Purposes: Pneumococcal pneumonia is the most common etiology for community-acquired pneumonia. The prevalence of lymphadenopathy seen on CT scans in these patients is uncertain. The purpose of this series was to assess the prevalence of intrathoracic lymphadenopathy in hospitalized patients with pneumococcal pneumonia. Materials and Methods: We retrospectively identified 35 hospitalized patients with pneumococcal pneumonia who had been evaluated with CT scanning between January 1998 and April 2002. There were 18 men and 17 women with a mean age of 56 years. The study inclusion criteria were as follows: blood culture positive for Streptococcus pneumoniae, clinical diagnosis of pneumonia, and chest CT scan. The study exclusion criteria were known causes of lymphadenopathy. Charts were reviewed for HIV status, smoking history, and comorbidities. CT scans were reviewed for the presence and degree of lymphadenopathy, and the characteristics of pneumonia. The number of enlarged lymph nodes was graded as few (one to two), moderate (three to five), or many (six or more). Pneumonias were described by location, the number of involved lobes, the presence of cavitation, and the presence of pleural effusion. Patients with different comorbidities and different imaging characteristics of pneumonia were compared. Results: Among the 35 patients with pneumococcal pneumonia, intrathoracic lymphadenopathy was present on CT scans in 54% of patients (19 of 35 patients). The lymphadenopathy was ipsilateral to the pneumonia in 100% of patients (19 of 19 patients). One patient also had contralateral lymphadenopathy. The lymphadenopathy was graded as few in 37% of patients (7 of 19 patients), moderate in 37% of patients (7 of 19 patients), and many in 26% of patients (5 of 19 patients). The pneumonia was right-sided in 26% of patients (9 of 35 patients), left-sided in 17% of patients (6 of 35 patients), and bilateral in 57% of patients (20 of 35 patients). Comorbidities included the following: HIV infection (n = 15); smoking (n = 21); emphysema (n = 5); hepatitis C (n = 5); and diabetes (n = 3). The characteristics of pneumonia included the following: multilobar (n = 25); pleural effusion (n = 25); and cavitation (n = 5). The prevalence of lymphadenopathy according to comorbidities and characteristics of pneumonia ranged from 40 to 100%. None of the differences in prevalence of lymphadenopathy among the subgroups was statistically significant. Conclusion: Lymphadenopathy is a common CT scan feature of pneumococcal pneumonia in hospitalized patients, in a variety of settings. Therefore, when a patient with pneumococcal pneumonia has lymphadenopathy seen on a CT scan, other etiologies for the lymphadenopathy need not be suspected. [ABSTRACT FROM AUTHOR]

We develop a conceptual framework for studying collective adaptation in complex socio-cognitive systems, driven by dynamic interactions of social integration strategies, social environments and problem structures. Going beyond searching for 'intelligent' collectives, we integrate research from different disciplines and outline modelling approaches that can be used to begin answering questions such as why collectives sometimes fail to reach seemingly obvious solutions, how they change their strategies and network structures in response to different problems and how we can anticipate and perhaps change future harmful societal trajectories. We discuss the importance of considering path dependence, lack of optimization and collective myopia to understand the sometimes counterintuitive outcomes of collective adaptation. We call for a transdisciplinary, quantitative and societally useful social science that can help us to understand our rapidly changing and ever more complex societies, avoid collective disasters and reach the full potential of our ability to organize in adaptive collectives.

We consider short-range Ising spin glasses in equilibrium at infinite system size, and prove that, for fixed bond realization and a given Gibbs state drawn from a suitable metastate, each translation and locally invariant function (for example, self-overlaps) of a single pure state in the decomposition of the Gibbs state takes the same value for all the pure states in that Gibbs state. We describe several significant applications to spin glasses.

We propose an approach toward understanding the spin glass phase at zero and low temperature by studying the stability of a spin glass ground state against perturbations of a single coupling. After reviewing the concepts of flexibility, critical droplet, and related quantities for both finite- and infinite-volume ground states, we study some of their properties and review three models in which these quantities are partially or fully understood. We also review a recent result showing the connection between our approach and that of disorder chaos. We then view four proposed scenarios for the low-temperature spin glass phase-replica symmetry breaking, scaling-droplet, TNT, and chaotic pairs-through the lens of the predictions of each scenario for the lowest-energy large-lengthscale excitations above the ground state. Using a new concept called σ-criticality, which quantifies the sensitivity of ground states to single-bond coupling variations, we show that each of these four pictures can be identified with different critical droplet geometries and energies. We also investigate necessary and sufficient conditions for the existence of multiple incongruent ground states.

