Sevoflurane has been widely used in clinical anesthesia as an inhalation anesthetic. With the development of medicine, there have been several new applications in recent years, such as daytime surgery, labor analgesia, and combined nerve block for some surgeries. Moreover, as research progresses, it has been found that it not only has potential organ protection effects but can also be used to treat severe asthma and relieve the tracheal spasm state. In addition, local administration can effectively treat vascular ulcers. We briefly review the organ protective effect of sevoflurane, its application in dental treatment, asthma treatment, vascular ulcer treatment and some new progress in clinical application. Background: According to Rome IV, reflux hypersensitivity (RH) represents a novel form of functional esophageal disorder. This study was designed to compare the clinical features of three types of endoscopic-negative heartburn: RH, nonerosive reflux disease (NERD), and functional heartburn (FH). Methods: Patients with heartburn in a medical center from 01/01/2017 to 10/31/2021 were included. This article presented a blinded retrospective analysis of 24 h MII–pH and HRM tracings from patients with NERD, RH, and FH to compare their clinical characteristics. Results: A total of 118 patients were included in the study. There were no significant differences in age, sex, BMI, smoking status, or drinking history among RH, NERD and FH. Functional dyspepsia (FD) symptoms were more prone to exist in FH than in NERD (P < 0.05), whereas hiatal hernia was more prevalent in NERD and RH than in FH (P < 0.05). The incidence of anxiety and depression gradually increased in the NERD, RH, and FH groups (P > 0.05). The distal MNBI and PSPW index of the NERD and RH groups were lower than those of the FH group (P < 0.05). The distal MNBI showed good diagnostic potential. Conclusions: Clear pathological alterations, which are distinct from those of other FGIDs, are observable in RH. It might be inappropriate to categorize RHs within FGIDs. [ABSTRACT FROM AUTHOR]

A solution to address the issues of environmental light interference in Remote Photoplethysmography (rPPG) methods is proposed in this paper. First, signals from the face's region of interest (ROI) and background noise signals are simultaneously collected, and the two signals are processed by a differential to obtain a more accurate rPPG signal. This method effectively suppresses background noise and enhances signal quality. Secondly, the singular spectrum analysis algorithm (SSA) is enhanced to further improve the accuracy of heart rate detection. The algorithm's parameters are adaptively optimized by integrating the spectral and periodic characteristics of the heart rate signal. Experimental results demonstrate that the method proposed in this paper effectively mitigates the effects of lighting changes on heart rate detection, thereby enhancing detection accuracy. Overall, the experiments indicate that the proposed method significantly improves the effectiveness and accuracy of heart rate detection, achieving a high level of consistency with existing contact-based detection methods. Flax, an important oil and fiber crop, is widely cultivated in temperate and sub-frigid regions worldwide. China is one of the major producers of flax, with Gansu Province predominantly practicing cultivation in hilly areas. However, common issues such as feeding difficulties, stem entanglement, and low threshing efficiency significantly restrict the improvement of planting efficiency. This study addresses the key technical challenges in flax combine harvesting in hilly regions by developing a discrete element model of the flax plant and utilizing DEM-FEA co-simulation technology. The performance of two threshing drum models (T1 and T2) was analyzed, focusing on motion trajectory, stress distribution, and threshing effects. The simulation results show that the T2 model, with its combination of rib and rod tooth design, significantly improves threshing and separation efficiency. The loss rate was reduced from 5.6% in the T1 model to 1.78% in the T2 model, while the maximum stress and deformation were significantly lower, indicating higher structural stability and durability. Field validation results revealed that the T1 model had a total loss rate of 3.32%, an impurity rate of 3.57%, and an efficiency of 0.09 hm2/h. In contrast, the T2 model achieved a total loss rate of 2.29%, an impurity rate of 3.39%, and an efficiency of 0.22 hm2/h, representing a 144.4% improvement in working efficiency. These findings indicate that the T2 model has a higher potential for flax harvesting in hilly and mountainous regions, especially in improving threshing efficiency and operational stability, providing an important theoretical basis for optimizing threshing equipment design. With the explosive development of the Internet of Things (IoT), it is convenient and important to collect health data from medical sensors and smart devices and construct medical knowledge graph. The knowledge graph contributes to investigating the connection between patient and disease, especially for epidemic surveillance. However, it is possible to cause the leakage of sensitive health information due to the untrusted data collector or various malicious attackers. In this paper, we attempt to utilise federated learning to construct a special knowledge graph, that is, individual‐symptom relationship diagram with local differential privacy (LDP‐ISRD), for epidemic risk surveillance, which presents the underlying infectious relationship among individuals. At first, we propose a federated learning‐based framework of LDP‐ISRD by utilising individuals' smart devices in IoT. Then, we leverage locations to determine the connection among individuals in terms of physical contact. Next, we propose a randomised algorithm PrivISRD to implement federated learning‐based LDP‐ISRD, which consists of symptom perturbation and aggregation. Finally, extensive experiments evaluate the impact of various parameters and results demonstrate that LDP‐ISRD has good performance. [ABSTRACT FROM AUTHOR]

