BACKGROUND: This Population, Intervention, Comparison, and Outcomes-guided systematic review assesses continuous lateral rotation therapy versus conventional position changes in mechanically ventilated critically ill adults, evaluating mortality, ICU length of stay (LOS), and hospital LOS as primary outcomes and respiratory function, mechanical ventilation duration, pulmonary complications, and adverse events as secondary outcomes. METHODS: This systematic review follows Preferred Reporting Items for Systematic Reviews and Meta-Analyses criteria (International Prospective Register of Systematic Reviews CRD42022384258). Searches spanned databases MEDLINE/PubMed, Embase, Scopus, ScienceDirect, Cochrane, CINAHL, and Web of Science, without language or publication year restrictions. Inclusion criteria involved randomized controlled trials (RCTs) and quasi-randomized trials, comparing continuous lateral rotation therapy (intervention) with conventional position changes (control). Risk of bias and quality of evidence for RCTs were assessed using the Cochrane Collaboration and Grading of Recommendations Assessment, Development, and Evaluation tools. For the quasi-randomized trials, the Risk of Bias in Non-Randomized Studies-of Interventions tool was used. RESULTS: In 18 studies with 1,466 participants (intervention, n = 700, 47.7%; control, n = 766, 52.2%), continuous lateral rotation therapy was predominantly used for prophylactic purposes, with protocols varying from 10-24 h/d. Meta-analysis (16 RCTs) favored continuous lateral rotation therapy for reduced mechanical ventilation duration (standardized mean difference [SMD] -0.17 [CI -0.29 to -0.04] d, P = .008) and lower nosocomial pneumonia incidence (odds ratio 0.39 [CI 0.29-0.52], P < .001). Continuous lateral rotation therapy showed no significant impact on mortality (odds ratio 1.04 [CI 0.80-1.34], P = .77), ICU LOS (SMD -0.11 [CI -0.25 to 0.02] d, P = .11), hospital LOS (SMD -0.10 [CI -0.31 to 0.11] d, P = .33), and incidence of pressure ulcers (odds ratio 0.73 [CI 0.34-1.60], P = .44). CONCLUSIONS: Continuous lateral rotation therapy showed no significant difference in primary outcomes (mortality, ICU and hospital LOS) but revealed significant differences in secondary outcomes (consistently reduced nosocomial pneumonia, with a minor effect on mechanical ventilation duration), supported by moderate certainty. Very low certainty for other outcomes highlights the need for current studies in diverse clinical settings and protocols to assess continuous lateral rotation therapy effectiveness. To assess a quality improvement project using alternating pressure air mattresses' impact on reducing full‐thickness pressure injuries by enhancing setting accuracy and device utilization. We retrospectively evaluated adult acute care unit patients with inclusion criteria (Braden scores ≤12 or existing full‐thickness pressure injuries) between May 2020 and August 2023. A wound team attempted to enhance the accuracy, utilization and effectiveness of alternating pressure air mattress implementation. The implementation outcomes were setting accuracy and accurate utilization rates. The clinical outcome was the full‐thickness pressure injury proportion. Utilization and allocation gaps were also calculated. The setting accuracy and accurate utilization rates increased (0.59 to 0.88 and 0.15 to 0.37, respectively). The full‐thickness pressure injury proportion decreased (0.17 to 0.06), with a strong negative correlation coefficient (−0.789) (p < 0.001) with accurate utilization rates. The full‐thickness pressure injury proportion declined faster during the project's complete phase than the partial phase (−0.0046 vs. −0.0016; p < 0.05). The utilization gap narrowed (99 to 60); however, the allocation gap increased (1 to 13), suggesting increased alternating pressure air mattress usage among ineligible patients. Targeting high‐risk patients for alternating pressure air mattress utilization and ensuring correct settings, both performed by a dedicated team, substantially reduces the full‐thickness pressure injury incidence. Sudden infant death syndrome (SIDS) is an unexplained and distressing phenomenon that has plagued parents for years until research determined there was a link between the infant's sleeping position and sudden death. The risk of SIDS doubled when infants were placed on their stomachs to sleep. In 1994, the "Back to Sleep" campaign was introduced to teach caregivers and parents about the dangers of sudden infant death and sleep positions. In expeditionary environments, the consistent availability of blood for casualty care is imperative yet challenging. Responding to evidence and the specific needs of its expeditionary context, the US Central Command (USCENTCOM) prioritized supplying stored low titer O whole blood (LTOWB) to its units from March, 2023 onward. A strategy was devised to set minimal LTOWB on-hand supply benchmarks, determined