Purpose Multigene panels are commercially available for the evaluation of prostate cancer (PCA) predisposition, which necessitates tailored genetic counseling (GC) for men. Here we describe emerging results of Genetic Evaluation of Men, prospective multigene testing study in PCA to inform personalized genetic counseling, with emerging implications for referrals, cancer screening, and precision therapy. Patients and Methods Eligibility criteria for men affected by or at high risk for PCA encompass age, race, family history (FH), and PCA stage/grade. Detailed demographic, clinical, and FH data were obtained from participants and medical records. Multigene testing was conducted after GC. Mutation rates were summarized by eligibility criteria and compared across FH data. The 95% CI of mutation prevalence was constructed by using Poisson distribution. Results Of 200 men enrolled, 62.5% had PCA. Eleven (5.5%; 95% CI, 3.0% to 9.9%) had mutations; 63.6% of mutations were in DNA repair genes. FH of breast cancer was significantly associated with mutation status ( P = .004), and FH that met criteria for hereditary breast and ovarian cancer syndrome was significantly associated with PCA (odds ratio, 2.33; 95% CI, 1.05 to 5.18). Variants of uncertain significance were reported in 70 men (35.0%). Among mutation carriers, 45.5% had personal/FH concordant with the gene. A tailored GC model was developed based on emerging findings. Conclusion Multigene testing for PCA identifies mutations mostly in DNA repair genes, with implications for precision therapy. The study highlights the importance of comprehensive genetic evaluation for PCA beyond metastatic disease, including early-stage disease with strong FH. Detailed FH is important for referrals of men for genetic evaluation. The results inform precision GC and cancer screening for men and their male and female blood relatives.

Purpose Multigene panels (MGPs) are increasingly being used despite questions regarding their clinical utility and no standard approach to genetic counseling. How frequently genetic providers use MGP testing and how patient-reported outcomes (PROs) differ from targeted testing (eg, BRCA1/2 only) are unknown. Methods We evaluated use of MGP testing and PROs in participants undergoing cancer genetic testing in the multicenter Communication of Genetic Test Results by Telephone study (ClinicalTrials.gov identifier: NCT01736345), a randomized study of telephone versus in-person disclosure of genetic test results. PROs included genetic knowledge, general and state anxiety, depression, cancer-specific distress, uncertainty, and satisfaction. Genetic providers offered targeted or MGP testing based on clinical assessment. Results Since the inclusion of MGP testing in 2014, 395 patients (66%) were offered MGP testing. MGP testing increased over time from 57% in 2014 to 66% in 2015 ( P = .02) and varied by site (46% to 78%; P < .01). Being offered MGP testing was significantly associated with not having Ashkenazi Jewish ancestry, having a history of cancer, not having a mutation in the family, not having made a treatment decision, and study site. After demographic adjustment, patients offered MGP testing had lower general anxiety ( P = .04), state anxiety ( P = .03), depression ( P = .04), and uncertainty ( P = .05) pre-disclosure compared with patients offered targeted testing. State anxiety ( P = .05) and cancer-specific distress ( P = .05) were lower at disclosure in the MGP group. There was a greater increase in change in uncertainty ( P = .04) among patients who underwent MGP testing. Conclusion MGP testing was more frequently offered to patients with lower anxiety, depression, and uncertainty and was associated with favorable outcomes, with the exception of a greater increase in uncertainty compared with patients who had targeted testing. Addressing uncertainty may be important as MGP testing is increasingly adopted.

