PurposeIn 2023 the American Urological Association (AUA) requested an Update Literature Review (ULR) to incorporate new evidence generated since the 2019 publication of this Guideline. The resulting 2024 Guideline Amendment addresses updated recommendations to provide guidance for the care of patients with incontinence after prostate treatment (IPT).Materials and methodsIn 2023, the IPT Guideline was updated through the AUA amendment process in which newly published literature is reviewed and integrated into previously published guidelines. There were 82 studies of interest initially identified in preliminary abstract review. Following full-text review, 17 studies met inclusion criteria and ultimately informed the statements of interest.ResultsThe Panel developed evidence- and consensus-based statements based on an updated review to provide guidance for the care of patients who experience IPT. These updates are detailed herein.ConclusionsAs prostate treatments are refined, a decreasing incidence of incontinence is anticipated. This Guideline will require further review as the diagnostic and treatment options for patients with IPT continue to evolve.

BACKGROUND: Age is the greatest risk factor for lower urinary tract symptoms attributed to benign prostatic hyperplasia (LUTS/BPH). Although LUTS/BPH can be managed with pharmacotherapy, treatment failure has been putatively attributed to numerous pathological features of BPH (eg, prostatic fibrosis, inflammation). Mitochondrial dysfunction is a hallmark of aging; however, its impact on the pathological features of BPH remains unknown. METHODS: Publicly available gene array data were analyzed. Immunohistochemistry examined mitochondrial proteins in the human prostate. The effect of complex I inhibition (rotenone) on a prostatic cell line was examined using quantitative polymerase chain reaction, immunocytochemistry, and Seahorse assays. Oleic acid (OA) was tested as a bypass of complex I inhibition. Aged mice were treated with OA to examine its effects on urinary dysfunction. Voiding was assessed longitudinally, and a critical complex I protein measured. RESULTS: Mitochondrial function and fibrosis genes were altered in BPH. Molecular profiling of clear cell renal cell carcinoma (ccRCC) tumors of patients in a clinical trial has identified distinct transcriptomic signatures with predictive value, yet data in non-clear cell variants (nccRCC) are lacking. We examined the transcriptional profiles of RCC tumors representing key molecular pathways, from a multi-institutional, real-world patient cohort, including ccRCC and centrally reviewed nccRCC samples. ccRCC had increased angiogenesis signature scores compared with the heterogeneous group of nccRCC tumors, while cell cycle, fatty acid oxidation/AMPK signaling, and fatty acid synthesis/pentose phosphate signature scores were increased in one or more nccRCC subtypes. Among both ccRCC and nccRCC tumors, T effector scores statistically correlated with increased immune cell infiltration and were more commonly associated with immunotherapy-related markers (PD-L1+/TMBhi/MSIhi). In conclusion, this study provides evidence of differential gene transcriptional profiles among ccRCC versus nccRCC tumors, providing insights for optimizing personalized and histology-specific therapeutic strategies for patients with advanced RCC.

IntroductionUrology residency prepares trainees for independent practice. The optimal operative chief resident year experience to prepare for practice is undefined. We analyzed the temporal arc of cases residents complete during their residency compared to their chief year in a multi-institutional cohort.MethodsAccreditation Council for Graduate Medical Education case logs of graduating residents from 2010 to 2022 from participating urology residency programs were aggregated. Resident data for 5 categorized index procedures were recorded: (1) general urology, (2) endourology, (3) reconstructive urology, (4) urologic oncology, and (5) pediatric urology. Interactions were tested between the trends for total case exposure in residency training relative to the chief resident year.ResultsFrom a sample of 479 resident graduates, a total of 1,287,433 total cases were logged, including 375,703 during the chief year (29%). Urologic oncology cases had the highest median percentage completed during chief year (56%) followed by reconstructive urology (27%), general urology (24%), endourology (17%), and pediatric urology (2%). Across the study period, all categories of cases had a downward trend in median percentage completed during chief year except for urologic oncology. However, only trends in general urology (slope of -0.68, P = .013) and endourology (slope of -1.71, P ≤ .001) were significant.ConclusionsOver 50% of cases completed by chief residents are urologic oncology procedures. Current declining trends indicate that residents are being exposed to proportionally fewer general urology and endourology cases during their chief year prior to entering independent practice.

Optimal ergonomics are essential to improving clinical performance and longevity among urologists, as poor ergonomics can contribute to work-related injury and physician burnout. While a majority of urologists experience muscular injury throughout their career, women and trainees are disproportionately affected. These disparities are exacerbated by the lack of formal ergonomics education within urologic training programs. This review provides an overview of practical approaches to optimize ergonomics across working environments for urologists and trainees. We highlight intraoperative techniques and novel devices which have been shown to reduce work-related injury, and we identify knowledge gaps to guide future areas of ergonomic research.

INTRODUCTION: Laparoscopic surgery requires significant training, and prior studies have shown that surgical residents lack key laparoscopic skills. Many educators have implemented simulation curricula to improve laparoscopic training. Given limited time for dedicated, in-person simulation center practice, at-home training has emerged as a possible mechanism by which to expand training and promote practice. There remains a gap in published at-home laparoscopic curricula employing embedded feedback mechanisms. METHODS: We developed a nine-task at-home laparoscopic curriculum and an end-of-curriculum assessment following Kerns six-step approach. We implemented the curriculum over 4 months with first- to third-year residents. RESULTS: Of 47 invited residents from general surgery, obstetrics/gynecology, and urology, 37 (79%) participated in the at-home curriculum, and 25 (53%) participated in the end-of-curriculum assessment. Residents who participated in the at-home curriculum completed a median of six of nine tasks (interquartile range: 3-8). Twenty-two residents (47%) responded to a postcurriculum survey. Of these, 19 (86%) reported that their laparoscopic skills improved through completion of the curriculum, and the same 19 (86%) felt that the curriculum should be continued for future residents. Residents who completed more at-home curriculum tasks scored higher on the end-of-curriculum assessment (p = .009 with adjusted R 2 of .28) and performed assessment tasks in less time (p = .004 with adjusted R 2 of .28). DISCUSSION: This learner-centered laparoscopic curriculum provides guiding examples, spaced practice, feedback, and graduated skill development to enable junior residents to improve their laparoscopic skills in a low-stakes, at-home environment.

