Islands.A prospective clinic-based study.A total of 507 patients with ocular trauma who were reviewed at the National Referral Hospital in Honiara or one of five provincial eye clinics were included.An interview-based questionnaire to determine the circumstances of ocular trauma, and an ocular examination to elicit the trauma sustained,infectious sequelae and the visual outcome.Visual acuity.Males were significantly more likely to have ocular trauma than females (P = 0.01). The major cause of ocular trauma in young boys and girls was being poked by a stick, followed by lime burns in young boys. For both genders, physical violence resulted in most injuries across all adult age groups. Microbial keratitis complicated 4.4% of ocular trauma. Monocular vision impairment (6/18) occurred in 5.5% of participants and was more likely to occur if female (P = 0.02).Ocular trauma is a significant cause of visual morbidity in the Solomon Islands. The results from this prospective study provide a basis for planning blindness prevention programmes in the Western Pacific.

BACKGROUND: To examine the prevalence, patterns and trends of vision impairment and its causes from 1990 to 2010 in Central and South Asia. METHODS: Based on the Global Burden of Diseases Study 2010 and ongoing literature searches, we examined prevalence and causes of moderate and severe vision impairment (MSVI ; presenting visual acuity

AIM: To estimate the magnitude, temporal trends and subregional variation in the prevalence of blindness, and moderate/severe vision impairment (MSVI) in sub-Saharan Africa. METHODS: A systematic review was conducted of published and unpublished population-based surveys as part of the Global Burden of Disease, Risk Factors and Injuries Study 2010. The prevalence of blindness and vision impairment by country and subregion was estimated. RESULTS: In sub-Saharan Africa, 52 studies satisfied the inclusion criteria. The estimated age- standardised prevalence of blindness decreased by 32% from 1.9% (95% CI 1.5% to 2.2%) in 1990 to 1.3% (95% CI 1.1% to 1.5%) in 2010 and MSVI by 25% from 5.3% (95% CI 0.2% to 0.3%) to 4.0% (95% CI 0.2% to 0.3%) over that time. However, there was a 16% increase in the absolute numbers with blindness and a 28% increase in those with MSVI. The major causes of blindness in 2010 were ; cataract 35%, other/unidentified causes 33.1%, refractive error 13.2%, macular degeneration 6.3%, trachoma 5.2%, glaucoma 4.4% and diabetic retinopathy 2.8%. In 2010, age- standardised prevalence of MSVI in Africa was 3.8% (95% CI 3.1% to 4.7%) for men and 4.2% (95% CI 3.6% to 5.3%) for women with subregional variations from 4.1% (95% CI 3.3% to 5.4%) in West Africa to 2.0% (95% CI 1.5% to 3.3%) in southern Africa for men ; and 4.7% (95% CI 3.9% to 6.0%) in West Africa to 2.3% (95% CI 1.7% to 3.8%) in southern Africa for women. CONCLUSIONS: The age-standardised prevalence of blindness and MSVI decreased substantially from 1990 to 2010, although there was a moderate increase in the absolute numbers with blindness or MSVI. Significant subregional and gender disparities exist.

Purpose: To describe a systematic review of population-based prevalence studies of visual impairment (VI) and blindness worldwide over the past 32 years that informs the Global Burden of Diseases, Injuries and Risk Factors Study. Methods: A systematic review (Stage 1) of medical literature from 1 January 1980 to 31 January 2012 identified indexed articles containing data on incidence, prevalence and causes of blindness and VI. Only cross-sectional population-based representative studies were selected from which to extract data for a database of age- and sexspecific data of prevalence of four distance and one near vision loss categories (presenting and best-corrected). Unpublished data and data from studies using rapid assessment methodology were later added (Stage 2). Results: Stage 1 identified 14,908 references, of which 204 articles met the inclusion criteria. Stage 2 added unpublished data from 44 rapid assessment studies and four other surveys. This resulted in a final dataset of 252 articles of 243 studies, of which 238 (98%) reported distance vision loss categories. A total of 37 studies of the final dataset reported prevalence of mild VI and four reported near VI. Conclusion: We report a comprehensive systematic review of over 30 years of VI/blindness studies. While there has been an increase in population-based studies conducted in the 2000s compared to previous decades, there is limited information from certain regions (eg, Central Africa and Central and Eastern Europe, and the Caribbean and Latin America), and younger age groups, and minimal data regarding prevalence of near vision and mild distance VI.

