BackgroundThe Pooled Uranium Miners Analysis (PUMA) study draws together information from cohorts of uranium miners from Canada, the Czech Republic, France, Germany and the USA.MethodsVital status and cause of death were ascertained and compared with expectations based upon national mortality rates by computing standardized mortality ratios (SMRs) overall and by categories of time since first hire, calendar period of first employment and duration of employment as a miner.ResultsThere were 51 787 deaths observed among 118 329 male miners [SMR = 1.05; 95% confidence interval (CI): 1.04, 1.06]. The SMR was elevated for all cancers (n = 16 633, SMR = 1.23; 95% CI: 1.21, 1.25), due primarily to excess mortality from cancers of the lung (n = 7756, SMR = 1.90; 95% CI: 1.86, 1.94), liver and gallbladder (n = 549, SMR = 1.15; 95% CI: 1.06, 1.25), larynx (n = 229, SMR = 1.10; 95% CI: 0.97, 1.26), stomach (n = 1058, SMR = 1.08; 95% CI: 1.02, 1.15) and pleura (n = 39, SMR = 1.06; 95% CI: 0.75, 1.44). Lung-cancer SMRs increased with duration of employment, decreased with calendar period and persisted with time since first hire. Among non-malignant causes, the SMR was elevated for external causes (n = 3362, SMR = 1.41; 95% CI: 1.36, 1.46) and respiratory diseases (n = 4508, SMR = 1.32; 95% CI: 1.28, 1.36), most notably silicosis (n = 814, SMR = 13.56; 95% CI: 12.64, 14.52), but not chronic obstructive pulmonary disease (n = 1729, SMR = 0.98; 95% CI: 0.93, 1.02).ConclusionsWhereas there are important obstacles to the ability to detect adverse effects of occupational exposures via SMR analyses, PUMA provides evidence of excess mortality among uranium miners due to a range of categories of cause of death. The persistent elevation of SMRs with time since first hire as a uranium miner underscores the importance of long-term follow-up of these workers.

BackgroundSurvival from metastatic cutaneous melanoma is substantially lower than for localized disease. Treatments for metastatic melanoma have been limited, but remarkable clinical improvements have been reported in clinical trials in the last decade. We described the characteristics of US patients diagnosed with cutaneous melanoma during 2001-2013 and assessed trends in short-term survival for distant-stage disease.MethodsTrends in 1-year net survival were estimated using the Pohar Perme estimator, controlling for background mortality with life tables of all-cause mortality rates by county of residence, single year of age, sex, and race for each year 2001-2013. We fitted a flexible parametric survival model on the log-hazard scale to estimate the effect of race on the hazard of death because of melanoma and estimated 1-year net survival by race.ResultsOnly 4.4% of the 425 915 melanomas were diagnosed at a distant stage, cases diagnosed at a distant stage are more commonly men, older patients, and African Americans. Age-standardized, 1-year net survival for distant-stage disease was stable at approximately 43% during 2001-2010. From 2010 onward, survival improved rapidly, reaching 58.9% (95% confidence interval = 56.6% to 61.2%) for patients diagnosed in 2013. Younger patients experienced the largest improvement. Survival for distant-stage disease increased in both Blacks and Whites but was consistently lower in Blacks.ConclusionsOne-year survival for distant-stage melanoma improved during 2001-2013, particularly in younger patients and those diagnosed since 2010. This improvement may be a consequence of the introduction of immune-checkpoint-inhibitors and other targeted treatments for metastatic and unresectable disease. Persistent survival inequalities exist between Blacks and Whites, suggesting differential access to treatment.