To produce a bibliographic source by compiling the most-cited articles about stress urinary incontinence to guide researchers for novel research topic ideas in urogynecology. We analyzed 200 most-cited articles (T200) between 1995–2022 by reviewing the Institution for Scientific Information (ISI) − Web of Science (WOS) and PubMed databases about the stress urinary incontinence. The time span-effect of publications was normalized by calculating average citations per item (ACI) index for each article. The publications were sub-classified by the publication year, journals, countries, surgical methods, and departments. The median for total citation numbers and the ACI indices of T200 were 35 and 5 respectively. The top-cited articles were mostly published in USA with 91 articles (46.5 %) followed by Italy (22, 11 %), and United Kingdom (21, 10.5 %). Among T200, 149 were original research, 44 reviews, 4 meta -analyses, and 3 editorial articles. Further, 59 were about surgical procedures (of which 28 were about TOT-TVT comparison). M. Serati and L. Bruker were the most contributing authors to T200 with 19 articles. On the top of T200, the article of Pierre A. Clavien et al. (2009) sits with 6201 citations, published in the journal of "Annals of Surgery". We suggest that the most-cited articles commonly conducted in the high-income countries, mostly published between 2014–2018. Best of our knowledge, this is the first study analyzing the most-cited 200 publications in urogynecology field under two different perspectives of two different disciplines, namely urology and obstetrics and gynecology. The article presents the case study of a 13-year-old boy diagnosed with metastatic embryonal rhabdomyosarcoma (RMS) of the prostate after presenting with urinary symptoms. Topics include the symptoms and initial findings, such as weakened urine stream, urinary frequency, and hematuria; the types and risk factors of RMS; and the age-related incidence pattern of RMS, which peaks in early childhood and adolescence.

It was a systematic review and meta-analysis that aimed to compare the efficiency and safety of robot-assisted urological surgery using both the KangDuo and da Vinci robotic systems. The PubMed, Embase, and Cochrane Library databases were searched for all papers published through September 1, 2024. The focus was on English-language papers comparing the KangDuo surgical robot and the da Vinci system in urological procedures. The screening method focused on RCTs and cohort studies and followed strict criteria. Three cohort studies and two randomised controlled trials with 300 adult urological surgery patients met the inclusion criteria. These 150 patients were operated on with the KangDuo robotic system and 150 with the da Vinci system. The investigation showed that whereas KangDuo operations were slightly longer, intraoperative blood loss, hospital stay, and postoperative complication rates were similar. The KangDuo robotic system performs urological surgery as well as the da Vinci system, although it takes longer. Urological cancers including those of the prostate, bladder, and kidney, are prevalent and often lethal malignancies besides other less common ones like testicular and penile cancers. Current treatments have major limitations like side effects, recurrence, resistance, high costs, and poor quality of life. Nanotechnology offers promising solutions through enhanced diagnostic accuracy, targeted drug delivery, controlled release, and multimodal imaging. This review reflects clinical challenges and nanomedical advances across major urological cancers. In prostate cancer, nanoparticles improve delineation and radiosensitization in radiation therapy, enable fluorescent guidance in surgery, and enhance chemotherapy penetration in metastatic disease. Nanoparticles also overcome bladder permeability barriers to increase the residence time of intravesical therapy and chemotherapy agents. In renal cancer, nanocarriers potentiate tyrosine kinase inhibitors and immunotherapy while gene vectors and zinc oxide nanoparticles demonstrate antiproliferative effects. Across modalities, urological applications of nanomedicine include polymeric, liposomal, and metal nanoparticles for targeted therapy, prodrug delivery, photodynamic therapy, and thermal ablation. Biosafety assessments reveal favorable profiles but clinical translation remains limited, necessitating further trials. In conclusion, nanotechnology holds significant potential for earlier detection, precise intervention, and tailored treatment of urological malignancies, warranting expanded research to transform patient outcomes. Background: Chat Generative Pre-Trained Transformer (ChatGPT) has previously been shown to accurately predict colon cancer screening intervals when provided with clinical data and context in the form of guidelines. The National Comprehensive Cancer Network® (NCCN®) guideline on non-muscle invasive bladder cancer (NMIBC) includes criteria for risk stratification into low-, intermediate-, and high-risk groups based on patient and disease characteristics. The aim of this study is to evaluate the ability of ChatGPT to apply the NCCN Guidelines to risk stratify theoretical patient scenarios related to NMIBC. Methods: Thirty-six hypothetical patient scenarios related to NMIBC were created and submitted to GPT-3.5 and GPT-4 at two separate time points. First, both models were prompted to risk stratify patients without any additional context provided. Custom instructions were then provided as textual context using the written versions of the NMIBC NCCN® Guidelines, followed by repeat risk stratification. Finally, GPT-4 was provided with an image of the NMIBC risk groups table, and the risk stratification was again performed. Results: GPT-3.5 correctly risk stratified 68% (24.5 of 36) of scenarios without context, slightly increasing to 74% (26.5 of 36) with textual context. Using GPT-4, the model had accuracy of 83% (30 of 36) without context, reaching 100% (36 of 36) with textual context (P =.025). GPT-4 with image context maintained similar accuracy to GPT-4 without context, with accuracy 81% (29 of 36). ChatGPT generally performed poorly when stratifying intermediate risk NMIBC (33%-63%). When risk stratification was incorrect, most responses were overestimations of risk. Conclusions: GPT-4 can accurately risk stratify patients with respect to NMIBC when provided with context containing guidelines. Overestimation of risk is more common than underestimation, and intermediate risk NMIBC is most likely to be incorrectly stratified. Background: Erectile dysfunction (ED) is a prevalent condition in urology, and studies on the effectiveness of platelet-rich plasma (PRP) for this condition have been conducted; however, the evidence remains inconclusive. This meta-analysis aimed to evaluate the effectiveness of PRP in treating ED. Methods: On May 17, 2024, a literature search was performed and evaluated using the Cochrane method. The primary outcome measured was the International Index of Erectile Function (IIEF) score, while the secondary outcomes included Minimal Clinically Important Difference (MCID) and peak systolic velocity (PSV). Results: A total of 12 controlled trials involving 991 patients and 11 single-arm trials with 377 patients were analyzed. The findings revealed that compared to the control group, the PRP group demonstrated better outcomes in terms of the IIEF score and MCID (SMD = 0.59 (95% CI: [0.34, 0.84]; RR = 1.94 (95% CI: [1.33, 2.83]), In the single-arm trials, a significant improvement in IIEF scores was observed following PRP treatment (SMD = -0.99 95% CI: [-1.53, 0.46]). Conclusion: PRP appears effective in treating erectile dysfunction, but further high-quality, large-sample trials with longer follow-up are needed to fully understand its effects. Background: Malignant ureteral obstruction (MUO) is a serious health condition in which a malignant tumor compresses the ureter. The optimal decompressive intervention in MUO remains unclear. This study was conducted to assess and compare renal function, the occurrence of ureterohydronephrosis (UHN), intraoperative, and postoperative complications among patients with MUO who underwent double J stenting (DJS) and percutaneous nephrostomy (PCN) in