To determine the feasibility of withdrawing canakinumab (CAN) in a large cohort of paediatric patients with colchicine-resistant familial Mediterranean fever (crFMF).This retrospective observational cohort study included paediatric crFMF patients that received CAN treatment for ≥6 months. Patient data were recorded at treatment onset (baseline), and at 1, 3, 6, 12, 18, and 24 months after initiation of treatment.The study included 114 patients that were followed-up for 2736 person-months. During the 24-month follow-up period, the CAN dose interval remained unchanged in 44 patients. The dose interval was extended in 58 patients within a median 6 months (range: 3-18 months) of treatment initiation. In all, 4 of these 58 patients had a new attack of crFMF after the dose interval was extended. CAN was withdrawn in 12 patients (in 5 at month 12 month and in 7 at month 18), of which 2 had a new attack within 3 months of withdrawal. In these 2 patients CAN was re-initiated with a dose interval of 8 weeks. The remaining 10 patients in which CAN was withdrawn did not report any symptoms throughout the remainder of the 24-month follow-up period. The median attack-free period in those treated with CAN was 669 d (95% CI: 644-696).The present findings show that it may be feasible to withdraw CAN or extend its dose interval in paediatric crFMF patients. Based on the present findings, we think that as the quantity of real-life data increases, standard CAN protocols may be developed.

Einleitung: Das Management von Tumorblutungen im Kopf-Halsbereich stellt für HNO-Ärzte eine Herausforderung dar. Häufig handelt es sich um Patienten mit Tumorrezidiven, infauster Prognose und reduziertem Allgemeinzustand. Dabei kann die interventionelle Tumorembolisation eine alternative Therapieoption zu rezidivierenden operativen Blutstillungen sein. Methodik: Es wurden 3 Patienten mit ausgedehnten Kopf Hals Tumoren im Zeitraum von 2013 bis 2014 aufgrund rezidivierender Tumorblutungen bei infauster Prognose embolisiert und retrospektiv untersucht. In allen Fällen waren die chirurgischen Interventionsmöglichkeiten ausgeschöpft. Es konnte gezeigt werden, dass in der Mehrheit der Patienten eine temporäre Blutstillung mittels Interventioneller Tumorembolisation möglich ist. In einem Fall war im weiteren Verlauf jedoch eine erneute Embolisation nach initial regredientem Tumor aufgrund der veränderten vaskulären Tumorversorgung keine therapeutische Option mehr. Ergebnisse: Die Tumorembolisation stellt eine sichere Methode zur Kontrolle von rezidivierenden Tumorblutungen von Patienten mit infauster Prognose dar und trägt damit deutlich zur Erhaltung der Lebensqualität in der Palliativsituation bei. In this study the sounds, the smallest unit of oral communication, are analyzed. In the study, children’s ability of Turkish consonants is analyzed living in Muş city center at the ages of between 7 age and 6 months and 10 age and 6 months. The answers of the children are saved in the answer keys prepared for them and the analysis of collected data is actualized with the help of SPSS 15 program (Statistical Package for the Social Sciences). In the evaluation of the data descriptive statistics methods are benefited, answers given to questions are transferred to table as frequency and percentage. After analysis it is determined that age and gender is significant variables in pronunciation. It is observed that the ability of girls’ and boys’ pronouncing changes depending on the age. It is determined that all the ages of children participated in the study, pronounced the consonants differently or mispronounced depending on the position. It is concluded that in most of the age group, some sounds are pronounced accurately and successfully. © 2017, Ankara University. All rights reserved.

