Experience with numbers of drug addicts has shown them to be singularly unmotivated to discontinue drug use. To develop motivation in the unmotivated, the treatment program described here implemented a four-stag, scheme of motivational progression: (1) confrontation of the patient with his addiction, (2) development in the patient of an Intellectual understanding of why he uses drugs, (3) development in the patient of an Intellectual conviction that drugs are harmful, and (4) development of an awareness in the patient of the role his feelings play in his addiction. Described here is the experience of the Drug Addiction Treatment Units of Boston State Hospital and Boston City Hospital, which were established in 1964. This approach to rehabilitation of the addict is three pronged—involving the disciplines of social work, medicine, and psychiatry—and has its major emphasis in a work training program aimed at providing the addict with job skills and regular employment. C. Andre St. Pierre, in his article "A Treatment Program for the Drug-Dependent Patient" in the October 1969 issue, creates the erroneous impression that the Drug Addiction Rehabilitation Program at Boston State Hospital is run on the lines he describes. The Boston State Hospital program was initiated in July 1964 and the Boston City Hospital program around August 1966. The orientation of the program is a drug- free environment, with Methadone used in the withdrawal of heroin abusers. The milieu is that of a therapeutic community, with intensive group confrontation of the "here and now". One-to-one therapy is rarely used during the early phase of treatment.
