The objective of this study was to test the precision of in-vivo indirect bracket placement via medium-soft, transparent, broad-coverage, computer-aided designed and manufactured transfer trays using an automated digital method.Seventeen patients requiring vestibular fixed appliances were consecutively recruited, and bonding accuracy was measured at each bracket, evaluating 3 linear (mesiodistal, buccolingual, and vertical) and 3 angular measurements (torque, tip, and rotation) with an automated method involving digital superimposition of individual teeth. Mean and standard deviation values were calculated for both arches, single arch, and tooth type, and the percentages of single deviations over the thresholds of 0.25 mm and 1° were calculated, as well as maximum and minimum values for each deviation and directional bias. Correlations between each variable (arch, tooth type, and single tooth) and deviations were investigated through classification and regression trees (CART) predictive models.Neither mean nor single linear deviations ever exceeded the set cutoff value of 0.25 mm. Mean angular deviations never exceeded 1°, but some individual angular deviations did, specifically 8.31% of torque, 13.16% of tip, and 7.16% of rotation deviations. The highest percentage of deviation was recorded for rotation of the maxillary incisors (18.11%). No evident trend in directional deviation bias was found. Tooth type appears to influence mesiodistal and torque deviations, whereas the single tooth variable influenced the percentage of rotation deviations exceeding 1° (P 0.05).This computer-aided designed and manufactured medium-soft, transparent transfer tray provides accurate bracket placement and could be recommended for routine fixed appliance treatment.

The research project »text sound«: mixed-methods-analysis of lyric poetry in text and tonal sound (funded by the Federal Ministry for Education and Research, BMBF) aims to undertake a systematic and diachronic investigation of the relationship between literary texts, especially lyric poetry from the Romantic period, and their phonetic realisation in recitations or musical performances. Ideas of orality, sound and voice, which are particularly associated with poetry, are investigated empirically and also theorised in the line with modern approaches to the analysis of lyric poetry. Of particular importance is the experimental approach of speech synthesis, i.e. using computers to artificially produce a human sounding voice; this approach makes it possible to explore an ideal-typical realisation of the text and to test the aesthetic peculiarity of human realisations.

ZUSAMMENFASSUNG Gegenstand und Ziel Viele Patienten mit posturalem Tachykardie-Syndrom (PoTS) erhalten eine Hautbiopsie, um eine mögliche Small Fiber Neuropathie (SFN) zu erkennen. Ziel der Fragebogenstudie war es, mögliche Nebenwirkungen nach Lokalanästhesie mit Adrenalinzusatz zu erkennen. Material und Methoden 52 Patienten (29 PoTS, 23 SFN) füllten die retrospektive Befragung aus. Es wurde eine qualitative deskriptive und quantitative Datenanalyse mit Zwischensubjektdesign durchgeführt. Ergebnisse Alle Nebenwirkungen wurden bei PoTS (32 Items, relative Häufigkeit 15,8 %) häufiger als bei SFN-Patienten (8 Items, 5,0 %), entweder während oder nach der Biopsie (p = 0,027) beschrieben. Nur Patienten mit PoTS beschrieben Hämatome und verlängerte Blutung. Schlussfolgerung In der klinischen Routine besteht Unsicherheit bei Patienten, ob die Hautbiopsie unter Lokalanästhesie mit Adrenalinzusatz Nebenwirkungen haben kann. In dieser Pilotstudie zeigten sich keine schwerwiegenden Nebenwirkungen, die die Durchführung der Hautbiopsien in der klinischen Diagnostik bei PoTS einschränken könnten. Klinische Relevanz Auf Basis dieser Daten wird eine prospektive Studie durchgeführt.

Fatigue is a common and disabling non-motor symptom (NMS) in Parkinson's disease (PD) patients. However, the effect of subthalamic nucleus (STN) deep brain stimulation (DBS) on fatigue has not been widely studied.To determine the effect of STN DBS on fatigue in PD patients, measured by the Non-motor symptoms scale (NMSS).Cross-sectional analysis of 50 patients with PD who underwent STN DBS at King's College Hospital and Salford Royal Hospital with fatigue scores (measured by question number 4 from domain 2 (sleep/fatigue) of the NMSS as the primary outcome measure. Secondary outcome measures included the PD Sleep Scale (PDSS), Scales for Outcome in PD (SCOPA)-motor examination, activities of daily living, motor complications, Hoehn and Yahr (HY) stage and changes in Levodopa Equivalent Daily Dose (LEDD).50 patients with a mean follow-up period of 1.98 ± 1.36 years were studied. Significant improvement in median fatigue scores (4.00 (0.75-9.00) to 1.00 (0.00-4.50);Even though open label and not using a validated fatigue scale, this observational analysis suggest that fatigue improves significantly after STN DBS with persisting benefits at two years follow-up.

