Abstract

We present the results of a search for variable stars in the globular cluster NGC 5286, which has recently been suggested to be associated with the Canis Major dwarf spheroidal galaxy. 57 variable stars were detected, only 19 of which had previously been known. Among our detections one finds 52 RR Lyrae (22 RRc and 30 RRab), 4 LPV's, and 1 type II Cepheid of the BL Herculis type. Periods are derived for all of the RR Lyrae as well as the Cepheid, and BV light curves are provided for all the variables. The mean period of the RRab variables is = 0.656 days, and the number fraction of RRc stars is N(c)/N(RR) = 0.42, both consistent with an Oosterhoff II (OoII) type -- thus making NGC 5286 one of the most metal-rich ([Fe/H] = -1.67; Harris 1996) OoII globulars known to date. The minimum period of the \RRab's, namely Pab,min = 0.513 d, while still consistent with an OoII classification, falls towards the short end of the observed Pab,min distribution for OoII globular clusters. As was recently found in the case of the prototypical OoII globular cluster M15 (NGC 7078), the distribution of stars in the Bailey diagram does not strictly conform to the previously reported locus for OoII stars. We provide Fourier decomposition parameters for all of the RR Lyrae stars detected in our survey, and discuss the physical parameters derived therefrom. The values derived for the RRc's are not consistent with those typically found for OoII clusters, which may be due to the cluster's relatively high metallicity -- the latter being confirmed by our Fourier analysis of the ab-type RR Lyrae light curves. We derive for the cluster a revised distance modulus of (m-M)V = 16.04 mag. (ABRIDGED), Comment: 15 pages, 8 figures (plus 57 light curve sets, in an Appendix). The Astronomical Journal, in press

Abstract

Since the 2016 Havana Peace Accord (HPA), for many Colombians security has hardly improved. This article argues that this is largely due to socially rooted marginalisation that constrains citizen participation and contributes to entrenched insecurity in the country, especially its border regions. We show how post-HPA community-level responses to insecurity, including constructive engagement with the state, can enhance security and kick-start processes of de-marginalisation. Drawing on original data from interviews in Cesar Department and a series of cross-stakeholder fora in 2019, we demonstrate the utility of the citizen security lens when examining processes of marginalisation, de-marginalisation, and their impact on perceptions of (in)security of marginalised communities.

Con el fin de evaluar el comportamiento productivo de conejos durante el período de crecimiento-engorde, se realizó un experimento que consistió en la sustitución parcial del alimento balanceado comercial (ABC) por el fruto de mango criollo (Mangifera indica) en una granja comercial del estado Monagas, Venezuela. El experimento tuvo una duración de 63 días, se utilizaron 32 conejos mestizos Nueva Zelanda x California de 30 días de edad alojados en jaulas. Se realizó un diseño completamente aleatorizado con cuatro tratamientos: T1: 120 g de ABC; T2: 80 g de ABC + 40 g de mango criollo; T3: 60 g de ABC + 60 g de mango criollo y T4: 40 g de ABC + 80 g de mango criollo. Las variables dependientes fueron: ganancia diaria de peso promedio (GDP), peso final a los 93 días de edad (PESOFIN), consumo total (CONSTOTAL) y conversión alimenticia con alimento balanceado comercial (CAABC). Se realizó un análisis de varianza por el método de mínimos cuadrados. Los resultados arrojaron diferencias significativas para las variables bajo estudio, siendo los mayores promedios en GDP para T1: 26,69 ± 1,34 g ; PESOFIN para T1: 2083,75 ± 26,09 g ; CONSTOTAL fue para T1: 7434, 37 ± 6,80 g . Las mejores CAABC se obtuvieron en los tratamientos que incorporaron el mango criollo. En general los resultados obtenidos permiten inferir la utilización del fruto del mango como recurso alternativo en la alimentación de conejos de engorde.

