In this study, we present a case of Grade IV diabetic foot ulcer (DFU) and its management with Vrana ropana chikitsa along with Ayurvedic precision medicines. A 72-year-old male patient having grade IV DFU was given the Vrana ropana chikitsa with precision medicines. The bacterial profile and antibiotic resistance, and wound characteristics were monitored before and after the treatment. After the treatment, significant progressions were found in the patient. There was a decrease in the growth of Staphylococcus aureus in the wound after the treatment. Doxycycline and erythromycin-resistant isolates were switched to antibiotic sensitive strains with Vrana ropana chikitsa. Progression of Grade IV ulcer to healing wound was found in the patient. A case of Grade IV DFU with drug-resistant S. aureus was managed with the alternative treatment of Vrana ropana chikitsa and showed improved wound healing.

Ayurveda’s distinctiveness lies in its use of Ahara-Vihara (dietic and daily regimes) as a therapeutic method. Sixty therapeutic protocols for wound healing have ahara (food) as one of the approach. Wound healing is a multifactorial mechanism where food plays a significant role in terms of micro and macro nutrients. In clinical practice, understanding Pathya Kalpana (conducive practises) is crucial as it considered as half of the treatment, Ardha Chikitsa (Partial Treatment) and it also stabilises digestive and metabolic fires (jataragni and dhatwagni). Prescribing medicines and ignoring a proper lifestyle and diet changes serve no purpose as it only defeats the symptoms of the disease without attending the root causative factors of a disease, as Apathya (non conducive practices) are the major causative factors of pathogenesis. Even a treatment protocol which can hit the bull’s eye would fail, when a patient has inappropriate diet and indulges in improper activities of routine. The knowledge of influence of shadrasa (six tastes) in healing, method of eating, conducive and non conducive foods and regimes given in scriptures, properties and effects of the wholesome foods and its indications is beneficial for establishing a structured treatment protocol. Various types of wounds and different phases of wound healing can be approached by a specifically designed diet and regimes.

Diabetic foot ulcer (DFU) is a serious complication of diabetes mellitus and a cause of significant morbidity, mortality and healthcare expenditure. Treatment of DFU includes multimodal approach like surgical debridement, infection control, vascular assessment, dressing etc. Multidisciplinary approach towards foot care is becoming a mainstay of therapy, and even with this comprehensive approach, there is still room for improvement in DFU outcomes. Integrative management includes the adoption of various systems of treatment with standard treatment for better outcomes. In the present case series, six cases of DFU were managed with the integration of Ayurveda and allopathic treatments. The cases were managed according to the standard diabetic foot ulcer management principles like surgical debridement, insulin therapy, along with incorporation of Ayurveda procedures like Vimlapana, Prakshalana, Bandhana etc. Standard assessment of ulcers at different time points was done using the Bates- Jensen Ulcer assessment tool. All the six DFU healed with minimal scar formation and in less time, lowering the risk of further amputation. Promising results were obtained in all six cases by adopting integrated Ayurveda and allopathic treatments, which indicates the potential benefits of alternative systems of medicine.

Healing of vrana (wound) is either primary or secondary by nature. Secondary healing requires more attention than primary healing. Basically, two things, shodhana (making free from undesirable healing factors) and ropana (closure of wound), are desirable for proper healing. Many drugs have been described in classics for healing, but none of them is capable of healing the wound individually. Hence, to fulfill the aim, manjishthadi ghrita was prepared with the help of 7 drugs having vrana ropana effects and was evaluated clinically for its healing properties in this study. It was used topically in postoperative wounds, mostly of ano-rectal cases, twice a day, for 21 days. The follow-up period was 1 month to observe the healing as well as vaikritapaham (reduced deformity) properties and any untoward effects of the drug. A randomized control clinical trial was done. Out of 45 patients, 24 patients in group A were treated with "Manjishthadi Ghrita" (treated group), while 21 patients in group B (standard group) treated with povidine iodine ointment. Better result was observed in the treated group in comparison to the standard group. No adverse effect was observed in any patient. Manjishthadi ghrita can be prescribed as a local healing agent for common wound.