The ability to 'sense' the social environment and thereby to understand the thoughts and actions of others allows humans to fit into their social worlds, communicate and cooperate, and learn from others' experiences. Here we argue that, through the lens of computational social science, this ability can be used to advance research into human sociality. When strategically selected to represent a specific population of interest, human social sensors can help to describe and predict societal trends. In addition, their reports of how they experience their social worlds can help to build models of social dynamics that are constrained by the empirical reality of human social systems.

Belief change and spread have been studied in many disciplines-from psychology, sociology, economics and philosophy, to biology, computer science and statistical physics-but we still do not have a firm grasp on why some beliefs change more easily and spread faster than others. To fully capture the complex social-cognitive system that gives rise to belief dynamics, we first review insights about structural components and processes of belief dynamics studied within different disciplines. We then outline a unifying quantitative framework that enables theoretical and empirical comparisons of different belief dynamic models. This framework uses a statistical physics formalism, grounded in cognitive and social theory, as well as empirical observations. We show how this framework can be used to integrate extant knowledge and develop a more comprehensive understanding of belief dynamics.

We study the problem of predictability, or "nature vs nurture," in several disordered Ising spin systems evolving at zero temperature from a random initial state: How much does the final state depend on the information contained in the initial state, and how much depends on the detailed history of the system? Our numerical studies of the "dynamical order parameter" in Edwards-Anderson Ising spin glasses and random ferromagnets indicate that the influence of the initial state decays as dimension increases. Similarly, this same order parameter for the Sherrington-Kirkpatrick infinite-range spin glass indicates that this information decays as the number of spins increases. Based on these results, we conjecture that the influence of the initial state on the final state decays to zero in finite-dimensional random-bond spin systems as dimension goes to infinity, regardless of the presence of frustration. We also study the rate at which spins "freeze out" to a final state as a function of dimensionality and number of spins; here the results indicate that the number of "active" spins at long times increases with dimension (for short-range systems) or number of spins (for infinite-range systems). We provide theoretical arguments to support these conjectures, and also study analytically several mean-field models: the random energy model, the uniform Curie-Weiss ferromagnet, and the disordered Curie-Weiss ferromagnet. We find that for these models, the information contained in the initial state does not decay in the thermodynamic limit-in fact, it fully determines the final state. Unlike in short-range models, the presence of frustration in mean-field models dramatically alters the dynamical behavior with respect to the issue of predictability.

We present a general geometrical approach to the problem of escape from a metastable state in the presence of noise. The accompanying analysis leads to a simple condition, based on the norm of the drift field, for determining whether caustic singularities alter the escape trajectories when detailed balance is absent. We apply our methods to systems lacking detailed balance, including a nanomagnet with a biaxial magnetic anisotropy and subject to a spin-transfer torque. The approach described within allows determination of the regions of experimental parameter space that admit caustics.

We present a technique to generate relations connecting pure state weights, overlaps, and correlation functions in short-range spin glasses. These are obtained directly from the unperturbed Hamiltonian and hold for general coupling distributions. All are satisfied in phases with simple thermodynamic structure, such as the droplet-scaling and chaotic pairs pictures. If instead nontrivial mixed-state pictures hold, the relations suggest that replica symmetry is broken as described by a Derrida-Ruelle cascade, with pure state weights distributed as a Poisson-Dirichlet process.

Protein distributions measured under a broad set of conditions in bacteria and yeast were shown to exhibit a common skewed shape, with variances depending quadratically on means. For bacteria these properties were reproduced by temporal measurements of protein content, showing accumulation and division across generations. Here we present a stochastic growth-and-division model with feedback which captures these observed properties. The limiting copy number distribution is calculated exactly, and a single parameter is found to determine the distribution shape and the variance-to-mean relation. Estimating this parameter from bacterial temporal data reproduces the measured distribution shape with high accuracy and leads to predictions for future experiments.