With the purpose of evaluating the productive behavior of rabbits during the period of fattening growth, I made an experiment that consisted on the partial substitution of the commercial balanced food by the Mango fruit (Mangifera indicates) in a commercial farm of the Monagas state, Venezuela . The experiment lasted 63 days, there were used 32 racially mixed rabbits from New Zealand and California being 30 days of age, lodged in cages. Four treatments were used, with a design completely randomized and four repetitions settled down. T1: 120 commercial balanced food (BFC), T2: 80 BFC g + 40 Mango g, T3: 60 BFC g + 60 Mango g and T4:40 BFC g + 80 Mango g. The dependent variables were: daily gain of weight average (DGW), final gain of weight (FGW), nutritional conversion with commercial balanced food (NCABFC), final weight at 93 days of age (FP), total consumption (TC). There was made an analysis of variance by the method of square minimums. The results showed significant differences for the variables under study. Being the greater averages in DGW for T1: 26,69 ± 1.34 g ; FP for T1: 2083,75 ± 7.85 kg ; TC was for T1: 7434,37 ± 11,23. The best NCABFC were obtained in the treatments that incorporated the Mango fruit. In general the results obtained allow to infer the use of the fruit as alternative resource in the feeding of fattening rabbits.

El PD es una rara condición, que se presenta como una dilatación anormal de uno o varios senos paranasales (SP) sin evidencia de compromiso óseo ni mucoso, que se asocia a dolor y deformidad facial. La tomografía computada (TC) permite hacer el diagnóstico; el tratamiento es quirúrgico y está orientado a descomprimir el o los SP comprometidos; con lo anterior se logra la erradicación del dolor, pero la deformidad facial persiste.

La tomografía computada (TC) permite hacer el diagnóstico; el tratamiento es quirúrgico y está orientado a descomprimir el o los SP comprometidos; con lo anterior se logra la erradicación del dolor, pero la deformidad facial persiste. Two clinical cases with a diagnosis ofpneumoslnus dilatans (PD) from the Servicio de Otorrinolaringología (SORL) of Hospital Barros Lucos-Trudeau (HBLT) are presented. PD is a rare condition in which an abnormal enlargement of one or several paranasal sinuses (PS) is present, with no evidence of bone or mucosa involvement. It is associated with facial pain and deformity. Diagnosis is based on CT scan; treatment is surgical and it is oriented to decompress the PS involved, eradicating pain but with a persisting facial deformity.

15. Smoking habit in school age children, in Chile

Abstract

Background: In Chile, there is no information about the prevalence of smoking among basic and high school students. Aim: To study the prevalence of smoking among school age population. Material and Methods: A cross sectional survey was conducted during 2001, assessing tobacco use and smoking habit in a representative sample of 15.119 randomly selected students. Results: Sixty four percent of students had smoked at least once in their lifetime. Eighteen percent of students smoked daily and 15%, occasionally. The figures for women were higher (19 and 18% respectively). The mean age for the first contact with smoking was 12.3 years and, for starting the smoking habit, 13.2 years. Smokers increased the amount of cigarettes during weekends. Higher prevalence rates of smoking were observed in students from the Southern region of the country and of lower socioeconomic levels. From the 3rd grade on, there is an increased risk of having contact with tobacco and smoking for the first time and from 6th grade on, this risk becomes permanent. The adjusted Odds Ratio to be a smoker is significantly higher when the mother is a current or irregular smoker (OR 1.9 95 CI; 1.7-2.0). Conclusions: High smoking prevalence rates were detected in this survey, mainly in women. The risk for smoking starts early during school life. Therefore, health promotion programs must include elementary and high school students (Rev Méd Chile 2004; 132: 171-82)

Abstract

Objective: To develop and validate practical prognostic models for death at 14 days and for death or severe disability six months after traumatic brain injury. Design: Multivariable logistic regression to select variables that were independently associated with two patient outcomes. Two models designed: "basic" model (demographic and clinical variables only) and "CT" model (basic model plus results of computed tomography). The models were subsequently developed for high and low-middle income countries separately. Setting: Medical Research Council (MRC) CRASH Trial. Subjects: 10 008 patients with traumatic brain injury. Models externally validated in a cohort of 8509. Results: The basic model included four predictors: age, Glasgow coma scale, pupil reactivity, and the presence of major extracranial injury. The CT model also included the presence of petechial haemorrhages, obliteration of the third ventricle or basal cisterns, subarachnoid bleeding, midline shift, and non-evacuated haematoma. In the derivation sample the models showed excellent discrimination (C statistic above 0.80). The models showed good calibration graphically. The Hosmer-Lemeshow test also indicated good calibration, except for the CT model in low-middle income countries. External validation for unfavourable outcome at six months in high income countries showed that basic and CT models had good discrimination (C statistic 0.77 for both models) but poorer calibration. Conclusion: Simple prognostic models can be used to obtain valid predictions of relevant outcomes in patients with traumatic brain injury.

