3. Implementation of an Innovative Design: Restructuring the Virginia Department of Education. Policy Issues.

Abstract

This paper describes the restructuring of the Virginia Department of Education, a process that began in 1990. Data were obtained from four sources: (1) Virginia Department of Education documents; (2) a prerestructuring study conducted by Price Waterhouse in 1989; (3) an evaluative report undertaken by the Joint Legislative Audit and Review Commission (JLARC); and (4) personal interviews with department staff. The Price Waterhouse study identified seven issues confronting the department: increasing student diversity; a changing workplace; local fiscal disparities; altered state-local relationships; the demands of state policymakers for outcome indicators; the activism of state policymakers nationwide in educational restructuring; and the need for a systemwide view of school improvement. The new structure features four hierarchical levels and three branches for student services; research, policy development, and information systems; and administrative services. A new request-for-proposal process, in which an "idea paper" can be generated by anyone within the department or local school division and is reviewed by top management in collaboration with experts, establishes an internal system of control. Although the change strategies appear to have worked, a midway evaluation report indicated the presence of poor staff morale, stress, and uncertainty. The recommendation is made to conduct a followup evaluation in approximately 2 years. One figure is included. (Contains 12 references.) (LMI)

Abstract

Background While previous research has suggested that health care assistants supporting palliative care work in the community regard the provision of emotional labour as a key aspect of their role, little research has explored the experiences of family carers who are the recipients of such support. Objective To explore the emotional labour undertaken by health care assistants working in community palliative care from the perspectives of both health care assistants and bereaved family carers. Design We conducted a qualitative interview study in 2011-2012 with bereaved family carers of cancer patients who had received the services of health care assistants in the community, and health care assistants who provided community palliative care services. Transcripts were coded and analysed for emergent themes using a constant comparative technique. Settings Three different research sites in the United Kingdom, all providing community palliative care. Participants and methods Semi-structured interviews were conducted with 33 bereaved family carers and eight health care assistants. Results Health care assistants view one of their key roles as providing emotional support to patients and their family carers, and family carers recognise and value this emotional support. Emotional support by health care assistants was demonstrated in three main ways: the relationships which health care assistants developed and maintained on the professional-personal boundary; the ability of health care assistants to negotiate clinical/domestic boundaries in the home; the ways in which health care assistants and family carers worked together to enable the patient to die at home. Conclusion t Trough their emotional labour, health care assistants perform an important role in community palliative care which is greatly valued by family carers. While recent reports have highlighted potential dangers in the ambiguity of their role, any attempts to clarify the ?boundaries? of the health care assistant role should acknowledge the advantages health care assistants can bring in bridging potential gaps between healthcare professionals and family carers.

Recovery-orientated services are recommended for adult mental health, but with little evidence base to support this. ObjectivesTo facilitate understanding and promotion of recovery in psychosis and bipolar disorder (BD), in a manner that is empowering and acceptable to service users. MethodThere were six linked projects using qualitative and quantitative methodologies: (1) developing and piloting a service user-defined measure of recovery; (2) a Delphi study to determine levels of consensus around the concept of recovery; (3) examination of the psychological factors associated with recovery and how these fluctuate over time; (4) development and evaluation of cognitive–behavioural approaches to guided self-help including a patient preference trial (PPT); (5) development and evaluation of cognitive–behavioural therapy (CBT) for understanding and preventing suicide in psychosis including a randomised controlled trial (RCT); and (6) development and evaluation of a cognitive–behavioural approach to recovery in recent onset BD, including a RCT of recovery-focused cognitive–behavioural therapy (RfCBT). Service user involvement was central to the programme. ResultsMeasurement of service user-defined recovery from psychosis (using the Subjective Experience of Psychosis Scale) and BD (using the Bipolar Recovery Questionnaire) was shown to be feasible and valid. The consensus study revealed a high level of agreement among service users for defining recovery, factors that help or hinder recovery and items which demonstrate recovery. Negative emotions, self-esteem and hope predicted recovery judgements, both cross-sectionally and longitudinally, whereas positive symptoms had an indirect effect. 13. Receiving end of life care at home: experiences of the bereaved carers of cancer patients cared for by health care assistants

Abstract

First paragraph: Many terminally ill cancer patients and their families prefer for death to occur at home rather than in an institution where the majority of care falls to the patient’s family and friends. As death approaches caring can become an increasing burden for the patient’s informal carers. This issue has long been recognized by health care professionals and also in current policy for end of life care, with the End of Life Care Strategy for England (DH, 2008) highlighting the need for community services to enable home death by supporting both patient and their family carers. Basic nursing, social and respite home care has frequently been provided by basically trained, unqualified nursing staff, including auxiliary nurses and health care assistants (HCA). Whilst increasing research has been undertaken into the needs of family carers (Stajduhar et al, 2010; Funk et al 2010), relatively little has focused on the care HCAs deliver (Herber & Johnston 2012) and very few studies have explored the experience of bereaved family carers of patients who have received such services.
