- Abstract
OBJECTIVE: Transit-time flow measurement (TTFM) represents a valid tool in the assessment of the quality of the anastomosis during coronary artery bypass graft (CABG). Nevertheless, a high variability limits its standardized use, lacking univocally accepted cutoff flow values. Our study analyzes TTFM data collected from a study population that underwent off-pump CABG (OP-CABG), with the aim to differentiate into subgroups according to the presence of diabetes mellitus. PATIENTS AND METHODS: Patients referred to the Cardiovascular Unit of S. Michele Hospital (Caserta, Italy) for coronary artery disease (CAD) and underwent OP-CABG between January 2015 and December 2019 were enrolled, and intraoperative TTFMs data were recruited and evaluated. Mean graft flow (MGF) and pulsatility index (PI) values were collected and analyzed. RESULTS: The study population was composed of 342 patients who underwent OP-CABG with TTFM data regarding 824 grafts. Diabetic patients shared a higher cardiovascular risk profile. The TTFM assessment showed better results for the use of the arterial grafts in diabetic patients, especially for those insulin-dependent; conversely, venous grafts showed worse data with lower MGF and higher PI values. In particular, the anastomoses of the saphenous vein graft with marginal obtuse (MO) coronary artery showed worse MGF results in the insulin-dependent rather than normoglycemic subgroup (28.66 vs. 38.44, p=0.003). CONCLUSIONS: Diabetic patients, especially in the insulin-dependent subgroups, have demonstrated lower MGF and higher PI values collected from venous anastomoses with, conversely, inverse results from the arterial one. These results might be correlated to an altered biological adaptability caused by the effects of the diabetic endocrine disorder. [ABSTRACT FROM AUTHOR]

Background: Dupilumab, a fully human monoclonal antibody targeting the alpha subunit of IL-4 was recently approved for the treatment of moderate-to-severe atopic dermatitis (AD) in adult patients. Objective: To assess dupilumab effectiveness and safety in adults with moderate-to-severe AD in a real-life Italian multicentre retrospective cohort. Methods: Adult moderate-to-severe AD patients, referring to 39 Italian centers, received dupilumab in the context of a national patient access program. Disease assessment was performed at baseline, after 4 and 16 weeks of treatment using Eczema-Area-and-Severity-Index (EASI) score, itch and sleep numerical-rating-score (itch-NRS, sleep-NRS) and Dermatology-Life-Quality-Index (DLQI). Results: A total of 109 (71 M/38F) patients was studied. There was a significant reduction in EASI score, itch-NRS, sleep-NRS and DLQI from baseline to week 4 and a further significant decline to week 16. EASI 50, EASI75 and EASI90 were achieved by 59.6%, 28.4% and 9.3% of patients at 4 weeks and by 87.2%, 60.6% and 32.4% of them at 16 weeks, respectively. Adverse events were experienced by 19.2% (21/109) of the patients and they were all mild in intensity, being conjunctivitis the most common side effect. Conclusions: Dupilumab significantly improved disease severity, pruritus, sleep loss and quality of life with an acceptable safety profile.

EASI 50, EASI75 and EASI90 were achieved by 59.6%, 28.4% and 9.3% of patients at 4 weeks and by 87.2%, 60.6% and 32.4% of them at 16 weeks, respectively. Adverse events were experienced by 19.2% (21/109) of the patients and they were all mild in intensity, being conjunctivitis the most common side effect. Conclusions: Dupilumab significantly improved disease severity, pruritus, sleep loss and quality of life with an acceptable safety profile.

Reducing the nuclear loads in the toroidal field (TF) coils is a main driver to the integrated design of the DEMO tokamak. Problems may arise from local peak values of nuclear heating in the winding packs, as well as neutron fluence and irradiation damage on the epoxy insulator and the copper stabilizer. In particular, the integration of various systems into the different vacuum vessel ports must consider in each design step the shielding of neutrons generated by the fusion plasma with the objective to limit radiation loads to below specific design limits. The pre-conceptual phase of the EU DEMO development is concluding in 2020 and the design of all essential ports have reached sufficient maturity to allow a first global assessment of the nuclear loads expected in TF coils. The analyses can thus also take into account the cross-talk between different ports. The results are compared to the radiation limits defined for the DEMO TF coils at the beginning of the pre-conceptual phase. Furthermore, the results will guide future work on shielding design and inform designers of systems yet to be integrated into DEMO of promising shielding strategies.

As indicated in the European Fusion Roadmap, the main objective of the Divertor Tokamak Test facility (DTT) is to explore alternative power exhaust solutions for DEMO so as to mitigate the risk that the conventional divertor based on detached conditions to be tested on the ITER device cannot be extrapolated to a fusion reactor. The issues to be investigated by DTT include: • demonstrate a heat exhaust system capable of withstanding the large load of DEMO in case of inadequate radiated power fraction; • close the gaps in the exhaust area that cannot be addressed by present devices; • demonstrate that the possible (alternative or complementary) solutions (e.g., advanced divertor configurations or liquid metals) can be integrated in a DEMO device. In this paper, we describe a proposal for such a DTT, presented by ENEA in collaboration with a European team of scientists. The selection of the DTT parameters (a major radius of 2.15 m, an aspect ratio of about 3, an elongation of 1.6-1.8, a toroidal field of 6 T, and a flat top of about 100 s) has been made according to the following specifications: • edge conditions as close as possible to DEMO in terms of dimensionless parameters; • flexibility to test a wide set of divertor concepts and techniques; • compatibility with bulk plasma performance. • an upper bound of 500 M€ for the investment costs. This paper illustrates this DTT proposal showing how the basic machine parameters and concept have been selected so as to make a significant step toward the accomplishment of the power exhaust mission.