Background: In Sushruta Samhita, various medicinal plants as single and compound formulations having Vrana-Shodhana (wound cleansing) and Vrana-Ropana (wound healing) potential are enumerated. There are no published data available on these wound-healing medicinal plants of Sushruta Samhita. The effectivee management of wound is necessary in the immunocompromised and chronic wounds patients as they take more time to heal. Aim: To review and systematically analyze wound-healing medicinal plants and their modus-operandi on the basis of pharmacodynamics attributes, i.e., Rasa (taste), Veerya (potency), and Vipaka (biotransformation) in the various stages of healing. Materials and methods: Review of Sushruta Samhita was done to gather wound-healing medicinal plants; pharmacodynamics attributes were gatherd from various Nighantus to understand their role in wound healing. The contemporary information about wound-healing mechanism was gathered from PubMed to interpitate the rational use of plants in the various stages of wound healing. Results: The study suggests that 43 medicinal plants have Vrana-Shodhana activity, 48 have Vrana-Ropana and 62 have both Vrana-Shodhana and Vrana-Ropana potential. Medicinal plants with Vrana-Shodhana category are having predominance of Katu (pungent), Tikta (bitter) and Kashaya (astriengent) Rasa. Plants under Vrana-Ropana are having Madhura, Kashaya Rasa (sweet taste), Madhura Vipaka (sweet biotransform) and Sheeta Veerya (cold potency). Conclusion: Plants having Tikta, Kashaya Rasa, Katu Vipaka, and Sheeta Veerya may be useful in the inflammatory stage, plants having Madhura, Kashaya Rasa and Sheeta Veerya may be useful in the prolifiratory stage and plants having Madhura Rasa, Sheeta Veerya, and Madhura Vipaka may be useful in the remodeling stage. In the era of globalization and evidence-based medicines, a systematic documentation of information by compiling the studies carried out in different parts of India could be useful for the clinicians of Ayurveda and to the ailing community. In this review, an attempt has been made to compile all such clinical research works carried out on Vrana Ropana (wound healing). A grey literature of post graduate (PG) and Doctorate (PhD) researches on Vrana Ropana from various Ayurvedic institutes were collected in the form of soft or/and hard copy as per the availability. The studies were found to be a combination of drug/drug formulations and various procedures mentioned under Shashti Upakramas in Sushruta Samhitha. The use of the Lepa (topical application), Avachoornana (sprinkling of medicated powder), Raktamokshana (bloodletting) like Jalauokavacharana (medicinal leech application) and Kshalana (therapeutic procedure in which the wounds are cleansed with medicated liquids) were cross reviewed from various research works. All these works were mainly targeted to find the best Shodhana (cleaning) and Ropana (healing) drugs for the treatment of wound. On the basis of the clinical evidences on the same drug with positive outcomes, one should further try it in multi-centres and develop that drug for wound management. Hence this review study would help to know the previous research works carried out on wound healing and design further trials on specific parameters or treatment protocol as a whole with local as well systemic management of wounded patients.

