2. Epicardial fat tissue, a hidden enemy against the early recovery of left ventricular systolic function after transcatheter aortic valve implantation

Background: Epicardial fat tissue (EFT) is an active organ that can affect cardiac function and structure through endocrine, paracrine, and proinflammatory mechanisms. We hypothesized that greater thickness of EFT may harm the recovery of left ventricular (LV) systolic function in patients with severe aortic stenosis (AS) and reduced LV ejection fraction (EF ≤ 50 %) undergoing transcatheter aortic valve implantation (TAVI). Methods: A sixty six patients with severe AS and 20 % ≥ LVEF ≤ 50 % who underwent TAVI were included. Patients were categorized into two groups based on LV systolic function recovery 30 days after TAVI defined by ≥ 20 % relative increase in LV Global longitudinal strain (GLS) from baseline. EFT was determined by ECG-gated contrast-enhanced multidetector computed tomography (MDCT). Results: Forty-five patients (68.0 %) showed LV systolic function recovery. EFT showed no significant correlation with the baseline LV-GLS but was associated with less likelihood of LV systolic function recovery (OR 0.7, 95 % CI 0.50 – 0.98, P = 0.04). In the multivariate analysis, higher LVMI (OR 1.05, 95 % CI 1.00–1.10, P = 0.02), lower LV-GLS (OR 0.55, 95 % CI 0.40–0.82, P = 0.002), and thinner EFT (OR 0.38, 95 % CI 0.20–0.73, P = 0.003) were independently associated with LV systolic function recovery after TAVI. Conclusion: EFT extent is associated with LV systolic function recovery in AS patients with impaired LVEF undergoing TAVI and therefore may help in risk stratification and management of these patients.

BackgroundDifficulties in emotional regulation are often observed in children and adolescents with attention-deficit/hyperactivity disorder (ADHD). Innovative complementary treatments, such as video games and virtual reality, have become increasingly appealing to patients. The Secret Trail of Moon (MOON) is a serious video game developed by a multidisciplinary team featuring cognitive training exercises. In this second randomized clinical trial, we evaluated the impact of a 20-session treatment with MOON on emotional regulation, as measured by the Strengths and Difficulties Questionnaire. ObjectiveWe hypothesize that patients with ADHD using MOON will show improvements in (1) emotional regulation, (2) core ADHD symptoms, (3) cognitive functioning, and (4) academic performance, compared to a control group; additionally, we anticipate that (5) changing the platform (from face-to-face using virtual reality to the web) will not affect emotional regulation scores; and (6) the video game will not cause any clinically significant side effects. MethodsThis was a prospective, unicentric, randomized, unblinded, pre- and postintervention study with block-randomized sequence masking. Participants included individuals aged between 7 and 18 years who had a clinical diagnosis of ADHD and were receiving pharmacological treatment. They were randomized into 2 groups using an electronic case report form: the MOON group, receiving standard pharmacological treatment plus personalized cognitive training via a serious video game, and the control group, receiving standard pharmacological treatment. We provided both the groups with psychoeducational support on ADHD. Analysis was conducted using the Student 2-tailed t test and 2-factor ANOVA. An independent monitor supervised the study. ResultsA total of 76 patients with ADHD participated in the trial, with an equal randomization (MOON: n=38, 50% and control: n=38, 50%) and a total dropout rate of 7. The primary hypothesis, a 3- or 4-point reduction in the global Strengths and Difficulties Questionnaire score, was not met. However, significant improvements were observed in material organization (P=.03), working memory (P=.04), and inhibition (P=.05), particularly among patients more engaged with the MOON treatment. ConclusionsSerious video games, when integrated into a multimodal treatment plan, can enhance outcomes for symptoms associated with ADHD. El estudio realiza una exploración exhaustiva sobre la integración de la Inteligencia Artificial en la educación, destacando la evolución de su rol en los procesos de enseñanza y aprendizaje dentro del marco de la Educación 4.0. Los resultados revelan que la Inteligencia Artificial ha pasado de ser una herramienta de enseñanza básica a sistemas avanzados que personalizan el aprendizaje y automatizan tareas administrativas, lo cual mejora la eficiencia educativa. El principal hallazgo es que la Inteligencia Artificial permite una adaptación en tiempo real del contenido educativo a las necesidades individuales de los estudiantes. La metodología empleada se basa en la PRISMA, la cual garantiza la transparencia y reproducibilidad al realizar una revisión sistemática y metaanálisis de la literatura relevante, siguiendo fases estructuradas de identificación, selección, elegibilidad e inclusión. Como resultado se evidencia que esta investigación tiene una gran relevancia a nivel global, con especial énfasis en Perú, donde el contexto educativo enfrenta desafíos únicos que pueden beneficiarse significativamente de las tecnologías emergentes. La conclusión enfatiza que, aunque la Inteligencia Artificial tiene el potencial de transformar la educación globalmente, su implementación efectiva requiere una revisión profunda de las infraestructuras educativas, así como una formación continua de los docentes.