the Palestinian practice. Methods: This study was conducted in retrospective design in one of the main tertiary care hospitals in the West Bank of Palestine. The data were collected from the electronic health information system of the hospital for the patients with MUO who received either DJS or PCN as a decompressive intervention from January 2018 to January 2024. Results: In this retrospective analysis, 62 patients who had stage 2 to stage 4 cancer and suffered MUO were included. The mean age of the patients was 60.8 ± 13.6 years. Of the patients, 40 (64.5%) were male and 22 (35.5%) were female. Of the patients, 26 (41.9%) had urinary bladder cancer. Of the patients, 23 (37.1%) had flank pain and 16 (25.8%) had lower urinary tract symptoms. Of the patients, 34 (54.8%) experienced bilateral UHN and 28 (45.2%) experienced unilateral UHN. In this study, 43 patients (69.4%) received PCN, and 19 (30.6%) received DJS as a decompressive intervention. Of the patients, 36 (58.1%) suffered postoperative complications. Stent migration/slip, UTIs, and urosepsis were the most commonly reported postoperative complications. There were no statistically significant differences in the occurrence of intraoperative complications, postoperative complications, time elapsed from receiving the decompression intervention to the diagnosis of complications, ICU admission, prognosis of UHN, serum creatinine, and serum BUN between both decompressive interventions. Conclusion: Despite improvements in renal functions, creatinine and BUN levels remained abnormal even after receiving a decompressive intervention. Postoperative complications were frequently reported among patients who received DJS or PCN as decompressive interventions. Larger prospective studies are still needed to determine the optimal interventions to improve outcomes, quality of life, and survival rates of patients with DJS or PCN. [ABSTRACT FROM AUTHOR]

The genetic landscape of urologic cancers has evolved with the identification of actionable mutations that impact diagnosis, prognosis, and therapeutic strategies. This narrative review consolidates existing literature on genetic mutations across key urologic cancers, including bladder, renal, prostate, upper tract urothelial, testicular, and penile. The review highlights mutations in DNA damage repair genes, such as BRCA1/2 and PTEN, as well as pathway alterations like FGFR and PD-L1 overexpression. These mutations influence tumor behavior and therapeutic outcomes, emphasizing the need for precision oncology approaches. Molecular profiling, through tools like next-generation sequencing, has revolutionized patient care by enabling targeted treatment strategies, especially in cancers with distinct molecular subtypes such as luminal or basal bladder cancer and clear cell renal carcinoma. Emerging therapies, including FGFR inhibitors and immune checkpoint blockade, offer new treatment avenues, although resistance mechanisms remain a challenge. We also emphasize the importance of biomarker identification for personalized management, especially in metastatic settings where treatment intensification is often required. Future research is needed to further elucidate our understanding of the genetics affecting urologic cancers, which will help develop novel, individualized therapies to enhance oncologic outcomes. Introduction and Objective: As part of the Endourological Society's (ES) initiative to continuously enhance the field of endourology, the second annual census was circulated after the World Congress of Endourology and Uro-Technology 2022 (WCET22). Methods: An anonymous survey was created using Qualtrics XM and was disseminated via email to all ES members (n = 1502) between October 4, 2022, and January 26, 2023. A total of 46 questions were included in the survey and covered different aspects, including demographics, practice patterns, satisfaction, impact of COVID-19, WCET22 attendance, and future opportunities. Results: A total of 404 (26.9%) ES members (91.8% male and 8.2% female), representing 63 different countries, participated in the survey. Fellowship-trained endourologists constituted 58.9% of respondents, and the most common practice setting was academic (55.2%). The most common practice scope was complex retrograde endoscopy (83.4%), followed by percutaneous nephrolithotomy (79.5%) and medical management of urolithiasis (72.5%). Work schedules were variable, with 51.1% working 40–60 hours/week and 35.3% working >60 hours/week. More than 80% were satisfied with t heir practice; however, 42.4% indicated that COVID-19 made satisfaction worse. Of the participants, 49.5% were satisfied with their compensation, and 7.3% plan to retire within the next 5 years. When asked about the future of endourology, 92.9% had a positive outlook. Of the respondents, only 36.8% attended WCET22, with the most chosen reason for attendance being an interest in learning new research and technology. For lack of attendance, the cost of travel and lodging was reported as a determining factor by 45.4%. Conclusion: These survey results report important trends within the field of endourology and demonstrate the robust outlook of ES members for the future. Background: Surgical bootcamps provide hands‐on learning opportunities for junior doctors and trainees in low‐pressure, non‐clinical environments. Participants develop skills, knowledge, and confidence in preparation for their registrar years. A number of international urology‐specific bootcamps have demonstrated improvements in these technical and non‐technical domains. However, there was no such opportunity for Australian doctors. In the first urology bootcamp in Australia, we aimed to assess participants' knowledge, technical skills, and confidence in managing urological conditions pre‐ and post‐bootcamp. Methods: Twenty‐five early‐career medical officers attended the bootcamp and completed a comprehensive written survey at the beginning and end of the bootcamp. Results: Results showed that self‐perceptions of knowledge, confidence, and technical skill all improved at the conclusion of the bootcamp (P = 0.01). All participants reported the bootcamp to be 'useful' or 'very useful' in furthering their urological training. Conclusion: This study demonstrates the success of an inaugural Australian urology bootcamp in preparing junior doctors and unaccredited registrars for their registrar training years. Several multimedia tools can be used to improve patients' understanding of urological conditions and procedures, such as simulation and models. Use of these aids for preoperative discussion enhances knowledge and patient satisfaction, which can lead to more realistic patient expectations and better informed consent. Discussions surrounding urological diagnoses and planned procedures can be challenging, and patients might experience difficulty in understanding the medical language, even when shown radiological imaging or drawings. With the introduction of virtual reality and simulation, informed consent could be enhanced by audiovisual content and interactive platforms. Our aim was to assess the role of enhanced consent in the field of urology. A systematic review of the literature was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines, using informed consent, simulation, and virtual reality in urology as the search terms. All original articles were screened. Thirteen original studies were included in the review. The overall quality of these studies was deemed good according to the Newcastle-Ottawa Scale. The studies analysed the application of different modalities for enhanced consent: 3D printed or digital models, audio visual multimedia contents, virtual simulation of procedures and interactive navigable apps. Published studies agreed upon a significantly improved effect on patient understanding of the diagnosis, including basic anatomical details, and surgery-related issues such as the aim, steps and the risks connected to the planned intervention. Patient satisfaction was unanimously reported as improved as a result of enhanced consent. Simulation and multimedia tools are extremely valuable for improving patients' understanding of and satisfaction with urological procedures. Objectives: The aim of the current review was to elucidate