Abstract

Background Corticosteroids have been used to treat head injuries for more than 30 years. In 1997, findings of a systematic review suggested that these drugs reduce risk of death by 1-2%. The CRASH trial—a multicentre international collaboration—aimed to confirm or refute such an effect by recruiting 20 000 patients. In May, 2004, the data monitoring committee disclosed the unmasked results to the steering committee, which stopped recruitment. Methods 10 008 adults with head injury and a Glasgow coma score (GCS) of 14 or less within 8 h of injury were randomly allocated 48 h infusion of corticosteroids (methylprednisolone) or placebo. Primary outcomes were death within 2 weeks of injury and death or disability at 6 months. Prespecified subgroup analyses were based on injury severity (GCS) at randomisation and on time from injury to randomisation. Analysis was by intention to treat. Effects on outcomes within 2 weeks of randomisation are presented in this report. This study is registered as an International Standard Randomised Controlled Trial, number ISRCTN74459797. Findings Compared with placebo, the risk of death from all causes within 2 weeks was higher in the group allocated corticosteroids (1052 [21·1%] vs 893 [17·9%] deaths; relative risk 1·18 [95% CI 1·09-1·27]; p=0·0001). The relative increase in deaths due to corticosteroids did not differ by injury severity (p=0·22) or time since injury (p=0·05). Interpretation Our results show there is no reduction in mortality with methylprednisolone in the 2 weeks after head injury. The cause of the rise in risk of death within 2 weeks is unclear.

Food insecurity is a critical issue in the Americas, with severe impacts in the Caribbean, Mesoamerica, and South America, particularly affecting older adults in Indigenous and rural contexts where it intersects with poverty, gender, and ethnicity. This study aims to provide an in-depth understanding of the current research about food insecurity among older Indigenous adults in Latin America and the Caribbean. A comprehensive literature review was conducted, utilizing specific search queries and the population, intervention, comparison, and outcome (PICO) strategy across multiple databases to identify the pertinent studies. The findings indicate an increase in academic output on this topic since 2018, with significant emphasis on the interplay between climate change and food insecurity. The review highlights the importance of developing targeted food programs, reforming policies, and fostering collaboration between academia and local communities to implement practical interventions. Despite the growing body of literature, a notable research gap persists in rural areas of Latin America and the Caribbean. This study underscores the necessity of balancing the geographic distribution of research and emphasizes the preservation of cultural practices and the adaptation of public policies to support traditional food practices. It advocates for culturally sensitive interventions and interdisciplinary collaboration to formulate comprehensive strategies. The originality and value of this study lie in its focused analysis of older Indigenous adults, contributing crucial insights to the international literature on food security.

Nutrient consumption during pregnancy makes it essential to have a healthy eating routine. While a carefully thought-out vegan diet may contain all the essential nutrients, some micronutrients need special attention, which may call for supplementation. The study delves into significant findings concerning nutritional adequacy and challenges in the discussion section, highlighting nutrients like calcium, iron, omega-3 fatty acids, vitamin D, and protein. It emphasizes medical professionals' need to monitor and assist vegan expectant mothers in meeting their nutritional needs. The analysis also examines the intricate connection between a mother's health and the outcomes of vegan pregnancies, such as decreased rates of gestational diabetes and hypertension. Examining fetal growth and development further highlights the complexity of this process, as do the contradicting data on birth weights. Furthermore, early data suggest that infants born to vegan moms may benefit cognitively, but further studies are required to prove a causal relationship. It carefully examines several subtypes, including hormone receptor-positive (HR+), HER2-positive (HER2+), and triple-negative breast cancer (TNBC), recognizing the illness' fundamental variety. The review also examines how precise genetic abnormalities like BRCA1/2 and PIK3CA mutations and molecular profiling facilitate therapy selection. Abstract

