As a digital social medium, blogs have transformed into arenas where individuals can express their perspectives, concepts, and feelings. This research delves into the spread of information in blog posts related to the Uyghur discourse, utilizing topic modeling to classify and prioritize pertinent content. The analysis reveals a significant emphasis on the Chinese Government's suppression of Uyghur Muslims, as demonstrated by the hyperlinks embedded within the blog network. We mapped the discovered topics onto a network of hyperlinks, forming a topic-induced hyperlink network. Additionally, we conducted a toxicity analysis, which revealed a high level of toxicity in blog posts during that period. A morality assessment was also performed, revealing exceedingly high scores for all vices in the blog posts. Furthermore, we conducted an emotion analysis on the text and image data, which underscored a dominant sentiment of sadness pervading the blog posts and corresponding images. This reflects the severity of the Uyghur persecution. The research highlights the effectiveness of topic modeling, toxicity evaluation, and hyperlink analysis in shedding light on intricate social issues. A mob is an event that is organized via social media, email, SMS, or other forms of digital communication technologies in which a group of people (who might have an agenda) get together online or offline to collectively conduct an act and then disperse (quickly or over a long period). In recent years, these events are increasingly happening worldwide due to the anonymity of the internet, affordability of social media, boredom, etc. Studying such a phenomenon is difficult due to a lack of data, theoretical underpinning, and resources. In this research, we use the Agent-Based Modeling (ABM) technique to model the mobbers and the Monte Carlo method to assign random values to the factors extracted from the theory of Collective Action and conduct many simulations. We also leverage our previous research on Deviant Cyber Flash Mobs to implement various scenarios the mobber could face when they decide to act in a mob or not. This resulted in a model that can simulate mobs, estimate the mob success rate, and the needed powerful actors (e.g., mob organizers) for a mob to succeed. We finally evaluate our model using real-world mob data collected from the Meetup social media platform. AbstractPurposeMethodsResultsConclusionsThe Rotterdam Scoring System (RSS) attempts to prognosticate early mortality and early functional outcome in patients with traumatic brain injury (TBI) based on non-contrast head computed tomography (CT) imaging findings. The purpose of this study was to identify the relationship between RSS scores and long-term outcomes in patients with severe TBI.Consecutively treated patients with severe TBI enrolled between 2008 and 2011, in the prospective, observational, Brain Trauma Research Center database were included. The Glasgow Outcome Scale (GOS) was used to measure long-term functional outcomes at three, six, 12, and 24 months. GOS scores were categorized into favorable (GOS = 4–5) and unfavorable (GOS = 1–3) outcomes. RSS scores were calculated at the time of image acquisition.Of the 89 patients included, 74 (83.4%) were male, 81 (91.0%) were Caucasian, and the mean age of the cohort was 41.9 ± 18.5 years old. Patients with an RSS score of 3 and lower were more likely to have a favorable outcome with increased survival rates than patients with RSS scores greater than 3.The RSS score determined on the head CT scan acquired at admission in a cohort of patients with severe TBI correlated with long-term survival and functional outcomes up to two years following injury. Spinal serotonin enables neuro‐motor recovery (i.e., plasticity) in patients with debilitating paralysis. While there exists time of day fluctuations in serotonin‐dependent spinal plasticity, it is unknown, in humans, whether this is due to dynamic changes in spinal serotonin levels or downstream signaling processes. The primary objective of this study was to determine if time of day variations in spinal serotonin levels exists in humans. To assess this, intrathecal drains were placed in seven adults with cerebrospinal fluid (CSF) collected at diurnal (05:00 to 07:00) and nocturnal (17:00 to 19:00) intervals. High performance liquid chromatography with mass spectrometry was used to quantify CSF serotonin levels with comparisons being made using univariate analysis. From the 7 adult patients, 21 distinct CSF samples were collected: 9 during the diurnal interval and 12 during nocturnal. Diurnal CSF samples demonstrated an average serotonin level of 216.6 ±$ \pm $ 67.7 nM. Nocturnal CSF samples demonstrated an average serotonin level of 206.7 ±$ \pm $ 75.8 nM. There was no significant difference between diurnal and nocturnal CSF serotonin levels (p =.762). Within this small cohort of spine healthy adults, there were no differences in diurnal versus nocturnal spinal serotonin levels. Tranexamic