Objective: The Imported Medicine List was published for the first time on October 11, 2013 by the Ministry of Health of the Republic of Türkiye. The purpose of importing drugs from abroad is to supply the human medicinal products that are not licensed in our country and/or that are not available in the market for various reasons despite being licensed, on prescription for use in the diagnosis and treatment of diseases in emergencies. The aim of this qualitative research is to examine the information presented about these medicines on the imported medicine list and to evaluate the possible ethical problems that may be caused by insufficient information presentation. Material and Methods: The medicines on the 25/02/2022 Turkish Medicines and Medical Devices Agency imported medicine list were included in the analysis. First, between the dates 07-27 February 2022, it was determined whether these medicines had short product information and instructions for use on the Turkish Medicines and Medical Devices Agency website by searching for active substances. Then, all medicines were searched in the RxMediaPharma database by the active substance name, and those not found by this method were searched by the ATC code. Finally, the ethical implications of the issue were discussed and suggestions were presented. Results: The change in the number of active substances in the imported medicine list by years and the presence of generic and medicine-specific information in different Informed Consent Forms have been determined. The ambiguity of the criteria for keeping the unlicensed essential drugs used to meet the priority health needs of the population in the foreign-dependent list has been determined. Conclusion: The risks to patients' right to autonomy have been determined as the result of the lack of sufficient, adequate, and accurate information in the Informed Consent Forms carries risks for patients' rights to autonomy., Amaç: Yurt Dışı İlaç Listesi, Türkiye Cumhuriyeti Sağlık Bakanlığı tarafından ilk kez 11 Ekim 2013 tarihinde yayımlanmıştır. Yurt dışından ilaç ithalinde amaç, ülkemizde ruhsatı olmayan ve/veya ruhsatlı olmasına rağmen çeşitli nedenlerle piyasada bulunmayan beşerî tıbbi ürünlerin acil durumlarda, hastalıkların tanı ve tedavisinde kullanılmak üzere reçeteli olarak yurt dışından teminini sağlamaktır. Bu nitel araştırmanın amacı, yurt dışı ilaç listesindeki ilaçlarla ilgili sunulan bilgileri incelemek ve yetersiz bilgi sunumundan kaynaklanabilecek olası etik sorunları değerlendirmek ve öneriler sunulmaktır. Gereç ve Yöntemler: Bu araştırmaya, 25.02.2022 tarihinde yayımlanan Türkiye İlaç ve Tıbbi Cihaz Kurumu yurt dışı ilaç listesindeki ilaçlar dâhil edilmiştir. İlk olarak, 07-27 Şubat tarihleri arasında bu ilaçların Türkiye İlaç ve Tıbbi Cihaz Kurumu internet sitesinde etken madde taraması yapılarak kısa ürün bilgisi ve kullanım talimatı olup olmadığı belirlenmiştir. Daha sonra RxMediaPharma veri tabanında tüm ilaçlar etken madde adına göre aranmış ve bu yöntemle bulunamayanlar ATC kodu ile taranmıştır. Son olarak, konunun etik sonuçlarıyla ilgili tartışma yürütülüp, öneriler sunulmuştur. Bulgular: Yurt dışı ilaç listesindeki etken madde sayısının yıllara göre değişimi ve Aydınlatılmış Onam Formları'nın bazılarında jenerik bazılarında da ilaca özgü bilgilerin bulunduğu belirlenmiştir. Nüfusun öncelikli sağlık ihtiyaçlarını karşılamak için kullanılan ruhsatsız esansiyel ilaçların yurt dışı bağımlı yaratan listede tutulmasına ilişkin kriterlerin belirsizliği tespit edilmiştir. Sonuç: Aydınlatılmış Onam Formları'nda yeterli ve doğru bilgilerin bulunmaması sonucunda hastaların özerklik haklarına yönelik riskler belirlenmiştir.