Objective: The objective of this study was to evaluate the effects of breastfeeding on the risk for fever after routine immunizations., Methods: A prospective cohort study was conducted at a pediatric vaccination center in Naples, Italy. The mothers of the infants scheduled to receive routine immunizations were instructed on how to measure and record infant temperature on the evening of the vaccination and for the subsequent 3 days. The information about the incidence of fever was obtained by telephone on the third day after vaccination. The relative risk for fever in relation to the type of breastfeeding was estimated in multivariate analyses that adjusted for vaccine dose, maternal education and smoking, and number of other children in the household., Results: A total of 460 infants were recruited, and information on the outcome was obtained for 450 (98%). Fever was reported for 30 (25%), 48 (31%), and 94 (53%) of the infants who were being exclusively breastfed, partially breastfed, or not breastfed at all, respectively (P < .01). The relative risk for fever among infants who were exclusively and partially breastfed was 0.46 (95% confidence interval: 0.33-0.66) and 0.58 (95% confidence interval: 0.44-0.77), respectively. The protection conferred by breastfeeding persisted even when considering the role of several potential confounders., Conclusions: In this study, breastfeeding was associated with a decreased incidence of fever after immunizations.

Abstract

Objective: To characterize intimate partner abuse and identify the main factors associated with violence in pregnancy in four highly deprived States in Mexico., Material and Methods: The data were taken from the National Survey on Violence against Women 2003 (ENVIM per its abbreviation in Spanish). Based on it a cross-sectional study was conducted on 1 949 women between 15 and 50 years of age, who were once pregnant and who utilized primary and secondary health care services from the Ministry of Health, Mexican Institute of Social Security, and the Institute for Social Security and Services for State Workers in Guerrero, Hidalgo, Oaxaca and Chiapas states, between November 2002 and March 2003. Logistic regression was used to assess the association between independent variables and violence during pregnancy., Results: 250 women (13%) suffered a type of violence (physical, psychological, sexual or economical) during any of their pregnancies. Of these women 76 (30.40%) were battered on their abdomen. In most of these cases (91.39%) the husband was the aggressor. The variables significantly associated with violence in pregnancy were: woman's illiteracy (OR 2.2; CI 95:% .1,4.4); history of violence in childhood (OR 3.2; CI 95% 1.9, 5.4) as well as sexual abuse in her childhood (OR 2.4, CI 95% 1.3, 4.4) and her partner's daily alcohol consumption (OR 6.5; CI 95% 3.3, 12.9)., Conclusions: The results show that violence during pregnancy is a regular event in the impoverished context and that its expression is more severe. These results point to the importance of continued study of the problem of violence against pregnant women in Mexico and the importance of identifying battered women in prenatal care.

Objective: To describe the methodology, the research designs used, the estimation and sample selection, variable definitions, collection instruments, and operative design and analytical procedures for the National Survey Violence Against Women in Mexico., Material and Methods: A complex (two-step) cross-sectional study was designed and the qualitative design was carried out using in-depth interviews and participant observation in health care units., Results: We obtained for the quantitative study a total of 26 240 interviews in women users of health services and 2 636 questionnaires for health workers; the survey is representative of the 32 Mexican states. For the qualitative study 26 in-depth interviews were conducted with female users and 60 interviews with health workers in the States of Quintana Roo, Coahuila and the Federal District.

Abstract

Objective: To identify the prevalence of violence against females among those who are health service beneficiaries in Mexico., Material and Methods: The National Survey on Violence against Women (ENVIM, per its Spanish abbreviation) was applied in 2003 to female users of public primary and secondary health care services. The sampling framework was based on a stratified, probabilistic sample in two stages. First the health care units were selected with probability proportional to the number of physicians' offices in the unit, from a list of possible care units. Second, women 15 years and older who sought care at the health care unit were selected for participation in the study through systematic sampling. Univariate analysis and then bivariate analysis were carried out on the data collected with a questionnaire., Results: The sample included 26 042 women between 15 and 92 years of age, with a mean age of 35.8 years. Physical violence during childhood was reported by 42% of the women. Only 7.8% answered yes to a general question about whether they experienced domestic partner violence, but 21.5% reported experiencing violence of any type during the last 12 months as measured by a scale including specific acts of psychological, economic, physical and sexual violence. The most frequently reported type of violence was psychological (19.6%). Of the women who had been pregnant, 14.1% reported having experienced violence during pregnancy, and 4.4% reported being hit in the abdomen. The prevalence of sexual violence was 17.3% and close to half reported being victims of this type of violence before age 15. Higher prevalence of violence was found among women with lower levels of formal education, living in a rented home, in areas with higher overcrowding indices, and users of Ministry of Health care services., Conclusions: Identifying and measuring violence is complex, given the diverse types of violence and how they are perceived and therefore reported by women themselves. This is an important public health problem, in view of the high frequency observed in this study and the immediate implications. These findings indicate the urgent need for interventions to prevent and treat violence.