We obtain that the $p+1$ non-isomorphic self-dual semisimple Hopf algebras of dimension $p^3$ classified by Masuoka have no non-trivial cocycle deformations, extending his previous results for the 8-dimensional Kac-Paljutkin Hopf algebra. Purpose To better understand the longitudinal risks and benefits of telephone disclosure of genetic test results in the era of multi-gene panel testing. Patients and Methods Adults who were proceeding with germline cancer genetic testing were randomized to telephone disclosure (TD) with a genetic counselor or in-person disclosure (IPD) of test results (i.e., usual care). All participants who received TD were recommended to return to meet with a physician to discuss medical management recommendations. Results 473 participants were randomized to TD and 497 to IPD. Assessing therapy response of breast cancer bone metastases is challenging. In retrospective studies, serial (18)F-FDG PET was predictive of time to skeletal-related events (tSRE) and time to progression (TTP). (18)F-NaF PET improves bone metastasis detection compared with bone scanning. We prospectively tested (18)F-FDG PET and (18)F-NaF PET to predict tSRE, TTP, and overall survival (OS) in patients with bone-dominant metastatic breast cancer (MBC). Methods: Patients with bone-dominant MBC were imaged with (18)F-FDG PET and (18)F-NaF PET before starting new therapy (scan1) and again at a range of times centered around approximately 4 mo later (scan2). Maximum standardized uptake value (SUV(max)) and lean body mass adjusted standardized uptake (SUL(peak)) were recorded for a single index lesion and up to 5 most dominant lesions for each scan. tSRE, TTP, and OS were assessed exclusive of the PET images. Univariate Cox regression was performed to test the association between clinical endpoints and (18)F-FDG PET and (18)F-NaF PET measures. mPERCIST (Modified PET Response Criteria in Solid Tumors) were also applied. Survival curves for mPERCIST compared response categories of complete response+partial response+stable disease versus progressive disease for tSRE, TTP, and OS. 10537 Background: LS is among the most common hereditary cancer (CA) syndromes. PVs in MSH6 are 2-4 fold more common in the population (1/758) than those in MLH1 (1/1946) or MSH2 (1/2841), and are increasingly regarded as lower penetrance for CRC due to published data supporting later mean age of CRC onset and lower CRC risk. Unlike for MLH1/MSH2, NCCN 2020 CA risk estimates recognize only endometrial CA (EC) and CRC risks in MSH6+ carriers as clearly above SEER population estimates. Further, risks of other LS manifestations such as skin disease/Muir-Torre, ovarian CA (OC), and possible rare tumors in LS like sarcoma, have been minimally characterized in MSH6+ carriers. Methods: Pedigree data for 44 MSH6+ index (first-evaluated family member by our program) pts consecutively ascertained since 2009 at Fox Chase (FCCC) were reviewed. 1 pt w/a rare MSH6 uncertain variant w/personal history (PHx) of MSH6-expression deficient EC (age 50) and MSH6-deficient sebaceous skin CA (age 50) and a strong family history (FHx) c/w LS is also included here. 34% (15/44) index pts were referred to FCCC for cascade testing due to a known MSH6 PV in the family. Of the remaining 29 index pts, ascertainment included: 14% w/positive universal LS tumor screening, 21% w/early-onset or synchronous LS CA, 14% w/multi-gene panel for PHx of OC, 10% w/incidental MSH6+ result (2 had testing for PHx breast CA, 1 tumor genomic profiling), and 28% w/PHx and/or FHx of LS CA warranting genetic testing. Age of CA onset and path data were verified in > 90% index pts. Results: Index pts had a mean age of 55.5 yrs, and 77% were female. Overall, 11% (5/44) of MSH6+ index pts were found to have LS at diagnosis of synchronous primary CAs (3 EC/OC, 1 CRC/CRC, 1 CRC/EC), and 4/5 of these occurred 2 metachronous LS CAs. OC was the presenting CA in 14% (6/44) female index pts; 2 additional index pts had rarer OC variants (Mullerian duct @ 41, primary peritoneal CA @ 50). Skin manifestations of LS were documented in 9.1% (4/44) index pts (3 sebaceous, 1 SCC in-situ/Bowen’s disease); 1 other family had documented sebaceous CAs in an FDR (father) but the 2 daughters seen @FCCC (both 30s) had yet to develop skin lesions. 2 index pts were found to have LS after developing early-onset breast CA (age 39) and contralateral breast CA (ages 50 and 54). Finally, 7% (3/44) index pts had a PHx of sarcoma: 2 were liposarcomas (ages 57 and 67), and 1 was a dermatofibrosarcoma. 2 other index pts had siblings w/childhood sarcomas. Conclusions: Our data, encompassing 44 MSH6+ pts evaluated in our clinic and consecutively ascertained, suggest MSH6 PV carriers develop synchronous primaries (11%), common and rare OC histologic types (18%), sarcomas (7%) and skin disease/Muir-Torre (9%). While common in the population and lower penetrance for CRC, MSH6 PV can behave in uncommon ways and may have significant extra-colonic CA risks such as OC, sarcoma and skin manifestations.

