Abstract: Neutrinos described as an open quantum system may interact with the environment which introduces stochastic perturbations to their quantum phase. This mechanism leads to a loss of coherence along the propagation of the neutrino $-$ a phenomenon commonly referred to as decoherence $-$ and ultimately, to a modification of the oscillation probabilities. Fluctuations in space-time, as envisaged by various theories of quantum gravity, are a potential candidate for a decoherence-inducing environment. Consequently, the search for decoherence provides a rare opportunity to investigate quantum gravitational effects which are usually beyond the reach of current experiments. In this work, quantum decoherence effects are searched for in neutrino data collected by the KM3NeT/ORCA detector from January 2020 to November 2021. The analysis focuses on atmospheric neutrinos within the energy range of a few GeV to $100\,\mathrm{GeV}$. Adopting the open quantum system framework, decoherence is described in a phenomenological manner with the strength of the effect given by the parameters $\Gamma_{21}$ and $\Gamma_{31}$. Following previous studies, a dependence of the type $\Gamma_{ij} \propto (E/E_0)^n$ on the neutrino energy is assumed and the cases $n = -2,-1$ are explored. No significant deviation with respect to the standard oscillation hypothesis is observed. Therefore, $90\,\%$ CL upper limits are estimated as $\Gamma_{21} < 4.6\cdot 10^{-21}\,$GeV and $\Gamma_{31} < 8.4\cdot 10^{-21}\,$GeV for $n = -2$, and $\Gamma_{21} < 1.9\cdot 10^{-22}\,$GeV and $\Gamma_{31} < 2.7\cdot 10^{-22}\,$GeV for $n = -1$, respectively., Comment: 17 pages, 5 figures

Abstract: We present new Australia Telescope Compact Array (ATCA) radio observations toward N49, one of the brightest extragalactic Supernova remnants (SNRs) located in the Large Magellanic Cloud. Our new and archival ATCA radio observations were analysed along with $Chandra$ X-ray data. These observations show a prominent `bullet' shaped feature beyond the southwestern boundary of the SNR. Both X-ray morphology and radio polarisation analysis support a physical connection of this feature to the SNR. The 'bullet' feature's apparent velocity is estimated at $\sim$1300 km s$^{-1}$, based on its distance ($\sim$10 pc) from the remnant's geometric centre and estimated age ($\sim$7600 yrs). we estimated the radio spectral index, $\alpha= -0.55 \pm 0.03$ which is typical of middle-age SNRs. Polarisation maps created for N49 show low to moderate levels of mean fractional polarisation estimated at 7$\pm$1% and 10$\pm$1% for 5.5 and 9 GHz, respectively. These values are noticeably larger than found in previous studies. Moreover, the mean value for the Faraday rotation of SNR N49 from combining CABB data is 212$\pm$65 rad m$^{-2}$ and the maximum value of RM is 591$\pm$103 rad m$^{-2}$., Comment: 16 pages, accepted for publication in Publications of the Astronomical Society of Australia

Abstract: We present the serendipitous discovery of a new radio-continuum ring-like object nicknamed Kyklos (J1802-3353), with MeerKAT UHF and L-band observations. The radio ring, which resembles the recently discovered odd radio circles (ORCs), has a diameter of 80 arcsec and is located just 6 deg from the Galactic plane. However, Kyklos exhibits an atypical thermal radio-continuum spectrum ({\alpha} = -0.1 +/- 0.3), which led us to explore different possible formation scenarios. We concluded that a circumstellar shell around an evolved massive star, possibly a Wolf-Rayet, is the most convincing explanation with the present data., Comment: 7 pages, 5 figures, accepted in A&A