Objetivos. Explorar la frecuencia e impacto de la violencia contra los trabajadores de salud de Argentina y compararlo con el resto de sus pares de Latinoamérica en el contexto de la pandemia por COVID-19. Materiales y métodos. Estudio de corte transversal a través de una encuesta electrónica al personal médico y no médico de Latinoamérica que desempeño tareas asistenciales desde marzo de 2020. Se utilizó una regresión de Poisson para estimar las Razones de Prevalencia crudas (RP) y ajustadas (RPa) con sus respectivos intervalos de confianza al 95%. Resultados. Un total de 3544 participantes de 19 países respondieron la encuesta; 1992 (56,0%) residían en Argentina. Entre los mismos, el 62,9% padeció al menos un hecho de violencia: 97,7% refirió violencia verbal y 11,8% violencia física. El 41,5% de los agredidos padecieron situaciones de violencia al menos una vez por semana. BackgroundAttention deficit hyperactivity disorder (ADHD) is the most common neurodevelopmental disorder in childhood and adolescence, with a prevalence of 5% and associated difficulties and worse prognosis if undetected. Multimodal treatment is the treatment of choice. However, sometimes treatment can be insufficient or have drawbacks. ObjectiveThis study protocol aims to demonstrate the effectiveness of cognitive training through the serious video game The Secret Trail of Moon (MOON) in improving emotional regulation in people with ADHD. MethodsThis is a prospective, unicenter, randomized, unblinded, pre- and postintervention study. The groups will be randomized (MOON vs control) via an electronic case report form. The MOON intervention will be performed 2 times per week for 10 weeks (30 minutes per session). The first 5 weeks (10 sessions) will be conducted face-to-face at the Puerta de Hierro University Hospital, and the remaining weeks will be conducted via the internet at the participants’ homes. The total sample consists of 152 patients aged between 7 and 18 years. All participants have a clinical diagnosis of ADHD under pharmacological treatment. Data collection will be used to obtain demographic and clinical data. The data will be recorded using REDCap. Measures will be made through clinical scales for parents and objective tests of cognitive functioning in patients. Additional information on academic performance will be collected. The study has a power greater than 80% to detect differences. Student t test, 2-factor analysis of variance (ANOVA), and Mann-Whitney analyses will be performed according to each variable’s characteristics. ResultsThe study was approved by the Research Ethics Committee of the Puerta de Hierro University Hospital on December 14, 2022. BackgroundIn recent decades, the relationship between emotional disorders (i.e., depression and anxiety) and alterations in physiological functions (i.e., inflammation or metabolism) have been well supported. However, studies on a symptom-based approach have provided mixed results. Our study aims to gain insight into how subclinical statuses, featured by elevated depressive and/or anxious symptoms, may influence immunometabolic alterations in the concurrent relationship; and the development of metabolic diseases at 10-year follow-up: diabetes, hypertension and hypercholesterolemia.MethodsData from 758 Greek adults [394 men (aged 41 ± 10 years) and 364 women (aged 37 ± 12 years)] were used. Four groups were created according to the levels of depressive and anxiety symptoms: (1) control group (CG), (2) depressive group (DG), (3) anxiety group (AG) and (4) depressive and anxiety group (DAG). Multi-indicator multi-causes (MIMIC) modeling was used to estimate metabolic function and inflammatory response scores, on a wide selection of blood biomarkers. Finally, a binary logistic regression was carried out to study the influence of symptoms on the development of the aforementioned metabolic diseases on a 10-year follow-up.ResultsGroup membership was not associated with metabolic function score. Conversely, DAG membership was related with higher inflammatory response score (B = 0.20, CI95 = 0.01, 0.40), with respect to the CG (p < 0.05). Both age and sex were significant variables in the calculation of both scores. Regarding disease at 10-year follow-up effect, risk of developing diabetes, hypertension and hypercholesterolemia was associated with age and socioeconomic status. Moreover, DG membership was significant for diabetes risk (OR = 2.08, CI95 = 1.00, 4.22) and DAG for hypercholesterolemia (OR = 1.68, CI95 = 1.16, 2.43).LimitationsData on anti-inflammatory drugs and psychopharmacological medication were not collected in this study.ConclusionsElevated symptoms of depression and anxiety accounts for inflammatory alterations at concurrent relationship and a higher risk of 10-year follow-up metabolic diseases.