In collaboration with the American Board of Internal Medicine (ABIM) Foundation, ASCP would like to recognize several individual clinicians, as well as clinician teams, and their commitment to the ABIM Foundation's Choosing Wisely campaign. Since ASCP joined the Choosing Wisely campaign in 2013, many of its members have worked to advance the campaign's ideals, which intend to reduce waste in health care, in part by encouraging clinicians and patients to question which tests are really necessary, in order to decrease test overuse and improve the nation's healthcare system. “The Choosing Wisely Champions program recognizes clinicians for their contributions to the campaign, inspires clinicians seeking to implement Choosing Wisely in their own practices, and demonstrates how clinicians are driving change in health care,” says Lee H. BACKGROUND Guidelines for the management of type 2 diabetes universally recommend that adults with type 2 diabetes and obesity be offered individualized interventions to encourage weight loss. Yet despite the existing recommendations, provision of weight management services is currently patchy around the United Kingdom and where services are available, high attrition rates are often reported. In addition, individuals often fail to take up services, that is, after discussion with a general practitioner or practice nurse, individuals are referred to the service but do not attend for an appointment. Qualitative research has identified that the initial discussion raising the issue of weight, motivating the patient, and referring to services is crucial to a successful outcome from weight management. OBJECTIVE Our aim was to evaluate the effectiveness of an Internet-based training program and practice implementation toolkit with or without face-to-face training for primary care staff. The primary outcome is the change in referral rate of patients with type 2 diabetes to National Health Service adult weight management programs, 3 months pre- and postintervention. METHODS We used the Behavior Change Wheel to develop an intervention for staff in primary care consisting of a 1-hour Internet-based eLearning package covering the links between obesity, type 2 diabetes, and the benefits of weight management, the treatment of diabetes in patients with obesity, specific training in raising the issue of weight, local services and referral pathways, overview of weight management components/ evidence base, and the role of the referrer. The package also includes a patient pamphlet, a discussion tool, a practice implementation checklist, and an optional 2.5-hour face-to-face training session. We have randomly assigned 100 practices in a 1:1 ratio to either have immediate access to all the resources or have access delayed for 4 months. An intention-to-treat statistical analysis will be performed. RESULTS Recruitment to the study is now complete. We will finalize follow-up in 2018 and publish in early 2019. CONCLUSIONS This protocol describes the development and randomized evaluation of the effectiveness of an intervention to improve referral and uptake rates of weight management programs for adults with type 2 diabetes. At a time when many new dietary and pharmacological weight management interventions are showing large clinical benefits for people with type 2 diabetes, it is vital that primary care practitioners are willing, skilled, and able to discuss weight and make appropriate referrals to services. As African economic development improves, the prevalence of non‐communicable diseases (NCDs) will increase, with cancer at the forefront. NCDs such as cardiovascular disease, cancer, chronic respiratory diseases, and diabetes are often associated with the “globalization of unhealthy lifestyles" such as unhealthy diets, physical inactivity, exposure to tobacco smoke, or the effects of alcohol.1 Over the next five years, the annual number of new cancer cases is predicted to increase to 1 million.2 The economic costs associated with cancer—both immediate healthcare costs and loss of productivity—are threatening continued development and prosperity. Deaths from preventable cancers mean catastrophic health expenditures, lower household income, and potential impoverishment. Globally, the World Economic Forum projects that the NCD epidemic will inflict $21.3 trillion in losses in developing countries over the next two decades.3 Pathology is important for diagnosing and treating cancer, but the countries in Sub‐Saharan Africa lack pathologists and trained laboratory personnel, infrastructure, and adequate opportunities for professional training. Telephone disclosure of cancer genetic test results is noninferior to in-person disclosure. However, how patients who prefer in-person communication of results differ from those who agree to telephone disclosure is unclear but important when considering delivery models for genetic medicine. Patients undergoing cancer genetic testing were recruited to a multicenter, randomized, noninferiority trial (NCT01736345) comparing telephone to in-person disclosure of genetic test results. We evaluated preferences for in-person disclosure, factors associated with this preference and outcomes compared to those who agreed to randomization. Among 1178 enrolled patients, 208 (18%) declined randomization, largely given a preference for in-person disclosure. These patients were more likely to be older (P = 0.007) and to have had multigene panel testing (P < 0.001). General anxiety (P = 0.007), state anxiety (P = 0.008), depression (P = 0.011), cancer-specific distress (P = 0.021) and uncertainty (P = 0.03) were higher after pretest counseling. After disclosure of results, they also had higher general anxiety (P = 0.003), depression (P = 0.002) and cancer-specific distress (P = 0.043). While telephone disclosure is a reasonable alternative to in-person disclosure in most patients, some patients have a strong preference for in-person communication. Patient age, distress and complexity of testing are important factors to consider and requests for in-person disclosure should be honored when possible.