Vision impairment is a leading and largely preventable cause of disability worldwide. However, no study of global and regional trends in the prevalence of vision impairment has been carried out. We estimated the prevalence of vision impairment and its changes worldwide for the past 20 years.Systematic review.A systematic review of published and unpublished population-based data on vision impairment and blindness from 1980 through 2012.Hierarchical models were fitted fitted to estimate the prevalence of moderate and severe vision impairment (MSVI; defined as presenting visual acuity6/18 but ≥ 3/60) and the prevalence of blindness (presenting visual acuity3/60) by age, country, and year.Trends in the prevalence of MSVI and blindness for the period 1990 through 2010.Globally, 32.4 million people (95% confidence interval [CI], 29.4-36.5 million people; 60% women) were blind in 2010, and 191 million people (95% CI, 174-230 million people; 57% women) had MSVI. The age-standardized prevalence of blindness in older adults (≥ 50 years) was more than 4% in Western Sub-Saharan Africa (6.0%; 95% CI, 4.6%-7.1%), Eastern Sub-Saharan Africa (5.7%; 95% CI, 4.4%-6.9%), South Asia (4.4%; 95% CI, 3.5%-5.1%), and North Africa and the Middle East (4.6%; 95% CI, 3.5%-5.8%), in contrast to high-income regions with blindness prevalences of ≤ 0.4% or less. The MSVI prevalence in older adults was highest in South Asia (23.6%; 95% CI, 19.4%-29.4%), Oceania (18.9%; 95% CI, 11.8%-23.7%), and Eastern and Western Sub-Saharan Africa and North Africa and the Middle East (95% CI, 15.9%-16.8%). The MSVI prevalence was less than 5% in all 4 high-income regions. The global age-standardized prevalence of blindness and MSVI for older adults decreased from 3.0% (95% CI, 2.7%-3.4%) worldwide in 1990 to 1.9% (95% CI, 1.7%-2.2%) in 2010 and from 14.3% (95% CI, 12.1%-16.2%) worldwide to 10.4% (95% CI, 9.5%-12.3%), respectively. When controlling for age, women's prevalence of blindness was greater than men's in all world regions. Because the global population has increased and aged between 1990 and 2010, the number of blind has increased by 0.6 million people (95% CI, -5.2 to 5.3 million people). The number with MSVI may have increased by 19 million people (95% CI, -8 to 72 million people) from 172 million people (95% CI, 142-198 million people) in 1990.The age-standardized prevalence of blindness and MSVI has decreased in the past 20 years. However, because of population growth and the relative increase in older adults, the blind population has been stable and the population with MSVI may have increased

To assess the prevalence of diabetes and diabetic retinopathy in indigenous Australians aged ≥40 years.The National Indigenous Eye Health Survey used a stratified, multistage cluster probability sampling frame to provide a representative sample of the indigenous Australian population.One thousand one hundred and eighty-nine eligible indigenous adults were examined using standardized procedures.Each participant underwent a comprehensive eye examination included presenting and best corrected visual acuity, visual field, fundus and lens photography.Diabetic retinopathy.The prevalence of diabetes in the 1189 eligible indigenous adults was 37.3% (95% confidence interval: 34.6-40.2%). The prevalence of diabetic retinopathy among 394 people with diabetes was 29.7% (95% confidence interval: 25.2-34.2%), and 17.8% (95% confidence interval: 14.0-21.6%), 8.9% (95% confidence interval: 6.1-11.7%) and 3.1% (95% confidence interval: 1.3-4.7%) for mild or moderate diabetic retinopathy, clinically significant macular oedema and severe or proliferative diabetic retinopathy, respectively. Diabetic retinopathy was presented in 6.3% in those who did not report diabetes. The risk of diabetic retinopathy increased with duration of diabetes (the adjusted odds ratios were 3.4 for 10-19 years, 6.1 for 20-29 years and 25.8 for ≥30 years).The prevalence of self-reported diabetes in indigenous Australians is more than eight times higher than that in non-indigenous Australians. The prevalence of diabetic retinopathy in people with diabetes is similar to that of non-indigenous Australians.

To survey the current diabetic retinopathy screening and management practices of Australian optometrists following the release of the 1997 National Health Medical Research Council Diabetic Retinopathy Management Guidelines.Cross-sectional national survey, primary care setting.1000 Australian optometrists across different states.A self-administered questionnaire was sent to 1000 optometrists across all states during 2007/2008.Use of retinal camera, screening practices/attitudes and behaviour in diabetic retinopathy management.568 optometrists (57%) responded to the survey. Patients' unpreparedness to drive post dilation (51%) and the fear of angle closure glaucoma (13%) were the two main barriers to optometrists not performing dilated ophthalmoscopy. Those who had strong desire to screen for diabetic retinopathy were more likely to use a retinal camera (p0.005). Use of a retinal camera was significantly associated with an increased confidence in detecting clinical signs of diabetic retinopathy including macular oedema (P0.001). Optometrists who read the guidelines at least once were 2.5-times (P0.001) more likely to have confidence in detecting macular oedema than those who had never read the guidelines. Although they may be confident in diagnosis, and may use retinal cameras for screening, nearly 60% of optometrists would not refer patients with macular oedema to an ophthalmologist.Despite their self-reported desire for involvement in diabetic retinopathy, the management of macular oedema by Australian optometrists needs improvement. The use of retinal cameras and promotion of the 2008 NHMRC guidelines should be encouraged to improve overall optometric diabetic retinopathy management, particularly with macular oedema.

