1. Towards a Unified User Interface for Visual Analysis of Retinal Data in Ophthalmology

Abstract

The visual analysis of retinal data contributes to the understanding of a wide range of eye diseases. For the evaluation of cross-sectional studies, ophthalmologists rely on workflows and toolsets established in their work environment. That is, they know what tools and data are needed at each step of their workflow. Yet, manually operating the various tools, including activation, data handling, or view arrangement, can be cumbersome and time-consuming. We thus introduce a new visualization-supported toolchaining approach that combines workflow, tools, and data. First, we provide access to the tools required for each step of the workflow. Second, we handle the exchange of data between these tools. Third, we organize the views of the tools on screen using suitable layouts. Fourth, we visualize the connection between workflow, tools, and data to support the data analysis. We demonstrate our approach with a use case in ophthalmic research and report on initial feedback from experts., Comment: 12 pages, 4 figures

Abstract

Advances in optical coherence tomography (OCT) have enabled noninvasive imaging of substructures of the human retina with high spatial resolution. OCT examinations are now a standard procedure in clinics and an integral part of ophthalmic research. The interpretation of the OCT helps ophthalmologists understand the impact of various retinal and systemic diseases on the structure of the retina in a way not previously possible. In the early stages of retinal diseases, however, the identification and analysis of small and localized substructural changes in the retina remains a challenge. We present an overview of novel visual analytics approaches for the interactive exploration of early retinal changes in single and multiple patients, the comparison of the changes with normative data, and automated quantification and measurement of diagnosis-relevant information. We developed these approaches in close collaboration with ophthalmology researchers and industry experts from a leading OCT device manufacturer. As a result, they not only significantly reduced the time and effort required for OCT data analysis, especially in the context of cross-sectional studies, but have also led to several new discoveries published in biomedical journals., Comment: 5 pages, 5 figures

Abstract

Transfer learning is a powerful tool to adapt trained neural networks to new tasks. Depending on the similarity of the original task to the new task, the selection of the cut-off layer is critical. For medical applications like tissue classification, the last layers of an object classification network might not be optimal. We found that on real data of human corneal tissues the best feature representation can be found in the middle layers of the Inception-v3 and in the rear layers of the VGG-19 architecture., Comment: Extendend Abstract with 3 pages and 2 figures. Abstract

The densely innervated human cornea is the only superficial tissue of the human body in which nerve fibres are accessible in vivo by corneal confocal microscopy (CCM). Morphological parameters of the corneal sub-basal nerve plexus (SNP) derived from CCM images can potentially serve as a sensitive biomarker for early diagnosis of various neurodegenerative diseases. The evaluation of a single image with a typical field of view of 0.16 mm2 is insufficient for robust morphometric assessment. Mosaicking approaches have therefore been proposed to examine the SNP on a larger scale. Here we present a highly automated technique that significantly facilitates the generation of mosaic images of the SNP and is suitable for clinical tests.

ZusammenfassungEine ausreichende Vergrößerung und die richtige Beleuchtung sind entscheidend für den Erfolg einer mikrochirurgischen Intervention. Operationsmikroskope sind daher aus der modernen Ophthalmochirurgie schon lange nicht mehr wegzudenken und haben sich zum Kernstück eines modernen Operationssaals entwickelt. Wir geben in diesem Beitrag zunächst eine kurze Übersicht über die historische Entwicklung ophthalmologischer Operationsmikroskope – von den ersten Entwicklungen der Mitte des 19. Jahrhunderts bis hin zum derzeitigen Stand der Technik mit leistungsfähigen Koaxialbeleuchtungen und glasfasergeführten Xenon- oder LED-Lichtquellen. Weiterhin gehen wir auf aktuelle Entwicklungen, insbesondere im Bereich der Workflowunterstützung und Integration zusätzlicher Technologien, wie intraoperative OCT oder „Augmented Reality“ bzw. dem Operateur nützliche Dateneinspiegelungen, ein. Im letzten Teil wird ein Ausblick auf künftige Entwicklungen gegeben, wobei hier vor allem die Digitalisierung der Bildkette und intelligente Assistenzautomatisierung im Vordergrund stehen.

Abstract

The in vitro and in vivo characterization of intravitreal injections plays an important role in developing innovative therapy approaches. Using the established vitreous model (VM) and eye movement system (EyeMoS) the distribution of contrast agents with different molecular weight was studied in vitro. The impact of the simulated age-related vitreal liquefaction (VL) on drug distribution in VM was examined either with injection through the gel phase or through the liquid phase. For comparison the distribution was studied ex vivo in the porcine vitreous. The studies were performed in a magnetic resonance (MR) scanner. As expected, with increasing molecular weight the diffusion velocity and the visual distribution of the injected substances decreased. Similar drug distribution was observed in VM and in porcine eye. VL causes enhanced convective flow and faster distribution in VM. Confirming the importance of the injection technique in progress of VL, injection through gelatinous phase caused faster distribution into peripheral regions of the VM than following injection through liquefied phase. VM and MR scanner in combination present a new approach for the in vitro characterization of drug release and distribution of intravitreal dosage forms.

Abstract

MR microscopy (MRM) is known as ultra‐high‐field (UHF) magnetic resonance imaging with an in‐plane spatial resolution of <100 μm, yields highly resolved non‐invasive anatomical imaging and allows longitudinal assessment of embryonic avian development. The aim of the present study was to evaluate the feasibility of in vivo anatomical MRI assessment of the developing upper extremity of the chicken. Thirty‐eight fertilized chicken eggs were examined at 7 Tesla acquiring high‐resolution T2‐weighted images with an in‐plane resolution of 74 × 74 μm. To reduce motion artefacts, the eggs were moderately cooled before and during MRI. Development of the upper extremity was anatomically and quantitatively assessed. Chondrification and ossification on MRI were correlated with histological examination. MRM allowed the identification of the embryo from stage D5 onwards. First chondrification of the upper extremity was visible at stage D7, and the differentiation of the forearm was possible from stage D9 throughout the developmental period with excellent correlation to histology. MRM also allowed the differentiation between cortical and medullary bone as well as the detection of chondrified areas. UHF MRM allows the in vivo and in ovo evaluation of the upper limb during embryonic development and provides non‐invasive longitudinal anatomical information. This technique allows longitudinal studies of the same embryo during the developmental period and may therefore provide further insights into the development of the upper extremity. With improved coil technique and increasing availability of UHF MR systems, there is great potential regarding several research topics in experimental musculoskeletal radiology. Abstract

In vivo large-area confocal laser scanning microscopy (CLSM) of the human eye using EyeGuidance technology allows a large-scale morphometric assessment of the corneal subbasal nerve plexus (SNP). Here, the SNP of a patient suffering from diabetes and associated late complications was analyzed. The SNP contained multiple clusters of large hyperintense, stellate-shaped, cellular-like structures. Comparable structures were not observed in control corneas from healthy volunteers. Two hypotheses regarding the origin of these atypical structures are proposed. First, these structures might be keratocyte-derived myofibroblasts that entered the epithelium from the underlying stroma through breaks in Bowman���s layer. Second, these structures could be proliferating Schwann cells that entered the epithelium in association with subbasal nerves. The nature and pathophysiological significance of these atypical cellular structures, and whether they are a direct consequence of the patient���s diabetic neuropathy/or a non-specific secondary effect of associated inflammatory processes, are unknown.
