Abstract

Introduction: The most common interventions in lower urinary tract obstruction are vesico-amniotic shunting (VAS) and serial amnioinfusions; however, their outcome remains uncertain. The aim of the study was to assess the kidney function as well as clinical complications in boys at the age of 6 months with posterior urethral valves (PUV), who underwent prenatal VAS and survived the neonatal period. Material and methods: The study group consisted of 14 boys and the mean time of the VAS procedure was 22 weeks of gestational age (GA). Children were born in 34 GA (29--39) with average birth weight of 2860 g. Four fetuses needed amnioinfusion. In the study we assessed physical development, prevalence and aetiology of urinary tract infections (UTIs), number of hospitalizations, estimated glomerular filtration rate, presence of hypertension, albuminuria, proteinuria and acidosis. Results: Average body weight at 6 months of life was 7.2 kg. 86% of children had at least one UTI; the same percentage were given antibacterial prophylaxis. 36% underwent urinary diversion. Median number of hospitalizations was 4. One patient required chronic renal replacement therapy, and one needed a short course of peritoneal dialysis. Median serum creatinine level was 0.65 mg/dl with estimated glomerular filtration rate (eGFR) 50.6 ml/min/1.73 m². Average serum cystatin C level was 1.50 mg/l with eGFR 48.5 ml/min/1.73 m². 43% developed acidosis. 71% had elevated albuminuria with a median urine albumin-to-creatinine ratio of 581.64 mg/g. 28.5% had overt proteinuria. Conclusions: Growth at the age of 6 month of infants with PUV after prenatal VAS was barely satisfactory. More than half had impaired kidney function, but the need for renal replacement therapy was uncommon. [ABSTRACT FROM AUTHOR]

Abstract

Posterior urethral valves (PUV) account for 17% of paediatric end-stage renal disease. A major issue in the management of PUV is prenatal prediction of postnatal renal function. Fetal ultrasound and fetal urine biochemistry are currently employed for this prediction, but clearly lack precision. We previously developed a fetal urine peptide signature that predicted in utero with high precision postnatal renal function in fetuses with PUV. We describe here the objectives and design of the prospective international multicentre ANTENATAL (multicentre validation of a fetal urine peptidome-based classifier to predict postnatal renal function in posterior urethral valves) study, set up to validate this fetal urine peptide signature. Participants will be PUV pregnancies enrolled from 2017 to 2021 and followed up until 2023 in >30 European centres endorsed and supported by European reference networks for rare urological disorders (ERN eUROGEN) and rare kidney diseases (ERN ERKNet). The endpoint will be renal/patient survival at 2 years postnatally. Assuming α = 0.05, 1-β = 0.8 and a mean prevalence of severe renal outcome in PUV individuals of 0.35, 400 patients need to be enrolled to validate the previously reported sensitivity and specificity of the peptide signature. In this largest multicentre study of antenatally detected PUV, we anticipate bringing a novel tool to the clinic. Based on urinary peptides and potentially amended in the future with additional omics traits, this tool will be able to precisely quantify postnatal renal survival in PUV pregnancies. The main limitation of the employed approach is the need for specialized equipment. Accurate risk assessment in the prenatal period should strongly improve the management of fetuses with PUV.

Abstract

Background: The complement and kallikrein-kinin systems (KKS) are activated during vascular inflammation. The aim of this study was to investigate if blockade of the KKS can affect complement activation on the endothelium during inflammation. Methods: Complement deposition on endothelial microvesicles was assayed in vasculitis patient plasma samples and controls. Plasma was perfused over glomerular endothelial cells and complement deposition assayed by flow cytometry. The effect of the kinin system was assessed using kinin receptor antagonists and C1-inhibitor. The in vivo effect was assessed in kidney sections from mice with nephrotoxic serum-induced glomerulonephritis treated with a kinin receptor antagonist. Findings: Vasculitis patient plasma had significantly more C3- and C9-positive endothelial microvesicles than controls. Perfusion of patient acute-phase plasma samples over glomerular endothelial cells induced the release of significantly more complement-positive microvesicles, in comparison to remission or control plasma. Complement activation on endothelial microvesicles was reduced by kinin B1- and B2-receptor antagonists or by C1-inhibitor (the main inhibitor of the classical pathway and the KKS). Likewise, perfusion of glomerular endothelial cells with C1-inhibitor-depleted plasma induced the release of complement-positive microvesicles, which was significantly reduced by kinin-receptor antagonists or C1-inhibitor. Mice with nephrotoxic serum-induced glomerulonephritis exhibited significantly reduced glomerular C3 deposition when treated with a B1-receptor antagonist. Interpretation: Excessive complement deposition on the endothelium will promote endothelial injury and the release of endothelial microvesicles. This study demonstrates that blockade of the KKS can reduce complement activation and thereby the inflammatory response on the endothelium. (C) 2019 The Authors. Published by Elsevier B.V. Abstract