We aimed to assess the effects of hyperparameter tuning and automatic image augmentation for deep learning-based classification of orthodontic photographs along the Angle classes. Our dataset consisted of 605 images of Angle class I, 1038 images of class II, and 408 images of class III. We trained ResNet architectures for classification of different combinations of learning rate and batch size. For the best combination, we compared the performance of models trained with and without automatic augmentation using 10-fold cross-validation. We used GradCAM to increase explainability, which can provide heat maps containing the salient areas relevant for the classification. The best combination of hyperparameters yielded a model with an accuracy of 0.63–0.64, F1-score 0.61–0.62, sensitivity 0.59–0.65, and specificity 0.80–0.81. For all metrics, it was apparent that there was an ideal corridor of batch size and learning rate combinations; smaller learning rates were associated with higher classification performance. Overall, the performance was highest for learning rates of around 1–3 × 10−6 and a batch size of eight, respectively. Additional automatic augmentation improved all metrics by 5–10% for all metrics. Misclassifications were most common between Angle classes I and II. GradCAM showed that the models employed features relevant for human classification, too. The choice of hyperparameters drastically affected the performance of deep learning models in orthodontics, and automatic image augmentation resulted in further improvements. Our models managed to classify the dental sagittal occlusion along Angle classes based on digital intraoral photos.

Objectives To investigate and compare the transfer accuracy of five different surgical guides (SGs) for the insertion of orthodontic mini-implants (OMIs) in the anterior palate. Materials and Methods Stereolithographic files of 10 maxillary patient models and their corresponding lateral cephalograms were virtually matched and used for planning the position of two parallel OMIs in the paramedian region of the anterior palate. For each patient model, three 3-dimensional (3D)–printed and two conventional SGs were manufactured from different materials, and a total of 96 OMIs were transferred to the anterior palates of the respective 50 molded resin models. The planned (T0) and the actual (T1) OMI positions were analyzed and compared after superimposition of the digitized models. The deviations between the OMI positions in T0 and T1 were described as the distance between the head and the tip, respectively, of each OMI in millimeters and the deviating angle between the OMI axes for each patient and SG. Results The conventionally manufactured SGs of Pattern Resin LS (GC Europe N.V., Leuven, Belgium) showed the highest linear and angular transfer accuracy for the insertion of OMIs. The highest deviations were found with the SGs made of IMPRIMO LC Splint (3D-printed; Scheu-Dental, Iserlohn, Germany) and Memosil 2 (conventional SG; Kulzer, Hanau, Germany). Conclusions The 3D-printed SGs did not reach the accuracy of the conventional SGs made of Pattern Resin but may provide sufficient accuracy for palatal OMI placement.

Pinholin S2168 triggers the lytic cycle of bacteriophage φ21 in infected Escherichia coli. Activated transmembrane dimers oligomerize into small holes and uncouple the proton gradient. Transmembrane domain 1 (TMD1) regulates this activity, while TMD2 is postulated to form the actual “pinholes.” Focusing on the TMD2 fragment, we used synchrotron radiation-based circular dichroism to confirm its α-helical conformation and transmembrane alignment. Solid-state 15N-NMR in oriented DMPC bilayers yielded a helix tilt angle of τ = 14°, a high order parameter (Smol = 0.9), and revealed the azimuthal angle. The resulting rotational orientation places an extended glycine zipper motif (G40xxxS44xxxG48) together with a patch of H-bonding residues (T51, T54, N55) sideways along TMD2, available for helix–helix interactions. Using fluorescence vesicle leakage assays, we demonstrate that TMD2 forms stable holes with an estimated diameter of 2 nm, as long as the glycine zipper motif remains intact. Based on our experimental data, we suggest structural models for the oligomeric pinhole (right-handed heptameric TMD2 bundle), for the active dimer (right-handed Gly-zipped TMD2/TMD2 dimer), and for the full-length pinholin protein before being triggered (Gly-zipped TMD2/TMD1-TMD1/TMD2 dimer in a line).

