Abstract

L’irrompere della pandemia ha rappresentato una possibilità di rinnovamento per il mondo accademico. Con la chiusura improvvisa di tutte le attività in presenza, le Università hanno dovuto fare i conti con la necessità di salvaguardare i percorsi formativi degli studenti e delle studentesse. Soprattutto quelle istituzioni (la maggior parte in Italia), che non avevano mai affrontato il tema dell’erogazione a distanza delle attività didattiche, sono state costrette a fronteggiare la novità di un mondo dell’apprendimento “a distanza” (Piazza, 2020). Ovviamente anche l’Università di Catania si è trovata a fronteggiare questa situazione. Nello specifico, il Corso di laurea in Scienze dell’Educazione e della formazione (CdS L-19), risolto velocemente il problema dell’erogazione dei corsi, ha cercato delle soluzioni maggiormente calibrate sulle necessità degli studenti per favorire la loro esperienza di tirocinio, considerata un’attività formativa essenziale per costruire la propria identità professionale come educatore/educatrice. Il testo che segue racconta dell’esperienza realizzata e di come, da un’emergenza, sia scaturita una proposta formativa valida, mantenuta anche al ritorno delle attività in presenza.

Abstract

Twenty years after the European Landscape Convention, the paper aims at reflecting on the role of landscape planning in contrasting peripheralization processes and risks in rural contexts. The issue is debated referring to the research studies developed in the frame of the ongoing review process of the Lombardy Regional Landscape Plan. The Lombardy context is considered as representative of a huge variety of landscape conditions, as well as the rural system is recognised as one of the main components structuring and defining the identity of the regional territory. The rural landscape is formed by the interaction between anthropic and natural elements that express the cultural and productive traditions characterising the different territorial contexts within the Region. At the same time, these rural contexts are potentially affected by several peripheralization risks due to abandonment, degradation, banalisation phenomena or land take and fragmentation processes. In this frame, the paper presents three main qualitative objectives defined to preserve, protect and valorise the different rural landscapes characterising the Lombardy Region context facing potential peripheralization risks.

7. Lithium treatment reverses irradiation-induced changes in rodent neural progenitors and rescues cognition

Abstract

Cranial radiotherapy in children has detrimental effects on cognition, mood, and social competence in young cancer survivors. Treatments harnessing hippocampal neurogenesis are currently of great relevance in this context. Lithium, a well-known mood stabilizer, has both neuroprotective, pro-neurogenic as well as antitumor effects, and in the current study we introduced lithium treatment 4 weeks after irradiation. Female mice received a single 4 Gy whole-brain radiation dose on postnatal day (PND) 21 and were randomized to 0.24% Li2CO3 chow or normal chow from PND 49 to 77. Hippocampal neurogenesis was assessed on PND 77, 91, and 105. We found that lithium treatment had a pro-proliferative effect on neural progenitors, but neuronal integration occurred only after it was discontinued. Also, the treatment ameliorated deficits in spatial learning and memory retention observed in irradiated mice. Gene expression profiling and DNA methylation analysis identified two novel factors related to the observed effects, Tppp, associated with microtubule stabilization, and GAD2/65, associated with neuronal signaling. Our results show that lithium treatment reverses irradiation-induced loss of hippocampal neurogenesis and cognitive impairment even when introduced long after the injury. We propose that lithium treatment should be intermittent in order to first make neural progenitors proliferate and then, upon discontinuation, allow them to differentiate. Our findings suggest that pharmacological treatment of cognitive so-called late effects in childhood cancer survivors is possible. © 2019, The Author(s).

14. Évaluation non invasive de la fibrose hépatique chez des patients atteints de psoriasis, polyarthrite rhumatoïde ou maladie de Crohn traités par méthotrexate

