Aims To evaluate the kinetics of inflammatory biomarkers in septic patients in order to identify the most reliable predictor of unfavorable outcome. Methods A prospective analysis of septic patients was performed. Median levels of neutrophil/lymphocyte count ratio, fibrinogen, C-reactive protein and procalcitonin were dynamically assessed and comparatively analyzed. Results Seventy-seven patients were included. Descendent kinetic patterns were registered for all biomarkers, except C-reactive protein. At 24 hours, neutrophil/lymphocyte count ratio significantly decreased in 42.85% of cases, procalcitonin in 37.33%, C-reactive protein in 16.12% and fibrinogen in 1.58% of cases. At 72 hours, procalcitonin decreased to one-half in 70% of cases and neutrophil/lymphocyte count ratio in 67.53% of cases. Conclusions Neutrophil/lymphocyte count ratio and procalcitonin significantly decreased in the first 72 hours, while C-reactive protein increased in the first 24 hours. The proportions of patients with major decrease of baseline values were higher for neutrophil/lymphocyte count ratio and procalcitonin.

Aims: To analyze the diagnostic role of neutrophil/lymphocyte count ratio (NLCR) in sepsis and to evaluate its usefulness in appreciating the prognosis of septic patients. Methods : A prospective analysis of patients diagnosed with sepsis between October 2015 and July 2017 was performed. A control group of patients with systemic inflammatory response syndrome (SIRS) with noninfectious causes was recruited. The sensitivity, specificity and AUC of NCLR for sepsis diagnosis were comparatively assessed with other biomarkers including fibrinogen (FIB), C-reactive protein (CRP) and procalcitonin (PCT). The baseline levels of NLCR were correlated with APACHE IV, APS and SOFA scores of severity. Results : We enrolled 105 patients with sepsis and 37 patients with noninfectious SIRS. NLCR had significantly higher levels in the first group (p---lt---0.05). For an optimal cut-off value of 8.18, the sensitivity, specificity and AUC of NLCR for sepsis diagnosis were 70.5%, 72.2% and 0.766, respectively. A cut-off value of 12.0 led to moderate sensitivity (58.7%) and moderate-to-high specificity (89%). For patients with SOFA score over 2 points, the characteristics did not significantly improve. The correlations between baseline NCLR and the severity scores were not statistically significant. Conclusion : NLCR levels were significantly higher in sepsis group compared to noninfectious SIRS group. NLCR had moderate sensitivity and specificity regarding sepsis diagnosis. Baseline levels of NLCR did not correlate with the severity scores, so its usefulness in sepsis prognosis was limited.

The purpose of this paper was to evaluate the postoperative clinical outcomes of combined anterior cruciate ligament (ACL) and medial collateral ligament (MCL) reconstruction and to briefly describe the surgical procedure. The current study is a retrospective observational analysis, which included 12 patients with acute ACL and grade III MCL injuries. Combined ACL-MCL reconstruction was performed using bone-tendon-bone patellar autograft and semitendinosus and gracilis autograft, respectively. Preoperative and postoperative assessments were performed for each patient using the same protocol. Combined ACL-MCL reconstruction is associated with improved postoperative results. No recurrent knee instability was reported at the end of follow-up period. In conclusion, acute ACL and grade III MCL injuries can be successfully treated by combined ACL-MCL reconstruction, which is an effective option for the treatment of these patients, associated with good clinical results.

The objective of this study was to evaluate the effectiveness of different surgical implants for the reconstruction of severe acetabular bone defects in revision arthroplasty of the hip. The current study is a retrospective study on 32 patients with Paprosky type IIIA or IIIB acetabular defects operated between January 2012-December 2015 in a single hospital. The mean follow-up was 21 months (12-43 months). Five different types of reconstruction methods were used: primary uncemented cups with or without screws, cemented acetabular cups, tantalum cups, metal augments and antiprotrusio cages. Bone allograft was available in all cases. Functional outcome after surgery was evaluated using Harris Hip Score. Based on Paprosky classification, the study included 16 type IIIA and 16 type IIIB acetabular defects. Bone graft was used in 71.8% of the cases (23 out of 32 patients). Tantalum cups were used in 15 cases (46.9%), being the preferred implant. Primary uncemented cups were used in 2 cases, cemented acetabular cups were used in 4 cases, trabecular metal augments were used in 5 cases and antiprotrusion cages were used in 6 cases. The mean Harris Hip Score improved from 37.3�7.4 pre-operatively to 82.1�7.2 at final follow-up. In conclusion, the current study demonstrates that various methods of reconstruction are efficient in the short and medium-term.

