Abstract

Between 1950 and 1985, a period now referred to as the "Sixties Scoop", over 24,000 Indigenous and Inuit children were removed from their families and placed into primarily non-Indigenous foster and adoptive homes across Canada. Whereas the Residential school system was explicitly racist and genocidal in its orientation, the Sixties Scoop racism was cloaked in accusations of neglect and child-saving rhetoric. Drawing on Hacking's concept of looping effects, we examine how social workers classify their work and the Indigenous children they removed from their homes. We analyse sixteen interviews with social workers working in child welfare during the Sixties Scoop who were involved with the removal of Indigenous children. This article links broader systems that are fraught with racism, to the rank-and-file individuals tasked with carrying out child protection policies, and it probes the interpretations and classifications used both then and now to understand Indigenous child removal. 35. Multilevel Resilience and Appointment Attendance Among African American/Black Adults with HIV: A Prospective Multisite Cohort Study.

Abstract

Background: Attending clinic appointments supports HIV viral suppression, yet racial disparities are documented. We assessed whether multilevel resilience resources were associated with appointment attendance among African American/Black (AA/B) adults living with HIV in the United States. Methods: We ascertained data from 291 AA/B clinical cohort participants from 2018 to 2021. We assessed resilience using the Multilevel Resilience Resource Measure. Binary outcomes were a nonrepeated indicator of attending ≥87.5% of scheduled HIV appointments over 12 months (i.e., visit adherence) and a repeated measure of attending appointments during two sequential 6-month follow-up windows (i.e., clinic attendance). Modified Poisson models estimated adjusted risk ratios (aRRs). Results: The aRR for clinic attendance among participants with greater versus lesser multilevel resilience resource endorsement was 0.95 (95% confidence interval: 0.88, 1.0). The aRR for visit adherence among participants with greater versus lesser multilevel resilience resource endorsement was 1.2 (0.95, 1.4). Conclusions: This analysis is one of the first to assess appointment attendance as a function of resilience. Findings should be confirmed in larger cohorts. 36. Intramuscular Adipose Tissue in the Lumbar Paraspinal Muscles Does Not Correlate With Low Back Pain in Medical Students.

Abstract

Objective: The objective of the study is to identify any intergroup differences or statistically significant correlations between paraspinal intramuscular adipose tissue (IntraMAT) deposition and variables such as low back pain, gender, self-reported activity level, body mass index (BMI), age, and academic class-year, as measured by sonographic intensity analysis. This study is specific to a cohort of first- and second-year medical students. Materials and Methods: The right lumbar paraspinal muscles, at the level of the fifth lumbar vertebrae, were investigated with sonography among a cohort of medical students. Image pixels, within the selected region of interest, were segregated by pixel intensity and analyzed for IntraMAT content. Participants reported on additional study variables by responding to a survey. The cohort responses were analyzed to seek correlations between those variables and IntraMAT. Results: The female participants were found to have more IntraMAT (P <.001). Most participants reported low back pain. There were no significant correlations between IntraMAT and reported low back pain, academic class year, or BMI. Moderate exercise increased IntraMAT deposition, and each year of increasing age decreased IntraMAT (P <.05). Second-year students experienced more self-reported low back pain than first-year students. Conclusion: A cohort of medical students who were reportedly sedentary had more low back pain than their peers. Back pain was more prevalent in second-year students, irrespective of age, suggesting that ill effects of a sedentary lifestyle may be cumulative. The validity of the sonographic pixel intensity analysis performed was supported by the highly significant increase in IntraMAT observed in female participants. 37. Age and Menopausal Status in Idiopathic Subglottic Stenosis.

Abstract

Objective: To evaluate the impact of age on disease recurrence in idiopathic subglottic stenosis (iSGS) patients. Methods: This was a retrospective chart review of iSGS patients treated with laser wedge excision. Patients did not have prior surgery. Survival rates free of recurrence were estimated using the Kaplan–Meier methods, and associations were evaluated using Cox Proportional Hazards models. Results: Eighty‐five female patients were included in the study. Most patients (68%) were postmenopausal, had first period at or before 13 years of age, had at least one full‐term pregnancy (82%), were not undergoing hormone replacement therapy (93%), were not using hormonal birth control (88%), and were either partially or completely compliant with triple therapy regime (80%). There was a statistically significant average reduction in risk of recurrence of 5% for every additional year of age (p < 0.0001). When compared to patients older than 65 years of age, patients less than 35 were nearly 10 times more likely to recur (p = 0.002), and patients 55–65 and 45–55 years of age were 8 and 5 times more likely to recur, respectively (p = 0.003 and 0.009). Additionally, females on birth control showed decreased risk of recurrence of 74% compared with their counterparts (p = 0.04). Conclusion: This is the first study to demonstrate an inverse relationship between age and disease recurrence in patients with iSGS following surgery. Using age as a surrogate for menopausal status, these results suggest that increased age and/or birth control provide a protective effect through decreased recurrence rates, possibly mediated by decreased estrogen levels. Abstract