Motivated by recent experimental advances (Stroock et al. 2002) in microfluidic mixers, we study the passive mixing and flow properties of a patterned microchannel by means of computational fluid dynamics (CFD). Such geometries overcome the low Reynolds number, high Peclet number boundaries to efficient mixing by creating a three-dimensional flow that yields chaotic trajectories for advected passive scalars. The flow seems to exploit an effective shearing mechanism, or 'ditch mixing,' rather than the alternating span-wise vorticity mechanism we anticipated. Further, we find superexponential mixing for a modification of the geometry. Leos Janacek (1854-1928) composed the Sinfonietta in the Spring of 1926 (autographed score, unusually, not dated). The work is in five movements and is scored for four flutes (fourth doubles piccolo), 2 oboes (2nd doubles cor anglais), 2 clarinets (2nd doubles clarinet in Eb), bass clarinet, 2 bassoons, 4 horns, 3 trumpets in F, 9 trumpets in C, 4 trombones, tuba , percussion, 2 tenor tubas in 2 bass trumpets, timpani, harp and strings. The orchestration has in itself been subject to some speculation as different publications of the score give variations such as 3 trombones and tuba (Eulenburg) - more usual in Janacek’s orchestral writing - and viola d'amore (as in the MS but later altered to viola) in the third movement (hence the omission of the instruction "con sordini": the first of these points has some impact on part-writing in the second and third movements. Even now speculation surrounds the speed of certain sections in this same movement - of the five main editions (e.g. Eulenburg , Wiener Urtext 1981) indicate specific metronome marks which differ from others (such as the Wiener Urtext (U.E.) 1930 and 1987) and which in many cases were also omitted by Janacek himself in the MS. It is generally accepted that Janacek did not proofread the original publication very closely in 1927 and that consequently a number of inaccuracies were never corrected until many years after his death, if at all. For the purpose of analysis the version used will be the Universal Edition (U.E.) based on the Urtext of 1930, although references will be made where appropriate to anomalies with others, such as the Eulenburg Edition (1979) and to the MS itself, as one or two of these anomalies are quite significant. The work is often presumed to be a single composition of five movements, each of which contains thematic motivic or rhythmic references to one or more of the other movements: the most obvious of these is the re -introduction of the entire first movement during the latter part of the fifth movement, albeit with the restatement being “accompanied” by the full orchestra (which is not used in the first movement at all) and the re-introduction of the nine trumpets in C three bars from the end (a suggestion of Klemperer’s in a letter to Janacek of 19 Sept, 1927 which Janacek appears to have sanctioned, despite the retention - perhaps an oversight on Janacek’s part - of the instruction "(tacet)”, five bars earlier. Historically, however, the first movement was not initially conceived by Janacek in the context of a multi-movement work at all - the MS title is “Fanfary” for the first movement with the remainder of the work being entitled separately as “Sinfonietta vojenska” with programmatic titles about aspects of contemporary life in Brno where Janacek lived. Indeed, it is possible to tell by looking at the first page of the MS of the second movement that the right hand vertical “I” of “II” was added later - the pen stroke is darker and thicker than the left hand stroke. In addition both the fourth and fifth movements in the MS are labelled as IV. The “Fanfary” was intended to be used in the National Games of 1926 in Brno (hence the large number of trumpets suggesting an outdoor performance) and the remaining four movements were added later in the same year, and not a year later as is suggested in "Janacek - Leaves from his Life" edited and translated by Vilem and Margaret Tausky (Kahe and Averill, London 1980, p 159). Consequently, although there are motivic cells which can be said to be common to the first and second movements, each is best analysed separately and with in different parameters.