Abstract

Introduction L’hepatotoxicite rapportee avec le methotrexate souligne la necessite d’une evaluation repetee, non invasive et fiable de la fibrose hepatique. Nous avons depiste, par une strategie non invasive chez des patients traites par methotrexate, la prevalence et les facteurs predictifs d’une fibrose hepatique significative (fibrose ≥ F2). Materiel et methodes La fibrose a ete depistee prospectivement en utilisant 9 tests non invasifs chez des patients consecutifs atteints de psoriasis, de polyarthrite rhumatoide ou de maladie de Crohn. La fibrose significative a ete evaluee sans biopsie hepatique en definissant une « methode specifique » (resultat donne par la majorite des tests) et une « methode sensible » (au moins un test indiquant un stade ≥ F2). Resultats Un total de 131 patients (66 psoriasis, 40 polyarthrite rhumatoide et 25 maladie de Crohn) ont ete recrutes, dont 83 recevaient du methotrexate. Sept tests ont ete realises en moyenne par patient, avec une concordance complete dans 75 % des cas. Le Fibroscan® etait interpretable chez seulement 61 % des patients. Les meilleures performances (AUROC > 0,9) pour predire une fibrose significative ont ete obtenues par des tests dedies a la steatohepatite (FibroMeter NAFLD, NFS et FPI). La prevalence de la fibrose ≥ F2 selon la methode « specifique » ou « sensible » de la fibrose etait respectivement de 10 % et 28 %. L’exposition au methotrexate n’influencait pas le stade de la fibrose. Selon notre « methode sensible », les facteurs independamment associes a une fibrose significative etaient l’âge, le sexe masculin et le syndrome metabolique. Discussion Nous avons donc defini chez les patients traites par methotrexate, une approche non invasive pour identifier la fibrose hepatique significative (≥ F2) en utilisant 8 tests biochimiques et le Fibroscan®. Dans cette population, le risque de fibrose importante etait lie a l’âge, au sexe masculin et a la presence d’un syndrome metabolique, mais n’etait pas influence par le methotrexate.

16. Promotion of the mediterranean diet incancer long-survivors by means of the Med-Food Anticancer Program: a pilot study

Abstract

The intervention "Med-Anticancer Food Program" has proven to be effective in promoting the Mediterranean Diet, significantly increasing the Mediterranean Adequacy Index in healthy subjects. There are no studies that have investigated the effectiveness of this intervention in individuals who have had a diagnosis of cancer.To perform a pilot study to assess the opportunity of employing the methodology of the Med-Anticancer Food Program in order to encourage "long-term cancer survivors" to adhere to the Mediterranean Diet, as well as healthy people, and this in order to apply the program to larger groups.From the residents' register of Foggia, a city in southern Italy, forty adults of both sexes, over 25 years of age, were recruited at random and assigned (1:1) as follows: - Twenty healthy subjects to the intervention-1 group - Twenty long-term cancer survivors to the intervention-2 group. The Med-Anticancer Food Program was applied to both groups with an articulated intervention 11 weeks long, followed by a 52-week period of follow up. By means of a food diary of the last 3 days, the Mediterranean Adequacy Index values were calculated before intervention (T0), after a period of 11 weeks of interventions (T1) and at the end of the 52 weeks of follow-up period (T2). The H0 hypothesis of the study was that there are no differences between the two interventions in reaching by T1, and maintaining at T2, values of Mediterranean Adequacy Index around 7, considered the optimum for adherence to the Mediterranean diet.Out of the subjects assigned to the intervention-1 group (n = 20), 11 subjects have completed the 52-months follow-up (55.0% ); for intervention-2, 16 (80%) out of 20 have completed it. The average age of subjects was 52.1 years. The Mediterranean Adequacy Index, of intervention-1 group significantly increased from 2.8 (T0) to 9.2 (T1) and to 9.0 (T2) (p0.0001); whereas, in the intervention-2 group, Mediterranean Adequacy Index moved from 2.4 (T0) to 10.2 (T1) and to 9.3 (T2) (p0.0001). The difference of Mediterranean Adequacy Index between the two study groups at T1 and T2 was not significant. Such non-significance persists also after the stratification by sex and age obtained with Mantel-Haenszel procedure. The performance of the values of the laboratory parameters considered (folic acid, total cholesterol, alkyl resorcinol) was similar in the subjects of both intervention 1 and 2, without any difference, while considered at a basal level T0, at T1 and at the end of the follow-up period (T2).The results of our work suggest the feasibility of conducting the Med-Anticancer Food Program in long-term cancer survivors. The results of the pilot study show that such intervention, carried on a small number of long term cancer survivors, is adequate to assess its feasibility but, due to the limited size of our study, a confirmation is required through larger nutritional prevention intervention studies.

