Empirical constraints of fundamental properties of protoplanetary disks are essential for understanding planet formation and planetary properties (1,2). Carbon monoxide (CO) gas is often used to constrain disk properties (3). However, estimates show that the CO gas abundance in disks is depleted relative to expected values (4,5,6,7), and models of various disk processes impacting the CO abundance could not explain this depletion on observed 1Myr timescales (8,9,10,11,12,13,14). Here we demonstrate that surface energy effects on particles in disks, such as the Kelvin effect, that arise when ice heterogeneously nucleates onto an existing particle can efficiently trap CO in its ice phase. In previous ice formation models, CO gas was released when small ice-coated particles were lofted to warmed disk layers. Our model can reproduce the observed abundance, distribution and time evolution of gaseous CO in the four most studied protoplanetary disks (7). BackgroundPatients with severe asthma may present with characteristics representing overlapping phenotypes, making them eligible for more than one class of biologic. Our aim was to describe the profile of adult patients with severe asthma eligible for both anti-IgE and anti-IL5/5R and to compare the effectiveness of both classes of treatment in real life.MethodsThis was a prospective cohort study that included adult patients with severe asthma from 22 countries enrolled into the International Severe Asthma registry (ISAR) who were eligible for both anti-IgE and anti-IL5/5R. The effectiveness of anti-IgE and anti-IL5/5R was compared in a 1:1 matched cohort. Exacerbation rate was the primary effectiveness endpoint. There was some evidence to suggest that patients treated with anti-IL5/5R experienced fewer asthma-related hospitalizations (IRR 0.64; 95% CI 0.38, 1.08), but not ER visits (IRR 0.94, 95% CI 0.61, 1.43). Conclusions: In real life, both anti-IgE and anti-IL5/5R improve asthma outcomes in patients eligible for both biologic classes; however, anti-IL5/5R was superior in terms of reducing asthma exacerbations and LTOCS use.

The CONQUEST target population includes patients diagnosed with COPD, and those undiagnosed but with COPD-like exacerbations, who are at high but modifiable risk of COPD exacerbations, increased morbidity, and mortality. Timely diagnosis and optimized management to reduce these risks is vital. There is a need for a quality improvement program (QIP) that enables long-term improvement of patient clinical outcomes via integration of the program into routine clinical care. Core to the CONQUEST program is the adoption of four specifically designed, globally applicable, and expert-agreed quality standards (QS) for modifiable high-risk COPD patients. Translation of the QS into clinical practice, and implementation of the QIP, is guided by the CONQUEST global operational protocol provided to sites meeting the minimum healthcare system requirements. Initial analyses of current practices are conducted to generate baseline assessments of need within healthcare systems and sites looking to implement the QIP. Implementation is supported by the provision of CONQUEST resources and tools, such as clinical decision support, that promote prompt identification and treatment of patients. Utilization of electronic medical record (EMR) and patient-reported data are integral components of the QIP. Regular, automated collection and analysis of data, combined with a cyclical review of the implementation process, will be conducted for long-term, continuous improvement and health impact evaluation. The CONQUEST QIP will be an important resource in the identification and management of patients with modifiable high-risk COPD. Embedding the CONQUEST QS into routine clinical practice with regular evaluation and feedback will result in long-term quality of care improvement. Background Phenotypic characteristics of patients with eosinophilic and noneosinophilic asthma are not well characterized in global, real-life severe asthma cohorts. Research Question What is the prevalence of eosinophilic and noneosinophilic phenotypes in the population with severe asthma, and can these phenotypes be differentiated by clinical and biomarker variables? Study Design and Methods This was an historical registry study. Adult patients with severe asthma and available blood eosinophil count (BEC) from 11 countries enrolled in the International Severe Asthma Registry (January 1, 2015-September 30, 2019) were categorized according to likelihood of eosinophilic phenotype using a predefined gradient eosinophilic algorithm based on highest BEC, long-term oral corticosteroid use, elevated fractional exhaled nitric oxide, nasal polyps, and adult-onset asthma. Demographic and clinical characteristics were defined at baseline (ie, 1 year before or closest to date of BEC). Results One thousand seven hundred sixteen patients with prospective data were included; 83.8% were identified as most likely (grade 3), 8.3% were identified as likely (grade 2), and 6.3% identified as least likely (grade 1) to have an eosinophilic phenotype, and 1.6% of patients showed a noneosinophilic phenotype (grade 0). Eosinophilic phenotype patients (ie, grades 2 or 3) showed later asthma onset (29.1 years vs 6.7 years; P Interpretation According to this multicomponent, consensus-driven, and evidence-based eosinophil gradient algorithm (using variables readily accessible in real life), the severe asthma eosinophilic phenotype was more prevalent than previously identified and was phenotypically distinct. This pragmatic gradient algorithm uses variables readily accessible in primary and specialist care, addressing inherent issues of phenotype heterogeneity and phenotype instability. Identification of treatable traits across phenotypes should improve therapeutic precision.

