4. Risk Factors for Extubation Failure After Pediatric Cardiac Surgery and Impact on Outcomes: A Multicenter Analysis.

Abstract

IMPORTANCE: Extubation failure (EF) after pediatric cardiac surgery is associated with increased morbidity and mortality. OBJECTIVES: We sought to describe the risk factors associated with early (< 48 hr) and late (48 hr ≤ 168 hr) EF after pediatric cardiac surgery and the clinical implications of these two types of EF. DESIGN SETTING AND PARTICIPANTS: Retrospective cohort study using prospectively collected clinical data for the Pediatric Cardiac Critical Care Consortium (PC4) Registry. Pediatric patients undergoing Society of Thoracic Surgeons benchmark operation or heart transplant between 2013 and 2018 available in the PC4 Registry were included. MAIN OUTCOMES AND MEASURES: We analyzed demographics and risk factors associated with EFs (primary outcome) including by type of surgery. We identified potentially modifiable risk factors. Clinical outcomes of mortality and length of stay (LOS) were reported. RESULTS: Overall 18,278 extubations were analyzed. Unplanned extubations were excluded from the analysis. The rate of early EF was 5.2% (948) and late EF was 2.5% (461). Cardiopulmonary bypass time, ventilator duration, airway anomaly, genetic abnormalities, pleural effusion, and diaphragm paralysis contributed to both early and late EF. Extubation during day remote from shift change and nasotracheal route of initial intubation was associated with decreased risk of early EF. Extubation in the operating room was associated with an increased risk of early EF but with decreased risk of late EF. Across all operations except arterial switch, EF portrayed an increased burden of LOS and mortality. CONCLUSION AND RELEVANCE: Both early and late EF are associated with significant increase in LOS and mortality. Study provides potential benchmarking data by type of surgery. Modifiable risk factors such as route of intubation, time of extubation as well as treatment of potential contributors such as diaphragm paralysis or pleural effusion can serve as focus areas for reducing EFs.

Abstract

Person re-identification (re-ID) is of great importance to video surveillance systems by estimating the similarity between a pair of cross-camera person shorts. Current methods for estimating such similarity require a large number of labeled samples for supervised training. In this paper, we present a pseudo-pair based self-similarity learning approach for unsupervised person re-ID without human annotations. Unlike conventional unsupervised re-ID methods that use pseudo labels based on global clustering, we construct patch surrogate classes as initial supervision, and propose to assign pseudo labels to images through the pairwise gradient-guided similarity separation. This can cluster images in pseudo pairs, and the pseudos can be updated during training. Based on pseudo pairs, we propose to improve the generalization of similarity function via a novel self-similarity learning:it learns local discriminative features from individual images via intra-similarity, and discovers the patch correspondence across images via inter-similarity. The intra-similarity learning is based on channel attention to detect diverse local features from an image. The inter-similarity learning employs a deformable convolution with a non-local block to align patches for cross-image similarity. 18. Demand and Influencing Factors for Community Health Services during Chemotherapy of Patients with Advanced Cancer

Abstract

Background How to implement effective and reasonable support and comprehensive services for patients with advanced cancer during chemotherapy is an issue worthy of attention in community health services at present. According to the latest data, the mortality rate of malignant tumors among residents in Shanghai has become the second place after cardiovascular and cerebrovascular diseases. The current attention to patients with advanced cancer during chemotherapy is also gradually increasing. Therefore, this study tries to understand the demand for primary care among such patients and the influencing factors. Objective To investigate the demand and influencing factors for community health services during chemotherapy of partial patients with advanced cancer in Shanghai, and to explore how to develop community interventions and services to address the demand of such patients. Methods Patients with advanced cancer during chemotherapy who regularly visited or hospitalized at Shanghai Ninth People's Hospital, Wusong Hospital and Shanghai Baoshan Hospital of Integrated Traditional Chinese and Western Medicine from December 2021 to March 2022 were selected as the study subjects. Based on previous relevant questionnaires, government specifications and interview results, the final version of the demand for community health services of partial patients with advanced cancer during chemotherapy in Shanghai was formed, with a total of 3 demand dimensions (psychological, medical care, social support) and 38 demand items. The contents include patients' general information (demographic and sociological information: gender, age, religious beliefs, education level, marital status, source of medical expenses, disposable monthly household income, whether to participate in a patient group; tumor diagnosis: type and time of diagnosis, pain score, combining with other diseases), 6 items of psychological and psychological demand, 24 items of medical care demand, and 8 items of social support demand. A 3-point scale was adopted: 1 was unnecessary, 2 was necessary, and 3 was very necessary. The higher the score, the higher the degree of patients' demand, which was ranked according to the average score of each demand item. Univariate and multivariate Logistic regression analyses were used to explore the influencing factors of the demand for community health services of patients with advanced cancer during chemotherapy. Results The average scores of each demand dimension were ranked in order of psychological demand (2.31 points), medical care demand (2.27 points) and social support demand (2.18 points). The top five average scores of all demand items were "preparation of biochemical tests such as routine blood, liver and kidney functions before chemotherapy" (2.48 points), "education on chemotherapy knowledge" (2.48 points), "care of PICC catheterization during chemotherapy" (2.45 points), "management of myelosuppression after chemotherapy" (2.43 points) " and "providing of updated information on treatment, examination and rehabilitation" (2.42 points), mainly focus on the medical care demand dimension. Abstract

Our institution developed and continuously improved a Neurodevelopmental Reflex (NDR) algorithm to help physicians with genetic test ordering for neurodevelopmental disorders (NDDs). To assess its performance, we performed a retrospective study of 511 patients tested through NDR from 2018 to 2019. SNP Microarray identified pathogenic/likely pathogenic copy number variations in 27/511 cases (5.28%). Among the 484 patients tested for Fragile X FMR1 CGG repeats, a diagnosis (0.20%) was established for one male mosaic for a full mutation, a premutation, and a one-CGG allele. Within the 101 normocephalic female patients tested for "MECP2," two patients were found to carry pathogenic variants (1.98%). This retrospective study suggested the NDR algorithm effectively established diagnoses for patients with NDDs with a yield of 5.87%.

Abstract

Light field modulation matrix is closely related to the quality of reconstructed image in ghost imaging. The orthogonality light field modulation matrix with better noise immunity and high quality reconstructed image is urgently needed in the practical application of ghost imaging. In this work, we propose a Gold matrix ghost imaging method with excellent imaging and anti-noise performance, which is proud of the pseudo-randomness and orthogonality of the deterministic Gold matrix. Numerical simulation and experimental results show that the Gold matrix has the characteristics of both random speckle and Hadamard matrix, and the reconstructed images are superior to the two modulation matrixes under the condition of noise, which shows better imaging robustness., Comment: 9 pages, 7 figures
