7. A simple model to predict early recurrence of hepatocellular carcinoma after liver resection.

Abstract

Purpose: Multiple studies have reported models for predicting early recurrence of hepatocellular carcinoma (HCC) after liver resection (LR). However, these models are too complex to use in daily practice. We aimed to develop a simple model. Method: We enrolled 1133 patients with newly diagnosed HCC undergoing LR. The Kaplan − Meier method and log-rank test were used for survival analysis and Cox proportional hazards analysis to identify prognostic factors associated with early recurrence (i.e., recurrence within two years after LR). Results: Early recurrence was identified in 403 (35.1%) patients. In multivariate analysis, alpha-fetoprotein (AFP) 20–399 vs. < 20 ng/ml (HR = 1.282 [95% confidence interval = 1.002–1.639]; p = 0.048); AFP ≥ 400 vs. < 20 ng/ml (HR = 1.755 [1.382–2.229]; p < 0.001); 7th edition American Joint Committee on Cancer (AJCC) stage 2 vs. 1 (HR = 1.958 [1.505–2.547]; p < 0.001); AJCC stage 3 vs. 1 (HR = 4.099 [3.043–5.520]; p < 0.001); and pathology-defined cirrhosis (HR = 1.46 [1.200–1.775]; p < 0.001) were associated with early recurrence. We constructed a predictive model with these variables, which provided three risk strata for recurrence-free survival (RFS): low risk, intermediate risk, and high risk, with two-year RFS of 79%, 57%, and 35%, respectively (p < 0.001). 38. Endoscopic submucosal dissection and radiofrequency ablation for patients with flat‐type esophageal squamous cell neoplasia

Abstract

Several studies have demonstrated that endoscopic submucosal dissection (ESD) and radiofrequency ablation (RFA) are effective for patients with flat‐type esophageal squamous cell neoplasia (ESCN). In the present study, we aimed to report the outcomes achieved using these two endoscopic treatments. Patients who underwent ESD or RFA for flat‐type ESCN at Kaohsiung Chang Gung Memorial Hospital between January 2011 and December 2016 were included in this retrospective chart review. Treatment outcome, procedure time, and complication and recurrence rates were compared. Among 35 patients with flat‐type ESCN enrolled, 31 were treated with ESD and 4 with RFA. Overall mean age was 58.34 ± 11.5 years; 32 patients were men and 3 were women. The mean procedure times were 138.13 ± 101.47 minutes for ESD and 26.75 ± 11.1 minutes for RFA (P< .05). Complications occurred in 2 of the 31 patients (6.5%) in the ESD group—a perforation and a stricture. Further, one of the four patients after single RFA (25%) experienced a stricture. Recurrences occurred in three out of the four patients (75%) after single RFA; however, none occurred in patients in the ESD group. Of these recurrences, one was a submucosal invasion that was cured using photodynamic therapy followed by esophagectomy, whereas the other two were small focal epithelial recurrences that were cured using cap‐assisted endoscopic mucosal resection. No further recurrences were noted. ESD for flat‐type ESCN is effective for the eradication of ESCN. RFA for flat‐type ESCN carries a high risk of recurrence.

Abstract

Abstract Background The significant advances in microarray and proteomics analyses have resulted in an exponential increase in potential new targets and have promised to shed light on the identification of disease markers and cellular pathways. We aim to collect and decipher the HCC-related genes at the systems level. Results Here, we build an integrative platform, the Encyclopedia of Hepatocellular Carcinoma genes Online, dubbed EHCO http://ehco.iis.sinica.edu.tw, to systematically collect, organize and compare the pileup of unsorted HCC-related studies by using natural language processing and softbots. Among the eight gene set collections, ranging across PubMed, SAGE, microarray, and proteomics data, there are 2,906 genes in total; however, more than 77% genes are only included once, suggesting that tremendous efforts need to be exerted to characterize the relationship between HCC and these genes. Of these HCC inventories, protein binding represents the largest proportion (~25%) from Gene Ontology analysis. In fact, many differentially expressed gene sets in EHCO could form interaction networks (e.g. HBV-associated HCC network) by using available human protein-protein interaction datasets. To further highlight the potential new targets in the inferred network from EHCO, we combine comparative genomics and interactomics approaches to analyze 120 evolutionary conserved and overexpressed genes in HCC. 47 out of 120 queries can form a highly interactive network with 18 queries serving as hubs. Conclusion This architectural map may represent the first step toward the attempt to decipher the hepatocarcinogenesis at the systems level. Targeting hubs and/or disruption of the network formation might reveal novel strategy for HCC treatment.