acid (TXA) has long been utilized in spine surgery and can be administered through intravenous (IV) and topical routes. Although, topical and IV administration of TXA are both effective in decreasing blood loss during spine surgery, complications like deep vein thrombosis (DVT) and pulmonary embolism have been reported with the use of intravenous TXA (ivTXA). These potential complications may be mitigated through the use of topical TXA (tTXA). To assess optimal dosing protocols and efficacy of topical TXA in spine surgery, Embase, Ovid-MEDLINE, Scopus, Cochrane, and clinicaltrials.gov were queried for original research on the use of tTXA in adult patients undergoing spine surgery. Data parameters analyzed included blood loss, transfusion rate, thromboembolic, and other complications. Data was synthesized and confidence evaluated according to the Grades of Recommendation, Assessment, Development, and Evaluation approach. Nineteen studies were included in the final analysis with 2197 patients. Of the 18 published studies, 9 (50%) displayed high levels of evidence. Topical TXA showed a trend towards a lower risk of transfusion and complications. Protocols that used 1g tTXA showed a significantly reduced risk for transfusion when compared to controls (risk ratio −1.05, 95% CI (−1.62, −0.48); P = 0.94, I2 = 0%). Complications associated with tTXA included DVTs and wound infections. Topical TXA was non-inferior to intravenous TXA with similar efficacy and complication profiles for bleeding control in spine surgery; however, more studies are needed to discern benefits and risks. Background Chiari malformations, usually congenital, can rarely be associated with arteriovenous (AV) fistulas. We present the first case involving a type IV dural AV fistula with a Chiari type I malformation. Methods Retrospective chart review was performed to obtain pertinent details regarding history and examination, pathologic findings, and treatment course. Results A 63-year-old woman with a 2-year history of migraines presented with 5 months of occipital, right-sided headaches and neck pain exacerbated by Valsalva maneuvers. Computed tomography (CT) and magnetic resonance imaging (MRI) of the head showed a possible right occipital AV malformation, bilateral cerebellar subdural hygromas, and tonsillar crowding at the foramen magnum indicating an acquired Chiari type I malformation. Angiography demonstrated a Cognardtype IV right posterior occipital dural AV fistula supplied by bilateral middle meningeal and posterior meningeal arteries. Conclusion After treatment of the dural AV fistula, hygroma evacuation, and decompression of the acquired Chiari malformation, the patient's Valsalva-induced headaches abated. [ABSTRACT FROM AUTHOR]

Background and Objectives: The "July Effect" hypothesizes increased morbidity and mortality after the addition of inexperienced physicians at the beginning of an academic year. However, the impact of newer members on neurosurgical teams managing patients with traumatic brain injury (TBI) has yet to be examined. This study conducted a nationwide analysis to evaluate the existence of the "July Effect" in the setting of patients with TBI., Methods: The Healthcare Cost and Utilization Project Central Distributor's National Inpatient Sample data set was queried for patients with TBI using International Classification of Diseases (ICD)-9 and ICD-10 codes. Discharges were included for diagnoses of traumatic epidural, subdural, or subarachnoid hemorrhages. Only patients treated at teaching hospitals were included to ensure resident involvement in care. Patients were grouped into July admission and non-July admission cohorts. A subgroup of patients with neurotrauma undergoing any form of cranial surgery was created. Perioperative variables were recorded. Rates of different complications were assayed. Groups were compared using χ2 tests (qualitative variables) and t-tests or Mann-Whitney U-tests (quantitative variables). Logistic regression was used for binary variables. Gamma log-linked regression was used for continuous variables., Results: The National Inpatient Sample database yielded a weighted average of 3 160 452 patients, of which 312 863 (9.9%) underwent surgical management. Patients admitted to the hospital in July had a 5% decreased likelihood of death (P = .027), and a 5.83% decreased likelihood of developing a complication (P < .001) compared with other months of the year. July admittance to a hospital showed no significant impact on mean length of stay (P = .392) or routine discharge (P = .147). Among patients with TBI who received surgical intervention, July admittance did not significantly affect the likelihood of death (P = .053), developing a complication (P = .477), routine discharge (P = .986), or mean length of stay (P = .385)., Conclusion: The findings suggested that there is no "July Effect" on patients with TBI treated at teaching hospitals in the United States., (Copyright © Congress of Neurological Surgeons 2024.