Abstract: KM3NeT/ORCA is a water Cherenkov neutrino detector under construction and anchored at the bottom of the Mediterranean Sea. The detector is designed to study oscillations of atmospheric neutrinos and determine the neutrino mass ordering. This paper focuses on an initial configuration of ORCA, referred to as ORCA6, which comprises six out of the foreseen 115 detection units of photo-sensors. A high-purity neutrino sample was extracted, corresponding to an exposure of 433 kton-years. The sample of 5828 neutrino candidates is analysed following a binned log-likelihood method in the reconstructed energy and cosine of the zenith angle. The atmospheric oscillation parameters are measured to be $\sin^2\theta_{23}= 0.51^{+0.04}_{-0.05}$, and $ \Delta m^2_{31} = 2.18^{+0.25}_{-0.35}\times 10^{-3}~\mathrm{eV^2} \cup \{-2.25,-1.76\}\times 10^{-3}~\mathrm{eV^2}$ at 68\% CL. Abstract: The Tarantula Nebula in the Large Magellanic Cloud is known for its high star formation activity. At its center lies the young massive star cluster R136, providing a significant amount of the energy that makes the nebula shine so brightly at many wavelengths. Recently, young massive star clusters have been suggested to also efficiently produce high-energy cosmic rays, potentially beyond PeV energies. Here, we report the detection of very-high-energy $\gamma$-ray emission from the direction of R136 with the High Energy Stereoscopic System, achieved through a multicomponent, likelihood-based modeling of the data. This supports the hypothesis that R136 is indeed a very powerful cosmic-ray accelerator. Moreover, from the same analysis, we provide an updated measurement of the $\gamma$-ray emission from 30 Dor C, the only superbubble detected at TeV energies presently. The $\gamma$-ray luminosity above $0.5\,\mathrm{TeV}$ of both sources is $(2-3)\times 10^{35}\,\mathrm{erg}\,\mathrm{s}^{-1}$. This exceeds by more than a factor of 2 the luminosity of HESS J1646$-$458, which is associated with the most massive young star cluster in the Milky Way, Westerlund 1. Furthermore, the $\gamma$-ray emission from each source is extended with a significance of $>3\sigma$ and a Gaussian width of about $30\,\mathrm{pc}$. For 30 Dor C, a connection between the $\gamma$-ray emission and the nonthermal X-ray emission appears likely. 6. The TRAPUM Small Magellanic Cloud pulsar survey with MeerKAT: I. Discovery of seven new pulsars and two Pulsar Wind Nebula associations