Objective: Clothes are very functional on the effectiveness of the persons. It is not well investigated that the effect of the physician's white coat to their patient. Our aim was to find out the contemplating and attitude of peoples about the physician's white coat in Aydın City of Turkey. Method: We reached 443 people by a categorized and randomized sampling method at the hospitals and drugstores in Aydın City and around. We offered them to fill out a specific questioner. They would not be written their name on this questioner but all of them signed that form. Results: All people we reached are participated in our study. Their age were between the 15-87 (mean 37, 27±13, 17) years and they was 250 women (56,4%) and 193 (43,6%) men. While the 67,1% of them declared that they not effected by the white coat of their physicians, 49,2% of them declared that the white coat of their physicians were not important. In spite of this findings 65,4% of participants said “yes”, 25% of participants said “it's not matter” and 9,6% of them said “not” as an answer to our last question “must your physician wear white coat or not”. These findings were not different between the sex groups but we saw that the psychological effects of the white coat were increasing parallel with the educational level of the person. Conclusion: According to our results and the related literature, it is concluded that to conduct more detailed studies on this topic are needed. Hekimlerin beyaz önlük giymelerinin hastalarını nasıl etkilediği ülkemizde pek araştırılmamış bir konudur. Bu araştırmamızda, Aydın İli'ndeki hasta ve yakınlarının hekimlerin beyaz önlük giymesi konusundaki tutum ve davranışlarını ortaya çıkarmak amaçlanmıştır. Yöntem: Aydın İli'nde merkez ve çevre yerleşim birimlerindeki hastanelerle eczanelere gelen 443 kişiye tabakalanmış rastlantısal yöntemle ulaşıldı. Kendilerinden araştırmamızla ilgili tarafımızdan hazırlanmış bir sormaca belgesini doldurup imzalamaları istendi. İsim yazma zorunlu tutulmadı. Bulgular: Ulaşılan herkes araştırmaya katıldı. Bu 443 kişi 15-87 yaş arası, yaş ortalaması 37,27±13,17 olan, 250 bayan (%56,4) ve 193 erkekten (%43,6) oluşmuştur. Katılımcıların %67,1'i hekimlerinin beyaz önlük giymesinin ilişkilerini etkilemediğini söylerken %49,2'si hekimin beyaz önlük giymesinin bir şey farkettirmeyeceğini bildirdi. Buna rağmen “Doktorunuz beyaz önlük giymeli mi?” sorusuna, katılımcıların %65,4'ü giymeli diye yanıt verirken %25.0 farketmez ve %9,6'sının giymemeli diye yanıt verdiği görüldü. Bu konularda cinsler arasında bir düşünce farkı saptanmazken eğitim-öğretim düzeyi arttıkça beyaz önlükten etkilenirliğin arttığı gözlendi. Sonuç: Sonuçlarımız ve tıbbi yazıncadaki diğer bulgular uyarınca klinisyenlerin beyaz önlük giymelerinin etkileri konusunda daha ileri araştırmaların yapılması gerektiği kanısındayız. . Regional meteorological anomalies induce different types of mass movements. In May 1998, the Western Black Sea Region of Turkey has experienced such a type of meteorological anomaly. Numerous residential and agricultural areas and, engineering lifelines were buried under the floodwaters. Besides the reactivation of many previously delineated landslides, thousands of smallscale landslides mostly in earthflow type occurred in all over the region. The earthflows were mainly developed in flysch type units, which have already presented high landslide concentrations. In this study, three different catchments namely Agustu, Egerci and Kelemen were selected because these subcatchments have the most landslide prone geological units of the region. The targets of this proceeding are to describe the spatial distributions and statistical assessments of the shallow earthflows triggered. The unique condition units (UCU) were employed during the statistical analyses. The numbers of the unique condition units are 4052 for the Agustu subcatchment, 13241 for the Egerci subcatchment and 12314 for the Kelemen subcatchment. The earthflow intensity is the highest in the Agustu subcatchment (0.038 flow/UCU) while this value is the lowest for the Egerci subcatchment (0.0035 flow/UCU).