Study Design Questionnaire-based survey. Objectives Surgical site infection (SSI) is a common complication in spine surgery but universal guidelines for SSI prevention are lacking. The objectives of this study are to depict a global status quo on implemented prevention strategies in spine surgery, common themes of practice and determine key areas for future research. Methods An 80-item survey was distributed among spine surgeons worldwide via email. The questionnaire was designed and approved by an International Consensus Group on spine SSI. Consensus was defined as more than 60% of participants agreeing to a specific prevention strategy. Results Four hundred seventy-two surgeons participated in the survey. Screening for Staphylococcus aureus (SA) is not common, whereas preoperative decolonization is performed in almost half of all hospitals. Body mass index (BMI) was not important for surgery planning. In contrast, elevated HbA1c level and hypoalbuminemia were often considered as reasons to postpone surgery. Cefazoline is the common drug for antimicrobial prophylaxis. Alcohol-based chlorhexidine is mainly used for skin disinfection. Double-gloving, wound irrigation, and tissue-conserving surgical techniques are routine in the operating room (OR). Local antibiotic administration is not common. Wound closure techniques and postoperative wound dressing routines vary greatly between the participating institutions. Conclusions With this study we provide an international overview on the heterogeneity of SSI prevention strategies in spine surgery. We demonstrated a large heterogeneity for pre-, peri- and postoperative measures to prevent SSI. Our data illustrated the need for developing universal guidelines and for testing areas of controversy in prospective clinical trials.

To present an image-processing measurement algorithm to evaluate the transfer accuracy of indirect bonding (IDB) trays, exemplified by a CAD/CAM-based IDB tray integrated into a digital orthodontic workflow.Plaster casts of 24 patients with full dentition and different malocclusions were scanned with an intraoral scanner (Trios; 3Shape, Copenhagen, Denmark) to obtain digital models, which served for the virtual placement of orthodontic brackets in simulation software (OnyxCeph; Image Instruments, Chemnitz, Germany). The resulting STL files were sent to a dental laboratory (CA Digital; Hilden, Germany) for the production of INDIVIDUA IDB trays. These trays were used to transfer the brackets to the respective plaster casts. Finally, a second scan was performed to record the actual bracket positions. The transfer accuracy was then analyzed by a measurement algorithm scripted to automation, which calculated the deviations of the planned and real bracket positions with a local best-fit alignment, resulting in three linear and three angular measurements for each bracket.In total, 622 brackets and tubes were transferred successfully. The presented algorithm analyzed the transfer accuracy and demonstrated that the linear measurements were 98.3% within the range of the American Board of Orthodontics standard. The angular measurements were 86.7% within this range when the INDIVIDUA IDB tray was used.Scripted measurement algorithms facilitated the evaluation of present and future materials and designs for IDB trays to obtain an efficient solution for orthodontic practice. The INDIVIDUA IDB tray is a digital alternative to conventional IDB trays (Int J Comput Dent 2022;25(3):295-302; doi: 10.3290/j.ijcd.b2599775).

Objective: This study aims to investigate the transfer accuracy of two different design versions for 3D-printed indirect bonding (IDB) trays. Materials and Methods: Digital plaster models of 27 patients virtually received vestibular attachments on every tooth using OnyxCeph³™ (Image Instruments, Chemnitz, Germany). Based on these simulated bracket and tube positions, two versions of transfer trays were designed for each dental arch and patient, which differed in the mechanism of bracket retention: Variant one (V1) had arm-like structures protruding from the tray base and reaching into the horizontal and vertical bracket slots, and variant two (V2) had a pocket-shaped design enclosing the brackets from three sides. Both tray designs were 3D-printed with the same digital light processing (DLP) printer using a flexible resin-based material (IMPRIMO® LC IBT/Asiga MAX™, SCHEU-DENTAL, Iserlohn, Germany). Brackets and tubes (discovery® smart/pearl, Ortho-Cast M-Series, Dentaurum, Ispringen, Germany) were inserted into the respective retention mechanism of the trays and IDB was performed on corresponding plaster models. An intraoral scan (TRIOS® 3W, 3Shape, Copenhagen, Denmark) was performed to capture the actual attachment positions and compared to the virtually planned positions with Geomagic© Control (3D Systems Inc., Rock Hill, SC, USA) using a scripted calculation tool, which superimposed the respective tooth surfaces. The resulting attachment deviations were determined in three linear (mesiodistal, vertical and orovestibular) and three angular (torque, rotation and tip) directions and analyzed with a descriptive statistical analysis. A comparison between the two IDB tray designs was conducted using a mixed model analysis (IBM, SPSS® Statistics 27, Armonk, NY, USA). Results: Both design versions of the 3D-printed IDB trays did not differ significantly in their transfer accuracy (p > 0.05). In total, 98% (V1) and 98.5% (V2) of the linear deviations were within the clinically acceptable range of ± 0.2 mm. For the angular deviations, 84.9% (V1) and 86.8% (V2) were within the range of ± 1 degrees. With V1, most deviations occurred in the mesiodistal direction (3.3%) and in rotation (18%). With V2, most deviations occurred in the vertical direction (3.8%) and in palatinal and lingual crown torque (16.3%). Conclusions: The transfer accuracies of the investigated design versions for 3D-printed IDB trays show good and comparable results albeit their different retention mechanisms for the attachments and are, therefore, both suitable for clinical practice.