Multiplex genetic testing, including both moderate- and high-penetrance genes for cancer susceptibility, is associated with greater uncertainty than traditional testing, presenting challenges to informed consent and genetic counseling. We sought to develop a new model for informed consent and genetic counseling for four ongoing studies.Drawing from professional guidelines, literature, conceptual frameworks, and clinical experience, a multidisciplinary group developed a tiered-binned genetic counseling approach proposed to facilitate informed consent and improve outcomes of cancer susceptibility multiplex testing.In this model, tier 1 "indispensable" information is presented to all patients. More specific tier 2 information is provided to support variable informational needs among diverse patient populations. Clinically relevant information is "binned" into groups to minimize information overload, support informed decision making, and facilitate adaptive responses to testing. Seven essential elements of informed consent are provided to address the unique limitations, risks, and uncertainties of multiplex testing.A tiered-binned model for informed consent and genetic counseling has the potential to address the challenges of multiplex testing for cancer susceptibility and to support informed decision making and adaptive responses to testing. Future prospective studies including patient-reported outcomes are needed to inform how to best incorporate multiplex testing for cancer susceptibility into clinical practice.Genet Med 17 6, 485-492.

An academic librarian and the coordinator of a campus tutoring and writing center recently relocated to the library researched their value to second-year students. Differences in the amount and type of available data called for conducting in-depth interviews with students about their research and writing processes. The researchers also reviewed relevant material regarding similar collaborative efforts at other college and universities. The gaps revealed in the environmental scan along with the best practices of librarian/writing center collaboration helped determine future steps needed for both units to move from mere co-location to working in true collaboration.

The new, ASCP-led Partners for Cancer Diagnosis and Treatment in Africa, a Clinton Global Initiative Commitment to Action, has launched its first phase: providing patients in underserved areas of Sub-Saharan Africa access to rapid cancer diagnostics and appropriate care and treatment. In mid-January, the coalition delivered histotechnology equipment to Butaro District Hospital, in the city of Butaro in northern Rwanda. The equipment enables rapid diagnostics and review of patient material for therapy with a team of pathologists in the United States. Currently, there is no permanent pathologist in Butaro. Clinicians rely on either static image telepathology, visiting pathologists from the United States, or visits from local Rwandan pathologists, which takes them away from providing services in their own communities. Automated histopathology and telepathology systems are delivered to the Butaro District Hospital in the city of Butaro, Rwanda. Equipment was rolled into the lab space on logs. During this initial phase, four other hospitals in Rwanda also now have access to the equipment that enables them to share access to rapid diagnoses and advance consultants. [Graphic][1] Marilyn L. Slovak, PhD, FACMG, MT(ASCP), began her distinguished career as a medical technologist. After earning her PhD in regulatory biology, she went on to serve as cytogenetic laboratory director at the City of Hope Comprehensive Cancer Center in Duarte, Calif., for over 20 years before becoming medical director of Cytogenetics at Sonora Quest Laboratory in Tempe, Ariz., in 2012. She retired July 2015 and now does consulting. Dr. Slovak served on the International Cytogenetics Committee for Myelodysplastic Syndromes (MDSs) and in collaboration with Dr. Peter Greenberg and the International Working Group for Prognosis in MDS (IWG-PM) revised the International Prognostic Scoring System, or IPSS (1997), for MDS. The IPSS-R (revised) is regarded as the international gold standard classification system for MDS. This scoring system estimates overall survival and risk of transformation to acute myeloid leukemia (AML) in patients with primary MDS. The IPSS-R introduces several novel components, including five rather than three cytogenetic prognostic subgroups of IPSS (1997) with specific and new classifications of a number of less common cytogenetic subsets, splitting the low marrow blast percentage value, and depth (instead of the number) of cytopenias. Dr. Slovak also served on the faculty that developed the curriculum for the four MDS Summit workshops funded by an education grant from Celgene, that ASCP has co-hosted with the American Society of Hematology (ASH). Background Germline genetic testing is standard practice in oncology. Outcomes of telephone disclosure of a wide range of cancer genetic test results, including multigene panel testing (MGPT) are unknown. Methods Patients undergoing cancer genetic testing were recruited to a multicenter, randomized, noninferiority trial (NCT01736345) comparing telephone disclosure (TD) of genetic test results with usual care, in-person disclosure (IPD) after tiered-binned in-person pretest counseling. Primary noninferiority outcomes included change in knowledge, state anxiety, and general anxiety. Secondary outcomes included cancer-specific distress, depression, uncertainty, satisfaction, and screening and risk-reducing surgery intentions. To declare noninferiority, we calculated the 98.3% one-sided confidence interval of the standardized effect; t tests were used for secondary subgroup analyses. Only noninferiority tests were one-sided, others were two-sided. Results A total of 1178 patients enrolled in the study. Two hundred eight (17.7%) participants declined random assignment due to a preference for in-person disclosure; 473 participants were randomly assigned to TD and 497 to IPD; 291 (30.0%) had MGPT. TD was noninferior to IPD for general and state anxiety and all secondary outcomes immediately postdisclosure. TD did not meet the noninferiority threshold for knowledge in the primary analysis, but it did meet the threshold in the multiple imputation analysis. In secondary analyses, there were no statistically significant differences between arms in screening and risk-reducing surgery intentions, and no statistically significant differences in outcomes by arm among those who had MGPT. In subgroup analyses, patients with a positive result had statistically significantly greater decreases in general anxiety with telephone disclosure (TD -0.37 vs IPD +0.87, P = .02). Conclusions Even in the era of multigene panel testing, these data suggest that telephone disclosure of cancer genetic test results is as an alternative to in-person disclosure for interested patients after in-person pretest counseling with a genetic counselor.