Paraliparis hawaiiensis n.sp. is described from the north-western Hawaiian Islands from two specimens collected at 2196 and 3055 m. It differs from other North Pacific Ocean species in its chin pore arrangement, tooth pattern and body proportions. Although liparid specimens have previously been collected from Hawaii, they were undescribed and are now lost. Therefore, this is the first liparid species described from the archipelago. Consider a dynamical many-body system with a random initial state subsequently evolving through stochastic dynamics. What is the relative importance of the initial state ("nature") versus the realization of the stochastic dynamics ("nurture") in predicting the final state? We examined this question for the two-dimensional Ising ferromagnet following an initial deep quench from T=∞ to T=0. We performed Monte Carlo studies on the overlap between "identical twins" raised in independent dynamical environments, up to size L=500. Our results suggest an overlap decaying with time as t(-θ)(h) with θ(h)=0.22 ± 0.02; the same exponent holds for a quench to low but nonzero temperature. This "heritability exponent" may equal the persistence exponent for the two-dimensional Ising ferromagnet, but the two differ more generally.

Objective: To describe the clinical course of a patient with a nonresectable pheochromocytoma during urgent hip surgery., Methods: To describe the clinical management and postoperative outcome of the patient and review the relevant literature., Results: An 85-year-old male with a nonresectable pheochromocytoma required urgent hip surgery following a traumatic hip fracture. He was perioperatively managed with phenoxybenzamine, metyrosine, and metoprolol to avoid potential pheochromocytoma-related complications. He remained hemodynamically stable and recovered from the surgery without complications., Conclusions: This case illustrates the successful management of a patient requiring urgent surgery in the setting of a nonresectable pheochromocytoma, which is rarely described in the literature.

Exit times for stochastic Ginzburg-Landau classical field theories with two or more coupled classical fields depend on the interval length on which the fields are defined, the potential in which the fields deterministically evolve, and the relative stiffness of the fields themselves. The latter is of particular importance in that physical applications will generally require different relative stiffnesses, but the effect of varying field stiffnesses has not heretofore been studied. In this paper, we explore the complete phase diagram of escape times as they depend on the various problem parameters. In addition to finding a transition in escape rates as the relative stiffness varies, we also observe a critical slowing down of the string method algorithm as criticality is approached.

The BD Phoenix (BD Diagnostics, Sparks, MD) and Vitek 2 (bioMérieux, Durham, NC) automated susceptibility testing systems have implemented the use of cefoxitin to enhance the detection of methicillin (meticillin)-resistant Staphylococcus aureus (MRSA). To assess the impact of this change, 620 clinically significant S. aureus isolates were tested in parallel on Phoenix PMIC/ID-102 panels and Vitek 2 AST-GP66 cards. The results for oxacillin and cefoxitin generated by the automated systems were compared to those generated by two reference methods: mecA gene detection and MICs of oxacillin previously determined by broth microdilution according to CLSI guidelines. Testing of isolates with discordant results was repeated to attain a majority or consensus final result. There was 100% final agreement between the results of the two reference methods. For the 448 MRSA and 172 methicillin-susceptible S. aureus isolates tested, the rates of categorical agreement of the results obtained with the automated systems with those obtained by the reference methods were 99.8% for the Phoenix panels and 99.7% for the Vitek 2 cards. A single very major error occurred on each instrument (0.2%) with different MRSA isolates. The only major error was attributed to the Vitek 2 system overcalling oxacillin resistance. In 16 instances (9 on the Phoenix system, 7 on the Vitek 2 system), an oxacillin MIC in the susceptible range was correctly changed to resistant by the expert system on the basis of the cefoxitin result. The inclusion of cefoxitin in the Phoenix and Vitek 2 panels has optimized the detection of MRSA by both systems.

We report on the experimental observation of the phase angle of a particle escaping over a periodically modulated potential barrier. Optical tweezers and back-focal plane position detection were used to record particle trajectories in the entire double-well potential. These measurements provide a sensitive test of theories proposed in the past decade of escape driven by random thermal noise from a periodically modulated potential. The observed phase shifts as a function of modulation frequency are consistent with those calculated using existing theories.

A spatially extended classical system with metastable states subject to weak spatiotemporal noise can exhibit a transition in its activation behavior when one or more external parameters are varied. Depending on the potential, the transition can be first or second order, but there exists no systematic theory of the relation between the order of the transition and the shape of the potential barrier. In this paper, we address that question in detail for a general class of systems whose order parameter is describable by a classical field that can vary in both space and time, and whose zero-noise dynamics are governed by a smooth polynomial potential. We show that a quartic potential barrier can have only second-order transitions, confirming an earlier conjecture [D. L. Stein, J. Stat. Phys. 114, 1537 (2004)]. We then derive, through a combination of analytical and numerical arguments, both necessary and sufficient conditions to have a first-order vs a second-order transition in noise-induced activation behavior, for a large class of systems with smooth polynomial potentials of arbitrary order. We find in particular that the order of the transition is especially sensitive to the potential behavior near the top of the barrier.