Ayurveda is a time-tested science that systematically explains its concepts through eight branches. Grahas Chikitsa (Ayurvedic treatment of microbes) is one of these branches, and it is well-explained in the texts. "Graha" refers to a tiny, invisible living organism that may undergo several stages of development and can be pathogenic or antigenic to the body. Microorganisms can cause generalised pathogenic alterations in the body that affect multiple systems, similar to the concept of Grahas. This suggests a deeper understanding of microbes in relation to Grahas. This review paper aims to explore the age-old concept of Balgrahas (microbes) and its relevance in modern times. The review was conducted using published articles on paediatric disorders, electronic databases, textbooks on paediatrics, and traditional Ayurvedic writings related to Kauamarbhritya (Paediatrics). A distinguishing feature of this study is its compilation of information on Balgraha from classical texts, covering clinical characteristics, causal factors, and various internal and external treatments. It also discusses Ayurvedic treatment modalities such as Daiva Vyapashraya Chikitsa (Spiritual therapy) and Yukti Vyapashraya Chikitsa (Rational therapy). In this review, authors discussed several formulations for treating conditions associated with Balgraha (microbes) that have not been detailed in previous studies. ABSTRACT: Burn injuries are often prevalent and among the most catastrophic injuries encountered in casualty or surgery departments, involving skin and other tissues. These injuries typically result from heat sources such as solids, liquids, or fire, while scalds arise from hot liquids, electricity, chemicals, cold, friction, or radiation. Despite varying causes, the male-female ratio of burn cases remains consistent. In India, the annual incidence of burns is estimated at six to seven million. Topical antimicrobial agents are crucial for treatment. This case study focuses on an 83-year-old woman with acute burn wounds on her right leg and abdomen, accompanied by pain, discharge, foul odor, edema, and skin discoloration. Treatment involved Panchagavya Chikitsa , utilizing local applications of Gomutra twice daily for wound cleaning and Jatyadi Ghrita twice daily for wound healing over 60 days. Additionally, 15 ml of Gomutra Arka was administered internally twice daily for 15 days. Significant improvements were observed, including complete wound healing and associated symptom resolution. The positive outcomes may be attributed to the local and systemic effects of Gomutra , Jatyadi Ghrita , and Gomutra Arka , which possess antimicrobial, antiseptic, scrapping, and wound-healing properties. The findings suggest that Panchagavya formulations effectively and safely manage burn wounds, even in geriatric patients. Introduction: Prevalence of diabetes and its complications have been a burden to the society from the ancient times, in the present and also will be in the future unless proper measures are taken to prevent its manifestation. There have been an increasing number of death associated-amputation cases which are mainly caused by nonhealing wounds. These facts urge researchers to develop new, more effective wound treatments for diabetic patients. Aim: To evaluate and compare the effect of Katupila Kalka (Securinega leucopyrus [Willd.] Muell. leaf paste) and Tila Taila (Sesamum indicum oil) in Madhumehajanya Vrana (diabetic wounds/ulcers) with Betadine ointment. Materials and Methods: A total of 23 patients of Madhumehajanya Vrana were chosen and randomly divided into two groups (Group A and B). Patients of Group A were treated with local application of Katupila Kalka with Tila Taila, whereas, in Group B, Betadine ointment was applied on the affected parts, once a day in the morning for 30 days. The relief in signs and symptoms were assessed on scoring pattern. Results: In Group A, diabetic wounds treated with Katupila paste got healed within 28 days with minimal scar formation without any complications, whereas in Group B, wound was healed completely only in two patients within 28 days. In both the groups, no patients reported any adverse drug reaction during the entire course of treatment as well as in follow-up period. Rasatarangini, one of the esteemed treatises of Ayurvediya Rasasastra has mentioned numerous malahara (ointment) preparations with herbal and herbo-mineral origin. The term malahara itself mean those which remove mala (impurities) from vrana (ulcers), vidradhi (abscesses), twak vikara (skin diseases) etc. Acharya Sadananda Sharma, author of Rasatarangini has mentioned Sindhooradhya malahara a preparation with potent action on vranashodhana ropana (wound cleansing and healing) and bhootasanga prashamana (antimicrobial) properties. Acharya has mentioned two malahara preparations named as Prathama and Dwiteeya Sindhooradhya malahara with only difference in single drug. Both preparations are indicated in skin manifestations like pama and kandu, which may be due to invasion of some external organisms or microbes. WHO has declared that antimicrobial resistance