Aims: Postoperative delirium (POD) is a common neurological complication in elderly patients after anesthesia/surgery. The main purpose of this study is to explore the effect of circRNA‐targeted miRNA regulating SIRT3 on mitochondrial function through ceRNA mechanism under the surgical model of tibial fracture and to further explore the potential mechanism of postoperative delirium mediated by circRNA, so as to provide new ideas for clinical diagnosis and prevention of POD. Methods: The surgical model of tibial fracture under sevoflurane anesthesia caused acute delirium‐like behavior in elderly mice. We observed that the decrease of SIRT3 and mitochondrial dysfunction was related to POD, and miRNA and circRNA (circRNA_34414) related to SIRT3 were further studied. Through luciferase and RAP, we observed that circRNA_34414, as a miRNA sponge, was involved in the regulation of SIRT3 expression. Results: Postoperative delirium in elderly mice showed decreased expression of hippocampal circRNA_34414, increased expression of miR‐6960‐5p, decreased expression of SIRT3, and impaired mitochondrial membrane potential. Overexpression of circRNA_34414, or knockdown of miR‐6960‐5p, or overexpression of SIRT3 in hippocampal CA1 glutamatergic neurons significantly upregulated hippocampal SIRT3 expression, increased mitochondrial membrane potential levels, and significantly ameliorated postoperative delirium in aged mice; CircRNA_34414 ameliorates postoperative delirium in mice, possibly by targeting miR‐6960‐5p to upregulate SIRT3. Conclusions: CircRNA_34414 is involved in the improvement of postoperative delirium induced by anesthesia/surgery by upregulating SIRT3 via sponging miR‐6960‐5p. [ABSTRACT FROM AUTHOR]

Objective To explore the clinical characteristics of 2 668 inpatients in emergency ward, and to provide reference for emergency management. Methods Total of 2 668 patients hospitalized in the emergency department of xuanwu hospital from January 2015 to June 2018 were selected and the study data were retrospectively collected, including gender, age, comorbidities involvement system, hospitalization cost, length of hospital stay, and self-care ability score before hospitalization and before discharge. Results 1)The average age of male patients was younger than that of females (67.2±17.1 vs 70.7±17.2 years old)(P＜0.01). 2)2 408(90.25%) patients admitted to the hospital with more than 3 kinds of co-morbidities, and the co-morbidities involved mainly in respiratory, circulation, and endocrine systems. 3)The immediate living ability score was significantly lower than that before discharge (69.42±25.09 vs 78.12±26.13) (P＜0.001). 4)Barthel score was negatively correlated with length of stay, cost of hospitalization, decreased albumin, and anemia. Conclusions The types of diseases in the emergency ward are complicated, involving multiple disciplines and specialties, and the critical condition is variabl. Comprehensive and comprehensive evaluation is necessary in the process of clinical diagnosis, treatment and nursing care to improve the clinical outcomes of patients.

[Objectives] To reveal the genetic structure and diversity of 13 geographical populations of Conopomorpha sinensis in China by analyzing variation in the CO I gene fragment. [Methods] CO I gene fragments of 258 C. sinensis specimens from 13 geographical populations were amplified using PCR and sequenced. Based on the sequences obtained, haplotype composition, genetic diversity and genetic differentiation, of the geographic populations were analyzed with bioinformatic methods. [Results] The length of the CO I gene fragment amplified from the 258 specimens was 1 349 bp. 56 haplotypes were identified, including 14 common haplotypes and 42 population-specific haplotypes, which indicates a relatively high level of gene flow and genetic differentiation among geographic populations. Haplotype 4 is a common haplotype among all 13 geographical populations, indicating that it is relatively primitive, adaptable and stable in C. sinensis. Genetic diversity analysis indicates that geographical populations of C. sinensis have high haplotype diversity (Fst) and low nucleotide diversity (Nm). The fixed coefficient (Fst) and gene flow (Nm) for all specimens were 0.085 and 5.382, respectively, indicating a high level of gene flow among populations. [Conclusion] There is evidence of frequent gene exchange between different geographical populations of C. sinensis. The total population may have experienced rapid growth in the past, but there has been insufficient time to accumulate nucleotide mutations, resulting in a relatively high level of haplotype diversity, but a low level of nucleotide diversity.
