In 296 nonindigent nulliparous women wearing the Dalkon Shield, the pregnancy and medical removal rates calculated at the end of 12 months were 5.6 and 28.7, respectively, per 100 women. The continuation rate was 58.3. Removal of the shield was most frequent during the first 24 hours after insertion, usually because of pain. Previous early abortion did not lessen the probability of medical removal. Metrorrhagia occurred in 22 per cent and increased menstrual flow was observed in 34 per cent of women who used the shield successfully 6 months or longer. The effect of the device on dysmenorrhea was inconsistent. The advantage of the Dalkon Shield is its low expulsion rate.