Die Mesopausenregion in einer Höhe von 80-100 km ist ein Bereich der Atmosphäre, der schwer zu erforschen ist, da direkte Messungen kaum möglich sind. Eine Möglichkeit bieten aber Messungen des Airglows. Airglow entsteht, wenn elektromagnetische Strahlung durch chemische Reaktionen ausgesendet wird. Diese kann dann mit Satelliten oder vom Boden aus gemessen werden. Eine Reaktion bei der Airglow entsteht ist die Reaktion von Natrium mit Ozon. Bei dieser Reaktion, die auch Chapman-Mechanismus genannt wird, entsteht angeregtes Natrium mit zwei unterschiedlichen Gesamtdrehimpulsquantenzahlen. Die Abregung in den Grundzustand führt dann zu den zwei bekannten Natrium D-Linien (D2=589,0 nm; D1=589,6 nm). Wie hoch der Anteil des Natriums, der durch die Reaktion in den angeregten Zustand übergeht ist, ist dabei noch nicht genau geklärt. Das Verhältnis von Natrium im Grundzustand und Natrium im angeregten Zustand wird durch die branching ratio f beschrieben. Deren Wert wird in der aktuellen Forschung zwischen 5 und 16 % geschätzt. In dieser Arbeit soll, durch Vergleich verschiedener Messungen von unabhängigen Instrumenten, dieser Wert weiter eingegrenzt werden. Im Zentrum stehen dabei die Messungen des Instrumentes OSIRIS auf dem Satelliten ODIN und der Vergleich zu Messungen, die mit verschiedenen bodengestützten Natrium-Lidars gemacht worden sind. Das Ergebnis ist eine Schätzung der branching ratio auf ca. 6 %.

Sodium airglow is generated when excited sodium atoms emit electromagnetic radiation while they are relaxing from an excited state into a lower energetic state. This electromagnetic radiation, the two sodium D lines at 589.0 and 589.6 nm, can usually be detected from space or from ground. Sodium nightglow occurs at times when the sun is not present and excitation of sodium atoms is a result of chemical reaction with ozone. The detection of sodium nightglow can be a means to determine the amount of sodium in the earth's mesosphere and lower thermosphere (MLT). In this study, we present time series of monthly mean sodium concentration profiles, by utilizing the large spatial and temporal coverage of satellite sodium D-line nightglow measurements. We use the OSIRIS/Odin mesospheric limb measurements to derive sodium concentration profiles and vertical column densities and compare those to measurements from SCIAMACHY/Envisat and GOMOS/Envisat. Here we show that the Na D-line limb emission rate (LER) and volume emission rate (VER) profiles calculated from the OSIRIS and SCIAMACHY measurements, although the OSIRIS LER and VER profiles are around 25 % lower, agree very well in shape and overall seasonal variation. The sodium concentration profiles also agree in shape and magnitude, although those do not show the clear semi-annual cycle which is present in the LER and VER profiles. The comparison to the GOMOS sodium vertical column densities (VCDs) shows that the OSIRIS VCDs are of the same order of magnitude although again the semi-annual cycle is not as clear. We attribute the differences in the LER, VER and sodium profiles to the differences in spatial coverage between the OSIRIS and SCIAMACHY measurements, the lower signal-to-noise ratio (SNR) of the SCIAMACHY measurements and differences in local time between the measurements of the two satellites.

Using a combination of different measurement techniques is important to understand the numerous processes happening in the MLT-region. One of those processes is the excitation of atomic sodium by reaction with ozone which leads to emission of electromagnetic radiation: a phenomenon called Airglow. Although the sodium excitation mechanism was already proposed in 1939 by Sidney Chapman and further investigation was done by a great number of scientists, there are still some key parameters that are not well-known today. One of those parameters is the branching ratio fA which determines the amount of sodium in the excited state. Exact knowledge of this value would offer the opportunity to use Na-nightglow measurements to determine sodium profiles in the MLT-region. In this study we used both, satellite measurements and ground-based Lidar measurements to help approach a more reliable branching ratio fA. By comparing measurements that were made by the two instruments OSIRIS on Odin (Satellite) and the Lidar of the Colorado State University (ground-based) we found a branching ratio fA of 0.064 +- 0.028.