Abstract

Background The management of inflammatory rheumatisms and psoriasis has largely evolved over the last 15 years with the emergence of biotherapies whose main adverse effect is the increased infection risk. The prevalence of metabolic syndrome is increasing and has been estimated at 30% in patients with rheumatoid arthritis with an excess of 45% compared to healthy subjects. One of the major complications of the metabolic syndrome is the Non Alcoholic Fatty Liver Disease (NAFLD), which prevalence is 25% in the global population, and 30% in a cohort of patients with rheumatoid arthritis. The main complication of NAFLD is the development of cirrhosis, known to increase infectious risk. Surprisingly, there are no data on the safety of TNF antagonists in patients with chronic inflammatory disease and NAFLD or cirrhosis. Objectives To assess infectious and liver safety of the main TNF blockers used in chronic inflammatory diseases in patients with cirrhosis or NAFLD. Methods A systematic review of the literature, following the Prisma recommendations, was conducted on the Pubmed and Embase databases with the following keywords: ”adalimumab”, ”infliximab”, ”etanercept”, ”certolizumab”, ”golimumab” ”TNF -blockers “AND” liver cirrhosis ”,” non alcoholic fatty liver disease “. We selected only studies including patients treated with TNF blockers and with cirrhosis or ultrasonically characterised NAFLD. We excluded animal models and non-English articles. Results We identified 39 articles and only 11 fulfilled the inclusion criteria. 9 Case Reports have reported the tolerance of TNF-blockers in patients with cirrhosis and one controlled study (44 patients) assessed the safety of etanercept in case of NAFLD. One study (7 patients) investigated the impact of infliximab in refractory autoimmune hepatitis. The cause of cirrhosis was primary biliary cholangitis (5 cases), alpha1 antitrypsin deficiency (4 cases) and post hepatitis B cirrhosis (2 cases). All cirrhosis were compensated. Inflammatory diseases requiring the introduction of TNF blocker were rheumatoid arthritis (n=5), psoriatic arthritis (n=4), ulcerative colitis (n=2) and psoriasis (n=44). The TNF-blockers prescribed were adalimumab in 2 patients, infliximab in 11 patients and etanercept in 49 patients. The duration of treatment ranged from 6 to 24 months. For the 9 cirrhotic patients with an inflammatory disorder, no infection was reported and two of them (2 cases of primary biliary cholangitis) even had an improvement in liver function secondary to the introduction of biotherapy. For the 7 patients receiving infliximab to treat auto-immune hepatitis, 5 patients presented recurrent infections. For patients with NAFLD, no infectious event was reported and an improvement of the hepatic biological parameters was observed, suggesting an improvement in liver function. Conclusions The results of this review suggest that in case of compensated cirrhosis, TNF-blockers were not deleterious for the liver and did not increase the infectious risk. In case of auto-immune cirrhosis, TNF-blockers increased the infectious risk. In case of NAFLD, TNF-blockers might improve liver function and prevent fibrosis. Disclosure of Interest None declared

Abstract

International audience; Background In liver transplant recipients with hepatitis C virus recurrence, there is concern about renal safety of sofosbuvir-based regimens. Changes in serum creatinine or in the estimated glomerular filtration rate (eGFR) under treatment are used to look for possible renal toxicity. However, serum creatinine and eGFR are highly variable. Aim To analyse renal function trajectory with numerous assays of serum creatinine over a long period of time. Methods In a multicentre cohort of 139 patients, the eGFR was obtained from serum creatinine using the Chronic Kidney Disease-Epidemiology Collaboration (CKD-EPI) equation. Slopes of eGFR were defined as a change in eGFR during a period divided by time. Pre-treatment, on-treatment and post-treatment periods were 9 months, 3-9 months and 4.5 months. Interactions between eGFR slopes and the pre-treatment eGFR, use of ribavirin or mycophenolate mofetil, and stage of fibrosis were addressed. On-treatment eGFR slopes were separated in tertiles. Pre- and post-treatment eGFR slopes were compared globally and according to tertiles. Results The post-treatment eGFR slope was significantly better than pre-treatment eGFR slope (+0.18 (IQR -0.76 to +1.32) vs -0.11 (IQR -1.01 to +0.73) mL/min/1.73 m(2)/month, P=0.03) independently of the pre-treatment eGFR (P=0.99), ribavirin administration (P=0.26), mycophenolate mofetil administration (P=0.51) and stage of fibrosis (F3 and F4 vs lower stages, P=0.18; F4 vs lower stages, P=0.08; F4 Child-Pugh B and C vs lower stages, P=0.38). Tertiles of on-treatment eGFR slopes were -1.71 (IQR -2.54 to -1.48), -0.78 (IQR -1.03 to -0.36) and +0.75 (IQR +0.28 to +1.47) mL/min/1.73 m(2)/month. Pre- and post-treatment eGFR slopes were not significantly different according to tertiles (respectively, P=0.34, 0.08, 0.73). Conclusion The eGFR varies during treatment and gives a confusing picture of the renal safety of sofosbuvir-based regimens. In contrast, longitudinal assessment of the eGFR shows a rising trajectory over longer time, meaning that these therapies are safe for the kidneys in our cohort of liver transplant recipients.