the clinical context and presentation of sexual dysfunction (SD) and gynecomastia in rheumatological patients undergoing methotrexate treatment. Moreover, we aimed also to make physicians aware of the occurrence of these side effects, to adequately inform the patient before starting treatment. Methods: Systematic review (PROSPERO id: CRD42022358275) was performed according to preferred reporting items for systematic reviews and meta-analyses. Studies (1 January 1995 to 31 May 2022) were identified by highly sensitive searches of electronic databases (Medline, Embase, Cochrane Library databases). Key terms included: (("sexual dysfunction" OR "erectile dysfunction" OR "impotence") AND ("methotrexate" OR "MTX")) and (("gynecomastia" OR "male breast") AND ("methotrexate" OR "MTX")). Results: A total of seven papers (seven case reports), involving a total of eleven patients (min one, max three), were included in the final analysis. The age of the patients ranged from 19 to 68 years (median: 50.9 years). Rheumatoid arthritis (RA) was the most frequent rheumatic disease reported (n = 8, 72.7%). No patients reported erectile dysfunction (ED) and/or gynecomastia before using MTX. Time to onset of SD and gynecomastia after MTX ranged from 2 to 104 weeks (median: 22.7 weeks). ED (n = 5, 45.4%) and gynecomastia (n = 3, 27.2%) were the most common forms of presentation. Conclusions: Future prospective controlled studies with a large sample size and long follow-up as well as randomized controlled trials are needed to confirm this association, investigate its pathophysiological basis, assess the safest dosages, evaluate the most appropriate management, and provide clear recommendations. Chatbots, or conversational robots, have become a strategy or support tool for urologic patient care, diagnostic communication, and treatment. With regard to patients, studies have shown that chatbots can answer medical questions with similar percentages of acceptability as urologists. In addition, they can contribute to patient education, allowing them to ask questions that do not arise during medical consultation. They have also proven to be good tools for health promotion and disease prevention. These benefits can also serve doctors, as robots can support medical consultation and the reading of medical records, making patient care more efficient; however, there are several limitations, including the accuracy of bot-generated answers and the acceptability that urologists give to this type of tool. The article focuses on the challenges of diagnosing and managing non-clear cell renal cell carcinoma (nccRCC), a heterogeneous and less-studied subtype of kidney cancer. Endourology plays an important role in modern urological practice. Compared to open surgery, it offers many advantages. In Africa, endourology is not widely practiced or non‐existent in some referral centres. Several factors have been linked to this challenge. This article explores and proposes strategies to improve endourology practice in the African context. Recognising the unique challenges and opportunities in the region, the document discusses key initiatives and recommendations to promote the growth and development of endourological practices, including the identification of local needs, training, technological adaptation, etc. It aims to provide valuable information on the advancement of endourology in Africa. Social media (SoMe) is now a core part of modern-day life with increased use among both patients and urologists. The interplay of SoMe between these two parties is complex. From a patient perspective, SoMe platforms can serve as educational tools as well as communication portals to support networks and patient communities. However, studies report the educational value of content online is often poor and may contain misinformation. For urologists, SoMe can lead to research collaborations, networking and educational content but areas of concern include the potential negative impact SoMe can have on mental health and sharing of patient images without appropriate consent. The world of complementary and alternative medicine (CAM) encompasses a wide range of practices, treatments, and products that fall outside the realm of conventional (mainstream) medicine. The use of complementary and CAM has become increasingly popular in Western nations. People are turning to CAM for a variety of reasons, including managing chronic diseases, relieving symptoms of various conditions, and improving their overall health and well-being. There's a growing trend of people using and showing interest in complementary and alternative medicine therapies, especially in Western countries. CAM encompasses a wide range of treatments, some offering complete alternatives to conventional medicine, while others aim to complement existing medical approaches. Urologists should stay informed about CAM to guide their patients effectively to treat patients in a modern and personalized way. Objectives: Bladder-Sparing Approach was presented in patients who are not willing or not suitable for Radical Cystectomy (RC). There have been inconsistencies in the literature regarding the comparison of survival rates of these two methods. Our objective is to evaluate the survival rate of patients with muscle-invasive bladder cancer (MIBC) undergoing different treatment methods. Design: Retrospective cross-sectional study. Setting: A secondary care, multicenter study in Kerman, Iran 2008 to 2016. Participants: All 200 patients who were diagnosed with Muscle Invasive Bladder Cancer and were admitted to our hospitals. Patients with inaccessible medical files and patients with pathologies other than TCC were excluded. Main outcome measures: Radical cystectomy and different methods of bladder preservation were compared based on their survival rate. Interventions: Radical cystectomy or bladder preservation Results: Overall survival of the patients was 2 years [95% CI: 1.37–2.63]. The overall 5-year survival rate of patients with MIBC was 32%. Having a 6.4 years overall survival, the RC group showed the highest survival compared with others (p = 0.01); the overall survival of patients undergoing TMT, TURT, chemotherapy, or radiotherapy monotherapy was 3.15 years [95% CI: 2.242–4.061], 4.06 [95% CI: 3.207–4.931], 2.58 [95% CI: 1.767–3.399], and 3.14 [95% CI: 1.614–4.672] years, respectively. Patients younger than 65 undergoing RC had an overall survival of 7 years, compared with 2 years for the TMT group. (p = 0.0001). Conclusions: The Bladder-Preservation method, as a replacement for RC, showed a lower overall survival rate in our study. A prospective randomized clinical trial may declare the best treatment. Background and Objectives: Bladder cancer in patients under 45 is poorly characterized and rarely described, with variabilities in clinical outcomes and tumor properties. Our study aimed to elucidate the clinical and pathological features and outcomes of bladder cancer in this younger demographic to better inform management strategies. Materials and Methods: We conducted a retrospective analysis at the Urology Department of "Pius Brînzeu" County Emergency Clinical Hospital in Timișoara, Romania, on 60 patients aged 18–45 who underwent transurethral resection of bladder tumor (TURBT) during a 9-year period. Results: The cohort had a mean age of 38.5 ± 5.6 years with a male predominance (70%). Most tumors were non-muscle-invasive (NMIBC; 80%), with 16.7% being papillary urothelial neoplasms of low malignant potential (PUNLMP), 50% stage pTa, and 30% stage pT1. High-grade tumors were present in 43.3% of the patients. Recurrence occurred in 40% of the patients, while progression was observed in 16.7%. The 3-year overall survival rate was 93.3%, and the progression-free survival rate was 83.3%. Patients with high-grade tumors had a significantly higher recurrence rate (61.5% vs. 23.5%, p = 0.003) and lower survival rates compared to those with low-grade tumors. Conclusions: Young patients predominantly present with low-to-intermediate-stage tumors, yet a significant portion exhibit high-grade tumors associated with poorer outcomes. These findings suggest that while bladder cancer in younger patients tends to be less invasive, aggressive follow-up and treatment are crucial in those with high-grade tumors. [ABSTRACT FROM AUTHOR]

