Giant hydronephrosis in children, and particularly in infants, is frequently responsible for abdominal masses. High close intravenous urography has proved very useful in studying the degree of pyelocalyceal dilatation, as well as the amount of residual renal parenchyma and the possible recovery of these kidneys. Reparative surgery, rather than primary nephrectomy, should always be considered. 8. The Relationship of Kidney Disease to Hypertension in Childhood.

This article presents a discussion among health professionals that examines the relationship of kidney disease to hypertension in childhood. The patient for discussion is a six and one-half-year-old white girl from Nashua, New Hampshire. She is in good health until about one month prior to admission in a hospital. The pattern describes by the parents is that she would get up in the morning, vomit several times, feel better, go to school, and play normally the rest of the day. However, thirteen days prior to admission, the girl is seen by her family physician who noted that she is severely dehydrated and has a blood pressure of 180 over 110 mm.

This article examines cases of sudden death in the nephrotic syndrome. Sudden death unrelated to renal failure during the course of the nephrotic syndrome is not unknown. There has been a report that two patients died during adrenocortical steroid therapy, after developing hypertension and hyponatremia. Other similar cases are also reported to be successfully treated with infusions containing sodium chloride or with serum albumin. In this article, three children with the nephrotic syndrome in whom salt depletion could have been important in leading to unexpected death, is described. Awareness of this possibility and attention to the sodium metabolism of children with the nephrotic syndrome may prevent similar deaths in the future.

The following classical description was given by Blackfan
14 in 1924; ‘It is rarely seen in infancy or early childhood, but is found with increasing frequency until after puberty, after which time it diminishes rapidly and becomes infrequent in adult life. The children who have this condition are usually tall and slender. It is much more marked if the patient assumes the lordotic position. The prognosis is excellent. Some cases have been reported in which the albuminuria persisted throughout adult life but in a majority it disappears during adolescence.’ Thus it appears from the literature that the incidence of proteinuria in children may be 15 to 30 per cent or more. The maximum reported in a controlled series was 46.5 per cent. It is entirely possible that one-fourth and perhaps even one-third of all individuals may have orthostatic proteinuria sometime during adolescence, and especially those who are tall and thin. The condition seems to reach a peak at about the age of 15 or 16 years and declines rapidly after that. It occurs in only about 5 per cent of 17 and 18 year old youths and drops to about 2 per cent during the third decade. From 90 to 95 per cent of the proteinuria in children is orthostatic in type. It is more common in summer than in winter. With the exception of one author (who studied primarily adults) all follow up studies of this condition which we have reviewed have indicated a good prognosis, and in some series the follow up has been for 25 or more years. The incidence of subsequent renal disease has been very low. Most subjects have remained in good health. The high incidence in older children and adolescents, with a much lower incidence in adults, indicates that this proteinuria clears before adult life. As stated by R. J. Slator49 of the University of Toronto; ‘There seems no doubt there exists a phenomenon of benign postural proteinuria unassociated with renal disease, a physiologic variant of normal.’ [ABSTRACT FROM AUTHOR]- Published

Dialysis and transplantation are now acceptable modes of therapy for infants and children with end-stage renal disease. This necessitates a consideration of the appropriate role of the pediatric nephrology team in the care of these children. The passage of Federal legislation amending the Social Security Act to cover the extensive costs of dialysis and transplantation and the likelihood of the establishment of regional centers for adults and children further emphasizes the need to establish guidelines for personnel and facilities caring for infants, children and adolescents. Initially, we must assess the goals of dialysis and transplantation in children. The most desirable and obvious goal is to return renal function to normal through transplantation whenever possible.

Dr. James' text fills an important void in our educational program, particularly for those being trained under an organ-system-oriented curriculum. I have supplied the text to a number of junior and senior medical students during their elective in pediatric nephrology and have had a uniformly enthusiastic response by the students. The book is necessarily superficial in dealing with areas such as renal physiology and certain of the disease conditions considered, but even here gives adequate background and reference material for more in depth study by the interested student. The book is well illustrated by appropriate drawings and photomicrographs which add significantly to understanding. I feel that the text should also be very useful to physicians in practice who wish to obtain more current and often more in depth understanding of specific renal diseases than is provided by current textbook references.

This book is an excellent summary of patients with renal disease studied by the author during the "presteroid" era from the years 1929-1944 in New York, and from 1944-1952 in Los Angeles. It is an historical book and the author has beautifully summarized the earlier development of pediatric nephrology. This is by no means a textbook of pediatric nephrology, and may not be of practical use to the practicing pediatrician in terms of current methods of diagnosis and treatment since it lacks the more recent classification of kidney disease as based on renal pathology and immunofluorescent and electron microscopic studies of biopsy material. Current forms of therapy for patients with kidney disease are not discussed in detail. This book summarizes some of the problems associated with infection in patients with kidney disease prior to the penicillins. The book deals with those diseases with which the author has had fullest experience

This book reflects the coming of age of pediatric nephrology, as separate from the not-so-old specialty of nephrology. The text, intended as a "concise practical guide for pediatricians, urologists, general practitioners, and residents," fulfills its purpose well, in a restricted sense. The opening section on anatomy is well written, and the diagrams are satisfactory (although I prefer actual specimens to diagrams). The section concerned with function is given in places to mechanical explanations that suffer from oversimplifying a dynamic phenomenon. Before taking up specific renal disorders, the author describes their symptoms, signs, and laboratory tests. The treatment is quite conventional, but I believe useful to his chosen audience. Here and there I disagree, eg, a fundoscopic examination in patients with renal disease often is helpful well before exudates develop. A poor correlation between specific gravity and osmolality all but cancels the usefulness of specific gravity, except as a qualitative indicator

This volume took origin as a syllabus which accompanied a course in pediatric nephrology given at the Centre International de l'Enfance in 1961. Subsequent demand for copies of that syllabus led to its publication in a revised, expanded version. In its present form, this is virtually a textbook of pediatric nephrology, and one which should find a wide and appreciative audience. It is stated in the preface that this is a book by pediatricians and for pediatricians. Certainly its orientation is clinical, but there is much of value here for all who deal with problems of renal disease in childhood, whether at the bedside or in the laboratory. The text is divided into two large sections. The first 339 pages deal with "new aspects of renal pathology in childhood," the remainder of the book being devoted to therapeutic considerations. An introductory chapter deals with methods of investigation of renal function