Abstract

Etat de la question Le role de l’endotoxemie liee a la translocation bacterienne sur l’aggravation de la cirrhose fait l’objet de nombreuses etudes, mais son impact sur la mortalite au cours de la cirrhose est mal connu. Le but de ce travail etait d’etudier l’impact de la concentration sanguine de 3-hydroxymyristate (3-HM) total (composant specifique des lipopolysaccharides, endotoxine), sur le risque de deces et de complications des malades cirrhotiques. Materiel et methodes Au total, 593 patients atteints de cirrhose alcoolique ou virale, sans carcinome hepatocellulaire ont ete inclus entre 2008 et 2012 dans six centres hospitalo-universitaires francais. Le dosage de 3-HM total, libre et lie aux lipoproteines a ete realise en l’absence de complication aigue de la cirrhose par une methode innovante et brevetee de dosage massique des LPS. Le suivi annuel pendant cinq ans a permis de relever la survenue de complications ou du deces. L’analyse de survie a ete realisee par un modele de Cox multivarie avec stratification sur le centre et la severite de la cirrhose. Resultats Les patients avec une concentration de 3-HM elevee avait une cirrhose plus severe que les autres selon les scores de Child-Pugh et de Meld (p Conclusion Une concentration elevee de 3-HM, marqueur specifique de l’endotoxemie, est associee a un sur-risque de deces a cinq ans chez les patients avec cirrhose alcoolique ou virale compensee (Child Pugh A) suggerant un role majeur de la translocation bacterienne dans l’aggravation de la cirrhose.

Abstract

This population-based study aimed to assess thedeterminants of the outcome of chronic hepatitis C withanalysis of the impact of antiviral therapy with or withoutsustained virological response (SVR) on cirrhosis decom-pensation, hepatocellular carcinoma, liver-related and non-liver-related mortality. A total of 1159 HCV-positive patientsnewly detected between 1994 and 2001 were included. Foreach outcome, the prognostic effect of patients baselinecharacteristics was estimated by time-dependent Cox modelsusing age as the time-scale and adjusting for treatmentreceived during follow-up. The impact of antiviral therapywas assessed by using a propensity score in a sampleincluding 184 patients treated in the first 24 months fol-lowing diagnosis who were matched to 184 untreatedpatients. At the end of a 59-month median follow-up, 100cases of compensated disease, 58 liver cancer and 163deaths (55 liver related) were recorded. The 5-year rates ofdecompensated cirrhosis, hepatocellular carcinoma, liver-related and non-liver-related death were 4.4%, 2.7%, 5.0%and 8.9%, respectively. Multivariate analyses identified twovariables with pejorative influence: alcohol consumption(RR = 4.29 for CD; RR = 5.76 for HCC; RR = 6.69 for liver-related death; P < 0.0001); HCV diagnosis unrelated tosystematic screening (RR = 2.25 for CD; RR = 3.05 forHCC; RR = 4.31 for liver-related death, P < 0.03). In thematched subset, no significant benefit of antiviral therapywas observed. Nevertheless, among the 144 patients whoachieved SVR, no death was observed. This population-basedstudy showed substantial rates of decompensated cirrhosis,hepatocellular carcinoma and non-liver-related mortality.Alcohol consumption and absence of systematic screeningwere significant determinants of poor outcome, whereastreatment did not have significant influence.Keywords: antiviral therapy, hepatitis C, liver-related mor-tality, non-liver-related mortality, outcome, population-based cohort study, survival bias.