The aim of the study was to examine the knowledge and practices of nurses working in urology clinics on fluid–electrolyte monitoring and management. The study sample for this descriptive cross‐sectional research comprised 79 nurses employed in clinics providing care to urology patients from June 2023 to June 2024. These nurses had internet access and consented to participate in the study on a voluntary basis. The data were obtained through the 'Google Form', which uses a descriptive characteristics form, a fluid–electrolyte monitoring and management knowledge form, and a practice form for fluid–electrolyte monitoring and management. The mean score of the nurses on the fluid–electrolyte monitoring and management knowledge form was 7.48 ± 1.74. The most common practice related to fluid–electrolyte monitoring and management was "I monitor and record additional fluid losses related to vomiting, diarrhoea, fever and drains". When the mean scores of the fluid–electrolyte monitoring and management knowledge form were compared according to the individual characteristics of the nurses, no statistically significant difference was found between the mean scores of the knowledge form according to the characteristics of the nurses (p > 0.05). In this study, the mean knowledge scores of the nurses were above average. When the practices of the nurses for the prevention of fluid–electrolyte imbalances were examined, most of them stated that they monitored additional fluid losses related to vomiting, diarrhoea, fever and drains. Chronic pornography use is a public health problem that has considerable effects on men's health, especially on their sexual performance, having effects that can be encompassed from a biological, social, and psychological approach. Our objective was to conduct a literature review to evaluate the consequences in terms of sexual performance of young men who have chronic consumption of pornography. Chronic pornography consumption was related to erectile dysfunction, premature ejaculation, and alterations in genital anatomy and physiology. Social and psychological effects were also found, such as increased rates of violence towards women, problems with partners, and intimacy. However, benefits of controlled consumption of pornography have also been identified, such as reaffirmation of sexual orientation, stimulation of self‐discovery of the body, and improved communication with partners. Chronic consumption of pornography has direct effects on the sexual performance of young people and should be evaluated by a multidisciplinary team that includes physiological, social, and psychological elements. Aims: Urogynecology and Reconstructive Pelvic Surgery (URPS) fellowship can be pursued after completion of either a urology (URO) or obstetrics and gynecology (GYN) residency. Our aim is to determine differences in graduating fellow cohort (GFC) case logs between URO‐ and GYN‐based URPS programs. Methods: Accreditation Council for Graduate Medical Education case logs for URPS GFCs in both GYN‐ and URO‐based programs were analyzed for the 2019–2023 academic years (AY). Unpaired t‐tests with Welch's correction were used to compare annual mean logged cases between URO‐ versus GYN‐based GFCs for select surgical categories and the top 11 most logged index cases. Results: GYN‐based GFCs logged more cases for all pelvic organ prolapse (POP) categories including surgery on apical POP, anterior wall POP, and posterior wall POP (all p < 0.01), while URO‐based GFCs logged more cases for surgery on the urinary system (p = 0.03). For the top 11 logged procedures, URO‐based GFCs logged more sacral neuromodulation cases (p = 0.02), whereas GYN‐based GFCs logged more slings, vaginal hysterectomies, minimally‐invasive hysterectomies, vaginal apical POP, vaginal posterior POP, vaginal anterior POP, and minimally‐invasive apical POP cases (all p < 0.01). There was no difference between URO‐ and GYN‐based GFCs for complex urodynamics, cystoscopy with botox injection, or periurethral injection cases. Conclusions: URO‐based URPS fellows tend to graduate with more surgery on the urinary system and sacral neuromodulation cases, while GYN‐based fellows perform more slings, hysterectomies, and POP surgery. These findings may help fellowships better understand potential differences in training among graduates from URO‐ and GYN‐based programs and encourage collaboration to lessen these discrepancies. Purpose: Overactive bladder (OAB) is a common syndrome in children, which may affect the patient's quality of life (QoL). This study aimed to evaluate the effect of standard urotherapy (SU) given on the quality of life for children diagnosed with OAB. Methods: The study was conducted on 107 children aged 2 to 13 years who applied to the urology clinic of a hospital in China and were diagnosed with OAB. The QoL was assessed using the Pediatric Incontinence Questionnaire (PINQ). The QoL was evaluated before and 1 month later SU given. Results: Of the 107 children, at baseline, the children with OAB revealed a total mean PINQ score of 27.40 ± 13.19, and the total mean PINQ score was 13.72 ± 9.63 at 1 month later. PINQ score showed a statistically significant difference between before and after 1 month treatment (P < 0.001). Conclusions: The QoL of children with OAB increased may due to SU provided outpatient clinics. Purpose: Mirroring global trends, artificial intelligence advances in medicine, notably urolithiasis. It promises accurate diagnoses, effective treatments, and forecasting epidemiological risks and stone passage. This systematic review aims to identify the types of AI models utilised in urolithiasis studies and evaluate their effectiveness. Methods: The study was registered with PROSPERO. Pubmed, EMBASE, Google Scholar, and Cochrane Library databases were searched for relevant literature, using keywords such as 'urology,' 'artificial intelligence,' and 'machine learning.' Only original AI studies on urolithiasis were included, excluding reviews, unrelated studies, and non-English articles. PRISMA guidelines followed. Results: Out of 4851 studies initially identified, 71 were included for comprehensive analysis in the application of AI in urolithiasis. AI showed notable proficiency in stone composition analysis in 12 studies, achieving an average precision of 88.2% (Range 0.65–1). In the domain of stone detection, the average precision remarkably reached 96.9%. AI's accuracy rate in predicting spontaneous ureteral stone passage averaged 87%, while its performance in treatment modalities such as PCNL and SWL achieved average accuracy rates of 82% and 83%, respectively. These AI models were generally superior to traditional diagnostic and treatment methods. Conclusion: The consolidated data underscores AI's increasing significance in urolithiasis management. Across various dimensions—diagnosis, monitoring, and treatment—AI outperformed conventional methodologies. High precision and accuracy rates indicate that AI is not only effective but also poised for integration into routine clinical practice. Simple Summary: This review paper explores advanced imaging technologies for prostate cancer because traditional methods often miss or misclassify cancers. The aim of this study is to provide a comprehensive overview of three key technologies, MRI, micro-ultrasound, and prostate-specific membrane antigen (PSMA) PET scans, and how they can improve prostate cancer detection and management. The findings showed that these techniques have improved our ability to detect prostate cancer; however, further studies are needed to determine each imaging technique's specific role in the diagnosis of prostate cancer. This review discusses the principles, benefits, and clinical applications of multiparametric magnetic resonance imaging (mpMRI), micro-ultrasound (microUS), and prostate-specific membrane antigen positron emission tomography–computed tomography (PSMA PET/CT) in localized prostate cancer. Methods: We conducted a comprehensive literature review of recent studies and guidelines on mpMRI, microUS, and PSMA PET/CT in prostate cancer diagnosis, focusing on their applications in biopsy-naïve patients, those with previous negative biopsies, and patients under