We considered two distinct AAA models, namely, an idealized model and a patient-specific one under different physiological flow and pressure conditions.ResultsThe efficacy of the proposed setup for the investigation of AAA hemodynamics was confirmed by observing velocity and vorticity fields across multiple flow rate scenarios and regions of interest.ConclusionThe findings of this study underscore the potential impact of Hybrid-MCL integrated with a LED-PIV system on enhancing the affordability, accessibility, and safety of in-vitro CVD investigations.

Background: When treating aortic aneurysm patients with complex anatomical features, preprocedural planning aided by 3D-printed models offers valuable insights for endovascular intervention. This study highlights the use of stereolithographic (SLA) 3D printing to fabricate a phantom of a challenging aortic arch aneurysm with a complex neck anatomy. Clinical Case: A 75-year-old female presented with a 58 mm descending thoracic aortic aneurysm (TAA) extending to the distal arch, involving the left subclavian artery (LSA) and the left common carotid artery (LCCA). The computed tomography (CT) scans underwent scrutiny by radiology and vascular teams. Nevertheless, the precise spatial relationships of the ostial origins proved to be challenging to ascertain. To address this, a patient-specific phantom of the aortic arch was fabricated utilizing an SLA printer and a biomedical resin. The thoracic endovascular aortic repair (TEVAR) procedure was simulated using fluoroscopy on the phantom to enhance procedural preparedness. Subsequently, the patient underwent a right carotid-left carotid bypass and a right carotid-left subclavian bypass. After a 24-hour interval, the patient underwent the TEVAR procedure, during which a 37 mm × 150 mm stent graft (CTAG, WL Gore and Associates, Flagstaff, AZ, USA) and a 40 mm × 200 mm stent graft (CTAG, WL Gore and Associates, Flagstaff, AZ, USA) were deployed, effectively covering the LSA and LCCA. Notably, the aneurysm exhibited complete sealing, with no indications of endoleaks or graft infoldings. At the 12-month follow-up, the patient remains in good health, with no evidence of endoleaks or any other surgery-related complication. Conclusion: This report showcases the successful use of a 3D-printed endovascular phantom in guiding the decision-making process during the preparation for a TEVAR procedure. The correct estimation of the distensibility of deformable aorta replicas is a challenging issue, in particular when its local characterization is necessary. We propose a combined in-vitro and in-silico approach to face this problem. First, we tested an aortic silicone arch in a pulse-duplicator analyzing its dynamics under physiological working conditions. The aortic flow rate and pressure were measured by a flow meter at the inlet and two probes placed along the arch, respectively. Video imaging analysis allowed us to estimate the outer diameter of the aorta in some sections in time. Second, we replicated the in-vitro experiment through a Fluid-Structure Interaction simulation. Observed and computed values of pressures and variations in aorta diameters, during the cardiac cycle, were compared. Results were considered satisfactory enough to suggest that the estimation of local distensibility from in-silico tests is reliable, thus overcoming intrinsic experimental limitations. The aortic distensibility (AD) is found to vary significantly along the phantom by ranging from 3.0 × 10−3 mmHg−1 in the ascending and descending tracts to 4.2 × 10−3 mmHg−1 in the middle of the aortic arch. Interestingly, the above values underestimate the AD obtained in preliminary tests carried out on straight cylindrical samples made with the same material of the present phantom. Hence, the current results suggest that AD should be directly evaluated on the replica rather than on the samples of the adopted material. Summary: Background: Evaluation of arterial deformation and mechanics using strain analysis on ultrasound greyscale images has gained increasing scientific interest. The aim of this study was to validate in vitro measurements of circumferential strain by velocity vector imaging (VVI) and speckle‐tracking echocardiography (STE) against sonomicrometry as a reference method. Method: Two polyvinyl alcohol phantoms sized to mimic the descending aorta were constructed and connected to a pulsatile flow pump to obtain high‐resistance flow profiles. The ultrasound images of the phantom used for strain analyses were acquired with a transesophageal probe. Global and regional circumferential strains were estimated using VVI and STE and were compared with the strain acquired by sonomicrometry. Results: Global circumferential peak strain estimated by VVI and STE correlated well to sonomicrometry (r = 0·90, P≤0·001; and r = 0·97, P≤0·01) with a systematic bias of −0·78% and +0·63%, respectively. The reference strain levels were 1·07–2·54%. Circumferential strain values obtained by VVI