Objective: To evaluate the impact of methotrexate and rituximab therapy on highly recurrent idiopathic subglottic stenosis (iSGS) patients with a negative antineutrophil cytoplasmic antibody titer cANCA(−). Methods: This was a retrospective cohort study of highly recurrent iSGS patients who recurred within 1 year or less and were treated with methotrexate (MTX), and rituximab (RTX), or a combination of both at different time points (MTX/RTX). Average surgical durations before and after drug treatment were summarized, and the differences were calculated. Results: A total of 21 female patients with median age of 62 years were included. Fifteen patients were treated with MTX, three were treated with RTX, and five treated with both. Patients treated with immunosuppressants showed a trend toward longer intervals between operations (mean pre‐drug interval: 338; mean post‐drug interval: 697 days) (p‐value = 0.25). Three patients did not recur following drug initiation with median follow‐up of 1265 days. All three treatment groups demonstrated a trend toward increased post‐drug recurrence intervals (MTX: 444 days, RTX: 374 days, MTX/RTX: 55 days), that was not statistically significant. Patients with prior dilations demonstrated longer post‐drug recurrence intervals (mean pre‐drug interval: 341 days, mean post‐drug interval 978 days) (p‐value = 0.17). Four patients in the cohort with the highest recurring disease improved from mean 129 days between operations to 509 days with drug therapy. The most common drug side effect was nausea (16%). Conclusion: MTX and RTX may be treatment options for some highly recurrent iSGS patients. Initial results are variable and demonstrate a need for further research on drug candidacy. 40. Occupational and Hobby Exposures Associated With Myositis Phenotypes in a National Myositis Patient Registry

Abstract

The objective of this study was to investigate occupational and hobby exposures to silica, solvents, and heavy metals and the odds of having the idiopathic inflammatory myopathy (IIM) phenotypes dermatomyositis (DM) and polymyositis (PM) versus inclusion body myositis (IBM), lung disease plus fever or arthritis (LD+), and systemic autoimmune rheumatic disease–associated overlap myositis (OM). The sample included 1,390 patients (598 with DM, 409 with PM, and 383 with IBM) aged ≥18 years from a national registry. Of these, 218 (16%) were identified with LD+, and 166 (12%) with OM. Of these, 218 (16%) were identified with LD+, and 166 (12%) with OM. We calculated adjusted odds ratios (ORs) and 95% confidence intervals (CIs) and explored joint effects with smoking. High silica exposure was associated with increased odds of having DM (OR 2.02, 95% CI 1.18–3.46, compared to no exposure; Ptrend = 0.004), LD+ (OR 1.75, 95% CI 1.10–2.78, vs no LD; Ptrend = 0.005), and OM (OR 2.07, 95% CI 1.19–3.61, Ptrend = 0.020). Moderate to high heavy metals exposure was associated with greater odds of having LD+ (OR 1.49, 95% CI 1.00–2.14, Ptrend = 0.026) and OM (OR 1.59, 95% CI 0.99–2.55, Ptrend = 0.051). Greater odds of having LD+ were seen among smokers with moderate to high silica exposure versus nonsmokers with low or no exposure (high‐certainty assessment OR 2.53, 95% CI 1.31–4.90, Pinteraction = 0.061). These findings, based on a systematic exposure assessment, suggest that occupational and hobby exposures to silica and heavy metals contribute to adult IIM phenotypes, including DM, OM, and LD+, a possible marker for antisynthetase syndrome or other autoantibody‐associated lung diseases.

Abstract

One of the arguments in support of the legalization of cannabis is that it would help alleviate racial disparities in the criminal justice system. Using UCR data from Colorado and Washington, we explore trends in cannabis arrests disaggregated by rates using interrupted time-series analysis, linear mixed models, and data visualizations. The results demonstrate a general decline in cannabis arrests for nearly all racial groups, yet these declines were not consistent across racial groups or even across states. Moreover, substantial racial disparities persist following legalization, especially in Colorado. Overall, evidence suggests that while legalization has likely had a net positive effect on overrepresented populations by decreasing criminal justice contact, it is not a panacea and may only be minimally important for addressing disparities.