Resumen: Las visitas dentales previas tienen un gran impacto en el niño como en sus padres y/o cuidadores para su juicio frente a consultas dentales posteriores. Objetivo: Evaluar si existe diferencias al comparar los factores para elegir un odontopediatra y la consulta dental previa en Lima Metropolitana. Material y métodos: Estudio transversal con una muestra de 294 padres. Se recolectaron los datos sobre la consulta dental previa del niño en odontopediatría entre 0 a 5 años, con categorías de respuesta de Sí y No; los factores para elegir un odontopediatra, con categorías de respuesta de muy importante, muy útil, no importante y no útil. Para ello se utilizó un cuestionario virtual de 29 ítems. Se utilizó la prueba exacta de Fisher y Chi-cuadrado para el análisis bivariado. Resultados: Se encontraron diferencias estadísticamente significativas en 4 de 5 factores tales como, recomendaciones, experiencias previas, tipos de información buscada y logísticas. Por otro lado, se encontró que 50.68% de los niños menores de 5 años sí habían tenido una consulta dental con un especialista, donde el 20.07% habían asistido en el rango de 1 a 3 años y sólo el 12.59% asistió antes del primer año a la consulta dental con el odontopediatra. Conclusiones: Existen diferencias al comparar los factores para elegir un odontopediatra y la consulta dental previa en Lima Metropolitana. Por último, se evidenció que los padres de familia no llevan a sus hijos a su primera visita dental antes del primer año, recomendación indicada por la Asociación dental Americana.

Bioelectrogenesis allows the transformation of chemical energy into electrical energy by means of microbial fuel cells.The research aimed to determine the amount of energy generated by bioelectrogenesis using Pseudomonas aeruginosa and Aeromonas hydrophila bacteria. Four double chamber H-type glass microbial fuel cells with a capacity of 500 mL and 2 control cells were constructed, using graphite or aluminium rod electrodes at the anode and graphite rod at the cathode for all the cells. 325 mL of anaerobic sludge and 75 mL of wastewater from a wastewater treatment plant were inoculated as substrate for the bacteria in the anode, where 50 mL of the aforementioned bacteria strain were inoculated, respectively. The experimental part was carried out in 20 days; the conditions of the cells were evaluated in terms of temperature and pH, characteristics of the bacteria and the behaviour of the voltage generated. It was established that the bacteria that generated the highest voltage was Pseudomonas aeruginosa with 0.8960V, using an aluminum electrode in the anode chamber. The results indicate that bioelectrogenesis using bacteria in anaerobic sludge and wastewater is a promising technology for obtaining clean and low-cost energy.

The use of energy is fundamental in the main industrial, communications and other activities, it has become an essential resource in people's lives and in the development of peoples; However, some sources of its production have negative impacts on the environment, especially fossil and mineral sources that cause a greater negative impact. In Peru, in 2019, the energy sources were natural gas (including liquids) at 64.4%, mineral coal at 0.4%, crude oil at 10%; those from natural resources: hydropower 12.7%, firewood 8.8%, bagasse 2.1%, dung with yareta 0.5% and solar energy 0.4%. The objective of the research was to obtain biofuel and characterize it from benthic marine flora, specifically from the alga Macrocystis pyrifera. The method consisted of extracting the oil from three samples of 30 g of algae with three repetitions each, followed by a transesterification process and then separating it into phases with sodium methoxide, obtaining the biofuel. The results of the characterization of the product indicated a density of 0.8999 g/mL, kinematic viscosity 2.3314 cSt at 40 ° C, acid number of 0.344 mg KOH/g, carbon residue of 0.024% m/m, sulphated ash of 0.017%. m/m calorific value from 39.40 to 39.43 MJ/kg. These characteristics meet the specifications of EN 14214, ASTM D6751, ANP N ° 7/08 and NTP 321.125.2019 (an exception for kinematic viscosity that only met the first two named). In this way, it was shown that this natural resource turns out to be a viable alternative to be used as a source of bioenergy and with a low negative impact on the environment.

