SummaryFibrinogen, fibrinogen-fibrin breakdown products (b.d.p.), fibrin stabilizing factor (FSF), plasminogen, alpha-2 macroglobulin and alpha-1 antitrypsin concentrations were estimated immunologically in normal and in pathological synovial fluids from patients with RA, gout, monoarthritis, rheumatic fever, osteoarthritis and traumatic arthritis. The fibrinolytic activity was determined by the fibrinplate method.Normal synovial fluid contained no fibrinogen, b.d.p., FSF or alpha-2 macroglobulin, but traces of plasminogen and alpha-1 antitrypsin. By increasing inflammatory reactions increasing amounts of the various factors were demonstrable. Summary1) Normal rabbits injected intra-articularly (i.a.) with 0.5 ml Duck Anti-Rabbit Kidney Serum (DARKS) twice weekly for 6 weeks developed a progressive monoarthritis following a latent period averaging 10 weeks. Control rabbits treated similarly with Normal Duck Serum (NDS) failed to develop arthritis. 2) Rabbits with a remote history of immunization against duck serum developed an accelerated arthritis within one week following i.a. injections of either DARKS or NDS. 3) Nonimmunized animals preconditioned with isotonic NaCl or KC1 given instead of drinking water, developed accelerated arthritis within 2 to 4 weeks following i.a. injections not only of DARKS but of normal duck, calf or horse serum as well. 4) Circulating anti-duck serum precipitins always preceded development of arthritis induced by i.a. injections of duck serum. 5) The arthritic changes consisted of villous hypertrophy and infiltration of the synovium with lymphocytes and plasma cells. Monoarthritis is a common symptom in rheumatological practice. This study comprises 65 patients with monoarthritis having a duration of more than 3 months. The average observation time has been 3 years. The great majority in this material constitute the group with aseptic arthritis. The diagnosis at follow-up was: Benign arthritis 46 cases, villonodular synovitis 7, traumatic arthritis 5, rheumatoid arthritis 4, spondylarthritis anchylopoietica 1, tuberculosis 1, cancer arthritis 1. It is not possible by routine examination to differentiate between benign arthritis and rheumatoid arthritis. Histologically, we found a difference in degree between monoarthritis and classic rheumatoid arthritis evidenced as more pronounced fibrin deposits in rheumatoid arthritis. Comparison between operated and non-operated patients is not possible in this material. Summary1. Control animals injected with a similar dose of normal duck serum (NDS) failed to develop arthritis within a 6 months observation period. 3. The intra-articular injections of either NDS or DARKS produced high serum antibody titers precipitating both, NDS and DARKS prior to development of the arthritis. The titer disappeared as the arthritis developed in rabbits treated with DARKS but persisted in the NDS-treated control animals. 4. When either, DARKS or NDS, was injected into the joint of rabbits previously immunized with NDS, an accelerated arthritis developed within one week. 5.