UV-SCOPE is a mission concept to determine the causes of atmospheric mass loss in exoplanets, investigate the mechanisms driving aerosol formation in hot Jupiters, and study the influence of the stellar environment on atmospheric evolution and habitability. As part of these investigations, the mission will generate a broad-purpose legacy database of time-domain ultraviolet (UV) spectra for nearly 200 stars and planets. The observatory consists of a 60 cm, f/10 telescope paired to a long-slit spectrograph, yielding simultaneous, almost continuous coverage between 1203 {\AA} and 4000 {\AA}, with resolutions ranging from 6000 to 240. The efficient instrument provides throughputs > 4% (far-UV; FUV) and > 15% (near-UV; NUV), comparable to HST/COS and much better than HST/STIS, over the same spectral range. A key design feature is the LiF prism, which serves as a dispersive element and provides high throughput even after accounting for radiation degradation. The use of two delta-doped Electron-Multiplying CCD detectors with UV-optimized, single-layer anti-reflection coatings provides high quantum efficiency and low detector noise. From the Earth-Sun second Lagrangian point, UV-SCOPE will continuously observe planetary transits and stellar variability in the full FUV-to-NUV range, with negligible astrophysical background. All these features make UV-SCOPE the ideal instrument to study exoplanetary atmospheres and the impact of host stars on their planets. UV-SCOPE was proposed to NASA as a Medium Explorer (MidEx) mission for the 2021 Announcement of Opportunity. If approved, the observatory will be developed over a 5-year period. A relationship between chromospheric activity and age is calibrated for FGK dwarf stars using GALEX$FUV$ F U V magnitudes and Gaia$(G_{BP} - G)$ ( G B P − G ) colors. Such a calibration between GALEX$FUV$ F U V magnitudes and stellar age has utility in population studies of dwarfs for further understanding of the chemical evolution of the Milky Way. As an illustration of one such application we have investigated a population of Sun-like, solar neighborhood stars for their metallicities and velocity dispersions; a cross-matched sample of FGK type dwarf stars from Casagrande et al. (2011) with the Gaia and GALEX catalogs. Using calibrated relationships between $FUV$ F U V magnitudes and age, we determined a chromospheric activity indicator, $Q$ Q , and stellar age, $\tau $ τ , for each dwarf. We constructed age-velocity (AVR) and age-metallicity (AMR) relations with empirically-determined $FUV$ F U V ages. Power law fits to AVR plots are consistent with heating mechanism models within the literature. We further demonstrate that perigalactic distance and eccentricity versus $FUV$ F U V -age plots are consistent with an “inside out” formation history model.

Background : MP- AzeFlu (azelastine/fluticasone propionate in one spray) is the most effective allergic rhinitis (AR) treatment available. Its effect on asthma control in co- morbid patients is unknown. We examined whether MP- AzeFlu altered asthma control in patients with AR and asthma. Method : This pre- post historical cohort study, carried out using the Optimum Patient Care Database, included patients aged ≥12 years from UK general practice with active asthma in the year before MP- AzeFlu initiation. Active asthma was defined as a recorded diagnosis, with ≥ 1 prescription for reliever or controller inhaler in the year prior to and including the initiation date. The primary endpoint was change in number of acute respiratory events (i.e. an exacerbation or an antibiotic course for a respiratory event) between baseline and outcome years. Study outcomes were assessed by Wilcoxon signed rank test or McNemar ' s test. Results : 1,188 patients were included (average age: 47 yrs; 58.4% female). Most were prescribed intranasal corticosteroids (60.4%) and Inhaled corticosteroid (ICS)/long- acting β2 - agonist (59.3%) at baseline. MP- AzeFlu initiation was associated with significantly fewer acute respiratory events ( P = .013). Significantly more patients had well- controlled asthma in the year after MP- AzeFlu initiation ( P = .004). These asthma control benefits were noted despite significant reduction in short- acting β2 - agonist (SABA) dose ( P 2 SABA puffs/week ( P < .001) and the average ICS dose remaining stable (64.0% of patients) or reduced (20.4% of patients). Conclusion : MP- AzeFlu initiation is associated with reduced acute respiratory events, improved asthma control and reduced SABA and maintenance ICS dose in patients with AR and asthma. Regional and/or national severe asthma registries provide valuable country-specific information. However, they are often limited in scope within the broader definitions of severe asthma, have insufficient statistical power to answer many research questions, lack intraoperability to share lessons learned, and have fundamental differences in data collected, making cross comparisons difficult. What is missing is a worldwide registry which brings all severe asthma data together in a cohesive way, under a single umbrella, based on standardized data collection protocols, permitting data to be shared seamlessly. The International Severe Asthma Registry (ISAR; http://isaregistries.org/) is the first global adult severe asthma registry. It is a joint initiative where national registries (both newly created and preexisting) retain ownership of their own data but open their borders and share data with ISAR for ethically approved research purposes. Its strength comes from collection of patient-level, anonymous, longitudinal, real-life, standardized, high-quality data (using a core set of variables) from countries across the world, combined with organizational structure, database experience, inclusivity/openness, and clinical, academic, and database expertise. This gives ISAR sufficient statistical power to answer important research questions, sufficient data standardization to compare across countries and regions, and the structure and expertise necessary to ensure its continuance and the scientific integrity and clinical applicability of its research. ISAR offers a unique opportunity to implement existing knowledge, generate new knowledge, and identify the unknown, therefore promoting new research. The aim of this commentary is to fully describe how ISAR may improve our understanding of severe asthma.

