The present study is aimed at assessing the presence and prevalence of subclinical entheseal changes in Psoriasis (PsO) patients using musculoskeletal ultrasonography (US), conjoined with the analysis of possible differences in terms of demographic, clinical, or biological features. We carried out an observational study on 54 patients with PsO and 40 controls. Subclinical enthesopathy, according to OMERACT definitions, was identified in 20 of the psoriasis patients (37.03%), a significantly difference compared to the controls (5 patients; 10.20%). A comparison between US examinations for psoriasis patients and controls indicates that all the examined areas manifested changes in a significantly higher percentage of patients than the controls. The most common structural changes were represented by thickened tendon (85%), calcification (65%), erosions (35%), power Doppler (PD) signal (20%), and bursitis (5%). The difference in mean MASEI (Madrid Sonographic Enthesitis Index) score between the psoriasis and control groups was statistically significant (10.56 + 2.96 vs. 2.9 + 2.20; p < 0.0001). In conclusion, ultrasound is an easily accessible and vital follow-up method for psoriasis patients to enable an early, subclinical detection of entheseal involvement, i.e., the first red-flag sign for a future transition to psoriatic arthritis (PsA). The accurate diagnosis of gout frequently constitutes a challenge in clinical practice, as it bears a close resemblance to other rheumatologic conditions. An undelayed diagnosis and an early therapeutic intervention using uric acid lowering therapy (ULT) is of the utmost importance for preventing bone destruction, the main point of managing gout patients. Advanced and less invasive imaging techniques are employed to diagnose the pathology and ultrasonography (US) stands out as a non-invasive, widely accessible and easily reproducible method with high patient acceptability, enabling the evaluation of the full clinical spectrum in gout. The 2023 EULAR recommendations for imaging in diagnosis and management of crystal-induced arthropathies in clinical practice state that US is a fundamental imagistic modality. The guidelines underline its effectiveness in detecting crystal deposition, particularly for identifying tophi and the double contour sign (DCS). Its utility also arises in the early stages, consequent to synovitis detection. US measures of monosodium urate (MSU) deposits are valuable indicators, sensitive to change consequent to even short-term administration of ULT treatment, and can be feasibly used both in current daily practice and clinical trials. This paper aimed to provide an overview of the main US features observed in gout patients with reference to standardized imaging guidelines, as well as the clinical applicability both for diagnosis accuracy and treatment follow-up. Our research focused on summarizing the current knowledge on the topic, highlighting key data that emphasize gout as one of the few rheumatological conditions where US is recognized as a fundamental diagnostic and monitoring tool, as reflected in the most recent classification criteria. Objectives: The study has as main objective the evaluation of the potential roles of vitamin D, the neutrophil to lymphocyte ratio (NLR), and the systemic inflammation index (SII) as future biomarkers regarding the classification of flares in systemic lupus erythematosus (SLE). Material and Methods: Individuals diagnosed with SLE were encompassed in this observational study. The current applicable criteria, namely The European Alliance of Associations for Rheumatology (EULAR)/American College of Rheumatology (ACR) 2019 criteria had to be fulfilled. The participants underwent specific musculoskeletal examination, paraclinical investigations including complete blood count (CBC), determination of serum creatinine levels, as well as liver enzymes, and also the markers of inflammation. The fractions of the serum complement (C3 and C4) were also evaluated, together with serum vitamin D concentrations. Safety of Estrogens in Lupus National Assessment-Systemic Lupus Erythematosus Disease Activity Index (SELENASLEDAI) was required in order to analyze the research group's disease activity. Results: NLR and SII demonstrated validity, having statistically significant correlations with SELENA-SLEDAI (p value less than 0.001). The ROC analysis proved a strong discriminative power for NLR (AUC=0.96) and SII (AUC=0.963) in predicting severe disease flares. Optimal cut-off values were 3.45 for NLR and 877,002.19 for SII. Serum vitamin D concentrations had a weak association with the SLEDAI score (p=0.048, r=0.213). Conclusions: NLR and SII can be considered reliable biomarkers for discriminating between the levels of disease activity in SLE individuals. Psoriasis is a chronic recurrent inflammatory autoimmune pathology with a significant genetic component and several interferences of immunological cells and their cytokines. The complex orchestration of psoriasis pathogenesis is related to the synergic effect of immune cells, polygenic alterations, autoantigens, and several other external factors. The major act of the IL-23/IL-17 axis, strongly influencing the inflammatory pattern established during the disease activity, is visible as a continuous perpetuation of the pro-inflammatory response and keratinocyte activation and proliferation, leading to the development of psoriatic lesions. Genome-wide association studies (GWASs) offer a better view of psoriasis pathogenic pathways, with approximately one-third of psoriasis's genetic impact on psoriasis development associated with the MHC region, with genetic loci located on chromosome 6. The most eloquent genetic factor of psoriasis, PSORS1, was identified in the MHC I site. Among the several factors involved in its complex etiology, dysbiosis, due to genetic or external stimulus, induces a burst of pro-inflammatory consequences; both the cutaneous and gut microbiome get involved in the psoriasis pathogenic process. Cutting-edge research studies and comprehensive insights into psoriasis pathogenesis, fostering novel genetic, epigenetic, and immunological factors, have generated a spectacular improvement over the past decades, securing the path toward a specific and targeted immunotherapeutic approach and delayed