Abstract

The concept of phased arrays can be used in noninvasive ultrasound surgery based on magnetic resonance imaging (MRI). The technique allows the focus to be moved electronically and the focal size to be increased. The arrays can provide enough power to necrose living tissues in 10 sec. The technique is based on two arrays which comprise a spherically curved concentric ring array and a spherically curved 16 square element array. The former helps to move the focus along the central axis, and the latter helps to make the focus larger.

31. Hyperpolarized (129)Xe T (1) in oxygenated and deoxygenated blood

Abstract

The viability of the new technique of hyperpolarized (129)Xe MRI (HypX-MRI) for imaging organs other than the lungs depends on whether the spin-lattice relaxation time, T(1), of (129)Xe is sufficiently long in the blood. In previous experiments by the authors, the T(1) was found to be strongly dependent upon the oxygenation of the blood, with T(1) increasing from about 3 s in deoxygenated samples to about 10 s in oxygenated samples. Contrarily, Tseng et al. (J. Magn. Reson. 1997; 126: 79-86) reported extremely long T(1) values deduced from an indirect experiment in which hyperpolarized (129)Xe was used to create a 'blood-foam'. They found that oxygenation decreased T(1). Pivotal to their experiment is the continual and rapid exchange of hyperpolarized (129)Xe between the gas phase (within blood-foam bubbles) and the dissolved phase (in the skin of the bubbles); this necessitated a complicated analysis to extract the T(1) of (129)Xe in blood. In the present study, the experimental design minimizes gas exchange after the initial bolus of hyperpolarized (129)Xe has been bubbled through the sample. This study confirms that oxygenation increases the T(1) of (129)Xe in blood, from about 4 s in freshly drawn venous blood, to about 13 s in blood oxygenated to arterial levels, and also shifts the red blood cell resonance to higher frequency. Abstract

Several biocompatible carrier agents, in which xenon is highly soluble and has a long T(1), were tested, and injected in living rats. These included saline, Intralipid suspension, perfluorocarbon emulsion and (129)Xe gas-filled liposomes. The T(1) of (129)Xe in these compounds ranged from 47 to 116 s. Vascular injection of these carrier agents was tolerated well, encouraging their use for further experiments in live animals. In vivo spectra, obtained from gas-filled liposomes and perfluorocarbon solutions, suggest that these carrier agents have potential for use in angiography and perfusion imaging. Abstract

Laser-polarized 129Xe dissolved in a foam preparation of fresh human blood was investigated. The NMR signal of 129Xe dissolved in blood was enhanced by creating a foam in which the dissolved 129Xe exchanged with a large reservoir of gaseous laser-polarized 129Xe. The dissolved 129Xe T1 in this system was found to be significantly shorter in oxygenated blood than in deoxygenated blood. The T1 of 129Xe dissolved in oxygenated blood foam was found to be approximately 21 (+/-5) s, and in deoxygenated blood foam to be greater than 40 s. To understand the oxygenation trend, T1 measurements were also made on plasma and hemoglobin foam preparations. The measurement technique using a foam gas-liquid exchange interface may also be useful for studying foam coarsening and other liquid physical properties.

Abstract

We report the case of a 27-week premature infant in whom magnetic resonance imaging (MRI) at 4 postnatal weeks (postconceptional age, 31 weeks), term, and 6 months of age documented the postnatal postmigrational evolution of bilateral perisylvian polymicrogyria. The polymicrogyria was readily detected by ultrafine 1.5-mm coronal slices on three-dimensional, Fourier-transformed, spoiled gradient-recalled and T2-weighted MRI sequences. These MRI sequences provide the first in vivo documentation of the postmigrational evolution of polymicrogyria. The likelihood that the polymicrogyria was related to an ischemic encephaloclastic mechanism is supported by the simultaneous occurrence of periventricular leukomalacia.

Abstract

A new fast non rigid registration algorithm is presented. The algorithm estimates a dense deformation field by optimizing a criterion that measures image similarity by mutual information and regularizes with a linear elastic energy term. The optimal deformation field is found using a Simultaneous Perturbation Stochastic Approximation to the gradient. The implementation is parallelized for symmetric multi-processor architectures. This algorithm was applied to capture non-rigid brain deformations that occur during neurosurgery. Segmentation of the intra-operative data is not required but preoperative segmentation of the brain allows the algorithm to be robust to artifacts due to the craniotomy., Anglais

Abstract

One of the key unmet needs to improve long‐term outcomes of heart transplantation is to develop accurate, noninvasive, and practical diagnostic tools to detect transplant rejection. Early intragraft inflammation and endothelial cell injuries occur prior to advanced transplant rejection. We developed a novel diagnostic imaging platform to detect early declines in microvascular perfusion (MP) of cardiac transplants using contrast‐enhanced ultrasonography (CEUS). The efficacy of CEUSin detecting transplant rejection was tested in a murine model of heart transplants, a standard preclinical model of solid organ transplant. As compared to the syngeneic groups, a progressive decline in MPwas demonstrated in the allografts undergoing acute transplant rejection (40%, 64%, and 92% on days 4, 6, and 8 posttransplantation, respectively) and chronic rejection (33%, 33%, and 92% on days 5, 14, and 30 posttransplantation, respectively). Our perfusion studies showed restoration of MPfollowing antirejection therapy, highlighting its potential to help monitor efficacy of antirejection therapy. Our data suggest that early endothelial cell injury and platelet aggregation contributed to the early MPdecline observed in the allografts. High‐resolution MPmapping may allow for noninvasive detection of heart transplant rejection. The data presented have the potential to help in the development of next‐generation imaging approaches to diagnose transplant rejection. High‐resolution perfusion mapping using contrast‐enhanced ultrasound detects transplant rejection.

Abstract

We present a new algorithm for the nonrigid registration of three-dimensional magnetic resonance (MR) intraoperative image sequences showing brain shift. The algorithm tracks key surfaces of objects (cortical surface and the lateral ventricles) in the image sequence using a deformable surface matching algorithm. The volumetric deformation field of the objects is then inferred from the displacements at the boundary surfaces using a linear elastic biomechanical finite-element model. Two experiments on synthetic image sequences are presented, as well as an initial experiment on intraoperative MR images showing brain shift. The results of the registration algorithm show a good correlation of the internal brain structures after deformation, and a good capability of measuring surface as well as subsurface shift. We measured distances between landmarks in the deformed initial image and the corresponding landmarks in the target scan. Cortical surface shifts of up to 10 mm and subsurface shifts of up to 6 mm were recovered with an accuracy of 1 mm or less and 3 mm or less respectively.

Abstract

PURPOSE: Fast spin echo (FSE) images were compared with conventional images in 46 patients with intradural spinal disease to determine their accuracy in the detection and delineation of lesions. MATERIALS AND METHODS: The images were interpreted by two neuroradiologists, who read individually. A total of 720 blinded readings formed the basis for this evaluation. A gold standard for each patient was selected, based on the blinded readings of the conventional studies. RESULTS: In the sagittal plane, the FSE sequences were found to have an accuracy of 93% and 93% for the first reader and 93% and 85% for the second reader. For the axial plane, the corresponding figures were 86% and 82% for the first reader and 64% and 77% for the second reader. These figures compared favorably with conventional sequences. Similar delineation of lesions was noted in 78% of cases. In the remaining cases, there were no significant trends. CONCLUSION: Because of these findings, FSE sequences appear as accurate as conventional sequences. In this study, they were capable of supplanting conventional sequences in the evaluation of intradural pathology of the spine in the sagittal plane, although conventional sequences were still preferred in the axial plane.