Introduction: Streptococcal infection can be responsible for multiple complications, such as toxic, septic or allergic disorders. Toxin-mediated complications (TMC) can appear during the acute phase of disease and can involve any organ, causing carditis, arthritis, nephritis, hepatitis etc. Case presentation: The case of a young woman without a history of recurrent streptococcal tonsillitis, admitted to “Matei Balş” National Institute for Infectious Diseases, Bucharest, Romania, presenting with fever, sore throat and exudative tonsillitis, is detailed. The initial test for Streptococcus pyogenes was negative. The patient had leukopenia with severe neutropenia, high values of inflammatory biomarkers and electrocardiographic (ECG) changes with inverted T waves in leads V1-4 and flattened T waves in V5-6. There were no other cardiac signs or symptoms. The patient received cefuroxime for two days. On admission, the patient was prescribed Penicillin G and dexamethasone which resulted in a rapid recovery. The leucocytes count returned to normal as did the ECG abnormalities. At the time of discharge, the antistreptolysin O titre was high. Conclusions: The case highlights the toxin-mediated complications (TMC) of streptococcal infection which occur from the outset of the disease.

Sepsis represents a severe pathology that requires both rapid and precise positive and differential diagnosis to identify patients who need immediate antimicrobial therapy. Monitoring septic patients′ outcome leads to prolonged hospitalisation and antibacterial therapy, often accompanied by substantial side effects, complications and a high mortality risk. Septic patients present with complex pathophysiological and immunological disorders and with a predominance of pro-inflammatory or anti-inflammatory mediators which are heterogeneous with respect to the infectious focus, the aetiology of sepsis or patients′ immune status or comorbidities. Previous studies performed have analysed inflammatory biomarkers, but a test or combinations of tests that can quickly and precisely establish a diagnosis or prognosis of septic patients has yet to be discovered. Recent research has focused on re-analysing older accessible parameters found in the complete blood count to determine the sensitivity, specificity, positive and negative predictive values for the diagnosis and prognosis of sepsis. The neutrophil/lymphocyte count ratio (NLCR), mean platelet volume (MPV) and red blood cells distribution width (RDW) are haemogram indicators which have been evaluated and which are of proven use in septic patients′ management.

The author maintains that the atrophy of the sural triceps muscle, the retraction of the Achille tendon and the osteoporosis of the calcaneum and of the tarsian bones, occurring after the traumatic sectioning of the Achille tendon, are the results of an inadequate treatment. The cause of these sequellae is the immobilization of the foot in an equinus position, which relaxes the sural triceps and as a result of the lack of mechanical traction factor, leads to local circulatory disturbances followed by a modification in the structure of the bone and of the muscle. On the basis of this pathophysiological concept the author has excluded post-operative immobilization in the equinus position, and in fact any type of immobilization, recommending in contrast a mobilization of the foot from the very first days after the suture of the tendon. The clinical results obtained confirm the value of the hypothesis.

The position in which the articulation is immobilized is in fact the pattern in which capsular fibrosis and of the connective tissue is settled. It will also determine the degree of remanent articular mobility. The author does not recommend immobilization in the so-called "functional" position since this does not prevent the developement of articular rigidity and is even responsible for their developement. In view of maintaining the articular function when the articulation itself is involved or when it is immobilized due to lesions in its vicinity, immobilization should be made in a position allowing for maximal elongation of the connective fibers in the area which depends from the vascular and nervous viewpoint from the traumatic lesion, or that which is involved by surgery, that is in positions which are quite different from those used at present.

The author maintains that the osteoma of the brachial muscle as well as post-traumatic periarticular calcifications, occur in the muscle mass or in the tendon that prolongs it, or in the articular capsule, as a result of surgical treament and post-operative immobilization, and only exceptionally following orthopaedic treatment of traumatic lesions. Articular mobilization in the first week after osteosynthesis, or in the frame of conservative treatment, is not followed by osteoma of the brachial muscle or by calcifications surrounding the articulation, a fact that suggest that the presently applied therapeutic methods, which recommend strict and prolonged articular immobilization should be avoided. The development of ossification can be explained by post-traumatic changes in the muscular microcirculation, rendered more severe by surgical treatment and maintained by muscular inactivity. Early articular mobilization is the most efficient method of prophylaxis of these complications, which are determined not by the traumatic lesions themselves but by the present therapeutic methods applied.