active surveillance. Results: MpMRI has demonstrated high sensitivity and negative predictive value in detecting clinically significant prostate cancer (csPCa). MicroUS, a newer technology, has shown promising results in early studies, with sensitivity and specificity comparable to mpMRI. PSMA PET/CT has emerged as a highly sensitive and specific imaging modality, particularly valuable for staging and detecting metastatic disease. All three technologies have been incorporated into urologic practice for prostate cancer diagnosis and management, with each offering unique advantages in different clinical scenarios. Conclusions: Advanced imaging techniques, including mpMRI, microUS, and PSMA PET/CT, have significantly improved the accuracy of prostate cancer diagnosis, staging, and management. These technologies enable more precise targeting of suspicious lesions during biopsy and therapy planning. However, further research, especially randomized controlled trials, is needed to fully establish the optimal use and inclusion of these imaging modalities in various stages of prostate cancer care. Erectile dysfunction (ED), the impairment of achieving and maintaining an erection for satisfactory sexual intercourse, is a common pathology that men experience for a variety of different factors. Conservative treatment for ED includes changing medications, lifestyle modifications, and psychotherapy. Pharmaceutical and nonsurgical interventions include phosphodiesterase‐5 inhibitors(PDE‐5i), intracavernosal medication injections, and vacuum devices. Surgical treatment options for ED have evolved over time and currently include the use of inflatable penile prosthesis (IPP) and malleable penile prosthesis. IPP insertion is usually met with good patient satisfaction. However, complications of device insertion can include corporal perforation, urethral injury, cylinder erosion or extrusion, infection, and mechanical failure, to name a few. Our patient presented with device malfunction and intraoperative assessment showed the IPP tubing twisted at the levels of the reservoir on the first operation and the level of the cylinder and scrotal pump on the second operation. The twisting of the tubing resulted in a nonfunctioning IPP as the fluid was unable to fill the cylinders resulting in an erection. The patient was managed with complete device explanation and reinsertion of a new three‐piece IPP per the patient and partner's request. This is the first case report highlighting this specific complication, and we hope to provide clinicians with the resources to recognize this rare complication. [ABSTRACT FROM AUTHOR]

Aim: To compare and analyze the clinical efficacy of modified Koyanagi and staged Duckett for proximal hypospadias. Method: The clinical and prognostic data of children were treated and underwent surgery (modified Koyanagi or stage Duckett) in the Department of Urology, Kunming Children's Hospital from January 2020 to January 2023 were retrospectively analyzed. According to different surgical methods, the subjects were divided into the modified Koyanagi group and the staged Duckett group. Patients in both groups were followed up for more than 6 months after surgery. The success rate and complications of postoperative surgery in both groups were analyzed. Result: A total of 63 patients were included in this study, 34 in the modified Koyanagi group and 29 in the staged Duckett group. A total of 14 patients in the modified Koyanagi group experienced postoperative complications, the success rate of the surgery was 58.82%, and 5 among them experienced more than two kinds of complications. A total of 5 children in the staging Duckett group experienced postoperative complications, and the success rate of the operation was 82.75%. There were significant differences in the incidence of overall complications and fistula between the two surgical methods (P = 0.028). Conclusion: Compare with modified Koyanagi, staged Duckett can significantly reduce the incidence of overall complications and urethral fistula in patients, and have obvious advantages in the treatment of proximal hypospadias. (1) Background: This study aimed to quantify and evaluate the publication rate and discrepancies of functional urology abstracts from international conferences, and to explore the interdisciplinary contributions of urology and gynecology to the field. (2) Methods: A retrospective bibliometric and content analysis was conducted on abstracts presented between 2015 and 2019 at the EAU and ICS congresses, focusing on functional-urological keywords. A discrepancy scoring system ranging from 0 (minor discrepancies) to 3 (significant discrepancies) assessed the consistency between conference abstracts and full-text publications, and an in-depth analysis determined the disciplinary origin of these publications. (3) Results: Between 2015 and 2019, 53% of EAU and 57% of ICS congress abstracts were published as full-text articles, with minor discrepancies in 38% of EAU and 49% of ICS publications, and significant discrepancies in 17% from both. Urology departments dominated publications, contributing 68% at EAU and 55% at ICS, whereas gynecology contributed only 1% at EAU and 12% at ICS. (4) Conclusions: This study illuminates the need for improved reporting standards and interdisciplinary collaboration in functional urology, as well as increased gynecology research in functional urology-related fields, suggesting that addressing these issues is crucial for advancing the field and enhancing patient care. Key Clinical Message: Penile fracture is a urological emergency that requires prompt surgical intervention for optimal outcomes. Early diagnosis based on clinical presentation and imaging if needed is crucial. Timely repair ensures the preservation of penile function and minimizes complications. Penile fracture (PF) is a rare but significant urological emergency characterized by the rupture of the tunica albuginea, often resulting from blunt trauma during sexual intercourse or other activities. The typical presentation includes sudden and severe penile pain, an audible "cracking" sound followed by swelling and deformity. While PF generally does not involve the urethra, bleeding from the urethral meatus suggests an associated urethral lesion. Imaging such as ultrasonography or magnetic resonance imaging (MRI) can assess the extent of the injury. We report a case of a 28‐year‐old male who presented with sudden‐onset penile pain, swelling, and blood dripping from the urethral meatus after a traumatic sexual encounter. The injury occurred during vigorous intercourse when his erect penis struck his partner's buttocks. He heard a cracking sound followed by immediate detumescence, penile curvature, and urethral bleeding. At the hospital, the patient exhibited tachycardia and elevated blood pressure. Examination revealed deviation of the penis to the right, urethrorrhagia, and a tender, swollen penis. A clinical diagnosis of bilateral corpora cavernosa injury and partial rupture of the penile urethra was made. A urethral catheter was placed with difficulty, draining initially reddish urine followed by clear urine. Laboratory tests showed normal hemoglobin and coagulation profiles with mild bacteriuria and hematuria. Due to clear clinical signs, no radiological imaging was performed before surgery. Surgical exploration performed 48 h postinjury revealed a PF involving the right corpus cavernosum and a complete rupture of the spongy urethra. The corpus cavernosum was repaired using Vicryl 2–0 sutures followed by urethroplasty with Vicryl 5–0. Postoperative care included antibiotics and analgesics. The patient had an uneventful recovery with preserved penile sensitivity and function. PF is a rare urological emergency that necessitates prompt diagnosis and management to prevent long‐term complications. Although clinical presentation often suffices for diagnosis, imaging can be valuable in ambiguous cases. Surgical intervention is the preferred treatment, offering the best functional and cosmetic outcomes. Background: Urologic emergencies are common and complications from their management are high. Simulation‐based education (SBE) is a highly effective training method, allowing trainees to learn both technical and nontechnical