were significantly lower than those obtained by STE (bias −1·41%, P≤0·001). Conclusion: Global circumferential strain measured by VVI and STE correlates well with sonomicrometry. However, strain values obtained by VVI and STE differ significantly, which should be taken into consideration when comparing results from studies using different software for aortic strain measurements. Purpose: To design and characterize a magnetic resonance imaging (MRI)-compatible aortic phantom simulating normal and aortic coarctation (AoCo) conditions and to compare its hemodynamics with healthy volunteers and AoCo patients.Materials and Methods: The phantom is composed of an MRI-compatible pump, control unit, aortic model, compliance chamber, nonreturn, and shutoff valves. The phantom without and with AoCo (13, 11, and 9 mm) was studied using 2D and 3D phase-contrast data and with a catheterization unit to measure pressures. The phantom data were compared with the mean values of 10 healthy volunteers and two AoCo patients.Results: Hemodynamic parameters in the normal phantom and healthy volunteers were: heart rate: 68/61 bpm, cardiac output: 3.5/4.5 L/min, peak flow and peak velocity (Vpeak) in the ascending aorta (AAo): 270/357 mL/s (significantly, P < 0.05) and 97/107 cm/s (not significantly, P = 0.16), and pressure in the AAo of the normal phantom of 131/58 mmHg. Hemodynamic parameters in the 13, 11, and 9 mm coarctation phantoms and Patients 1 and 2 were: heart rate: 75/75/75/97/78 bpm, cardiac output: 3.3/3.0/2.9/4.0/5.8 L/min, peak flow in the AAo: 245/265/215/244/376 mL/s, Vpeak in the AAo: 96/95/81/196/187 cm/s, Vpeak after the AoCo: 123/187/282/247/165 cm/s, pressure in the AAo: 124/56, 127/51, 133/50, 120/51 and 87/39 mmHg, and a trans-coarctation systolic pressure gradient: 7, 10, 30, 20, and 11 mmHg.Conclusion: We propose and characterize a normal and an AoCo phantom, whose hemodynamics, including velocity, flow, and pressure data are within the range of healthy volunteers and patients with AoCo. The aim of the present study was to define biomechanical parameters of the healthy human abdominal aorta, usable to develop materials for the aortic phantom production. Such phantoms used in the training of endovascular treatment must describe the same morphology and mechanical behavior properties as the patient's aorta. To accurately identify these biomechanical parameters, ex vivo experiments in uniaxial tensile and dynamic simple shear tests were performed on six human healthy abdominal aortas (6 males, between 12 and 69 years old). A solid generalized Maxwell model including Yeoh expression for the elastic part was used to describe the hyper-viscoelastic behavior of the aorta. The results obtained from uniaxial tensile tests show an exponential-like increase in stiffness, which can be described by three hyperelastic parameters ( C 1 , C 2 and C 3 ). From dynamic shear experiments, the viscous part of the global biomechanical behavior was expressed in a specific angular-frequency range (1 to 315 rad/s). Three Maxwell elements ( β 1 , β 2 , and β 3 ) put on three constant times ( τ 1 = 0.003 s , τ 2 = 0.03 s , and τ 3 = 0.3 s ) respectively, were necessary to describe it. As this relatively high number of viscoelastic parameters may be difficult to control in the development of materials, we suggest defining the viscous behavior with the global viscosity η 0 that combines the viscoelastic contributions of each Maxwell element. In conclusion, four biomechanical parameters: C 1 , C 2 , C 3 and η 0 , must be considered for the development of materials used in the aortic phantom production. [ABSTRACT FROM AUTHOR]

Purpose: To describe rapid prototyping or 3-dimensional (3D) printing of aneurysms with complex neck anatomy to facilitate endovascular aneurysm repair (EVAR). Case Report: A 75-year-old man had a 6.6-cm infrarenal aortic aneurysm that appeared on computed tomographic angiography to have a sharp neck angulation of ∼90°. However, although the computed tomography (CT) data were analyzed using centerline of flow, the true neck length and relations of the ostial origins were difficult to determine. No multidisciplinary consensus could be reached as to which stent-graft to use owing to these borderline features of the neck anatomy. Based on past experience with rapid prototyping technology, a decision was taken to print a model of the aneurysm to aid in visualization of the neck anatomy. The CT data were segmented, processed, and converted into a stereolithographic format representing the lumen as a 3D volume, from which a full-sized replica was printed within 24 hours. The model demonstrated that the neck was adequate for stent-graft repair using the Aorfix device. Conclusion: Rapid prototyping of aortic aneurysms is feasible and can aid decision making and device delivery. Further work is required to test the value of 3D replicas in planning procedures and their impact on procedure time, radiation dose, and procedure cost. [ABSTRACT FROM AUTHOR]