Abstract

Neoplastic proliferation of plasma cells results in a population of immunologically homogeneous cells that can produce diffuse (multiple myeloma) or localized (extramedullary plasmacytomas and solitary plasmacytoma of bone) disease. In otorhinolaryngologic literature these neoplasms are rarely described and their nosological arrangement is often confused. The presence of a plasma cell neoplasm can be a surprise and sometimes a diagnostic challenge to the head and neck surgeon. Proper management of such lesions needs to be individualized according to their expected biologic behaviour. The recent observation of a case of maxillary sinus plasmacytoma suggested the Authors to carefully review the literature, drawing their attention mainly on the current histogenetic hypotheses and their consequences in therapeutic strategy. The correct diagnostic procedure is also explained, highlighting the difficulties due to both the protean nature of the disease and the still existing nosological confusion. The possibility of a plasma cell tumour should be never forgotten in presence of an head and neck neoplasm. Because these neoplasms may signal the presence of multiple mieloma, full evaluation is required to exclude disseminated disease. In light of recent histogenetic acquisitions it is suggested that extramedullary plasmacytomas can be classified among the so-called "mucosa-associated" lymphomas. Possible following differences in therapeutic approach and long-term follow-up are also indicated, stressing the role of surgery in managing these disorders. Surgical excision of extramedullary plasmacytomas followed by complementary radiotherapy on the site of tumour is proposed as the best treatment for these kind of neoplasms. This is in opposition with "classical" statement considering radiotherapy the only treatment for this kind of disorders.

A case of squamous cell carcinoma with sebaceous metaplasia arising in the epiglottis is reported. Extracutaneous neoplasms that show sebaceous differentiation have been mainly described in the major salivary glands. To date only one case of prominent sebaceous differentiation in a carcinoma arising in the vallecula has been reported. In the present paper we describe a similar case arising in the larynx.

Purpose: To define MR accuracy in the evaluation of the T stages of tumors of the tongue and floor of the mouth., Methods: Fifty-two patients affected by squamous cell carcinoma were studied with a superconductive system at 1.5 T. The study was performed with spin-echo T1- and T2-weighted images before contrast and short spin-echo T1-weighted and gradient-echo sequences after gadopentetate dimeglumine administration. The results obtained with MR were compared with pathologic data., Results: Good correlation of T stages (TNM system) was obtained in 45 of 52 cases. MR did not show four superficial lesions. In one case, infiltration of the cortical bone of the mandible was not demonstrated (MR, T2; pathologic, T4), and in another the lesion was classified as T2 instead of T3, as it was pathologically. One lesion was classified as T4 on MR because of infiltration of the alveolar ridge but was classified as T2 at surgery. In 46 patients who underwent surgery, the accuracy of MR was excellent for predicting the relationship of tumor to midline and oral floor musculature. The results obtained with gadopentetate dimeglumine were better than those obtained in noncontrast studies in 32 (62%) of 53 cases., Conclusion: MR showed high accuracy in the study of tumors of the tongue and floor of the mouth.