skills in a safe environment. Training in the management of urological emergencies is limited in many healthcare settings, so we performed a needs assessment for a urological emergencies SBE course in Ethiopia. Methods: This study presents data from a descriptive cross‐sectional, survey‐based survey of consultants and residents in the Ethiopian surgical community. The survey was disseminated using online Google Forms, through social media (WhatsApp), and to colleagues in the College of Surgeons of East, Central and Southern Africa (COSECSA) via email and social media. Results: One hundred‐seven results were received; two were discarded due to incomplete data. Fifty three of the respondents were general surgeons and 38 of the respondents were urologists. Sixty nine respondents strongly agreed that simulation‐based training was important for first‐year surgical residents, whereas twenty‐five respondents agreed and nine respondents strongly disagreed; one respondent disagreed and one was neutral. Eighty seven respondents suggested a 3‐day training course, whereas 17 respondents suggested a two‐day course. More than 80 of the respondents rated training in the management of acute urinary retention, acute scrotum, urethral, and suprapubic catheterization as extreme or very important and 79 respondents wanted education about urologic trauma and Fournier's gangrene. Intraoperative ultrasound is described widely in multiple pathological scenarios in adult practice and in image-guided interventions in children. We aim to describe the technique and range of potential uses of intraoperative ultrasound in paediatric urological surgery, from outlining the process of case selection, preparation, and logistics to demonstrating the ranging benefits of real-time, high spatial resolution ultrasound during resection. At our centre, we use intraoperative ultrasound to assist in a variety of operations. These include guiding excision margins in nephron-sparing surgery, assessing for vascular infiltration in renal tumours, and identifying salvageable testicular tissue in orchidectomy. By exhibiting these scenarios, we hope to display the unique value that intraoperative ultrasound can have to the paediatric surgeon and inspire additional uses further afield. INTRODUCTION: Nonagenarians represent a rapidly growing patient population in Canada and have unique health concerns. With the goal of preparing urologists to manage this complicated patient population in the future, we sought to characterize referral patterns, diagnoses, investigations, treatments, and associated complications in a cohort of nonagenarians. Our second goal was to review anticholinergic burden (ACB) and rates of anticoagulation in this patient population and to assess the risk of hematuria in those who were anticoagulated. METHODS: This was a single-center, retrospective chart review of a sample of nonagenarians referred to our tertiary care center between 2009 and 2017. Demographic information, referral patterns, investigations, treatment plans, and outcomes were assessed. We assessed medication lists to calculate ACB scores at the time of referral, in addition to rates of anti-coagulation use. RESULTS: Data was collected for 154 nonagenarians. Hematuria was the most common reason for referral (n=43, 27.9%). Urinary retention and lower urinary tract symptoms (LUTS) were seen in 22 and 36 patients, respectively. The majority of patients underwent routine investigations; however, treatment decisions were frequently based on age and frailty. Mild, moderate and severe ACB scores were seen in 76.6%, 9.33%, and 14.0% of patients, respectively. Of those referred for hematuria, 78.1% were on anticoagulation therapy. CONCLUSIONS: The most common reasons for urologic referral of nonagenarians include hematuria and LUTS. Most nonagenarians are offered routine investigations, and many are offered minor interventions for common benign and malignant urologic diagnoses. INTRODUCTION: We sought to identify predictors of failed retrograde ureteric stent (FRS) placement in the setting of obstructing ureteric calculi. In addition to patient- and stone-specific characteristics, we also considered computed tomography (CT) measures of ureteric wall thickness (UWT), as it has shown clinical potential in predicting outcomes of shockwave lithotripsy, ureteroscopy, and spontaneous stone passage. METHODS: We performed a retrospective, case-control study comparing patients who had successful retrograde stent (SRS) insertions with those who failed stent placement and ultimately required nephrostomy tube (NT) insertion (2013--2019). Patients were identified using administrative data from a shared electronic medical record (capturing all urology patients in our geographic area) and a prospective database capturing all institutional interventional radiology procedures. Patient demographics, as well as clinical and stone characteristics, were then collected, and imaging manually reviewed. Statistical analysis was performed using univariate and multivariate logistic regression analysis in collaboration with a statistician. RESULTS: A total of 109 patients met inclusion for analysis (34 FRS, 75 SRS). The most common indication for stent insertion included sepsis (79%). On multivariate analysis, both acute kidney injury as primary indication for stent insertion (odds ratio [OR] 9.16, 95% confidence interval [CI] 1.91--44.00, p=0.006) and UWT (OR 0.34, 95% CI 0.15--0.74, p=0.007) were found to be significantly associated with FRS placement. A receiver operator characteristic curve analysis demonstrates an optimal UWT cutoff of 3.2 mm (sensitivity 60.6%, specificity 83.3%). CONCLUSIONS: Elevated UWT and acute kidney injury as an indication for urgent urinary decompression in the setting of obstructing ureteric stones are predictive of FRS placement. INTRODUCTION: Generative artificial intelligence (AI) has proven to be a powerful tool with increasing applications in clinical care and medical education. ChatGPT has performed adequately on many specialty certification and knowledge assessment exams. The objective of this study was to assess the performance of ChatGPT 4 on a multiple-choice exam meant to simulate the Canadian urology board exam. METHODS: Graduating urology residents representing all Canadian training programs gather yearly for a mock exam that simulates their upcoming board-certifying exam. The exam consists of written multiple-choice questions (MCQs) and an oral objective structured clinical examination (OSCE). The 2022 exam was taken by 29 graduating residents and was administered to ChatGPT 4. RESULTS: ChatGPT 4 scored 46% on the MCQ exam, whereas the mean and median scores of graduating urology residents were 62.6%, and 62.7%, respectively. This would place ChatGPT's score 1.8 standard deviations from the median. The percentile rank of ChatGPT would be in the sixth percentile. ChatGPT scores on different topics of the exam were as follows: oncology 35%, andrology/benign prostatic hyperplasia 62%, physiology/anatomy 67%, incontinence/female urology 23%, infections 71%, urolithiasis 57%, and trauma/reconstruction 17%, with ChatGPT 4's oncology performance being significantly below that of postgraduate year 5 residents. CONCLUSIONS: ChatGPT 4 underperforms on an MCQ exam meant to simulate the Canadian board exam. To assess the panel composition of the 2 most important guideline developers in urology as equity and acceptability, important domains in clinical guideline development, require broad stakeholder representation. Following a predefined protocol, we identified all current AUA and EAU guideline documents. Two authors independently abstracted data including guideline topic, number and roles of panel members, voting status, and academic rank. We determined panel member's gender (woman, man, or nonbinary) and racialization (White or non-White) status based on name, internet picture, pronouns used, bios available, and gender listed on their profile. We identified 31 AUA and 20 EAU guidelines for inclusion. Median panel size was 19 (interquartile range [IQR]: 17; 21) with 12 (IQR: 10; 14) voting members. The average composition of voting panels was predominantly male (81.8%) and White (86.8%). Eleven guideline panels (21.6%) did not include