Many organisms avoid habitats posing risks of parasitism. Parasites are not generally conspicuous, however, which raises the question of what cues individuals use to detect parasitism risk. Here, we provide evidence in alpine newts (Ichthyosaura alpestris) that non-visual cues from parasite-exposed conspecifics inform habitat avoidance. Alpine newts breed in aquatic habitats and occasionally move among adjacent terrestrial habitat during breeding seasons. We completed experiments with newts whereby individuals had access to both habitats, and the aquatic habitats varied in prior occupancy by conspecifics with different histories of exposure to the parasitic skin fungus, Batrachochytrium dendrobatidis (Bd). Continuous filming of newt activity for 2 days provided little evidence that prior use of aquatic habitats by conspecifics, regardless of their Bd exposure history, immediately influenced newt habitat use. However, newts that encountered aquatic habitats used specifically by Bd-exposed conspecifics on day 1 spent less time aquatic on day 2, whereas other newts did not alter habitat use. Responses could have been elicited by cues generated by Bd stages on the conspecifics or, perhaps more likely, cues emitted by the conspecifics themselves. In either case, these observations suggest that newts use non-visual cues sourced from exposed conspecifics to detect Bd risk and that those cues cause newts to avoid aquatic habitats. Bd may therefore influence host behavior in early phases of interactions, and possibly before any contact with infectious stages is made, creating potential for non-consumptive effects.

Microbial activity involved in the cocoa beans fermentation process is essential to maintain and improve the organoleptic and nutritional qualities of chocolate; therefore, the aim of this investigation was to search and select microbial isolates with the potential to improve the quality of cocoa beans. Fermentation experiments were conducted on farms located in Maceo (Antioquia), San Vicente de Chucurí (Santander), and Rivera and Algeciras (Huila), Colombia. Yeast, lactic acid bacteria (LAB), acetic acid bacteria (AAB), and mesophilic aerobic microorganisms were obtained from different fermentation batches. The growth of these microorganisms was tested in six treatments as follows: 50% cocoa pulp agar (CPA), high concentrations of glucose (10%), ethanol (5%), and acetic acid (7%), an acidic pH of 3.0, and a high temperature of 50oC for 24 h. The isolates with the highest growth were identified by 18S and 16S rRNA gene analysis, revealing a high diversity of species associated with cocoa fermentation, including eight species of yeasts (Debaryomyces hansenii, Meyerozyma guillermondii, Wickerhanomyces anomalus, Pichia guillermondii, Pichia kudriavzevii, Trichosporon asahii, Candida parapsilosis, and Pichia manshurica), six species of LAB (Pediococcus acidilactici, Lactobacillus brevis, Lactobacillus plantarum, Lactobacillus farraginis, Lactobacillus rhamnosus, and Leuconostoc mesenteroides), four species of AAB (Gluconobacter japonicus, Acetobacter tropicalis, Acetobacter pasteurianus, and Acetobacter malorum/tropicalis), and three species of Bacillus spp. (Bacillus aryabhattai /megaterium, Bacillus subtilis, and Bacillus coagulans). In general, microbial populations increased in cocoa batches after 12 h of fermentation and decreased after 84-96 h. All the yeast isolates grew in 10% glucose and CPA, 85.7% in 5% ethanol, and 95% at a pH of 3.0. All the yeast isolates were affected by 7% acetic acid and incubation at 50oC for 24 h. Eighty-five percent of the LAB grew in 10% glucose, 100% in 5% ethanol, 42.8% in CPA, 64% at a pH of 3.0, and 35.7% grew after being exposed to 50oC for 24 h; all were affected by 7% acetic acid. As for the AAB, 100% grew in 10% glucose, 71% in 7% ethanol, 100% grew in CPA, in 7% acetic acid, and at a pH of 3.0, while 100% were affected by incubation at 50oC. Three yeast isolates, W. anomalus, D. hansenii and M. guillermondii, three LAB isolates, P. acidilactici, L. brevis, and L. plantarum, and three AAB isolates, A. tropicalis, A. pasteurianus and G. japonicus, were selected as promising strains to be used in a microbial starter culture for cocoa bean fermentation to improve the organoleptic quality of cocoa.