Abstract: The sensitivity of the MeerKAT radio interferometer is an opportunity to probe deeper into the population of rare and faint extragalactic pulsars. The TRAPUM (TRAnsients and PUlsars with MeerKAT) collaboration has conducted a radio-domain search for accelerated pulsars and transients in the Small Magellanic Cloud (SMC). This partially targeted survey, performed at L-band (856-1712 MHz) with the core array of the MeerKAT telescope in 2-h integrations, is twice as sensitive as the latest SMC radio pulsar survey. We report the discovery of seven new SMC pulsars, doubling this galaxy's radio pulsar population and increasing the total extragalactic population by nearly a quarter. We also carried out a search for accelerated millisecond pulsars in the SMC Globular Cluster NGC 121 using the full array of MeerKAT. This improved the previous upper limit on pulsed radio emission from this cluster by a factor of six. Our discoveries reveal the first radio pulsar-PWN systems in the SMC, with only one such system previously known outside our galaxy (the "Crab pulsar twin" in the Large Magellanic Cloud, PSR J0540$-$6919). We associate the 59 ms pulsar discovery PSR J0040$-$7337, now the fastest spinning radio pulsar in the SMC, with the bow-shock Pulsar Wind Nebula (PWN) of Supernova Remnant DEM S5. We also present a new young pulsar with a 79 ms period, PSR J0048$-$7317, in a PWN recently discovered in a MeerKAT radio continuum image. Using the multi-beam capability of MeerKAT, we localised our pulsar discoveries, and two previous Murriyang discoveries, to a positional uncertainty of a few arcseconds., Comment: 32 pages, 14 figures, 10 tables. Abstract: New advancements in radio data post-processing are underway within the SKA precursor community, aiming to facilitate the extraction of scientific results from survey images through a semi-automated approach. Several of these developments leverage deep learning (DL) methodologies for diverse tasks, including source detection, object or morphology classification, and anomaly detection. Despite substantial progress, the full potential of these methods often remains untapped due to challenges associated with training large supervised models, particularly in the presence of small and class-unbalanced labelled datasets. Self-supervised learning has recently established itself as a powerful methodology to deal with some of the aforementioned challenges, by directly learning a lower-dimensional representation from large samples of unlabelled data. The resulting model and data representation can then be used for data inspection and various downstream tasks if a small subset of labelled data is available. In this work, we explored contrastive learning methods to learn suitable radio data representation from unlabelled images taken from the ASKAP EMU and SARAO MeerKAT GPS surveys. We evaluated trained models and the obtained data representation over smaller labelled datasets, also taken from different radio surveys, in selected analysis tasks: source detection and classification, and search for objects with peculiar morphology. Abstract: Gamma-ray bursts are promising candidate sources of high-energy astrophysical neutrinos. The recent GRB 221009A event, identified as the brightest gamma-ray burst ever detected, provides a unique opportunity to investigate hadronic emissions involving neutrinos. The KM3NeT undersea neutrino detectors participated in the worldwide follow-up effort triggered by the event, searching for neutrino events. In this letter, we summarize subsequent searches, in a wide energy range from MeV up to a few PeVs. No neutrino events are found in any of the searches performed. Abstract: HESS J1813$-$178 is a very-high-energy $\gamma$-ray source spatially coincident with the young and energetic pulsar PSR J1813$-$1749 and thought to be associated with its pulsar wind nebula (PWN). Recently, evidence for extended high-energy emission in the vicinity of the pulsar has been revealed in the Fermi Large Area Telescope (LAT) data. This motivates revisiting the HESS J1813$-$178 region, taking advantage of improved analysis methods and an extended data set. Using data taken by the High Energy Stereoscopic System (H.E.S.S.) experiment and the Fermi-LAT, we aim to describe the $\gamma$-ray emission in the region with a consistent model, to provide insights into its origin. We performed a likelihood-based analysis on 32 hours of H.E.S.S. data and 12 years of Fermi-LAT data and fit a spectro-morphological model to the combined datasets. These results allowed us to develop a physical model for the origin of the observed $\gamma$-ray emission in the region. In addition to the compact very-high-energy $\gamma$-ray emission centered on the pulsar, we find a significant yet previously undetected component along the Galactic plane. With Fermi-LAT data, we confirm extended high-energy emission consistent with the position and elongation of the extended emission observed with H.E.S.S. These results establish a consistent description of the emission in the region from GeV energies to several tens of TeV. This study suggests that HESS J1813$-$178 is associated with a $\gamma$-ray PWN powered by PSR J1813$-$1749. A possible origin of the extended emission component is inverse Compton emission from electrons and positrons that have escaped the confines of the pulsar and form a halo around the PWN., Comment: 13+5 pages, 13+11 figures. Abstract: The Crab Nebula is a unique laboratory for studying the acceleration of electrons and positrons through their non-thermal radiation. Observations of very-high-energy $\gamma$ rays from the Crab Nebula have provided important constraints for modelling its broadband emission. We present the first fully self-consistent analysis of the Crab Nebula's $\gamma$-ray emission between 1 GeV and $\sim$100 TeV, that is, over five orders of magnitude in energy. Using the open-source software package Gammapy, we combined 11.4 yr of data from the Fermi Large Area Telescope and 80 h of High Energy Stereoscopic System (H.E.S.S.) data at the event level and provide a measurement of the spatial extension of the nebula and its energy spectrum. We find evidence for a shrinking of the nebula with increasing $\gamma$-ray energy. Furthermore, we fitted several phenomenological models to the measured data, finding that none of them can fully describe the spatial extension and the spectral energy distribution at the same time. Especially the extension measured at TeV energies appears too large when compared to the X-ray emission. Our measurements probe the structure of the magnetic field between the pulsar wind termination shock and the dust torus, and we conclude that the magnetic field strength decreases with increasing distance from the pulsar. Abstract: Monochromatic gamma-ray signals constitute a potential smoking gun signature for annihilating or decaying dark matter particles that could relatively easily be distinguished from astrophysical or instrumental backgrounds. We provide an updated assessment of the sensitivity of the Cherenkov Telescope Array (CTA) to such signals, based on observations of the Galactic centre region as well as of selected dwarf spheroidal galaxies. We find that current limits and detection prospects for dark matter masses above 300 GeV will be significantly improved, by up to an order of magnitude in the multi-TeV range. This demonstrates that CTA will set a new standard for gamma-ray astronomy also in this respect, as the world's largest and most sensitive high-energy gamma-ray observatory, in particular due to its exquisite energy resolution at TeV energies and the adopted observational strategy focussing on regions with large dark matter densities. Throughout our analysis, we use up-to-date instrument response functions, and we thoroughly model the effect of instrumental systematic uncertainties in our statistical treatment. Abstract: Generation of science-ready data from processed data products is one of the major challenges in next-generation radio continuum surveys with the Square Kilometre Array (SKA) and its precursors, due to the expected data volume and the need to achieve a high degree of automated processing. Source extraction, characterization, and classification are the major stages involved in this process. In this work we focus on the classification of compact radio sources in the Galactic plane using both radio and infrared images as inputs. To this aim, we produced a curated dataset of ~20,000 images of compact sources of different astronomical classes, obtained from past radio and infrared surveys, and novel radio data from pilot surveys carried out with the Australian SKA Pathfinder (ASKAP). Radio spectral index information was also obtained for a subset of the data. We then trained two different classifiers on the produced dataset. The first model uses gradient-boosted decision trees and is trained on a set of pre-computed features derived from the data, which include radio-infrared colour indices and the radio spectral index. The second model is trained directly on multi-channel images, employing convolutional neural networks. Using a completely supervised procedure, we obtained a high classification accuracy (F1-score>90%) for separating Galactic objects from the extragalactic background. Individual class discrimination performances, ranging from 60% to 75%, increased by 10% when adding far-infrared and spectral index information, with extragalactic objects, PNe and HII regions identified with higher accuracies. 13. Radio Continuum Study of the Large Magellanic Cloud Supernova Remnant Honeycomb Nebula