Lymphangitic spread of metastases has distinct patterns on pulmonary scintigraphy and radiographic imaging. A 43-year-old woman with metastatic breast cancer presented with shortness of breath. Symptomatology was studied with ventilation and perfusion (V/Q) scintigraphy, chest radiograph, and computed tomography (CT). The scintigraphic pattern is consistent with prior descriptions of lymphangitic spread of tumor. The chest x-ray and CT were both performed within the prior 2 months and were confirmatory.

Thermally induced conductance jumps of metal nanowires are modeled using stochastic Ginzburg-Landau field theories. Changes in radius are predicted to occur via the nucleation of surface kinks at the wire ends, consistent with recent electron microscopy studies. The activation rate displays nontrivial dependence on nanowire length, and undergoes first- or second-order-like transitions as a function of length. The activation barriers of the most stable structures are predicted to be universal, i.e., independent of the radius of the wire, and proportional to the square root of the surface tension. The reduction of the activation barrier under strain is also determined.

We study chaotic size dependence of the low-temperature correlations in the Sherrington-Kirkpatrick (SK) spin glass. We prove that as temperature scales to zero with volume, for any typical coupling realization, the correlations cycle through every spin configuration in every fixed observation window. This cannot happen in short-ranged models as there it would mean that every spin configuration is an infinite-volume ground state. Its occurrence in the SK model means that the commonly used "modified clustering" notion of states sheds little light on the replica symmetry breaking (RSB) solution of SK, and, conversely, the RSB solution sheds little light on the thermodynamic structure of Edwards-Anderson models.

Klebsiella pneumoniae isolates from Taiwan medical centers (50 strains; 1998 to 2000) with a CTX-M resistance phenotype (ceftazidime susceptible and ceftriaxone or cefotaxime nonsusceptible) were selected for initial isoelectric focusing analysis. beta-Lactamases with pIs of 7.9 (n = 22) and 8.4 (n = 28) in addition to 5.4 and/or 7.6 were detected. DNA gene sequencing identified the beta-lactamases with pIs of 7.9 and 8.4 as CTX-M-14 and CTX-M-3, respectively. Molecular typing suggested inter- and intrahospital clonal dissemination of these Taiwanese CTX-M-producing Klebsiella strains.

We study a spatially extended model of noise-induced magnetization reversal: a classical Ginzburg-Landau model, restricted to a bounded interval and perturbed by weak spatiotemporal noise. By adapting the Coleman-Langer approach to false vacuum decay, we determine the dependence of the activation barrier and Kramers rate prefactor on the interval length. As it increases, a transition between activation regimes occurs, at which the prefactor diverges. Similar transitions between activation regimes should occur in many other bistable, spatially extended classical models.

We derive exact expressions for a number of aging functions that are scaling limits of nonequilibrium correlations, R(t(w),t(w)+t) as t(w)-->infinity, t/t(w)-->theta, in the 1D homogenous q-state Potts model for all q with T = 0 dynamics following a quench from T = infinity. One such quantity is (0)(t(w));sigma-->(n)(t(w)+t)> when n/square root of ([t(w))-->z. Exact, closed-form expressions are also obtained when an interlude of T = infinity dynamics occurs. Our derivations express the scaling limit via coalescing Brownian paths and a "Brownian space-time spanning tree," which also yields other aging functions, such as the persistence probability of no spin flip at 0 between t(w) and t(w)+t.

We present a general theorem restricting properties of interfaces between thermodynamic states and apply it to the spin glass excitations observed numerically by Krzakala and Martin and separately by Palassini and Young in spatial dimensions d = 3,4. We show that such excitations, with interface dimension d(s) < d, cannot yield regionally congruent thermodynamic states. More generally, zero density interfaces of translation-covariant excitations cannot be pinned (by the disorder) in any d but rather must deflect to infinity in the thermodynamic limit. Additional consequences concerning regional congruence in spin glasses and other systems are discussed.

We treat the noise-activated escape from a one-dimensional potential well of an overdamped particle, to which a periodic force of fixed frequency is applied. Near the well top, the relevant length scales and the boundary layer structure are determined. We show how behavior near the well top generalizes the behavior determined by Kramers, in the case without forcing. Our analysis includes the case when the forcing does not die away in the weak-noise limit. We discuss the relevance of scaling regimes, defined by the relative strengths of the forcing and the noise, to recent optical trap experiments.