progression to inflammatory arthritis. This review aimed to offer insight into various domains that underline the pathogenesis of psoriasis and how they influence disease development and evolution. The pathogenesis mechanism of psoriasis is multifaceted and involves an interplay of cellular and humoral immunity, which affects susceptible microbiota and the genetic background. Osteoarthritis (OA) is a persistent joint condition resulting from the disturbance of joint homeostasis due to various systemic and biomechanical factors. This condition is marked by the deterioration of cartilage and other joint tissues, accompanied by low-grade inflammation that can lead to symptoms such as pain, diminished function, and disability. Objective. The study assessed the impact of frequently prescribed pharmacological treatments on structural changes in the knee among participants experiencing progressive OA using MRI. Material and Methods. The study was conducted on a number of 34 patients with bilateral knee OA (22 women and 12 men), aged between 35 and 78 years. Selected patients were evaluated at the initial visit, then 6 months after administration of glucosamine, chondroitin sulfate and Harpagophytum procumbens therapy. From a clinical point of view, morning stiffness and pain intensity (visual analogue scale – VAS), knee mobility and functionality were quantified with the help of the WOMAC and the quality-of-life questionnaire (HAQ – 20 items). In terms of imaging evaluation bilateral knee MRI was performed. Measurements of the cartilage thickness were performed both at the level of the lateral and medial condyles, as well as intercondylar. Outcomes. The average age of onset of OA was 55.06 ± 10.05 years. The mean duration of the disease at the time of inclusion was 6.32±3.45 years. The mean VAS value at baseline was 73 ± 9.90 mm. The degree of OA was quantified on the Kellgren-Lawrence scale up to grade III disease severity. The use of slow-acting symptomatic agents (SYSADOA) resulted in a reduced need for administration of analgesic and anti-inflammatory therapy, leading to diminished side effects associated with these drugs. Thus, spontaneous pain and assessed functionality improved significantly, with symptom improvement reaching up to 25%. In addition, SYSADOA therapy resulted in a delay in imaging progression of knee OA, with improvement in femoral hyaline cartilage thickness in all compartments. The MRI results did not show a statistically significant difference between the thickness of the femoral hyaline cartilage, between the first and the second imaging evaluation. Thus, between baseline and the 6-month visit, femoral cartilage thickness appears to be maintained, even showing a slight increase after SYSADOA treatment. Conclusions. SYSADOA administration delays the radiographic progression of knee OA, with preservation, even modest improvement in hyaline cartilage thickness, making MRI an important tool in the evaluation and monitoring of OA. Background. Dermatomyositis (DM) is a rare disease that belongs to the idiopathic inflammatory myopathies, and is characterized by inflammation and weakness of proximal skeletal muscles, accompanied by skin rash. Furthermore, DM can manifest clinically as a paraneoplastic syndrome in patients with breast cancer. Managing the progression of DM is typically achievable with elevated doses of glucocorticoids in the majority of cases. Nonetheless, when faced with a severe manifestation of the paraneoplastic syndrome, initiation of cancer treatment is imperative. Methods. We report a clinical case of 34-year-old female, presenting heliotrope rash and Gottron’s papules. In addition, there was a development of severe proximal muscle weakness with severe dysphagia and dysphonia. The clinical features and laboratory results led to the diagnosis of DM. Concomitantly, the patient was diagnosed with a stage IV M1 HEP, HER2 positive breast neoplasm. Results. At first, high dose of glucocorticosteroids was administered as pulse therapy due to the severe symptoms of the paraneoplastic syndrome. Then anti-HER molecular targeted-therapy and chemotherapy were applied, as well as oral glucocorticoids for DM control. The patient showed significant improvement following three months of treatment, both in clinical and biological aspects. Conclusion. Background: Systemic sclerosis (SSc) is a complex connective tissue disease characterized by microangiopathy, immune dysregulation, and fibrosis. Early detection of microvascular abnormalities using nailfold videocapillaroscopy (NVC) is crucial in assessing disease progression and associated disease's involvement such as interstitial lung disease (ILD) and pulmonary arterial hypertension (PAH)., Objective: This study aims to explore the relationships correlation between NVC patterns, clinical manifestations, and systemic complications in SSc., Methods: We analyzed the data of 63 patients, predominantly female (95%), with a mean age of 49 years and an average disease duration of 42 months. Patients were categorized into early, active, and late patterns based on NVC findings. Clinical features, including digital ulcers (DU), ILD, and PAH, were assessed. Pearson correlation analyses were performed to evaluate the relationships between capillary loss, neoangiogenesis, ILD, and PAH., Results: The early pattern group (mean mRSS 2.36) exhibited minimal microvascular damage and systemic involvement, with no DUs. In the active pattern group (mean mRSS 10.40), 34.38% had diffuse cutaneous SSc (dcSSc), with 15.63% presenting DUs, 65.63% ILD, and 37.5% PAH. The late pattern group (mean mRSS 18.00) showed the most severe disease, with 80% having DUs, 70% dcSSc, 90% ILD, and 70% PAH. Pearson correlation analyses revealed strong correlations between capillary loss and ILD (r = 0.7255) and PAH (r = 0.6369). A moderate correlation was found between neoangiogenesis and PAH (r = 0.5592)., Conclusion: The study demonstrates that progressive microvascular damage in SSc, as visualized by NVC, correlates strongly with the severity of systemic complications. Early detection of capillary loss and neoangiogenesis using NVC is critical for timely interventions, which could improve patient outcomes by mitigating the progression of ILD and PAH., (© 2024 Camelia Palici et al., published by Sciendo.)