any women, and 9 (17.6%) panels had no representation of individuals from non-White groups. While gender distribution was similar among guidelines of the 2 organizations, the AUA included more voting members from non-White groups (14.3% vs 8.0%; P =.010). Analysis of the AUA panel composition over time revealed stable proportions of female and non-White individuals. Both AUA and EAU guidelines exhibit insufficient representation of females and non-White individuals, with no evident improvement observed over time. Implementing more transparent processes that advocate for diverse panel representation may enhance the incorporation of stakeholder values and preferences, thereby improving the dissemination and adoption of guidelines. Objective: To analyse the litigation trends and the reasons for claims within the specialty of Urology, within the UK National Health Service (NHS), over a 16‐year period. Materials and Methods: Data were requested from NHS Resolution under the Freedom of Information Act 2000. This included the total number of claims in Urology, the number of these that were successful (settled or closed), and the costs in damages paid out per financial year between 2006 and 2022. A breakdown of the successful claims by their primary cause was also collected. These were coded into the categories: 'non‐operative', 'intraoperative', 'postoperative', and 'other'. Results: A total of 4124 litigation claims were made between 2006 and 2022 and 60.9% (2511/4124) of these claims were successful. In all, £145 million (British pounds) was paid out in damages. The number of successful claims increased 2.9‐fold from the start to end of this 16‐year period, and the costs in damages paid out increased 10‐fold. Regarding primary causes for the successful claims, failure or delay in treatment (20.9%, 525/2511), failure or delay in diagnosis (14.5%, 364/2511), and intraoperative problems (9.1%, 229/2511) accounted for the highest proportion. Overall, non‐operative causes for successful claims accounted for 73.3% (1840/2511), intraoperative for 20.1% (504/2511), and postoperative for 3.9% (98/2511). Conclusions: The number of successful urological litigation claims, and their associated costs is rising. The majority are due to non‐operative causes, which may be partially explained by NHS waiting lists alongside the effects of the coronavirus disease 2019 (COVID‐19) pandemic. [ABSTRACT FROM AUTHOR]

Non-muscle invasive bladder cancer (NMIBC) is characterised by high rates of recurrence and progression, requiring substantial healthcare resources. In Latin America, the incidence of NMIBC is set to increase due to an aging population and lifestyle changes. To better understand the current challenges for NMIBC treaters and patients, a mixedmethods approach was leveraged combining secondary research with qualitative interviews from healthcare providers in Brazil, Colombia, Mexico and Argentina. Our analysis found that significant challenges persist across the region, particularly due to Bacillus Calmette-Guérin shortages, inconsistent adherence to clinical guidelines and significant socioeconomic disparities for patients accessing healthcare services. Addressing these challenges requires improved patient advocacy, strategic use of clinical trials and better resource distribution to enhance NMIBC management across Latin America. Penile skin zipper entrapment is an emergent medical condition in which the penile skin, scrotal skin, or foreskin gets caught within the teeth of a zipper or the slider itself. This can lead to complications such as urethral involvement, skin loss, or tissue necrosis. We propose a novel technique to aid in the release of entrapped skin utilizing wire cutters directed at the inferior portion of the zipper pull. To describe a novel technique to free entrapped penile skin and compare its performance to the well-established median bar technique in a simulated setting. A randomized cross-over design was used to compare techniques on successful release, time to release and tissue injury using an animal model of raw chicken skin entrapped in a zipper. Statistical significance was assessed at p < 0.05. Twenty-two participants were included. There was no statistically significant difference between the novel technique and the median bar technique regarding successful release (100% vs 95.5%, respectively), median time to release (29.1 vs 26.4 seconds, respectively), or frequency of tissue injury (22.7% vs 27.3%). Performance using our novel technique for removal of penile skin from a zipper is similar to the median bar release technique regarding. Objective: The concentration of albumin and globulin in the body can serve as indicators of both nutritional status and inflammation. The predictive significance of the albumin-to-globulin ratio (AGR) has been documented in multiple cancer types. Consequently, a meta-analysis was conducted in order to investigate the prognostic impact of AGR on survival outcomes among individuals diagnosed with renal cell carcinoma (RCC). Methods: A systematic search was conducted across four electronic databases to identify pertinent studies that evaluated the predictive significance of pre-treatment albumin-to-globulin ratio (AGR) in patients with renal cell carcinoma (RCC). The main outcome of interest in this study was overall survival (OS), whereas additional outcomes included cancer-specific survival (CSS), progression-free survival (PFS), and disease-free survival (DFS). The researchers utilized random-effect models to summarize the time-to-event outcomes, presenting the results as adjusted hazard ratios (aHR) along with their corresponding 95% confidence intervals (CI). Results: A total of 5,732 RCC patients in eight studies were included. Cut-off for AGR value varies among studies, with AGR higher than 1.1–1.47 regarded as normal. Pooled analysis from these studies showed that low AGR value was associated with shorter OS (aHR 1.84 (95% CI 1.35–2.51), p = 0.0001) and CSS (aHR 1.91 (95% CI 1.15–3.16), p = 0.01). Conclusions: This study suggests the role of AGR in predicting the OS and CSS of RCC. Background. The pre-operative evaluations of patients should not cause any mental or physical harm to the patient or waste the financial resources of the patient or the public health system by unnecessary actions. Health spending is one of the issues of concern in developed and developing countries. In Iran, 5–10% of government spending is allocated to this sector. The purpose of the present study was to compare the current process of preparing surgical candidates with accepted clinical guidelines. Methods. In this cross-sectional study, sampling was performed by the judgmental/purposive method among the mentioned population; finally, 134 patients were evaluated in this study. The data were categorized as 3 x 4 tables. On one side of the table, patients were divided into small, intermediate, and large surgical groups according to surgical complexity, and on the opposite side of the table, they were assigned to four groups at low, intermediate, high, and very high risk. As a result, each patient was housed in one of the houses on a 12-hole table. The clinical findings and evaluation results before surgery were extracted and evaluated retrospectively. Results. The mean age of the patients was 49.68 ± 16.96 years, and 74.6% were male. The most common type of surgery was transurethral lithotripsy. The incidence of hypertension (21.4%) among underlying diseases and comorbidities was the highest recorded item. The procedures were selected relatively in the same population size from three groups, namely, small, intermediate, and large procedures, with a frequency of 50 (37.3%), 34 (25.4%), and 50 (37.3%) cases, respectively. Conclusion. In the present study, the majority of patients with minor surgeries did not comply with the valuations recommended by guidelines. Among the patients with intermediate surgeries, in nearly half of the cases, and in the major or complex surgery group, most of the time, the evaluations were performed in accordance with the guidelines. Practical Implications. Physicians should consider adhering to current clinical guidelines when requesting preclinical tests, which can pave the way for minimizing unnecessary costs.