Advances in genome sequencing technology have fostered a new era of clinical genomic medicine. Genetic counselors, who have begun to support patients undergoing multi-gene panel testing for hereditary cancer risk, will review brief clinical vignettes, and discuss early experiences with clinical genomic testing. Their experiences will frame a discussion about how current testing may challenge patient understanding and expectations toward the evaluation of cancer risk and downstream preventive behaviors. J. As an essential department at a higher education institution and an informal learning space, little is known about how academic libraries contribute to student learning on campus. The Olin Library sought to learn the role of library space in our users' learning. We surveyed users about their learning behaviors in a specific space prior to a scheduled renovation and then in the same space after. We wanted to determine how the renovation changed users' perceptions of their learning behaviors in that space.

We once again benefitted from a strong partnership, co-led by AcademyHealth and the National Institutes of Health (NIH), with co-sponsorship from the Agency for Healthcare Research and Quality (AHRQ), the Patient Centered Outcomes Research Institute (PCORI), the Robert Wood Johnson Foundation (RWJF), the US Department of Veterans Affairs (VA), and the WT Grant Foundation. In addition, we benefitted from the collaboration of staff from the Centers for Disease Control and Prevention (CDC) and the World Health Organization (WHO). NIH and AcademyHealth again co-led the program planning committee, which focused on the development of the plenary sessions, and convened a scientific advisory panel to suggest speakers and advise on the overall conference development. The planning committee identified four key areas around which to focus the plenary panels and keynote address. Dr. America Bracho, M.D., M.P.H., Executive Director of Latino Health Access in Orange County, California, spoke about the opportunities for implementation science to inform efforts to improve community health and engage underserved populations. The three plenary panels each focused on a significant future direction for dissemination and implementation (D & I) research: the interface between D&I science and population health, emerging opportunities for global implementation science, and the challenges around implementation of precision medicine. The plenary sessions were complemented by facilitated lunchtime discussions on the same three topics, which offered participants an opportunity to identify key research questions for each and brainstorm next steps. Synopses of the lunchtime discussions are included in this supplement. Given the overwhelming success of the 2014 conference and the large number of abstracts received in 2014 (660), the program planning committee identified eight program tracks for abstract submitters to respond to, and through which the concurrent sessions of the conference would be organized. These tracks—Behavioral Health, Big Data and Technology for Dissemination and Implementation Research, Clinical Care Settings, Global Dissemination and Implementation, Promoting Health Equity and Eliminating Disparities, Health Policy Dissemination and Implementation, Prevention and Public Health, and Models, Measures and Methods— were designed to enable conference participants to follow a consistent theme across the multiple sessions of the conference and form the structure of this supplement. The call for abstracts, including individual paper presentations, individual posters and panel presentations, resulted in 515 submissions, spread across the eight thematic tracks. Over one hundred reviewers devoted their time to ensuring a comprehensive and expert review, and reviews were conducted within each track and coordinated by the track leads. For the final program, 64 oral presentations, 12 panels, and 263 posters were presented over the two-day meeting. Slides for the oral presentations and panels (with the agreement of the authors) were posted on the conference website (http://diconference.academyhealth.org/archives/2015archives) and all abstracts were included on the conference webapp (https://academyhealth.confex.com/academyhealth/2015di/meetingapp.cgi). This supplement has compiled the abstracts for presented papers, panel sessions, and lunchtime discussions from the 8th Annual Meeting on the Science of Dissemination and Implementation in Health: Optimizing Personal and Population Health. We are pleased to have the abstracts from the conference together in one volume once again, and look forward to the 9th Annual meeting, scheduled for December in Washington, D.C.