Abstract

The purpose of this study was to define MR accuracy in the evaluation of T Stage of tumors in the tonsillar region. Twenty-two patients with a squamous cell carcinoma of the tonsillar region were studied utilizing a superconductive scanner operating at 1.5 T. The study was performed with SE T1 and T2 images before contrast and short SE T1 after Gd-DPTA infusion. MR results were correlated with pathological data on T Stage (TNM classification) and on the relationships between tumors and surrounding structures. A positive correlation between MR and pathological data was obtained in 19/22 cases, with an MR accuracy of 86%. MR did not show the presence of 2 superficial lesions (MR T0, pathological T1), while one lesion was classified T2 with MR, instead of pathology T1. The accuracy of MR was 95% in the evaluation of the relationships between tumors and the base of the tongue and 100% for body of the tongue, retromolar trigone, valleculae, epiglottis, pre-epiglottis, parapharyngeal and masticator space. MR showed high accuracy in the evaluation of T Stage, above all utilizing Gd-DPTA infusion, with short SE T1 sequences. MR did not show the superficial lesions, but in these cases a deep extension of the disease was excluded.

Abstract

Poor survival rates and the limited palliation afforded by radiotherapy alone, together with progress made in reconstructive surgery in restoring mucosal continuity after large resections, make total glossectomy reasonably indicated for treatment of advanced carcinoma of the tongue. The Authors reviewed 19 cases (17 males, 2 females, mean age 58.4 years) of total and near total glossectomy without laryngectomy treated at National Cancer Institute "Regina Elena" of Rome from 1990 to 1993 in order to evaluate oncological and functional results. All patients were reconstructed immediately, 16 with a pectoralis major flap, 2 with a nasolabial flap and 1 with a radial forearm free flap and were available for follow-up from 6 to 45 months (mean 29 months). There was no operative mortality and no patient needed total laryngectomy for aspiration. The rate of local recurrencies was 52.6%, most of them (75%) in patients who had undergone total/near total glossectomy for recurrence. Survival rate was 61.5% after 1 year and 20% after 2 years. 94% of patients resumed swallowing and independent oral alimentation (48% of them without any dysphagia); 84%; of the patients were decannulated and 48% produced easily intellegible speech. Data from our experience let us conclude that, in the light of the acceptable functional results obtained with reconstructive flaps, total glossectomy should be considered as the primary treatment modality in advanced carcinoma of the tongue (including T2 > 3 cm exceeding midline), and should not be reserved only for salvaging hopeless situations.

Abstract

Mycoplasma pneumoniae is one of the most common bacteria causing respiratory diseases in other countries, specially in older children, adolescents and young adults and less frequently in the age group studied here, nevertheless the determination of its presence in this group was considered important. Two hundred and fifty throat swabs were taken from children, under five years of age, hospitalized with a diagnosis of acute respiratory infections (ARI) and to 50 children, same age, with no ARI (controls). The samples were placed in transport media and were incubated at 37 degrees C during 7 to 15 days. They were reinoculated in PPLO agar and typical colonies were looked for, 5 to 8 days later. The organisms were identified by biochemical tests. Eight Mycoplasma sp (3.2%) were obtained, five of them were M. pneumoniae (2.0%) and three M. hominis (1.2%). Only in 2 cases adenoviruses with M. hominis were found in the absence of other pathogens. It was shown that M. pneumoniae also infects children under five years old, so its present should be suspected, specially when the patient's health does not improve with the installed treatment. Some important suggestions for the isolation of mycoplasma are given.

Abstract

The authors analysed the data obtained from their experiences in extensive head and neck resections as well as reconstructive treatments using myocutaneous flaps. They tried to evaluate the reconstructive approach in terms of morbidity and functional results. Sixty-nine patients, treated from January 90 to November 92 for advances intra-oral cancer, were considered in this study. The reconstructive procedure in 68 cases was the pectoralis major myocutaneous flap while in 5 cases the trapezius flap was utilized. Even though the oncological results were poor, the functional ones were quite encouraging. In fact, the assessment of speech and swallowing was extremely satisfactory in 83% of the cases. The morbidity related to both flap procedures was low and the patients' life quality was good. Indeed, the use of a free flap offers additional advantages, without necessarily compromising safety of the reconstruction. Free flaps in general allow greater leeway in flap design and donor-site choice than pedicled flaps. Several reports have already demonstrated the versatility, usefulness and reliability of these flaps, especially in intra-oral reconstruction. The intricate nature of microsurgery and the expense of microsurgical equipment inhibit this kind surgery in many institutions around the world where the standard myocutaneous flaps would be a more practical procedure. In conclusion, even those patients with a poor prognosis may be considered potential candidates for demolitive and reconstructive treatment with which satisfactory results may be obtained.