Son olarak, “Zavallı Çocuklar” kitap içeriği, 1968 yılında ülkemiz basılmış olan ilk sosyal pediatri ders kitabının yazarı olan Dr. Sabiha Özgür’ün tanımladığı ‘gelişmekte olan ülkelerde sosyal pediatrinin dört ana hedefi’ kapsamında tartışılmıştır. Bulgular: “Zavallı Çocuklar” kitabının yazıldığı dönemde ülkemizde ve dünyada etkin olan ‘nature-nurture’ tartışmasında ‘nature’ kavramının ön planda olduğu göz önünde bulundurulursa, Dr. Peker’in çocuk ile kurulacak kuvvetli hissi bağın sağlığın korunması ve iyileştirilmesi için çok önemli olduğunu kendi klinik deneyimleri ile anlatmış olması özgün bir bulgudur. Dr. Peker’in hastalarına ve ailelerine son derece hakim olması, kitabın tamamında, anne ve babaya karşı yargıdan uzak olması ve çocuk ile etkileşimlerine ilişkin nedensel bir bağ kuruyor olması ise kitaba ilişkin diğer bulgulardandır. Sonuç: Dr. Peker’in, hekimlik yaptığı dönemdeki yaygın anlayışın aksine kendisine getirilen çocuk hastaların sıkıntılarının arkasındakini görebilecek hassasiyet, duyarlılık, zaman ve bilgisinin bulunması, onun çocuk psikiyatristi görüşünü ortaya koymaktadır., Object: The aim of this study is to Dr. Nuriye Peker's book titled "Zavallı Çocuklar-Poor Children" is to examine comparatively the place of social pediatrics in the development process in our country. Method: First, Dr. Peker's medical education and the factors and people who were influential in her orientation to social pediatrics were examined. Afterward, the book "Poor Children" was analyzed in terms of content, discourse, and methodology, and then it was examined how the approaches to social pediatrics in the book took place in the national and international legislation of the period. Finally, the content of "Poor Children" book has been discussed within the scope of “four main objectives of social pediatrics in developing countries” defined within the first social pediatrics textbook published in our country. Results: Considering that the concept of 'nature' was at the forefront of the 'nature-nurture' debate that was active in our country and in the world at the time the book “Poor Children-Zavallı Çocuklar” was written, it is an original finding that Dr. Peker explained with her own clinical experience that the strong emotional bond to be established with the child is very important for the protection and improvement of health. Other findings related to Dr. Peker’s book are that she has a great command of her patients and their families, that she is far from judgmental towards parents throughout the book, and that she establishes a causal link with her interactions with the child. Conclusion: Dr. Peker's sensitivity, time, and knowledge to see what is behind the problems of the pediatric patients brought to her, contrary to the common understanding during her practice as a physician, reveals her notion of a child psychiatrist.

Pandemi, insanlığın varlığını ve iyilik halini tümden tehdit eden bir küresel bir halk sağlığı acil durumudur. Halk sağlığı alanındaki etik sorunlara yönelik geliştirilmiş çözümleme metodolojileri bölgesel ve sınırlı halk sağlığı acillerinde bir miktar yol gösterici olabilse de, pandemi gibi küresel acil durumlarda yeterli rehberlik sağlayamamaktadır. COVID-19 pandemisi bu eksikliği belirgin bir şekilde görmemizi sağlamıştır. Bu makalede öncelikle halk sağlığı etiğinin (HSE) temel unsurları, sorun alanları ve çözüm yaklaşımlarına ilişkin bir çerçeve sunulacak, daha sonra bu çerçevenin küresel halk sağlığı acillerinde neden yeterli rehberlik sağlamadığı açıklanacak ve son olarak yeni bir etik çözümleme çerçevesi önerisinde bulunulacaktır., The pandemic is a global public health emergency that threatens the existence and wellbeing of humanity as a whole. While improved resolution methodologies for ethical issues in public health may provide some guidance in regional and limited public health emergencies, they do not provide adequate guidance in global emergencies such as pandemics. The COVID-19 pandemic has made this deficiency clear. In this article, firstly, a framework regarding the basic elements of public health ethics (HSE), problem areas and solution approaches will be presented, then it will be explained why this framework does not provide sufficient guidance in global public health emergencies, and finally, a new ethical analysis frame will be proposed.
