Abstract

Recent years have witnessed exponential growth in cardiac imaging technologies, allowing better visualization of complex cardiac anatomy and improved assessment of physiology. These advances have become increasingly important as more complex surgical and catheter-based procedures are evolving to address the needs of a growing congenital heart disease population. This state-of-the-art review presents advances in echocardiography, cardiac magnetic resonance, cardiac computed tomography, invasive angiography, 3-dimensional modeling, and digital twin technology. The paper also highlights the integration of artificial intelligence with imaging technology. While some techniques are in their infancy and need further refinement, others have found their way into clinical workflow at well-resourced centers. Studies to evaluate the clinical value and cost-effectiveness of these techniques are needed. For techniques that enhance the value of care for congenital heart disease patients, resources will need to be allocated for education and training to promote widespread implementation.

Abstract

During the peak phase of the COVID-19 pandemic, alterations of standard operating procedures were necessary for health systems to protect patients and healthcare workers and ensure access to vital hospital resources. As the peak phase passes, re-activation plans are required to safely manage increasing clinical volumes. In the context of cardiovascular magnetic resonance (CMR), re-activation objectives include continued performance of urgent CMR studies and resumption of CMR in patients with semi-urgent and elective indications in an environment that is safe for both patients and health care workers.

Abstract

This paper describes a possible way to improve computer security by implementing a program which implements the following three features related to a weak notion of artificial consciousness: (partial) self-monitoring, ability to compute the truth of quantifier-free propositions and the ability to communicate with the user. The integrity of the program could be enhanced by using a trusted computing approach, that is to say a hardware module that is at the root of a chain of trust. Abstract

This paper argues that mathematical objects are constructions and that constructions introduce a flexibility in the ways that mathematical objects are represented (as sets of binary sequences for example) and presented (in a particular order for example). The construction approach is then applied to searching for a mathematical object in a set, and a logarithm-time search algorithm outlined which applies to a set X of all binary sequences of length ordinal $\beta$ with a binary label appended to each sequence to indicate that sequence is a member of X or not. It follows that deciding membership of a set for a given binary sequence of length of binary sequence of cardinal length $\beta$ takes $\beta+1$ bits, which is shown to be equivalent to the Generalised Continuum Hypothesis on the assumption that information is minimised when a mathematical object is created., Comment: 7 pages, no figures. Abstract

We propose a new iterative segmentation model which can be accurately learned from a small dataset. A common approach is to train a model to directly segment an image, requiring a large collection of manually annotated images to capture the anatomical variability in a cohort. In contrast, we develop a segmentation model that recursively evolves a segmentation in several steps, and implement it as a recurrent neural network. We learn model parameters by optimizing the interme- diate steps of the evolution in addition to the final segmentation. To this end, we train our segmentation propagation model by presenting incom- plete and/or inaccurate input segmentations paired with a recommended next step. Our work aims to alleviate challenges in segmenting heart structures from cardiac MRI for patients with congenital heart disease (CHD), which encompasses a range of morphological deformations and topological changes. We demonstrate the advantages of this approach on a dataset of 20 images from CHD patients, learning a model that accurately segments individual heart chambers and great vessels. Abstract

This is a paper that aims to interpret the cardinality of a set in terms of Baire Category, i.e. how many closed nowhere dense sets can be deleted from a set before the set itself becomes negligible. . To do this natural tree-theoretic structures such as the Baire topology are introduced, and the Baire Category Theorem is extended to a statement that a $\aleph$-sequentially complete binary tree representation of a Hausdorff topological space that has a clopen base of cardinality $\aleph$ and no isolated or discrete points is not the union of $<\aleph+1$-many nowhere dense subsets for cardinal $\aleph\ge\aleph_{0}$, where a $\aleph$-sequentially complete topological space is a space where every function $f:\aleph\rightarrow\{0.1\}$ is such that $(\forall x)(x\in f\rightarrow x\in\in X)\rightarrow(f\in X)$. It is shown that if $\aleph<\left|X\right|\le2^{\aleph}$ for $\left|X\right|$ the cardinality of a set $X$, then it is possible to force $\left|X\right|-\aleph\times\left|X\right|\ne\emptyset$ by deleting a dense sequence of $\aleph$ specially selected clopen sets, while if any dense sequence of $\aleph+1$ clopen sets are deleted then $\left|X\right|-(\aleph+1)\times\left|X\right|=\emptyset$. This gives rise to an alternative definition of cardinality as the number of basic clopen sets (intervals in fact) needed to be deleted from a set to force an empty remainder. Abstract

This paper describes a type of infinitary computer (a hypercomputer) capable of computing truth in initial levels of the set theoretic universe, V. The proper class of such hypercomputers is called a universal hypercomputer. There are two basic variants of hypercomputer: a serial hypercomputer and a parallel hypercomputer. The set of computable functions of the two variants is identical but the parallel hypercomputer is in general faster than a serial hypercomputer (as measured by an ordinal complexity measure). Abstract

mRNA display is a powerful technology to screen libraries of >1012 cyclic peptides against a protein target, enabling the rapid discovery of high affinity ligands. These cyclic peptides are particularly well suited to challenging protein targets that have been difficult to drug with small molecules. However, target choice can still be limited as screens are typically performed against purified proteins which often demands the use of isolated domains and precludes the use of aggregation‐prone targets. Herein, we report a method to perform mRNA display selections in mammalian cell lysates without the need for prior target purification, vastly expanding the potential target scope of mRNA display. We have applied the methodology to identify low to sub‐nanomolar peptide binders for two targets: a NanoLuc subunit (LgBiT) and full‐length bromodomain‐containing protein 3 (BRD3). Our cyclic peptides for BRD3 were found to bind to the extraterminal (ET) domain of BRD3 and the closely related BRD proteins, BRD2 and BRD4. While many chemical probes exist for the bromodomains of BRD proteins, the ET domain is relatively underexplored, making these peptides valuable additions to the BRD toolbox. [ABSTRACT FROM AUTHOR]

Abstract

Physiology is a key element of "bioscience" education within pre-registration nursing programs, but there is a lack of clarity on what is included. Physiology and bioscience content and delivery are highly varied across both higher education institutions and the related programs in the United Kingdom (UK). Despite evidence highlighting concerns over nurses' lack of bioscience knowledge and unsafe practice, there is no universally agreed on curriculum with detailed outcomes of minimum levels of knowledge to support nurses in practice and patient care. This study aimed to inform the construction of discipline-specific physiology learning outcomes to clarify relevant physiological topics required in pre-registration nursing. Initially, 360 learning outcomes were identified from various sources. Using a modified Delphi approach, an expert panel from the Bioscience in Nurse Education group reviewed and modified the list to 195 proposed outcomes. These were circulated to universities in the UK who teach nursing (n = 65). Outcomes that had 80% consensus were automatically included in the next round, with others recommended with modification (response rate 22%). The panel reviewed the modifications, and 182 outcomes were circulated in the second questionnaire (response rate 23%), and further panel review resulting in 177 outcomes agreed. These learning outcomes do not suggest how they should be delivered, but gives the basic level required for qualification as a nurse commensurate with the Nursing and Midwifery Council new standards for the "future nurse."