Abstract

Introduction et but de l’etude Le carcinome hepatocellulaire (CHC) se developpe dans 80 % des cas sur un foie cirrhotique. En France, l’incidence augmente et les traitements restent peu efficaces. L’alimentation peut etre un moyen de prevention interessant mais les resultats d’etudes dans ce domaine restent rares et divergents. Notre objectif etait donc d’etudier les liens entre differents facteurs alimentaires et le risque de CHC dans une population francaise atteinte de cirrhose. Materiel et methodes Pour l’etude multicentrique Cirrhose et risque de carcinome hepatocellulaire dans le grand est (CiRCE), des patients cirrhotiques avec CHC histologiquement confirme (cas) et des patients cirrhotiques sans CHC a l’inclusion (temoins) ont ete recrutes entre 2008 et 2012 dans 6 hopitaux de l’Est de la France. Les consommations (g/j) de 208 aliments et 22 nutriments etaient obtenus a l’aide d’un questionnaire type histoire alimentaire et sa table de composition. La methode des residus et un decoupage en tertiles etaient ensuite utilises. Les Odds Ratio (OR) et leurs intervalles de confiance (CI) a 95 % etaient obtenus par regression logistique multivariee. Resultats et analyse statistique Les 181 cas avec CHC inclus etaient plus âges (64,4 ans vs 59 ans, p Conclusion Nos resultats preliminaires mettent en evidence des relations interessantes entre des facteurs alimentaires et le risque de CHC chez des patients atteints de cirrhose. L’analyse des profils de consommations alimentaires selon l’etiologie de la cirrhose et les liens avec certains parametres biologiques sont en cours. Nos resultats devront etre confirmes dans de plus larges etudes.

Abstract

Resume Le syndrome hepatopulmonaire est caracterise par une hypertension portale avec ou sans cirrhose, une augmentation du gradient alveolo-arteriel en oxygene en air ambiant superieure ou egale a 15 mmHg, et des dilatations vasculaires intrapulmonaires au niveau des vaisseaux capillaires. Chez les malades cirrhotiques, la prevalence du syndrome hepatopulmonaire est estimee a 20 %. Le diagnostic est fait par l’echographie cardiaque de contraste. La physiopathologie du syndrome hepatopulmonaire ferait intervenir essentiellement une augmentation de la production pulmonaire de monoxyde d’azote. L’hypoxemie s’aggrave progressivement et augmente la mortalite. Une surveillance rapprochee des patients hypoxemiques est necessaire pour envisager une transplantation hepatique dans des conditions optimales. En effet, les principaux facteurs de mortalite postgreffe sont une PaO2 inferieure ou egale a 50 mmHg seule ou associee a un shunt isotopique superieur a 20 %. Nombreux sont les medicaments qui ont ete testes dans le syndrome hepatopulmonaire, mais les resultats probants sont rares : la poudre d’ail et, plus recemment, les aerosols d’iloprost pourraient ameliorer de facon prolongee l’oxygenation des patients avant et apres la transplantation hepatique.

Abstract

Resume L’hepar lobatum carcinomatosum correspond a une dysmorphie hepatique majeure non cirrhotique acquise au cours de l’evolution d’un carcinome, le plus souvent d’origine mammaire, metastase au foie. Il s’associe souvent a une hypertension portale. Il est la consequence d’un envahissement des veinules portes par les cellules tumorales et d’une reaction desmoplastique au contact de la tumeur. Le pronostic est sombre en depit d’une impression de regression tumorale en imagerie. Nous rapportons un cas d’hepar lobatum carcinomatosum revele par une rupture de varices œsophagiennes. Le diagnostic a pu etre documente par les donnees de l’imagerie par resonance magnetique dans un contexte clinique evocateur.

Abstract

Summary Background A prevalence of 1.2% of coeliac disease (CD) in patients with chronic hepatitis C was recently reported, suggesting a possible epidemiological link between these two diseases. However, other studies have not found this relationship. Aim To conduct a French multicentre prospective study to assess the prevalence of CD in hepatitis C virus (HCV)-infected patients. Methods Between June 2003 and November 2005, 624 consecutive HCV-positive out-patients were tested for antiendomysial IgA antibodies (AEA), antigliadin IgA and IgG antibodies (AGA). Patients with positive AEA or IgA AGA and positive IgG AGA in a context of a high suspicion of CD were asked to undergo gastroscopy with duodenal biopsies. Results Isolated IgA AEA, IgA AGA and IgG AGA were 0.16%, 5.7% and 4.4%, respectively. Gastroscopy was required for 39 patients, 31 were performed (eight refusals), but only 25 duodenal biopsies were performed as six patients had cirrhosis. CD was never detected. Conclusions The prevalence of CD in HCV-positive patients was 0% (95% confidence interval: 0–0.59%), but there is a low prevalence of CD in the whole French population.