When Bara, a carefully crafted documentary about a hospital in Soweto, hit the South African state television screens in October 1979, its producer, Kevin Harris, was fired from the broadcaster within 24 hours and made headlines for weeks on end in South African newspapers. Taking the events around the broadcast as a point of departure, I analyse a South African discourse of authority and its challenges by moving across the levels of media production and textuality. I argue that the documentary highlighted the impossibility as well as the immorality of lived apartheid, a revelation which was often concealed in public secrecy. The chapter thus aims to understand the particular relationship between visibility and legitimacy in the regime set up by Hendrik Frensch Verwoerd, the psychology and sociology professor and newspaper editor who was to become South African Prime Minister in 1959. In the equation of two profiles of authorization, I place the politician and the filmmaker both within the category of media experts, who themselves produce media which in turn legitimate their positions.

Contamination of animal feed with Fusarium spp results in accumulation of mycotoxins including deoxynivalenol. In animals, deoxynivalenol is metabolized to de-epoxy deoxynivalenol (DOM-1), which is generally considered to be a non-toxic metabolite; however, recent studies demonstrated that DOM-1 can reduce steroid production and induce apoptosis in the bovine ovary. The objectives of this study were to assess the effects of DOM-1 on applied aspects of reproductive function in cattle, specifically sperm function and embryo development in vitro and follicle growth and superovulatory responses in vivo. The effect of naturally contaminated feed on superovulatory responses was assessed; a dose of 6 ppm deoxynivalenol increased blood DOM-1 concentrations to 20 ng/ml, but this did not alter the number of viable embryos recovered on day 7. However, intrafollicular injection of DOM-1 (100 ng/ml) directly into the growing dominant follicle resulted in cessation of follicular growth over the subsequent 3 days. Treatment with DOM-1 reduced motility of bull spermatozoa over a 10-h period in vitro. Addition of DOM-1 to oocytes in vitro during IVM did not alter rates of cumulus expansion and nuclear maturation, but treatment during IVF reduced the rate of blastocyst formation. These data illustrate that DOM-1 is more biologically active than previously thought and negatively impacted reproductive outcomes in cattle.

Over the course of the sustainable energy transition, distributed energy generation becomes increasingly important. Building residential energy installations requires resources (expertise, time, financial liquidity, space) not all citizens have at hand. Especially in urban areas, where land is scarce and many people live as tenants, only a minority qualifies. To include urban households in the energy transition, new smart and efficient solutions need to be developed. In this article, we examine an example of an innovative energy project in Zurich (Switzerland) that complements the concept of community energy, offering residents a simple and cost-effective way to participate in photovoltaic installations in their city. The aim of the study is to gain understanding of the project characteristics that trigger or hinder participation, drawing on qualitative data from semi-structured telephone interviews with participants and non-participants (n = 18). The main drivers for participation are the direct and tangible way of supporting local sustainable energy generation, and the desire to feel as a co-owner at little effort and expense. Conversely, reservations against photovoltaics and a lack of financial resources, knowledge, or interest act as barriers for participation. The study lays a foundation for further quantitative examination and for the development of other urban energy projects.

The Swiss energy system will undergo successive restructuring in the next decades as a result of recent decisions in energy policy (Switzerland’s Energy Strategy 2050) and the expected second step in the market liberalization process (free choice of provider for small customers). This transformation will probably lead to the following trends: an increasing share of renewable energies, a shift from centralized to distributed generation and the emergence of new players in the energy market. Because of the associated integration of renewables, the grid will increasingly face problems of intermittent loads, the need to integrate smart information and communication technology, and mechanisms for demand-side management. However, these key elements of the future energy system also raise issues concerning social acceptance, as they tend to interfere with basic human needs like autonomy and privacy. Additionally, market liberalization may reshuffle existing customer utility relationships considerably. This focus group study contributes to understanding residential consumers’ attitudes and expectations regarding four scenarios of future utility/customer interactions in the Swiss electricity sector (dynamic tariffs, direct load control, energy storage and novel energy services). The findings show that as to acceptance of future scenarios, there are no “one-fit-for-all” solutions, as the needs of different consumer groups are very diverse. However, the analyses reveal that for acceptance of novel models of interaction, transparent communication and a trusted relationship are crucial. The discussion touches on practical implications for research, policymaking and electric utilities’ market strategies.