Islahiye catchment, covering an area of 37.7 km P2 in the eastern Black Sea region, was heavily suffered from earthflows triggered by heavy rainfall in 1995 and 2002 (on July 23). Earthflows caused 17 causalities, four houses were demolished and some engineering lifeline structures were damaged in the catchment. Total 121 earthflows were recorded in the area. The weathering of the rock units, existence of steep slopes and human interaction can be evaluated as the main factors conditioning of earthflows. The main triggering factor of earthflows in the region is rainfall either intensive or long lasting. The majority of the earthflows were occurred in the weathered zones, which is mainly composed of inorganic and organic silts and silty clays of low plasticity. The slopes having more than 30 degrees constitute approximately 50 % of the entire catchment which varies between 1.5 and 4.0 km in width and 13 km in length. The minimum and maximum elevation values are 150 m and 1970 m, respectively. In order to identify other potential earthflow prone areas in the catchment, earthflow susceptibility assessments were performed using logistic regression analyses. The available raster data such as slope, aspect, curvature and elevation data were produced from digital elevation model (DEM) in GIS environment and were employed during the analyses. Two different mapping units namely pixel and unique condition units were used for performing logistic regression analyses. In addition, a comparison of performance of the earthflow susceptibility maps of Islahiye catchment was carried out. It can be said that the susceptibility map prepared based on unique condition units exhibits slightly better performance than that of pixel unit.

Objective: To evaluate the prevalence of asymptomatic cholelithiasis in type-II diabetes. Method: Abdominal ultrasonography was performed on 138 patients with type-II diabetes (100 women and 38 men) over 35 years old 270 people (190 women and 80 men) over 35 years old were examined as the control group. Results: 17 of the patients (12.3 %) and 28 controls (10.3 %) had cholelithiasis. These rates weren’t statisti-cally different (p>0.05). 13 % of female patients and 10.5 % of men patients were found that they had cholelithiasis. These rates were 11 % and 8.75 % in the control group . There was no statistically significant difference between these groups either (p > 0.05). Conclusion: The prevalance of asymptomatic cholelithiasis in type-II diabetes was not different than that of healthy persons.

Several studies demonstrated that certain fatty acids have specific effects on tumor cells. n-3 series fatty acids (alpha-linolenic acid, eicosapentaenoic acid and docosahexaenoic acid) may suppress the carcinogenesis, whereas n-6 series fatty acids (arachidonic acid, linoleic acid) may exert tumor promoting effects. In this study, 19 patients with various brain tumors and 12 control brain tissues were studied. n-3, n-6, n-9 unsaturated fatty acids and certain saturated fatty acids levels were measured in the plasma membrane of tumor or control brain tissues by capillary gas chromatography. We found that the level of docosahexaenoic acid from n-3 series fatty acids was significantly lower in gliomas and meningiomas than controls (p = 0.000). Total n-3 fatty acids level was also significantly lower in tumors than controls (p = 0.000). The levels of linoleic acid, arachidonic acid and dihomogamma linolenic acid from n-6 series were significantly higher in gliomas and meningiomas compared with controls (p = 0.000). Total n-6 fatty acids level was also significantly higher in tumors than controls (p = 0.000). Furthermore, in total n-9 fatty acids, total unsaturated fatty acids and total saturated fatty acids levels, there were no significant differences in gliomas and meningiomas compared with controls (p = 0.6840, p = 0.4388 and p = 0.4343, respectively). This findings suggest that n-6 fatty acids can act as a tumor-promoting agent in human brain tumors.

In this study, tissue fibronectin levels have been assayed in human prostatic cancer, benign prostatic hyperplasia and normal prostatic tissue. The mean tissue fibronectin levels for prostatic cancer, benign prostatic hyperplasia and normal groups were found to be 20.22 +/- 8.66 micrograms/mg protein and 11.77 +/- 6.74 micrograms/mg protein, respectively. In the malignant group, the mean fibronectin concentrations, appeared to be significantly higher than normal, (p0.05). On the other hand, fibronectin levels of benign prostatic hyperplasia were found to be statistically insignificant in comparison to the normal group (p0.05).