Abstract

SUMMARY High-dose omeprazole reduces the rate of recurrent bleeding after endoscopic treatment of peptic ulcer bleeding. However, the effectiveness of high-dose vs. standard-dose omeprazole in peptic ulcer bleeding has never been shown. Aim To compare the benefits of high-dose vs. standard-dose omeprazole in peptic ulcer bleeding. Methods We reviewed the medical files of patients admitted between 1997 and 2004 for high-risk peptic ulcer bleeding who had undergone successful endoscopic treatment. We distinguished 2 periods: before 2001, standarddose omeprazole (40 mg ⁄ day intravenously until alimentation was possible, then 40 mg ⁄ day orally for 1 week); after 2001, high-dose omeprazole (80 mg bolus injection, then 8 mg ⁄ h continuous infusion for 72 h, then 40 mg ⁄ day orally for 1 week). During both periods, patients subsequently received omeprazole, 20 mg ⁄ day, orally for 3 weeks. Results We enrolled 114 patients (period 1, n = 45, period 2, n = 69). Therapy with high-dose omeprazole significantly decreased the occurrence of poor outcome (27 vs. 12%, P = 0.04), rebleeding (24 vs. 7%, P = 0.01), mortality due to haemorrhagic shock (11 vs. 0%, P < 0.001) and need for surgery (9 vs. 1%, P = 0.05). Conclusions In this retrospective study, high-dose omeprazole reduced the occurrence of rebleeding, need for surgery and mortality due to hemorrhagic shock in patients with high-risk peptic ulcer bleeding, as compared with standard-dose omeprazole.

Abstract

The computing GRID infrastructure could benefit of techniques that can improve the over-all throughput of the system. It is possible that job submission will include different ontology in resource requests due to the generality of the GRID infrastructure. Such flexible resource request could offer the opportunity to optimize several parameters, from network load to job costs in relation to due time, more generally the quality of services. We present the result of the simulation of GRID jobs allocation. The search strategy for this input case does not converge to the optimal case inside the limited number of trial performed, in contrast with previous work on up to 24 jobs [Scheduling in a Grid Computing Environment using Genetic Algorithms, in: Proceedings of the International Parallel and Distributed Processing Symposium: IPDPS 2002 Workshops]. The benefits of the usage of the genetic algorithms to improve the quality of the scheduling is discussed. The simulation has been obtained using an environment GGAS suitable to study the scheduling of jobs in a distributed group of parallel machines. The modular structure of GGAS allows to expand its functionalities to include other first level schedule policy with respect to the FCFS that is considered. The result of this paper suggests the usage of local search strategy to improve the convergence when the number of jobs to be considered is as big as in real world operation.

Abstract

Background: Randomized controlled trials testing flumazenil in hepatic encephalopathy have shown conflicting results. Aim: To compare flumazenil and placebo in hepatic encephalopathy in patients with cirrhosis. Methods: An overview of randomized controlled trials comparing flumazenil and placebo in hepatic encephalopathy in patients with cirrhosis was performed. For each end-point, heterogeneity and treatment efficacy were assessed by Peto and Der Simonian methods. As most trials were crossover in nature, a sensitivity analysis was performed including the two treatment periods. Results: Six double-blind randomized controlled trials, including 641 patients (326 treated with flumazenil and 315 with placebo), were identified. The treatment duration ranged from 5 min to 3 days. Heterogeneity tests between control groups were not significant. The mean percentages of patients with clinical improvement (five trials) were 27% in treated groups and 3% in placebo groups. 38. Impact de l’infection à cytomégalovirus sur l’issue des patients cirrhotiques : une étude observationnelle

