1 Twelve patients with mild hypertension were treated with bumetanide for a six-month period. No evidence was found of hypokalaemia or decreased total exchangeable potassium in subjects with or without additional potassium supplements. 2 Bumetanide was well tolerated by all patients. Bumetanide is an effective diuretic for the treatment of ascites. Fifteen out of 17 patients with chronic liver disease responded satisfactorily and the incidence of hepatic encephalopathy and electrolyte disturbances was similar to that previously observed with frusemide. Colicky abdominal pain was reported in three patients but no other adverse effects were noted. An unexpected response was obtained in two patients who had diuresis without natriuresis. Oedema results from excessive reabsorption of filtered sodium for the renal tubules. The reabsorption is not due to intrinsic alterations in tubular function but secondary to changes in the peritubular environment. Diuretics however act by blocking reabsorption in the tubules themselves. The mechanisms by which they produce their effects are still poorly understood. Advances in knowledge of the patterns of sodium transport at different parts of the nephron have enabled us to localize their sites of action in man. Application of this knowledge in patients with resistant oedema enables us to decide how to alter the type or dose of diuretic or diuretic combination. [ABSTRACT FROM AUTHOR]

The clinical features of the Guillain-Barré syndrome in 34 Nigerian patients are reported. The patients were treated with oral prednisolone 20 mg three times daily for a minimum of six weeks. Twenty-seven patients recovered completely and five others showed considerable improvement, although three of them appeared to be `steroid dependent'. Two patients died. The good prognosis and the response to treatment with prednisolone in this series is in keeping with an allergic demyelinating neuropathy. [ABSTRACT FROM AUTHOR]