by the number of operating beds and point of injury teams. This transition led to a 54% reduction in orders for packed red blood cells. As a countermove, the Armed Services Blood Program (ASBP) enhanced LTOWB production at a conversion rate 2:1 from packed red blood cell to LTOWB. Consequently, there was a decline in expired blood products, and fulfillment rates for blood requests are projected to reach 100% consistently. This paper delves into the intricacies of the expeditionary blood supply, the rationale behind the LTOWB transition, the devised allocation strategy, and the subsequent impacts of this change. This study proposes porous block structures as a viable alternative to conventional pebble bed structures, aiming to overcome the structural drawbacks including low packing fractions, extensive stress concentrations, and low thermal conductivity. To validate the feasibility of the porous block structures, Gyroid Li 2 TiO 3 block breeders were prepared by the hybrid process of stereolithography and gelcasting. The resulting Gyroid Li 2 TiO 3 block breeders, measuring ∼18.8 × 18.8 × 18.8 mm3, achieved a high packing fraction of ∼78.8% through optimized slurry and sintering at 1100 °C for 3 h. These Gyroid Li 2 TiO 3 block breeders exhibited clear contours, integrated channels, a compressive strength of 8.78 MPa, and a low isotropic sintering shrinkage of ∼6%. The hybrid process demonstrated enormous advantages over the reported 3D printing for the preparation of porous block breeders with low shrinkage and large-sized dimensions. Prolonged mechanical loading of the skin and underlying soft tissue cause pressure ulceration. The use of special support surfaces are key interventions in pressure ulcer prevention. They modify the degree and duration of soft tissue deformation and have an impact on the skin microclimate. The objective of this randomized cross‐over trial was to compare skin responses and comfort after lying for 2.5 h supine on a support surface with and without a coverlet that was intended to assist with heat and moisture removal at the patient/surface interface. In addition, physiological saline solution was administered to simulate an incontinence episode on the mattress next to the participants' skin surface. In total, 12 volunteers (mean age 69 years) with diabetes mellitus participated. After loading, skin surface temperature, stratum corneum hydration and skin surface pH increased, whereas erythema and structural stiffness decreased at the sacral area. At the heel skin area, temperature, erythema, and stratum corneum hydration increased. These results indicate occlusion and soft tissue deformation which was aggravated by the saline solution. The Gini coefficient, Lorenz curve, Theil index and agglomeration degree were used to analyze the equity of the bed allocation, and the grey prediction model GM(1,1) was used to predict the development from 2022 to 2026.Results: From 2017 to 2020, the Gini coefficient of bed allocation of the department of stomatology in Chinese hospitals was below 0.2 by population. From 2017 to 2021, the Gini coefficient of beds was above 0.6 by geography and between 0.2 and 0.3 by economy. The Theil index of beds ranged from 0.022 to 0.056 by population, from 0.532 to 0.564 by geography, and from 0.042 to 0.047 by economy. The inequity in the allocation by population was mainly from between regions, and the inequity in the allocation by geography and economy was mainly from within regions. Health resource agglomeration degree (HRAD) was greater than 2 in the eastern and central regions and less than 1 in the western region. HRAD/ population agglomeration degree (PAD) was greater than 1 in the northeast, eastern, and central regions and less than 1 in the western region. According to the prediction, the number of beds of the department of stomatology in Chinese hospitals will continue to increase, reaching 47,862.485 in 2026.Conclusion: The equity of bed allocation was better by population and economy than by geography. The equity of beds in the western region is insufficient equity by population and geography, and the equity of beds in the eastern region is insufficient equity by economy.Keywords: department of stomatology, beds, equity, prediction, China