Iron tailings (IT) are by-products of iron ore mining, which have the characteristics of high stockpile and low cost. Recycling IT as alternatives for high-quality quartz sand (QS) in ultrahigh-performance mortar (UHPM) is an environmentally friendly and sustainable approach. However, the addition of IT changes the continuity of the material gradation, which in turn affects the dense structure of the UHPM. Therefore, in this paper, the effects of composite aggregates composed of different blends (0%–100%) of IT and QS on the working properties, compressive strength, chloride ion penetration, toxic leaching, and microstructure of UHPM were investigated. The chloride ion permeability and toxicity leaching of UHPM were tested using the electric flux method and the inductively coupled plasma method. The microstructure of UHPM was characterized using the mercury intrusion method and electron microscope. The results showed that a composite aggregate consisting of 60% IT and 40% QS could achieve the best strength of UHPM. When QS was completely replaced by IT, similar strength levels to the control group (100% QS) could still be achieved, while the electric flux was reduced by 24.8% and the leaching of harmful ions was negligible. What counts is that the IT replacement rate should be controlled within 60% to maintain the necessary working performance. The results of the close packing model indicate that the UHPM material is most closely packed when the IT content is about 60%. The random aggregate distribution model also indicates that the range of 60%–80% IT admixture is the optimal gradation interval for UHPM, and the results of the two models are consistent with the basic performance law. Background: This study aimed to assess how knee savers (KSs) and knee pads (KPs) alleviate risks of knee musculoskeletal disorders (MSDs) among roofers during various phases of shingle installation. These phases encompass (1) reaching for shingles, (2) placing shingles, (3) grabbing a nail gun, (4) moving to the first nailing position, (5) nailing shingles, (6) replacing the nail gun, and (7) returning to an upright position., Methods: In a laboratory setting, nine male participants simulated the shingle installation task on a slope-adjustable roof platform (0°, 15°, and 30° slopes) under four intervention conditions: no intervention (NO); with KPs only (KP); with KSs only (KS); and with both KPs and KSs (BO). Knee flexion, abduction, adduction, and internal/external rotations were measured to assess intervention impact through statistical analysis., Results: Phase 5 (nailing shingles), one of the riskiest phases, saw reduced knee rotations, with BO and KP interventions being the most effective. Phase 6 (replacing the nail gun) exhibited notable reductions in all knee rotations, primarily due to BO intervention. Significant improvements in certain knee angles for other phases were noted, particularly with BO intervention., Conclusions: BO and KP can lower knee strain by minimizing extreme knee postures and thereby reducing the risk of MSDs during the installation of shingles, especially at critical periods and on steeper slopes. Objective: It is of positive significance for clinical management to infer the changes of BPV and blood Hcy in the risk of cardiovascular disease progression in patients with hypertension by ambulatory blood pressure test. We aimed to explore the correlation of homocysteine and blood pressure variability with left ventricular hypertrophy in patients with hypertension and carotid atherosclerosis., Methods: A total of 333 elderly hypertensive patients admitted to Jinniu District People's Hospital in Chengdu from February 2020 to December 2021 were selected as the study subjects. They were divided into the pure hypertension group and hypertension combined with the carotid atherosclerosis group. In addition, the two groups were divided into the hypertension combined with left ventricular remodeling subgroup and the hypertension without left ventricular remodeling subgroup. Plasma Hcy levels were measured on a BECKMAN AU 680 automated biochemistry analyzer. Ambulatory blood pressure was measured by HINGMED ABP-03 monitoring system. IMT measurement was performed using a GE LOGIQ E9 ultrasound. Left ventricular diastolic function measurement was performed using a GE LOGIQ E9 ultrasound., Results: Blood Hcy, 24-h mean SBP, 24-h mean DBP, 24-h SBP coefficient of variation, and 24-h DBP coefficient of variation were significantly higher in hypertension combined with the carotid atherosclerosis group than the pure hypertension group (P < .05). Moreover, LVMI, E/A, and E/e were greater in the carotid atherosclerosis group than in the normal carotid group (P < .05). IMT, blood Hcy, 24-h mean SBP, 24-h mean DBP, 24-h SBP coefficient of variation, 24-h DBP coefficient of variation, E/A, and E/e were significantly higher in the hypertensive combined with left ventricular remodeling subgroup than in the non-left ventricular remodeling subgroup (P < .05). IMT was positively correlated with blood Hcy, 24-h SBP, 24-h DBP, 24-h SCV, 24-h DCV, LVMI, E/A, and E/e (P < .05)., Conclusion: There was a position correlation of Hcy and BPV with left ventricular hypertrophy in patients with hypertension combined with carotid atherosclerosis.

Bioactive hydrogels have emerged as promising artificial niches for enhancing stem cell-mediated tendon repair. However, a substantial knowledge gap remains regarding the optimal combination of niche features for targeted cellular responses, which often leads to lengthy development cycles and uncontrolled healing outcomes. To address this critical gap, an innovative, data-driven materiomics strategy is developed. This approach is based on in-house RNA-seq data that integrates bioinformatics and mathematical modeling, which is a significant departure from traditional trial-and-error methods. It aims to provide both mechanistic insights and quantitative assessments and predictions of the tenogenic effects of adipose-derived stem cells induced by systematically modulated features of a tendon-mimetic hydrogel (TenoGel). The knowledge generated has enabled a rational approach for TenoGel design, addressing key considerations, such as tendon extracellular matrix concentration, uniaxial tensile loading, and in vitro pre-conditioning duration. Remarkably, our optimized TenoGel demonstrated robust tenogenesis in vitro and facilitated tendon regeneration while preventing undesired ectopic ossification in a rat tendon injury model. These findings shed light on the importance of tailoring hydrogel features for efficient tendon repair.