Background Clinical characteristics of the international population with severe asthma are unknown. Intercountry comparisons are hindered by variable data collection within regional and national severe asthma registries. We aimed to describe demographic and clinical characteristics of patients treated in severe asthma services in the United States, Europe, and the Asia-Pacific region. Methods The International Severe Asthma Registry retrospectively and prospectively collected data in patients with severe asthma (≥ 18 years old), receiving Global Initiative for Asthma (GINA) Step 5 treatment or with severe asthma remaining uncontrolled at GINA Step 4. Baseline demographic and clinical data were collected from the United States, United Kingdom, South Korea, Italy, and the Severe Asthma Web-based Database registry (including Australia, Singapore, and New Zealand) from December 2014 to December 2017. Results We included 4,990 patients. Mean (SD) age was 55.0 (15.9) years, and mean (SD) age at asthma onset was 30.7 (17.7) years. Patients were predominantly female (59.3%) and white (72.6%), had never smoked (60.5%), and were overweight or obese (70.4%); 34.9% were at GINA Step 5; and 57.2% had poorly controlled disease. A total of 51.1% of patients were receiving regular intermittent oral corticosteroids, and 25.4% were receiving biologics (72.6% for those at GINA Step 5). Mean (SD) exacerbation rate was 1.7 (2.7) per year. Intercountry variation was observed in clinical characteristics, prescribed treatments, and biomarker profiles. Conclusions Using a common data set and definitions, this study describes severe asthma characteristics of a large patient cohort included in multiple severe asthma registries and identifies country differences. Whether these are related to underlying epidemiological factors, environmental factors, phenotypes, asthma management systems, treatment access, and/or cultural factors requires further study.

The role of charge exchange in shaping exoplanet photoevaporation remains a topic of contention. Exchange of electrons between stellar wind protons from the exoplanet's host star and neutral hydrogen from the planet's wind has been proposed as a mechanism to create "energetic neutral atoms" (ENAs), which could explain the high absorption line velocities observed in systems where mass loss is occurring. In this paper we present results from 3D hydrodynamic simulations of the mass loss of a planet similar to HD 209458b. We self-consistently launch a planetary wind by calculating the ionization and heating resulting from incident high-energy radiation, inject a stellar wind into the simulation, and allow electron exchange between the stellar and planetary winds. We predict the potential production of ENAs by the wind-wind interaction analytically, then present the results of our simulations, which confirm the analytic limits. Within the limits of our hydrodynamic simulation, we find that charge exchange with the stellar wind properties examined here is unable to explain the absorption observed at high Doppler velocities., arXiv admin note: text overlap with arXiv:1906.00075