Background: The Standard Inpatient Class Policy (KRIS) in Indonesia aims to provide equitable healthcare services, ensuring the same standards in inpatient wards across all hospitals in Indonesia. This policy has been issued since 2021 and is mandatory for all hospitals to implement by December 2024. The implementation of the KRIS policy impacts hospitals, necessitating analysis and mitigation of risks arising from its implementation. This study aims to provide an overview of the analysis and mitigation of risks in the standard inpatient class implemented by Hospital X. Method: Qualitative research using Focus Group Discussions (FGDs) in November 2023 involving 7 key informants, including Hospital Directors and Department Heads, regarding the mitigation of risks in the standard inpatient class using risk analysis matrices. Results: There are two criteria for the standard inpatient class that pose a very high level of risk, namely the building components lacking high porosity and the density of ward spaces and bed spacing. Conclusion: Hospital X implements risk mitigation by taking actions to reduce the impact of risks through room improvement based on related room reports, risk reduction mitigation by planning inpatient building renovations, and risk acceptance mitigation by allowing ward areas to be 9.8 square meters per bed. Two novel vaccines against malaria are proposed as a complementary control tool to prevent and reduce Plasmodium falciparum related disease and death in under-five children from moderate to high malaria transmission regions. The Democratic Republic of Congo (DRC) has committed to eradicate malaria by 2030, and significant efforts have been deployed to strengthen control and elimination measures. We aimed to understand factors influencing the malaria vaccine acceptability among the general population in eastern DRC. We conducted a survey among adult Congolese in Bukavu in March 2022. The questionnaire was adapted from the Behavioral and Social Drivers of vaccine uptake (BeSD) framework and was administered online and physically. Multivariate logistic regressions were built, and estimates were represented as adjusted odds ratios (aOR) and corresponding 95% confidence intervals (95%CI). Out of 1612 adults (median age: 39 years, 46.15% female) surveyed, only 7.26% were aware of the malaria vaccine. However, 46.53% expressed willingness to vaccinate themselves, and 52.60% were open to vaccinating their under-five children. Adjusting for confounding factors, non-student/non-healthcare worker professions (aOR = 0.58, 95%CI [0.42–0.78]) and middle-income status (aOR = 1.87, 95%CI [1.25–2.80]) were significantly associated with self-vaccination acceptance. Age played a role in under-five child vaccination acceptability, with 25 to over 64 years showing increased acceptability compared to the 18–24 age group. Additionally, non-student/non-healthcare worker professions (aOR = 1.88, 95%CI [1.37–2.59]), medium education levels (aOR = 2.64, 95%CI [1.29–5.79]), and residing in semi-rural areas (aOR = 1.63, 95%CI [1.27–2.10]) were predictors of under-five child vaccination acceptance. The acceptability of the malaria vaccine for self and for under-five children was suboptimal for effective malaria control in this community in the DRC. Our study constitutes a call for the Expanded Program on Immunization to closely work with various stakeholders to strengthen risk communication for community engagement prior to and during the introduction of this novel and lifesaving tool, malaria vaccination. Understanding the contributions of stream bank and bed erosion will allow us to implement the most effective management practices. The objective of this study was to assess different methods to measure bank and bed erosion at different scales, specifically the watershed, reach and plot. Innovative and traditional methods were utilized. At the watershed scale, indices based on free satellite images were used. For the reach scale, indices were used, but the images with higher accuracy were purchased and captured by unmanned aerial vehicles (UAVs). At the plot scale, erosion pins, cross-sections and laser scanning were applied. The watershed scale analysis showcased "hot spots". These "hot spots" were reaches vulnerable to erosion and deposition. The indices of the purchased images were applied to these "hot spots" and allowed us to narrow the length of the reaches where UAV flights took place. These flight images located where erosion and deposition occurred. Finally, at the plot scale, laser scanning provided more detailed and accurate data at a greater scale compared to the traditional methods. The implementation of these methods allows us to find the areas vulnerable to erosion and deposition. These are the areas where nature-based solutions should be implemented to effectively mitigate erosion problems. [ABSTRACT FROM AUTHOR]

