Three cases of lymphoid interstitial pneumonia (LIP) are described. Lung biopsies demonstrated a similar characteristic histologic appearance of diffuse interstitial lymphocytes. Two had increased IgM. One patient developed a reticulum cell sarcoma. All were treated with prednisone but only one improved.

The clinical history and histopathology of two patients with Caplan's syndrome are described. In one patient, whose recorded history extends for eighteen years, cavitation and pyogenic infection of the rheumatoid lesions was observed. A response to prednisolone and later to chloroquine therapy was demonstrated in a second patient, whose lesions resembled those seen in the category III pneumoconiosis. Various clinical and pathologic features of the nodular pulmonary manifestations of rheumatoid disease are discussed.

Since the description by Rosen, Castleman, and Liebow 1 of pulmonary alveolar proteinosis, various empiric methods of treatment have been proposed. Clinical improvement was noted in 3 of 21 cases during therapy with corticosteroids 1-10 and radiographic improvement on two 11,12 of seven patients 9,11,12,14 treated with potassium iodide. One of the two patients 12 who improved while receiving oral iodides also received streptokinase-streptodornase by aerosol. Aerosol therapy has been associated with clinical and rediographic improvement in three other cases—one received streptokinase-streptodornase 15 and two trypsin. 8,9 One patient 8 tolerated trypsin aerosol therapy for only six days, but the oral intake of 400 units of trypsin-chymotrypsin daily was associated with progressive clearing of the chest x-ray film. The efficacy of these regimens is still in question, however, since spontaneous remissions have been reported 1 and clinical observations in this rare disease have been, by necessity, limited and uncontrolled. Se utilizaron 101 vacas Holstein que dieron cría en 1968 en Palmira (hato 1) a 1.000 m.s.n.m. y en granja de Obonuco (hato 2) a 2.800 m.s.n.m. Se efectuaron 3 tratamientos: acetato de melengestrol suministrado por vía oral durante 10 días (1), acetato de melengestrol por 10 días y estrógeno 48 horas después de la última dósis de acetato, (2) vacas testigo no tratadas (3). En el hato 1, el manejo de las vacas fué en confinamiento y la alimentación a base de ensilaje (maíz y sorgo) con adición de concentrado. En el hato 2, el manejo fué en pastoreo y se suministraron concentrados. La combinación de acetato de melengestrol con benzoato de estradiol, en los 2 hatos, mostró un mayor control sobre la aparición del celo post-tratamiento, con diferencias significativas, en intervalo de días vacíos post-tratamiento. El tratamiento 2 también muestra cifras significativamente favorables. También se dan porcentajes elevados en la tasa de animales sincronizados como respuesta al tratamiento 2. SINCE the report, by Rosen, Castleman and Liebow,1 of the histologic entity of pulmonary alveolar proteinosis a variety of syndromes have been described. The clinical presentations have ranged from an asymptomatic pulmonary infiltrate, incidentally discovered,2 to an unexplained recurrent fever of one and a half years' duration.3 The roentgenograms of the chest in most reported cases have shown a perihilar, granular infiltrate radiating to the lower fields of the lung — the area of the lung with the greatest radiodensity. We have studied the lipid composition and protein content of 99 pulmonary washings from patients with asthma, chronic bronchitis, pulmonary adenomatosis, desquamative interstitial pneumonia, and pulmonary alveolar proteinosis. Phospholipid represented 31.1% to 47.0% of recovered lipid in patients with asthma and bronchitis. Patients with alveolar proteinosis had the highest lipid (mean value 129.4 mg/100 ml effluent) of which 56% was phospholipid. One patient with pulmonary adenomatosis had 42.4% phospholipid and one with desquamative interstitial pneumonia had 26%. Palmitic acid comprised 78% of the total fatty acids of phosphatidylcholine obtained from patients with alveolar proteinosis but only 62% in others. Repeated lung washings in alveolar proteinosis patients showed progressive decrease in lipid and protein concentration. Lipid and protein composition of human lung washings tends to reflect the nature of the underlying disease.