- Abstract
Purpose: Resective epilepsy surgery is a well-established, evidence-based treatment option in patients with drug-resistant focal epilepsy. A major predictive factor of good surgical outcome is visualization and delineation of a potential epileptogenic lesion by MRI. However, frequently, these lesions are subtle and may escape detection by conventional MRI (≤ 3 T). Methods: We present the EpiUltraStudy protocol to address the hypothesis that application of ultra-high field (UHF) MRI increases the rate of detection of structural lesions and functional brain aberrances in patients with drug-resistant focal epilepsy who are candidates for resective epilepsy surgery. Additionally, therapeutic gain will be addressed, testing whether increased lesion detection and tailored resections result in higher rates of seizure freedom 1 year after epilepsy surgery. Sixty patients enroll the study according to the following inclusion criteria: aged ≥ 12 years, diagnosed with drug-resistant focal epilepsy with a suspected epileptogenic focus, negative conventional 3 T MRI during pre-surgical work-up. Results: All patients will be evaluated by 7 T MRI; ten patients will undergo an additional 9.4 T MRI exam. Images will be evaluated independently by two neuroradiologists and a neurologist or neurosurgeon. Clinical and UHF MRI will be discussed in the multidisciplinary epilepsy surgery conference. Demographic and epilepsy characteristics, along with postoperative seizure outcome and histopathological evaluation, will be recorded. Conclusion: This protocol was reviewed and approved by the local Institutional Review Board and complies with the Declaration of Helsinki and principles of Good Clinical Practice. Results will be submitted to international peer-reviewed journals and presented at international conferences. Trial registration number: www.trialregister.nl: NTR7536.

We describe the results of a test run involving a Transition Radiation Detector that can both distinguish electrons from pions with momenta greater than 0.7 GeV/c and simultaneously track particles passing through the detector. The particle identification is accomplished through a combination of the detection of Transition Radiation from the electron and the differences in electron and pion energy loss (dE/dx) in the detector. The dE/dx particle separation is most efficient below 2 GeV/c while particle ID utilizing Transition Radiation is effective above 1.5 GeV/c. Combined, the electron-pion separation is better than 5 X 10 squared). The single-wire, track-position resolution for the TRD is approximately 230 micrometer.

Objective. Ovarian cancers comprise several histologically distinct tumour groups with widely different prognosis. We aimed to describe the worldwide distribution of ovarian cancer histology and to understand what role this may play in international variation in survival. Methods. The CONCORD programme is the largest population-based study of global trends in cancer survival. Data on 681,759 women diagnosed during 1995–2009 with cancer of the ovary, fallopian tube, peritoneum and retroperitonum in 51 countries were included.We categorised ovarian tumours into six histological groups, and explored the worldwide distribution of histology. Results. During 2005–2009, type II epithelial tumours were the most common. The proportion was much higher in Oceania (73.1%), North America (73.0%) and Europe (72.6%) than in Central and South America (65.7%) and Asia (56.1%). By contrast, type I epithelial tumours were more common in Asia (32.5%), compared with only 19.4% in North America. From 1995 to 2009, the proportion of type II epithelial tumours increased from 68.6% to 71.1%, while the proportion of type I epithelial tumours fell from 23.8% to 21.2%. The proportions of germ cell tumours, sex cord-stromal tumours, other specific non-epithelial tumours and tumours of non-specific morphology all remained stable over time. Conclusions. The distribution of ovarian cancer histology varieswidely worldwide. Type I epithelial, germcell and sex cord-stromal tumours are generally associated with higher survival than type II tumours, so the proportion of these tumours may influence survival estimates for all ovarian cancers combined. The distribution of histological groups should be considered when comparing survival between countries and regions.