We give some applications of a Hopf algebra constructed from a group acting on another Hopf algebra A as Hopf automorphisms, namely Molnar's smash coproduct Hopf algebra. We find connections between the exponent and Frobenius-Schur indicators of a smash coproduct and the twisted exponents and twisted Frobenius-Schur indicators of the original Hopf algebra A. Assessment of how library patrons use space and the evaluation of their needs has become a “hot” topic in library research. But determining the best way to obtain information about their activity can be a challenge. Two types of data collection are quantitative and qualitative. Quantitative data provides a numerical count of what activities students perform within the library and therefore can be measured. Qualitative data gives personal opinions, feedback and individual perspectives regarding a topic but is not measurable. In this study, we were interested to learn if we would get substantially different results from a user observation study, a quantitative method, than from the results of asking users about their library space needs, a qualitative method. Essentially, would the results from both methods provide complementary results enabling us to obtain a comprehensive picture about our patrons’ needs and redesign the space to improve their library experience?

Oncology nurses have long been aware of the significance of recognizing patients' hereditary risk of cancer. Obtaining an accurate family history is an integral part of patient assessment and has helped to guide referrals for genetic counseling and testing for hereditary breast and ovarian cancer syndrome (HBOC) and Lynch syndrome.

We give a direct combinatorial proof that a certain n-dimensional algebra A is a comodule algebra over the Taft Hopf algebra of dimension n 2.

We introduce two kinds of gauge invariants for any finite-dimensional Hopf algebra H. When H is semisimple over C, these invariants are respectively, the trace of the map induced by the antipode on the endomorphism ring of a self-dual simple module, and the higher Frobenius-Schur indicators of the regular representation. We further study the values of these higher indicators in the context of complex semisimple quasi-Hopf algebras H. We prove that these indicators are non-negative provided the module category over H is modular, and that for a prime p, the p-th indicator is equal to 1 if, and only if, p is a factor of dim H. As an application, we show the existence of a non-trivial self-dual simple H-module with bounded dimension which is determined by the value of the second indicator., additional references, fixed some typos, minor additions including a questions and some remarks

In a period of fiscal constraint, when assumptions about the library as place are being challenged, administrators question the contribution of every expense to student success. Libraries have been successful in migrating resources and services to a digital environment accessible beyond the library. What is the role of the library as place when users do not need to visit the building to utilize library services and resources? We argue that the college library building's core role is as a space for collaborative learning and community interaction that cannot be jettisoned in the new normal.

College students today are spending more time online accessing information and connecting with each other. Libraries and librarians have made changes in their collections and services so as to be easily available and accessible in the online world. Embedding a librarian in a course enhances student learning by connecting students with reliable resources. Most embedded librarian programs focus on online courses but should be offered in face-to-face (F2F) classes as well. Embedded librarians can provide instruction via online Webinars to F2F classes giving them an interactive learning experience. Future embedded librarians need to adopt online Webinars as a way to be where our users and resources are.
