Additionally, the recent Supreme Court decisions in Students for Fair Admissions, Inc. (SFFA) v. the University of North Carolina and SFFA v. President & Fellows of Harvard College, which ended race-conscious admissions, or affirmative action, and upheld holistic admissions practices, encouraged medical school admissions committee to reevaluate their approach to admissions.Methods: Data from six admissions cycles (2018– 2022), totaling 5799 interviewees, were analyzed to assess potential biases and the effectiveness of the admissions process. Spearman correlation examined relationships between Casper scores and both traditional interview and MMI outcomes. T-tests and Cohen’s d explored demographic differences across URM, African American, Hispanic, and disadvantaged applicants in interview and academic metrics to evaluate fairness.Results: When comparing MMIs and traditional interviews, both appear equal in reducing group differences (Underrepresented in Medicine(URM) versus non-URM, African American to non-URM, Hispanic to non-URM, and disadvantaged to non-disadvantaged). MMIs decreased demographic differences compared with traditional interviews for African-American candidates and slightly increased for URM, Hispanic, and disadvantaged candidates, but the effect size was small.Conclusion: Future work includes conducting rigorous data analysis to better assess the MMI’s utility, exploring the correlations between MMI scores, clinical evaluations, and objective structured clinical examination (OSCE) results. This multifaceted approach will provide a comprehensive view of how MMI performance aligns with real-world clinical assessments and standardized evaluation metrics, offering valuable insights into its effectiveness as a predictor of future medical proficiency.Keywords: medical student selection, race-neutral admissions, multiple-min-interview, holistic admissions

Black girls in the USA face disproportionate rates of sexually transmitted infections (STIs), HIV, and sexual violence, prompting research into the sociocultural factors such as adultification, race and gender discrimination, and stereotyped messaging, impacting Black girls' sexual development. While existing literature finds that Black female caregivers utilise protective strategies to mitigate potential harm to Black girls, little is known about the role of Black male caregivers. This qualitative study employed the Becoming a Sexual Black Woman framework and one-on-one interviews to examine how 30 Black male caregivers of girls conceptualised parental protection and elated strategies. The thematic analysis revealed key themes of: Being a Provider, Being a Present Role Model, Protecting Black Girls' Bodies, Limiting Exposure to Stereotyped Messages, and Providing Emotional Protection. Black male caregivers navigated traditional gendered expectations and limited resources while utilising parental strategies that reflected patriarchal, sometimes misogynistic, and non-patriarchal ideals and norms. The study highlights the attitudes and beliefs of Black male caregivers regarding protecting Black girls from sexual violence and other related adverse outcomes. The findings offer insights for family-based interventions and programmes that empower Black girls to support their sexual development and health in the USA.

Purpose: To evaluate the safety and efficacy of 0.75% phentolamine ophthalmic solution (POS), an α-adrenergic antagonist, in reversal of pharmacologically induced mydriasis., Design: Two phase 3, multicenter, placebo-controlled, randomized, double-masked clinical trials in healthy participants., Participants: Five hundred fifty-three healthy 12- to 80-year-old participants were randomized 1:1 (MIRA 2) and 2:1 (MIRA 3) to receive either POS or placebo eye drops in both eyes., Methods: Participants received POS or placebo administered 1 hour after mydriasis, induced by instillation of either 2.5% phenylephrine, 1% tropicamide, or 1% hydroxyamphetamine / 0.25% tropicamide., Main Outcome Measures: Percent of participants returning to within 0.2 mm of baseline pupil diameter in study eye 90 minutes after POS administration. Safety measures included treatment-emergent adverse events and tolerability measures, including conjunctival hyperemia., Results: A total of 553 participants were randomized to treatment with placebo (n = 215) or POS (n = 338). A statistically significant greater percentage of participants treated with POS showed reversal of mydriasis at 90 minutes compared to placebo (MIRA 2: 48.9% vs. 6.6% [P < 0.0001]; MIRA 3: 58% vs. 6% [P < 0.0001]) and as early as 60 minutes (MIRA 2: 27.7% vs. 2.2% [P < 0.0001]; MIRA 3: 42% vs. 2% [P < 0.0001]). Between 28% and 34% of participants receiving placebo did not returned to baseline PD at 24 hours after pharmacologic dilation compared with 8% to 11% of patients treated with POS (P < 0.0001)., Conclusions: Treatment with POS reduced PD within 60 to 90 minutes, with a statistically significant time savings of 5 to 6 hours to return to baseline PD compared with placebo. One or 2 drops of POS rapidly reversed mydriasis in all participants regardless of mydriatic agent or iris color. More participants receiving POS reported a benefit in the resolution of visual symptoms caused by pharmacologically induced mydriasis compared with placebo, with statistically significant differences noted as early as 1 hour. Common variable immunodeficiency (CVID) is a type of primary immunodeficiency that presents as a heterogenous disorder characterized by hypogammaglobinemia, poor response to vaccines, recurrent sinopulmonary infections, and can have noninfectious systemic manifestations. We performed a single-center, retrospective, observational study of five patients with noninfectious complications of CVID. All patients had CVID as defined by the European Society of Immunodeficiencies criteria and had received intravenous immunoglobulin therapy. There were multiple pulmonary manifestations of CVID including frequent pneumonias, bronchiectasis, granulomatous lung disease, and pulmonary hypertension. All our patients were treated with pulmonary vasodilators for severe precapillary pulmonary hypertension along with individualized immunosuppression regimen for interstitial lung disease. Despite treatment for interstitial lung disease and PH, their conditions worsened over 2-3 years with all patients progressing toward organ transplant evaluation. Idiopathic thrombocytopenia and non-cirrhotic portal hypertension were common, with three patients probably suffering from nodular regenerative hyperplasia. Noninfectious complications of CVID can affect different organs and progress despite advanced therapies.
