Abstract

Background and objective Rapid point-of-care tests (POCT) are likely to assist with the detection of group A streptococci (GAS), but their usefulness is determined by the presence of carriers of GAS. This is insufficiently explored in the wet tropics. Methods This study included 77 patients attending primary care in the wet tropics complaining of a sore throat, and 49 healthy controls. Carrier rates of GAS and the positive and negative etiological predictive values (P-EPV and N-EPV, respectively) of a POCT were calculated. Results The carrier rates were 8.3% among healthy children and 2.7% for adults. The P-EPV for children was 71% (95% confidence interval [Cl]: 0.0-100%) and for adults it was 85% (95% Cl: 0.0-100%). The corresponding N-EPV was 99% (95% Cl: 95-100%) for children and 99% (95% Cl: 98-100%) for adults. Discussion N-EPV, ruling out GAS, was sufficiently high with narrow Cis to allow for defining a stopping rule to avoid unnecessary antibiotic prescribing. [ABSTRACT FROM AUTHOR]

Abstract

OBJECTIVES: Exacerbations of chronic obstructive pulmonary disease (COPD) are common in emergency departments (ED). Guidelines recommend administration of inhaled bronchodilators, systemic corticosteroids and antibiotics along with non-invasive ventilation (NIV) for patients with respiratory acidosis. We aimed to determine compliance with guideline recommendations for patients with treated for COPD in ED in Europe (EUR) and South East Asia/Australasia (SEA) and to compare management and outcomes. METHODS: In each region, an observational prospective cohort study was performed that included patients presenting to EDs with the main complaint of dyspnoea during three 72-hour periods. This planned sub-study included those with an ED primary discharge diagnosis of COPD. Data were collected on demographics, clinical features, treatment, disposition and in-hospital mortality. We determined overall compliance with guideline recommendations and compared treatments and outcome between regions. RESULTS: 801 patients were included from 122 EDs (66 EUR and 46 SEA). Abstract

Background Historically it has been challenging to recruit and retain an appropriately trained medical workforce to care for rural and remote Australians. This paper describes the Queensland North West Hospital and Health Service ( NWHHS) workforce redesign, developing education strategies and pathways to practice, thereby improving service provision, recruitment and retention of staff. Concept The Mount Isa-based Medical Education Unit sought accreditation for a Rural Generalist ( RG) training pathway from Internship to Fellowship with the Australian College of Rural and Remote Medicine ( ACRRM) and the Regional Training Provider ( RTP). This approach enhanced the James Cook University ( JCU) undergraduate pathway for rurally committed students while improving recruitment and retention of RMOs/Registrars. Achievements Accreditation was achieved through collaboration with training providers, accreditation agencies, ACRRM and a local general practice. The whole pathway from ignore Internship to Fellowship is offered with the RG Intern intake as a primary allocation site beginning in 2016. Comprehensive supervision and excellent clinical exposure provide an interesting and rewarding experience - for staff at all levels. Results Since 2013 RMO locum rates have been <1%. Registrars on the ACRRM pathway and Interns increased from 0 to 7 positions each in 2015, with similar achievements in SMO staffing. Three RMOs expressed interest in a Registrar position, Conclusions Appropriate governance is needed to develop and advertise the program. This includes the NWHHS, the RG Pathway and JCU.