The water-cooled lithium-lead breeding blanket is a candidate option for the European Demonstration Power Plant (DEMO) nuclear fusion reactor. This breeding blanket concept relies on the liquid lithium-lead as breeder-multiplier, pressurized water as coolant, and EUROFER as structural material. The current design is based on DEMO 2015 specifications and represents the follow-up of the design developed in 2015. The single-module-segment approach is employed. This is constituted by a basic geometry repeated along the poloidal direction. The power is removed by means of radial-toroidal (i.e., horizontal) water cooling tubes in the breeding zone. The lithium-lead flows in a radial-poloidal direction. On the back of the segment, a 100-mm-thick plate is in charge of withstanding the loads due to normal operation and selected postulated initiating events. Water and lithium-lead manifolds are designed and integrated with a consistent primary heat transport system, based on a reliable pressurized water reactor operating experience, and the lithium-lead system. Rationale and features of the single-module-segment water-cooled lithium-lead breeding blanket design are discussed and supported by thermo-mechanic, thermo-hydraulic, and neutronic analyses. Preliminary integration with the primary heat transfer system, the energy storage system, and the balance of plant is briefly discussed. Open issues, areas of research, and development needs are finally pointed out. @EUROfusion Consortium

- Abstract
In parallel with the programme to optimize the operation with a conventional divertor based on detached conditions to be tested on the ITER device, a project has been launched to investigate alternative power exhaust solutions for DEMO, aimed at the definition and the design of a divertor tokamak test facility (DTT). The DTT project proposal refers to a set of parameters selected so as to have edge conditions as close as possible to DEMO, while remaining compatible with DEMO bulk plasma performance in terms of dimensionless parameters and given constraints. The paper illustrates the DTT project proposal, referring to a 6 MA plasma with a major radius of 2.15 m, an aspect ratio of about 3, an elongation of 1.6-1.8, and a toroidal field of 6 T. This selection will guarantee sufficient flexibility to test a wide set of divertor concepts and techniques to cope with large heat loads, including conventional tungsten divertors; liquid metal divertors; both conventional and advanced magnetic configurations (including single null, snow flake, quasi snow flake, X divertor, double null); internal coils for strike point sweeping and control of the width of the scrape-off layer in the divertor region; and radiation control. The Poloidal Field system is planned to provide a total flux swing of more than 35 Vs, compatible with a pulse length of more than 100 s. This is compatible with the mission of studying the power exhaust problem and is obtained using superconducting coils. Particular attention is dedicated to diagnostics and control issues, especially those relevant for plasma control in the divertor region, designed to be as compatible as possible with a DEMO-like environment. The construction is expected to last about seven years, and the selection of an Italian site would be compatible with a budget of 500 M.

- Abstract
As indicated in the European Fusion Roadmap, the main objective of the Divertor Tokamak Test facility (DTT) is to explore alternative power exhaust solutions for DEMO so as to mitigate the risk that the conventional divertor based on detached conditions to be tested on the ITER device cannot be extrapolated to a fusion reactor. The issues to be investigated by DTT include: • demonstrate a heat exhaust system capable of withstanding the large load of DEMO in case of inadequate radiated power fraction;• close the gaps in the exhaust area that cannot be addressed by present devices;• demonstrate that the possible (alternative or complementary) solutions (e.g., advanced divertor configurations or liquid metals) can be integrated in a DEMO device. In this paper, we describe a proposal for such a DTT, presented by ENEA in collaboration with a European team of scientists. The selection of the DTT parameters (a major radius of 2.15 m, an aspect ratio of about 3, an elongation of 1.6-1.8, a toroidal field of 6 T, and a flat top of about 100 s) has been made according to the following specifications: • edge conditions as close as possible to DEMO in terms of dimensionless parameters;• flexibility to test a wide set of divertor concepts and techniques;• compatibility with bulk plasma performance.• an upper bound of 500 M€ for the investment costs. This paper illustrates this DTT proposal showing how the basic machine parameters and concept have been selected so as to make a significant step toward the accomplishment of the power exhaust mission.