Congenital diaphragmatic hernia (CDH) is a malformation with a diverse clinical picture. Its severity can be assessed prenatally using the lung area to head circumference ratio (LHR). LHR lower than 1 as associated with high mortality is an indication for prenatal treatment involving occlusion of fetal trachea. Before such treatment other congenital malformations must be excluded. Assessment of circulatory system in these patients is difficult. Heart is compressed by viscera in the thorax that decreases blood return to the left atrium, and structures of the left heart may be smaller than in healthy fetuses. The echocardiographic picture may be similar to some congenital defects of the left heart. Differentiation between structural and functional cardiac abnormalities is difficult but essential for diagnostic and therapeutic decisions in the most severe type of CDH. The authors present problems with interpretation of hemodynamic disturbances in a neonate with prenatally diagnosed and treated severe CDH. [ABSTRACT FROM AUTHOR]

Abstract

Cardiac resynchronization therapy (CRT) is an established therapy for patients with refractory symptomatic chronic heart failure, severe left ventricular (LV) systolic dysfunction and left ventricular dyssynchrony. CRT has been shown to improve patient's clinical outcome and survival, reduce hospital admission and promote cardiac performance, but a significant percentage of patients (up to 30-40%) treated with CRT do not show a favourable response. One of the important determinants of CRT outcome is left ventricular dyssynchrony. Echocardiography is the most available method for the assessment of cardiac dyssynchrony. At present, apart from the conventional echocardiographic examinations, various imaging methods are available including echocardiography with tissue Doppler imaging (TDI) and 3-dimensional echocardiography. Recently, new techniques have been developed for the assessment of LV dyssynchrony: magnetic resonance imaging (MRI), 3-dimensional maps and nuclear imaging - SPECT. Studies observed that assessment of inter- and particularly intraventricular dyssynchrony may allow to identify the potential responders to CRT. This poor CRT outcome is associated with suboptimal LV lead placement which depends on the presence and localization of a myocardial scar at the region targeted by the LV lead and the LV lead pacing positioned away from the latest activated areas of the left ventricle. Additionally, another important preimplantation issue is knowledge of the cardiac venous anatomy of the candidate. Searching for an optimal position of LV lead and visualisating the anatomy of the coronary sinus and tributaries may improve responsiveness to CRT. [ABSTRACT FROM AUTHOR]

Abstract

In a minority patients (pts) the endocardial approach to the left ventricular fails due to advanced heart disease or anatomic factors. In such pts bifocal endocardial pacing in right ventricle might be the profitable alternative.The aim of the study was to assess the clinical and hemodymanic parameters in pts with with bifocal pacing and severe heart failure.Methods 9 pts (6 males, 3 female) with mean age 68±8 years, in heart failure (at NYHA functional class IV), with QRS duration > 130 ms, with LVEF < 35% were included to the study. In all pts endocardial left ventricular approach was failed. Then, bifocal pacing system (one lead to the apex of the right ventricle (RVA) and the other to the right ventricle outflow tract (RVOT) was implanted. Atrial lead was inserted to the right atrial appendage. Next, all leads were connected to the three chamber pacemaker (Biotronik Stratos LV). Echo study in every pts with A-Vdelay and V-V delay optimisation was performed. Following parameters before (Bimpl) and after procedure (Aimpl) (from 2 to 8 , mean 5,6±3 days) were estimated: NYHA functional class (NYHA), 6 minute walking test (6MWT), left ventricular ejection fraction (LVEF), left ventricular diastolic diameter (LVdD), left ventricular systolic diameter (LVsD), cardiac output (CI) and cardiac index (CI). Results The results are presented in table below: Conclusions During the short term follow up in selected pts in whom achievment of pacingthrough a venous branch of coronary sinus was not possible and bifocal system in right ventricle was implanted, significant improvment in NYHA functional class, 6 minute walking test and in the majority of hemodynamic parametrers was reported. Abstract