By connecting realistic spin glass models at low temperature to the highly disordered model at zero temperature, we argue that ordinary Edwards-Anderson spin glasses below eight dimensions have at most a single pair of physically relevant pure states at nonzero low temperature. Less likely scenarios that evade this conclusion are also discussed.

The pattern of activated trajectories in a symmetric double well system without detailed balance may contain cusps and other singularities, similar to the caustics of geometrical optics. We derive a scaling law and nonpolynomial "equations of state" that govern the bifurcation of an anomalous cusp (a cusp coinciding with the saddle) into conventional cusps. The bifurcation is reflected in the system quasipotential, much as a phase transition is reflected in the free energy of a thermodynamic system. The anomalous cusp is analogous to a nonclassical critical point. Besides showing how critical phenomena occur in noise-perturbed systems, our results extend classical catastrophe theory.

We rigorously rule out the appearance of multiple domain walls between ground states in 2D Edwards-Anderson Ising spin glasses (with periodic boundary conditions and, e.g., Gaussian couplings). This supports the conjecture that there is only a single pair of ground states in these models.

We study analytically M-spin-flip stable states in disordered short-ranged Ising models (spin glasses and ferromagnets) in all dimensions and for all M. Our approach is primarily dynamical, and is based on the convergence of sigma(t), a zero-temperature dynamical process with flips of lattice animals up to size M and starting from a deep quench, to a metastable limit sigma(infinity). The results (rigorous and nonrigorous, in infinite and finite volumes) concern many aspects of metastable states: their numbers, basins of attraction, energy densities, overlaps, remanent magnetizations, and relations to thermodynamic states. For example, we show that their overlap distribution is a delta function at zero. We also define a dynamics for M=infinity, which provides a potential tool for investigating ground state structure.

Capillary endothelial surface charge was investigated by perfusing the intestinal circulation of anesthetized rats in situ with 0.1 or 10 mg/ml native ferritin (NF, pI = 3.8-4.2) or with 0.1 mg/ml cationized ferritin (CF, pI greater than 10.0) for 5 min, with or without 5% Dextran 40. Ferritin binding was quantified by electron microscopy. All electron micrographs of capillaries perfused with NF showed some NF binding. Mean NF particle densities (particles/microns) were significantly greater at vesicle necks (PDv) than elsewhere on the endothelial surface (PD). Capillaries perfused with CF showed binding in only 60% of the transverse sections examined. The binding was very marked in a large proportion of these vessels. Mean CF particle densities were significantly greater at fenestrae (PDf) than elsewhere. These results demonstrate that mucosal capillaries have a variable negative electrostatic charge on the endothelial surface and support the hypothesis that some vesicle diaphragms act as preferential attractors for anionic macromolecules. Such structures could promote transendothelial vesicular transport of albumin. Dextran significantly decreased PD for NF from 25.4 +/- 2.4 (SEM) to 13.1 +/- 0.9 and PD, PDv, and PDf for CF from 45.9 +/- 4.6 to 31.0 +/- 2.7, from 62.1 +/- 6.2 to 43.6 +/- 5.4, and from 152.9 +/- 15.5 to 114.5 +/- 8.6, respectively. The above responses result from dextran's weak interaction with the endothelial surface. Dextran reduces access of ferritin molecules to the cell surface by steric hindrance and/or electrostatic shielding of the glycocalyx.

We discuss the generic time behavior of reaction-diffusion processes capable of modeling various types of biological transport processes, such as ligand migration in proteins and gating fluctuations in ion channel proteins. The main observable in these two cases, the fraction of unbound ligands and the probability of finding the channel in the closed state, respectively, exhibits an algebraic t-1/2 decay at intermediate times, followed by an exponential cutoff. We provide a simple framework for understanding these observations and explain their ubiquity by showing that these qualitative results are independent of space dimension. We also derive an experimental criterion to distinguish between a one-dimensional process and one whose effective dimension is higher.

We propose a mathematical model for the next stage in the origin of life after that treated in our earlier work. At this stage we introduce the possibility of the modification of the environment by the information-containing entities and feedback between the environment and the population of macromolecules and hence provide a model for the development of the Eigen hypercycle.

Computer simulations of a "spin glass" model for the origin of biological information are discussed. Selection is found to occur among a wide diversity of possible species, and in addition competition, adaptation, and hysteresis are all exhibited.