Housing has long been the quintessential rentier asset. But under financialized capitalism its enrolment into accumulation dynamics has greatly intensified. As investors increasingly turn to residential real estate in search of corporate rents, the logic of assetization is reaching novel locations in the housing process – extending to new scales, metrics and micro-morphologies. This paper argues one such novel location is that most intimate and familiar of places: the bed. Bringing together constructivist and political economy approaches to assets and drawing on the empirical case of co-living, the bed is identified as both a technical tool for projecting and enhancing income from real estate, and a strategy for de-risking investments by hyper-focusing on the necessities of life. The SiC reticulated porous ceramics (RPCs) fabricated via sponge replica method usually showed a poor thermal shock resistance due to their randomly distributed skeleton structure and hollow struts. The present work reports a simple approach to prepare SiC RPCs with 3D lattices structure using the replica of printed resin, followed by pre-heated in coke bed and vacuum infiltration. The increased Si content and flake graphite addition promoted the formation of SiC whiskers network structure on the surface and matrix of SiC preform after pre-heated at 1400 °C. As SiC preforms were applied by vacuum infiltration and sintering, the three-layer struts included dense coating, SiC skeleton layer and filling layer were generated. Furthermore, in-situ SiC whiskers eliminated the coating cracks, also optimized the microstructure of the SiC matrix, thereby enhancing the thermal shock resistance and strength of SiC RCPs, whose strength increased from 0.36 MPa to 1.53 MPa, also the residual strength retention rate increased from 0 % to 75.2 % after three cycles of water- quenching. The kelp forest and bed of Laminaria ochroleuca were found in October 2023 in Porticello (Scylla -Reggio Calabria, Italy) between 48 and 55 m depth. Samples were collected for future genetic and sedimentological analyses and identification of associated fauna and flora. The aim of the upcoming research campaigns is to update the current knowledge of Laminaria forests in the Messina Strait, estimating the action and consequences of climate change at the local level. The project will involve the community of technical divers in long term monitoring (3–5 years), in order to measure ecosystem services. To descriptively analyse Australian public sector General Mental Health Services (GMHS) expenditure, ambulatory, and inpatient services, including key performance indicators (KPIs) in comparison with other subspeciality mental health services (MHS).We descriptively analysed data published by the Australian Institute of Health and Welfare (AIHW), including inpatient, ambulatory services, expenditure, and KPIs.From 2017–18 to 2021–22, per capita expenditure for Australian GMHS (18–64) rose by an average annual inflation-adjusted change of 2%. Overall bed numbers remained static, with non-acute beds declining, and commensurate expansion of acute beds. Community GMHS had high outpatient utilisation, with high rates of schizophrenia, schizoaffective disorder, and bipolar affective disorders as primary diagnoses in mid-life. From 2017–18 to 2021–22, GMHS inpatient and ambulatory episodes had decreasing rates of significant improvement and increasing rates of significant deterioration.Although GMHS has the highest overall population and service utilisation, there has been static bed availability and relatively small increases in expenditure which are occurring concurrently with worsening clinical outcomes. Conventional patient monitoring in healthcare has limitations such as delayed identification of deteriorating conditions, disruptions to patient routines, and discomfort due to extensive wiring for bed-bound patients. To address these, we have recently developed an innovative IoT-based healthcare system for real-time wireless patient monitoring. This system includes a flexible epidermal patch that collects vital signs using low power electronics and transmits the data to IoT nodes in hospital beds. The nodes connect to a smart gateway that aggregates the information and interfaces with the hospital information system (HIS), facilitating the exchange of electronic health records (EHR) and enhancing access to patient vital signs for healthcare professionals. Our study validates the proposed smart bed architecture in a clinical setting, assessing its ability to meet healthcare personnel needs, patient comfort, and data transmission reliability. Technical performance assessment involves analyzing key performance indicators for communication across various interfaces, including the wearable device and the smart box, and the link between the gateway and the HIS. Also, a comparative analysis is conducted on data from our architecture and traditional hospital equipment. Usability evaluation involves questionnaires completed by patients and healthcare professionals. Prolonged sleeping postures or unusual postures can lead to the development of various ailments such as subacromial impingement syndrome, sleep paralysis in the elderly, nocturnal gastroesophageal reflux, sore development, etc Fibre Bragg Gratings (a variety of optical sensors) have gained huge popularity due to their small size, higher sensitivity and responsivity, and encapsulation flexibilities. However, in the present study, FBG Arrays (two FBGs with 10 mm space between them) are employed as they are advantageous in terms of data collection, mitigating sensor location effects, and multiplexing features. In this work, Liquid silicone encapsulated FBG arrays are placed in the head (E), shoulder (C, D), and lower half body (A, B) region for analyzing the strain patterns generated by different sleeping postures namely, Supine (P1), Left Fetus (P2), Right Fetus (P3), and Over stomach (P4). These strain patterns were analyzed in two ways, combined (averaging the data from each FBG of the array) and Individual (data from each FBG was analyzed separately). Both analyses suggested that the FBGs in the arrays responded swiftly to the strain changes that occurred due to changes in sleeping postures. 