In parallel with the programme to optimize the operation with a conventional divertor based on detached conditions to be tested on the ITER device, a project has been launched to investigate alternative power exhaust solutions for DEMO, aimed at the definition and the design of a divertor tokamak test facility (DTT). The DTT project proposal refers to a set of parameters selected so as to have edge conditions as close as possible to DEMO, while remaining compatible with DEMO bulk plasma performance in terms of dimensionless parameters and given constraints. The paper illustrates the DTT project proposal, referring to a 6 MA plasma with a major radius of 2.15 m, an aspect ratio of about 3, an elongation of 1.6–1.8, and a toroidal field of 6 T. This selection will guarantee sufficient flexibility to test a wide set of divertor concepts and techniques to cope with large heat loads, including conventional tungsten divertors; liquid metal divertors; both conventional and advanced magnetic configurations (including single null, snow flake, quasi snow flake, X divertor, double null); internal coils for strike point sweeping and control of the width of the scrape-off layer in the divertor region; and radiation control. The Poloidal Field system is planned to provide a total flux swing of more than 35 Vs, compatible with a pulse length of more than 100 s. This is compatible with the mission of studying the power exhaust problem and is obtained using superconducting coils. Particular attention is dedicated to diagnostics and control issues, especially those relevant for plasma control in the divertor region, designed to be as compatible as possible with a DEMO-like environment. The construction is expected to last about seven years, and the selection of an Italian site would be compatible with a budget of 500 M€.

SummaryBackground The nationwide prevalence of latent tuberculosis infection (LTBI) in Italian patients with psoriasis has never been investigated. Objectives To estimate the nationwide prevalence of LTBI in Italian patients with psoriasis who are candidates for systemic treatment. Methods Data were obtained from the Psocare Registry on those patients (n = 4946) with age > 18 years, systemic treatment at entry specified and tuberculin skin test (TST) performed according to the Mantoux method. LTBI diagnosis was based on a positive TST result in the absence of any clinical, radiological or microbiological evidence of active tuberculosis. Results Latent tuberculosis infection was diagnosed in 8·3% of patients with psoriasis (409 of 4946). The prevalence of LTBI was lower in patients on biologics than in those on conventional systemic treatments, ranging from 4·3% (19 of 444) of patients on adalimumab to 31% (eight of 26) of those on psoralen–ultraviolet A (P

Background/objective: Although 40% of psoriasis patients reported skin pain, this symptom is often underestimated. A new formula of calcipotriol plus betamethasone dipropionate (Cal/BD) has been recently approved for psoriasis treatment. Therefore, we aimed to evaluate the efficacy of Cal/BD aerosol foam on skin pain of patients with plaque psoriasis. Methods: A real‐life 4‐week prospective, open study on Cal/BD aerosol foam (not compared to vehicle or emollient cream) was performed in adult psoriasis patients attending three Dermatology units located in Campania region, Italy, between March and October 2018. Inclusion criteria were a history of skin pain over the last week and psoriatic involvement of the palmar area. Before (t0) and after a course of once daily application of Cal/BD aerosol foam for 4 weeks (t1), the following items were evaluated: Psoriasis Area and Severity Index (PASI), Physician Global Assessment (PGA) index of target plaque on palmar region, subjective skin pain features through Pain Qualities Assessment Scale questionnaire and skin pain threshold measured by pressure digital algometer at palmar psoriatic plaques. Results: Seventy‐five patients (43 male, mean age of 43.2 years) were enrolled. After 4 weeks of therapy with Cal/BD aerosol foam, a significant improvement in both PASI (mean: 6.5 ± 2.1 at t0 vs. 2.3 ± 1.6 at t1) and palmar plaques PGA (mean: 3.6 at t0 vs. 1.7 at t1) was observed (P < 0.001). The mean intensity score of skin pain decreased from 7.6 to 1.3 (P < 0.001); among skin pain qualities, intense, sensible, aching and unpleasant showed the highest rate of reduction (t0–t1: 6.3, 6.3, 6.1 and 5.8, respectively). Pain threshold of palmar skin lesions increased at t1. Conclusions: Our real‐life study suggested that Cal/BD aerosol foam may represent a valid topical anti‐psoriatic treatment, not only improving skin lesions, but also relieving cutaneous pain, thus contributing to ameliorate patients' quality of life. The water-cooled lithium-lead breeding blanket is a candidate option for the European Demonstration Power Plant (DEMO) nuclear fusion reactor. This breeding blanket concept relies on the liquid lithium-lead as breeder-multiplier, pressurized water as coolant, and EUROFER as structural material. The current design is based on DEMO 2015 specifications and represents the follow-up of the design developed in 2015. The single-module-segment approach is employed. This is constituted by a basic geometry repeated along the poloidal direction. The power is removed by means of radial-toroidal (i.e., horizontal) water cooling tubes in the breeding zone. The lithium-lead flows in a radial-poloidal direction. On the back of the segment, a 100-mm-thick plate is in charge of withstanding the loads due to normal operation and selected postulated initiating events. Water and lithium-lead manifolds are designed and integrated with a consistent primary heat transport system, based on a reliable pressurized water reactor operating experience, and the lithium-lead system. Rationale and features of the single-module-segment water-cooled lithium-lead breeding blanket design are discussed and supported by thermo-mechanic, thermo-hydraulic, and neutronic analyses. Preliminary integration with the primary heat transfer system, the energy storage system, and the balance of plant is briefly discussed.