The paper presents some basic data on treating patients with coronary heart disease and sexual dysfunctions. It is reported the opinion of the Second Princeton Consensus Panel (2005). Erectile dysfunction may be the earliest syndrome of cardiovascular disease. Patients after myocardial infarction feel grat prejudice while returning to their sexual activity. Patients fears are an important problem for cardiologist. Discussed reports show correlation between sudden death and taking cardiological medications with PDE-5 inhibitors at the same time. Sildenafil is recommended for men with erectile dysfunction and heart dissease. PDE-5 inhibitors should not be used in combination with alpha-blockers. Patients treated with nitrates should not take sildenafil and vardenafil in 24 hours, and tadalafil in 48 hours after a nitrate. Abstract

This paper discusses the cases of siblings that were born healthy, then diagnosed in their neonatal periods with cardiomyopathy and/or severe metabolic acidosis, which ran progressive courses and contributed to death in infancy. Molecular testing of the children confirmed the presence of an m.3303C>T point mutation in the mitochondrial DNA in the MT-TL1 gene, which was also present in their oligosymptomatic mother and their mother's sister, an asymptomatic carrier.

48. A second delivery after heart transplantation - a case study.

Abstract

Pregnancy after organ transplantation is becoming relatively common. We present the case of a heart transplant recipient who gave birth to a second child. Despite the fact that the transplanted heart seems to adapt well to the changes caused by pregnancy, gestation in patients after heart transplantation may be complicated by hypertension, pre-eclampsia, or preterm labor. In this article, we consider the issues of preterm uterine contractions, anemia, thrombocytopenia, and several other complications in pregnant patients with transplanted hearts. We also present current opinions regarding the use of glucocorticoids as a form of preventing breathing disorders in neonates as well as breast-feeding by mothers receiving immunosuppressive agents. Pregnancies in heart transplant recipients should be considered high-risk. A second successful delivery of a healthy child remains a challenge for such patients and their doctors.

Abstract

Introduction: Patients with severe aortic stenosis (AS), high mean gradient (HMG), and preserved left ventricular ejection fraction (LVEF) may present with paradoxical "low flow" (LF)., Objectives: The aim of the study was to assess the potential effect of cardiac collagen metabolism on the HMG/LF phenomenon in patients with severe AS and to determine a clinical and echocardiographic pattern of these patients., Patients and Methods: We assessed a clinical status of 89 patients, aged over 64 years, with severe AS, HMG, and preserved LVEF (≥50%). Cardiac structure and function as well as systemic arterial hemodynamics were assessed with echocardiography, conventional Doppler, and tissue Doppler imaging. Moreover, plasma levels of N-terminal pro-B-type natriuretic peptide (NT-proBNP), procollagen III N-terminal propeptide (PIIINP), carboxyterminal telopeptide of collagen type I, matrix metallopeptidase 9, and inhibitor of matrix metalloproteinase type 1 were evaluated. We analyzed 2 groups of patients: with normal flow (stroke volume index [SVI], ≥35 ml/m²; n = 70) and with LF (SVI, <35 ml/m²; n = 19)., Results: Patients with LF were older, had a larger left atrium and left atrial volume index, smaller aortic valve area, lower energy loss index, stroke work, mitral flow E velocity, mitral annular E' and S' velocities and systemic arterial compliance, higher relative left ventricular wall thickness, E/E', systemic arterial resistance and valvulo-arterial impedance. We observed a correlation between SVI and NT-proBNP, PIIINP, and selected parameters of cardiac structure and function., Conclusions: In patients with severe AS, HMG and preserved LVEF, the LF is related to a more severe obstruction, altered aortic hemodynamics, cardiac dysfunction, and higher blood levels of NT-proBNP. An inverse association between PIIINP and SVI may indicate enhanced tissue fibrosis as an underlying pathology.

Abstract

We present a case of the left atrial myxoma 6 years after atrial fibrillation ablation. The initial diagnosis of the mass revealed on echocardiography was a thrombus. Failure of anticoagulant treatment and transesophageal echocardiography led to diagnosis of myxoma, confirmed intraoperatively and histologically.