Conflicts between national and local governments over land use in floodplains have been well documented in the US and elsewhere. The US National Flood Insurance Program offers subsidized flood insurance to communities that agree to prevent further development in floodplains, but the requirements are poorly enforced and local governments are commonly reluctant to restrain development on flood-prone lands. In this paper we highlight this problem in particularly sensitive areas: the floodways (or flood bypasses) that are essential components of the Mississippi River flood control system. To properly operate the flood control system, the US Army Corps of Engineers must be able to divert flow from the mainstem Mississippi into the bypasses, thereby lowering stage in the main river, and thus minimizing flooding of cities and other vulnerable areas. However, operation of the Birds-Point-New Madrid Floodway in Missouri was compromised in 2011 by local opposition (and a legal challenge ultimately rejected by the US Supreme Court), and it was finally used to accommodate floodwaters. The West Atchafalaya Floodway in Louisiana experienced a threefold increase in the number of structures within the floodway from about 1970 to 2010. Because of the pattern of flooding, the West Atchafalaya Floodway was not needed in 2011, but if it is needed in the future, its operation may be compromised by the extensive encroachments within the floodway. Thus, operation of critical national infrastructure, designed to deal with floods on an interstate, river-basin scale, is compromised by land-use decisions made at the local level.

The widely accepted “continuing bonds” model of grief tells us that rather than bereavement necessitating the cessation of one's relationship with the deceased, very often the relationship continues instead in an adapted form. However, this framework appears to conflict with philosophical approaches that treat reciprocity or mutuality of some form as central to loving relationships. Seemingly the dead cannot be active participants, rendering it puzzling how we should understand claims about continued relationships with them. In this article, we resolve this tension by highlighting two fundamental aspects of paradigmatic loving relationships that can, and often do, continue in an adapted form following bereavement: love and mutual shaping of interests, choices, and self-concepts. Attention to these continuing features of relationships helps to capture and clarify the phenomenological and behavioral features of continuing bonds. However, love and mutual shaping must also change in important ways following bereavement. Love becomes unreciprocated, and although the dead continue to shape our interests, choices, and self-concepts, we predominantly shape their legacies and memories in return. These changes place important constraints upon the nature of our interpersonal connections with the dead.

BackgroundThe Emerald project's focus is on how to strengthen mental health systems in six low- and middle-income countries (LMICs) (Ethiopia, India, Nepal, Nigeria, South Africa and Uganda). This was done by generating evidence and capacity to enhance health system performance in delivering mental healthcare.A common problem in scaling-up interventions and strengthening mental health programmes in LMICs is how to transfer research evidence, such as the data collected in the Emerald project, into practice.AimsTo describe how core elements of Emerald were implemented and aligned with the ultimate goal of strengthening mental health systems, as well as their short-term impact on practices, policies and programmes in the six partner countries.MethodWe focused on the involvement of policy planners, managers, patients and carers.ResultsOver 5 years of collaboration, the Emerald consortium has provided evidence and tools for the improvement of mental healthcare in the six LMICs involved in the project. We found that the knowledge transfer efforts had an impact on mental health service delivery and policy planning at the sites and countries involved in the project.ConclusionsThis approach may be valid beyond the mental health context, and may be effective for any initiative that aims at implementing evidence-based health policies for health system strengthening.

Aphantasia has been described as the inability to voluntarily evoke mental images using the "mind's eye." We studied a congenital aphantasic subject using neuropsychological testsand 64 channel EEG recordings, in order to studycortical activity involved in perception and imagery evaluating event-related potentials(N170, P200, N250). The subject is in the normal range of the neuropsychological tests performed, except for specific imagery tests. The EEG results show that when he evokes the same mental image, he starts the evoking process from left temporal instead of frontal areas, he does not activate occipital visual nor left anterior parietal areas.