Purpose In neuro-oncology, 3 Tesla (3T) MRI is the current clinical standard for tumor localization, radiotherapy volume delineation and stereotactic (radio)surgery. With superior SNR and image resolution, anatomical 7T MRI can visualize micro-vascularization in glioblastomas potentially allowing improved target volume delineation. However, concerns regarding geometrical distortion (GD) with increasing field strength (B0) are detrimental for applications of 7T MRI in image-guided interventions. For high-precision treatment strategies, the spatial integrity of anatomical images needs to be warranted within ±1 mm. The aim of the study was to evaluate B0- and sequence-related GD in clinically relevant 7T pulse sequences and compare it to equivalent 3T sequences, and CT images. Material & Methods To quantify B0- and sequence-related GD in T1-GRE, T1-TFE, T2-TSE, T2-TSE FLAIR on 7T pulse sequences, a dedicated anthropomorphic head-phantom (CIRS Model 603A) was used. The phantom is composed of bone- /soft-tissue equivalent materials and contains a rigid 3D grid (3 mm rods spaced 15 mm apart). System-based distortion correction methods were applied to restore the gradient uniformity for 3T and 7T. For all CT and MR images, 436 points of interests (POIs) were defined by manual reconstruction of the 3D grid points in the respective images. GD was assessed in 3 ways. Firstly, global GD was estimated by the mean absolute difference (MADglobal) between the measured and the true Euclidian distances of all unique combinations of POIs, independent of location within the phantom. Secondly, local GD was estimated by MADlocal between the measured and the true Euclidian distances of all POIs relative to the magnetic field isocenter. Thirdly, a distortion map was created by evaluating 3D displacement vectors for each individual grid point. Results MADglobal in 3T and 7T images ranged from 0.19−0.75 mm and 0.27−1.91 mm, respectively, and was more pronounced than in CT images. - Abstract
Behavioral changes due to human predation can affect the functional role of targeted fishes and has implications for ecosystem and fisheries management. Wariness of targeted fishes towards divers, often measured as flight initiation distance (FID), has been shown to increase in areas where spear fishing pressure is higher. Additional research is required to understand how these patterns vary among different species and places. To support such studies, there is a need to explore the relationship of approach starting distance (SD) and other variables that influence measures of fish wariness. We used stereo video to record FID, a new direct measure of minimum approach distance (dMAD), and escape responses of targeted reef fishes in a marine reserve and heavily fished area in Hawai'i. We investigated the role of SD and 15 other variables in influencing FID and dMAD and tested differences between the reserve and fished area. SD explained a large portion of the variability in FID and somewhat less variability in dMAD. FID and dMAD were higher when fishes were travelling (vs feeding), increased with fish body length, and decreased with depth. When including these co-variates in models of FID and dMAD of targeted reef fishes, we found significant differences between study sites for dMAD, though not for FID. We also found differences in escape responses between sites. Based on these results, we recommend using stereo video to conduct measures of dMAD as a complement or alternative to FID, while simultaneously measuring SD and collecting additional data such as escape response. Relative effect sizes of SD and, to a lesser extent, other co-variates were large in comparison to site. Studies that use similar methods and do not control for or incorporate SD may produce biased results with regard to the effects of fishing pressure on flight behavior of targeted fishes.

OBJECTIVE: Ovarian cancers comprise several histologically distinct tumour groups with widely different prognosis. We aimed to describe the worldwide distribution of ovarian cancer histology and to understand what role this may play in international variation in survival. METHODS: The CONCORD programme is the largest population-based study of global trends in cancer survival. Data on 681,759 women diagnosed during 1995-2009 with cancer of the ovary, fallopian tube, peritoneum and retroperitonum in 51 countries were included. We categorised ovarian tumours into six histological groups, and explored the worldwide distribution of histology. RESULTS: During 2005-2009, type II epithelial tumours were the most common. The proportion was much higher in Oceania (73.1%), North America (73.0%) and Europe (72.6%) than in Central and South America (65.7%) and Asia (56.1%). By contrast, type I epithelial tumours were more common in Asia (32.5%), compared with only 19.4% in North America. From 1995 to 2009, the proportion of type II epithelial tumours increased from 68.6% to 71.1%, while the proportion of type I epithelial tumours fell from 23.8% to 21.2%. The proportions of germ cell tumours, sex cord-stromal tumours, other specific non-epithelial tumours and tumours of non-specific morphology all remained stable over time. CONCLUSIONS: The distribution of ovarian cancer histology varies widely worldwide. Type I epithelial, germ cell and sex cord-stromal tumours are generally associated with higher survival than type II tumours, so the proportion of these tumours may influence survival estimates for all ovarian cancers combined. The distribution of histological groups should be considered when comparing survival between countries and regions.