However, key opportunities to optimize treatment are often not realized due to unrecognized disease and delayed implementation of appropriate interventions for both diagnosed and undiagnosed individuals. The COllaboratioN on QUality improvement initiative for achieving Excellence in STandards of COPD care (CONQUEST) is the first-of-its-kind, collaborative, interventional COPD registry. It comprises an integrated quality improvement program focusing on patients (diagnosed and undiagnosed) at a modifiable and higher risk of COPD exacerbations. The first step in CONQUEST was the development of quality standards (QS). The QS will be imbedded in routine primary and secondary care, and are designed to drive patient-centered, targeted, risk-based assessment and management optimization. Our aim is to provide an overview of the CONQUEST QS, including how they were developed, as well as the rationale for, and evidence to support, their inclusion in healthcare systems.Methods: The QS were developed (between November 2019 and December 2020) by the CONQUEST Global Steering Committee, including 11 internationally recognized experts with a specialty and research focus in COPD. The process included an extensive literature review, generation of QS draft wording, three iterative rounds of review, and consensus.Results: Four QS were developed: 1) identification of COPD target population, 2) assessment of disease and quantification of future risk, 3) non-pharmacological and pharmacological intervention, and 4) appropriate follow-up. Each QS is followed by a rationale statement and a summary of current guidelines and research evidence relating to the standard and its components.Conclusion: The CONQUEST QS represent an important step in our aim to improve care for patients with COPD in primary and secondary care. They will help to transform the patient journey, by encouraging early intervention to identify, assess, optimally manage and followup COPD patients with modifiable high risk of future exacerbations.Keywords: identification, assessment, intervention, follow-up

Background: Allergy, eosinophilic inflammation, and epithelial dysregulation are implicated in severe asthma pathogenesis. Objective: We characterized biomarker expression in adults with severe asthma. Methods: Within the International Severe Asthma Registry (ISAR), we analyzed data from 10 countries in North America, Europe, and Asia, with prespecified thresholds for biomarker positivity (serum IgE ≥ 75 kU/L, blood eosinophils ≥ 300 cells/μL, and FeNO ≥ 25 ppb), and with hierarchical cluster analysis using biomarkers as continuous variables. Results: Of 1,175 patients; 64% were female, age (mean ± SD) 53 ± 15 years, body mass index (BMI) 30 ± 8, postbronchodilator forced expiratory volume in 1 second (FEV1) predicted 72% ± 20%. By prespecified thresholds, 59% were IgE positive, 57% eosinophil positive, and 58% FeNO positive. There was substantial inflammatory biomarker overlap; 59% were positive for either 2 or 3 biomarkers. Five distinct clusters were identified: cluster 1 (61%, low-to-medium biomarkers) comprised highly symptomatic, older females with elevated BMI and frequent exacerbations; cluster 2 (18%, elevated eosinophils and FeNO) older females with lower BMI and frequent exacerbations; cluster 3 (14%, extremely high FeNO) older, highly symptomatic, lower BMI, and preserved lung function; cluster 4 (6%, extremely high IgE) younger, long duration of asthma, elevated BMI, and poor lung function; cluster 5 (1.2%, extremely high eosinophils) younger males with low BMI, poor lung function, and high burden of sinonasal disease and polyposis. Conclusions: There is significant overlap of biomarker positivity in severe asthma. Distinct clusters according to biomarker expression exhibit unique clinical characteristics, suggesting the occurrence of discrete patterns of underlying inflammatory pathway activation and providing pathogenic insights relevant to the era of monoclonal biologics.

RationaleProgressive lung function (LF) decline in patients with asthma contributes to worse outcomes. Asthma exacerbations are thought to contribute to this decline; however, evidence is limited with mixed results.MethodsThis historical cohort study of a broad asthma patient population in the Optimum Patient Care Research Database, examined asthma patients with 3+eligible post-18th birthday peak expiratory flow rate (PEF) records (primary analysis) or records of forced expiratory flow in 1 s (FEV1) (sensitivity analysis). Adjusted linear growth models tested the association between mean annual exacerbation rate (AER) and LF trajectory.ResultsWe studied 1 09 182 patients with follow-up ranging from 5 to 50 years, of which 75 280 had data for all variables included in the adjusted analyses. For each additional exacerbation, an estimated additional −1.34 L/min PEF per year (95% CI −1.23 to –1.50) were lost. Patients with AERs >2/year and aged 18–24 years at baseline lost an additional −5.95 L/min PEF/year (95% CI −8.63 to –3.28) compared with those with AER 0. These differences in the rate of LF decline between AER groups became progressively smaller as age at baseline increased. The results using FEV1 were consistent with the above.ConclusionTo our knowledge, this study is the largest nationwide cohort of its kind and demonstrates that asthma exacerbations are associated with faster LF decline. This was more prominent in younger patients but was evident in older patients when it was related to lower starting LF, suggesting a persistent deteriorating phenotype that develops in adulthood over time. Earlier intervention with appropriate management in younger patients with asthma could be of value to prevent excessive LF decline.