To determine the prevalence of vision loss due to cataract in indigenous Australians.A national, stratified, random cluster sample was selected in 30 communities across Australia. Data collection was undertaken in 2008. Adults 40 years and older were examined using a standardized protocol that included a questionnaire. The presence of visually significant cataract was assessed.Response rates were good and 1189 indigenous adults were examined and overall recruitment was 72%. Low vision (6/12-6/60) due to cataract occurred in 2.52% (1.63-3.41%) and blindness (6/60) in 0.59% (95% CI: 0.24-1.21%). The cataract coverage rate (proportion of those with visually significant cataract who had been operated on) was 65.3% (95% CI: 55.0-74.6%). Projections suggest that there are 3234 indigenous adults with vision loss from cataract.Cataract remains a major cause of vision loss in Aboriginal and Torres Strait Islander peoples. There were no significant regional or state differences in the prevalence of cataract or of cataract surgical coverage, which suggests that increased cataract surgery services are required across the country to address cataract in indigenous Australians.

To assess, among children with low literacy and poor stereoacuity, the efficacy of two intervention programs on child vision and education compared to a control program.Eighty-eight children aged 8 to 13 years who had reading problems, and demonstrated poor stereoacuity as measured by the Titmus stereocircle test (100 seconds arc) or computerized assessment were randomized to one of two intervention programs: Lawson vision or Phono-Graphix, or a control group: Parental Literacy Support. Vision (Lang test, visual acuity, convergence insufficiency symptom survey) and education assessments (Woodcock Reading Mastery Tests-Revised) were conducted at baseline, intervention end (10 weeks), and 36 weeks. Analysis used intention to treat multi-level models.Compared to the parental literacy support group, convergence insufficiency symptoms were reduced 36 weeks post-randomization amongst those receiving the Lawson orthoptic intervention (mean difference -5.55; 95% confidence interval (CI): -11.1 to -0.05, P0.05). Stereoacuity, measured by the Lang test, improved for both the Lawson and Phono-Graphix interventions compared to the parental literacy support group (-1.01; 95% CI: -1.6 to -0.4, P = 0.001, and -0.77; 95% CI: -1.4 to -0.2, P = 0.01). At the 36 week follow-up assessment, word identification had also improved for the Lawson and Phono-Graphix groups but other educational outcomes did not improve.A formal randomized control trial was feasible in this setting. Intervention among children with poor stereoacuity and low literacy produced small improvements in stereopsis and convergence insufficiency symptom scores. Further randomized control trials should be conducted to clarify the role of orthoptic intervention on literacy in selected child populations.

To describe the psychometric characteristics of the Impact of Vision Impairment (IVI) Profile and evaluate its validity and reliability over time and between different forms of administration.The IVI is a 32-item questionnaire developed to measure the impact of vision impairment on restriction of participation in daily activities in five domains of functioning. Each item is rated on a six-level scale from "no difficulty" to "can't do because of vision." The IVI was administered by trained interviewers to 115 people with impaired vision (visual acuity less than 6/12 or visual field deficit) who attended the Royal Victorian Eye and Ear Hospital, a vision rehabilitation agency, or a self-help group for people with impaired vision. Data were also collected on demographic characteristics of participants, cause of vision impairment, and distance and near vision. General health status was assessed with the Short Form-12 (SF-12) of the Physical and Mental Health Summary Scales. A subset of participants completed the IVI twice, either 1 to 2 weeks apart or by different forms of administration (different interviewers or self).Internal consistency of total and domain average IVI scores was high (alpha = 0.80-0.96) and sequential elimination of items did not affect consistency. Total and domain average IVI scores correlated moderately with both near and distance vision (r = 0.21-0.31) but did not correlate with physical or general health or comorbidity. Total and domain average IVI scores correlated most closely with global measures of restriction of participation (r = 0.44-0.82). Principal-components analysis confirmed that all IVI items contribute to one underlying theme and tended to confirm two of the five domains: emotional reaction to vision loss and mobility. The first three components explained 43%, 8%, and 6% of the variation in the data. Guttman split-half reliability coefficients between different forms of administration and over time ranged from 0.73 to 0.94 for domain and total IVI scores. Mean absolute difference for domain and total scores between administrations was less than 1 step for all domains and the total score.This study provides support that the IVI has sufficient internal and construct validity to measure the effect of vision impairment on restriction of participation in daily activities. The IVI demonstrates acceptable reliability over a short period and yields consistent results between interviewers. The IVI can also be self-administered with assurance that the results will be comparable to those that would have been obtained by a trained interviewer. Therefore, the psychometric characteristics of the IVI support its use in assessment of the vision rehabilitation needs of people with impaired vision. Its stability over time indicates that it has potential to evaluate outcomes of intervention.