Abstract

Etat de la question Chez les patients cirrhotiques, les determinants du carcinome hepatocellulaire (CHC) et de la mortalite de cause hepatique sont imparfaitement connus. Le cytomegalovirus (CMV), via ses proprietes pro-oncogeniques, pro-inflammatoires et immunosuppressives, pourrait favoriser le developpement du CHC et une inflammation systemique deletere. Notre objectif etait d’evaluer si l’infection a cytomegalovirus est associee au CHC et a la mortalite de cause hepatique chez les patients cirrhotiques. Materiel et methodes Parmi les 1178 patients inclus entre juin 2008 et decembre 2012 dans l’etude CIRCE, une etude cas–temoin multicentrique du Grand-Est de la France concue pour identifier les facteurs de risques de CHC chez les patients cirrhotiques indemnes de pathologie extrahepatique engageant le pronostic vital, 432 patients disposant d’un statut serologique CMV connu ou de serums a l’inclusion ont ete identifies, dont 159 cas avec CHC et 273 patients cirrhotiques sans CHC. Nous avons mesure les facteurs associes au CHC a l’inclusion ou a la survenue de CHC incidents chez les temoins, ainsi que les facteurs associes a la mortalite de toute cause et liee au foie dans toute la population d’etude. Resultats Durant un suivi median de 31 mois, 25 CHC sont survenus chez des temoins, et 209 deces (162 de cause hepatique) ont ete enregistres. Il y avait 247 (57,2 %) patients CMV-seropositifs. La seropositivite CMV n’etait pas associee a une frequence plus importante de CHC a l’inclusion ni au cours du suivi, mais parmi les patients CMV-seropositifs avec CHC, la proportion de tumeurs multinodulaires, infiltrantes ou metastatiques etait plus elevee (73,8 % versus 57,3 %, p = 0,029). Dans des regressions de Cox multivariees ajustees sur l’âge, le score MELD, la presence d’un CHC a l’inclusion et le diabete, la seropositivite CMV predisait la mortalite de toute cause (HR = 1,45 ; IC95 % : 1,08–1,94 ; p = 0,013) et de cause hepatique (HR = 1,56 ; IC95 % : 1,04–2,30 ; p = 0,031). Conclusion Les patients cirrhotiques CMV-seropositifs sont a risque de deceder d’une cause hepatique, en raison de CHC plus agressifs ou de complications hepatiques graves.

Abstract

To investigate the utility of multidetector CT (MDCT) in helping to establish the underlying cause of acute colitis.All patients who had acute colitis with a well-identified cause and underwent abdomen 64-MDCT were included in the study. MDCT images were retrospectively analysed in a blinded fashion and the CT findings were correlated with the eventual aetiological diagnosis.The study population included 105 patients. Acute colitis was related to inflammatory bowel disease in 43 cases. MDCT was used to identify six relevant signs of inflammatory colitis: the "comb" sign (p0.001), enlarged lymph nodes (p0.001), abscess (p = 0.026), fibro-fatty infiltration (p = 0.007), small bowel involvement (p0.001), and the absence of an "empty colon" sign (p = 0.045). Multivariate logistic regression analysis identified three independent signs of inflammatory colitis: the "comb" sign, small bowel involvement, and enlarged lymph nodes. Acute colitis was related to bacterial infection in 35 cases. Five signs were significantly associated with infectious colitis: continuous distribution (p = 0.020), an "empty colon" sign (p = 0.002), absence of fat stranding (p = 0.013), and absence of a "comb" sign (p = 0.010) and absence of enlarged lymph nodes (p = 0.035). Multivariate analysis identified three independent signs: the "empty colon" sign and absence of fat stranding and of a "comb" sign. The remaining causes were ischaemic colitis (n = 21) and drug-related colitis (n = 6). MDCT examination provided five relevant signs of ischaemic colitis: fat stranding (p = 0.002), discontinuous distribution (p0.001), and absence of enlarged lymph node (p0.001), a "comb" sign (p = 0.006) and small bowel involvement (p = 0.037).MDCT provides certain suggestive signs that may be helpful in distinguishing the underlying aetiological cause of acute colitis.

Abstract

The natural history of hepatitis C virus (HCV) infection in human immunodeficiency virus (HIV)-infected patients has never been studied according to the concept of liver fibrosis progression. The aim of this work was to assess the fibrosis progression rate in HIV-HCV coinfected patients and in patients infected by HCV only. A cohort of 122 HIV-HCV coinfected patients was compared with a control group of 122 HIV-negative HCV-infected patients. Groups were matched according to age, sex, daily alcohol consumption, age at HCV infection, and duration and route of HCV infection. The fibrosis progression rate was defined as the ratio between fibrosis stage (METAVIR scoring system) and the HCV duration.