3D histograms were utilized to track the strain changes and analyze different sleeping postures. A discussion regarding closely related postures and long hour monitoring has also been included. Arrays in the lower half (A, B) and shoulder (C, D) regions proved to be pivotal in discriminating body postures. The average standard deviation of strain for the different arrays was in the range of 0.1 to 0.19 suggesting the reliable and appreciable strain-handling capabilities of the Liquid silicone encapsulated arrays., (© 2024 IOP Publishing Ltd. Sleep is a crucial aspect of geriatric assessment for hospitalized older adults, and implementing AI-driven technology for sleep monitoring can significantly enhance the rehabilitation process. Sleepsense, an AI-driven sleep-tracking device, provides real-time data and insights, enabling healthcare professionals to tailor interventions and improve sleep quality. This study explores the perspectives of an interdisciplinary hospital team on implementing Sleepsense in geriatric hospital care. Using the interpretive description approach, we conducted focus groups with physicians, nurses, care aides, and an activity worker. The Consolidated Framework for Implementation Research (CFIR) informed our thematic analysis to identify barriers and facilitators to implementation. Among 27 healthcare staff, predominantly female (88.89%) and Asian (74.1%) and mostly aged 30-50 years, themes emerged that Sleepsense is perceived as a timesaving and data-driven tool that enhances patient monitoring and assessment. However, barriers such as resistance to change and concerns about trusting the device for patient comfort and safety were noted, while facilitators included training and staff engagement. The CFIR framework proved useful for analyzing implementation barriers and facilitators, suggesting future research should prioritize effective strategies for interdisciplinary team support to enhance innovation adoption and patient outcomes in rehabilitation settings.

A key best practice to prevent and treat pressure injuries (PIs) is to ensure at-risk individuals are repositioned regularly. Our team designed a non-contact position detection system that predicts an individual's position in bed using data from load cells under the bed legs. The system was originally designed to predict the individual's position as left-side lying, right-side lying, or supine. Our previous work suggested that a higher precision for detecting position (classifying more than three positions) may be needed to determine whether key bony prominences on the pelvis at high risk of PIs have been off-loaded. The objective of this study was to determine the impact of categorizing participant position with higher precision using the system prediction F1 score. Data from 18 participants was collected from four load cells placed under the bed legs and a pelvis-mounted inertial measurement unit while the participants assumed 21 positions. The data was used to train classifiers to predict the participants' transverse pelvic angle using three different position bin sizes (45°, ~30°, and 15°). A leave-one-participant-out cross validation approach was used to evaluate classifier performance for each bin size. Results indicated that our prediction F1 score dropped as the position category precision was increased.

Bed-sharing is a controversial but common parenting practice with claimed benefits for emotional and behavioral development. Using data from the UK Millennium Cohort Study ( N  = 16,599), this prospective study investigated whether bed-sharing at 9 months is associated with childhood internalizing and externalizing symptom trajectories. Children were grouped by their patterns of co-developing internalizing and externalizing symptoms from 3 to 11 years of age using a parallel process latent class growth analysis. There were no associations between bed-sharing at 9 months of age and internalizing and externalizing symptom trajectories across childhood. This finding suggests that bed-sharing at 9 months has no positive or negative influence on the development of internalizing and externalizing symptoms across childhood. Clinicians should inform parents that bed-sharing during the second half of the first year is unlikely to have an impact on the later emotional and behavioral development of the children.