is one of the top 10 global public health threat facing humanity and thus the search for ayurvedic antimicrobials are progressing day by day. Since it is a topical antibiotic preparation, with very minimal ingredients proving its efficacy will be of great use for the society. The present study aims at the preparation and comparison between the antimicrobial activity of the two Sindhooradhya malahara named as Prathama and Dwiteeya respectively. The main findings are, antimicrobial study by both agar well diffusion and macro broth dilution showed that more action in Prathama Sindhooradhya malahara. Among the 4 tested strains, significant results are seen in case of Streptococcus pyogenes and Candida albicans whereas Dwiteeya Sindhooradhya malahara had no action in the tested strains of microbes. Introduction: Aphthous ulcer is seen to be one of the most commonly occurring ailments found among patients that dentists may come across on a daily basis. Taking into consideration, the agents that have been used for treatment of Aphthous stomatitis safety, least toxic properties with symptomatic relief and topical application are considered. These days there are few reliable natural remedies which are emerging as a therapy for aphthae. Herbal and polyherbal medicines will alleviate pain, induce healing, prevent infection and lessen inflammation in aphthous ulcers without any adverse effects. Aim: To evaluate and compare the efficacy of Jati Ghrita and 1% chlorhexidine gluconate on aphthous stomatitis patients. Materials and Methods: A randomised control trial will be carried out for an expected time of 3-4 months, in the Department of Oral Medicine and Radiology, Sharad Pawar Dental College and Dattatray Ayurvedic Rasashala, Wardha, Maharashtra, India. The study population consisting of 30 patients (age 18-65 years) with Aphthous stomatitis will be divided randomly into two groups i.e., Jati Ghrita and 1% Chlorhexidine gluconate, each group consisting of 15 patients. Efficacy will be assessed by patient's response through Visual Analogue Scale (VAS) and oral physician's observation-5 point Investigator's Global Assessment (IGA) scale. The intergroup comparison will be done by independent sample t-test. Expected Results: Jati Ghrita is expected to be effective in faster healing of aphthous stomatitis with reduction in pain. Jati is also expected to be more effective than 1% chlorhexidine gluconate. Ghee is widely considered as the Indian name for clarified butterfat and processing of ghee with therapeutic herbs i.e., ghrita is renowned for augmenting their medicinal properties. The wound is considered as a challenging clinical problem with early and late complications. To reduce the burden of wounds with the shortest period and minimum scaring, an attempt was made to prepare and evaluate the wound healing potential of ghee based polyherbal formulation. Based on local ethnic tribal claims, Semecarpus anacardium L., Argemone mexicana L., Cocculus hirsutus L., and Woodfordia fruticosa K. were collected from Western Ghats of India. The polyherbal Bhallatakadi Ghrita (BG) formulation was prepared as per Ayurvedic procedure and assessed for its wound healing potential using incision and excision wound animal models. BG treated group showed a complete contraction of wounds (99.82 ± 0.10%) (p<0.001) with 15.17 ± 0.40 days re-epithelization time and breaking strength (531.50 ± 5.89) (p<0.05). The hydroxyproline content of BG was found to be significantly higher i.e., 4.23 ± 0.21 (p<0.05). Quantitative estimation of BG exhibited 54.7 ± 3.7 mg100 g−1 of polyphenols and 42.3 ± 5.4 mg.100 g−1 flavonoids in terms of gallic acid and quercetin respectively. HPLC analysis revealed the presence of gallic acid and quercetin whereas the presence of fatty acids was confirmed by GC-MS analysis. It may conclude that the presence of quercetin, gallic acid, and fatty acids could have accelerated the healing rate of the ghrita formulation, as they have already been known for their potential wound healing properties. Stasis ulcers of the gaiter zone are difficult to heal due to deoxygenated blood congestion in peripheral areas and lack of oxygenated blood supply. Venous ulcers occur due to insufficiency of distal peripheral veins in legs. This manuscript refers to three cases of nonhealing venous leg ulcers treated with local application of Apamarga Kshara Taila along with some oral drugs. Three patients of nonhealing venous ulcer of the gaiter zone were treated with ayurvedic treatment protocol. In all patients after Panchavalkala Kwatha wound wash, Apamarga Kshara Taila was applied locally on wound once daily up to complete wound healing. In this series, wound healing was achieved in 96 days, 27 days, and 38 days, with a unit healing time of 4.311, 7.287, and 3.677 days/cm3, respectively. For the past 8 months, all three patients are living symptom-free life without any reoccurrence. On the basis of these cases, it can be said that Ayurveda principles of ulcer management have shown a promising role in wound healing and resuming improved quality of life without any untoward effects.