Abstract: We present the first and deepest Australia Telescope Compact Array radio continuum images of the Honeycomb Nebula at 2000 and 5500 MHz solely from archival data. The resolutions of these images are 3.6 x 2.8 arcsec2 and 1.3 x 1.2 arcsec2 at 2000 and 5500 MHz. We find an average radio spectral index for the remnant of -0.76 +- 0.07. Polarisation maps at 5500 MHz reveal an average fractional polarisation of 25 +- 5% with a maximum value of 95 x 16. We estimate the equipartition field for Honeycomb Nebula of 48 +- 5 {\mu}G, with an estimated minimum energy of Emin = 3 x 1049 erg. The estimated surface brightness, {\Sigma}1 GHz , is 30 x 10-20 W m-2 Hz-1 sr-1; applying the {\Sigma}-D relation suggests this supernova remnant is expanding into a low-density environment. Finally, using Hi data, we can support the idea that the Honeycomb Nebula exploded inside a low-density wind cavity. We suggest that this remnant is likely to be between late free expansion stage and early Sedov phase of evolution and expanding into a low-density medium.

Abstract: The KM3NeT/ARCA neutrino detector is currently under construction at 3500 m depth offshore Capo Passero, Sicily, in the Mediterranean Sea. The main science objectives are the detection of high-energy cosmic neutrinos and the discovery of their sources. Simulations were conducted for the full KM3NeT/ARCA detector, instrumenting a volume of 1 km$^3$, to estimate the sensitivity and discovery potential to point-like neutrino sources and an all-sky diffuse neutrino flux. This paper covers the reconstruction of track- and shower-like signatures, as well as the criteria employed for neutrino event selection. By leveraging both the track and shower observation channels, the KM3NeT/ARCA detector demonstrates the capability to detect the diffuse astrophysical neutrino flux within half a year of operation, achieving a 5$\sigma$ statistical significance. With an angular resolution below 0.1$^\circ$ for tracks and under 2$^\circ$ for showers, the sensitivity to point-like neutrino sources surpasses existing observed limits across the entire sky., Comment: 20 pages, 30 figures Submitted to EPJ-C

Abstract: SS 433 is a microquasar, a stellar binary system with collimated relativistic jets. We observed SS 433 in gamma rays using the High Energy Stereoscopic System (H.E.S.S.), finding an energy-dependent shift in the apparent position of the gamma-ray emission of the parsec-scale jets. These observations trace the energetic electron population and indicate the gamma rays are produced by inverse-Compton scattering. Modelling of the energy-dependent gamma-ray morphology constrains the location of particle acceleration and requires an abrupt deceleration of the jet flow. We infer the presence of shocks on either side of the binary system at distances of 25 to 30 parsecs and conclude that self-collimation of the precessing jets forms the shocks, which then efficiently accelerate electrons., Comment: Submitted 20th Apr. 2023, published 25th January 2024 (accepted version)

Abstract: We present new radio continuum images and a source catalogue from the MeerKAT survey in the direction of the Small Magellanic Cloud (SMC). The observations, at a central frequency of 1.3 GHz across a bandwidth of 0.8 GHz, encompass a field of view ~7 x 7 degrees and result in images with resolution of 8 arcsec. The median broad-band Stokes I image Root Mean Squared noise value is ~11 microJy/beam. The catalogue produced from these images contains 108,330 point sources and 517 compact extended sources. We also describe a UHF (544-1088 MHz) single pointing observation. We report the detection of a new confirmed Supernova Remnant (SNR) (MCSNR J0100-7211) with an X-ray magnetar at its centre and 10 new SNR candidates. This is in addition to the detection of 21 previously confirmed SNRs and two previously noted SNR candidates. Our new SNR candidates have typical surface brightness an order of magnitude below those previously known, and on the whole they are larger. The high sensitivity of the MeerKAT survey also enabled us to detect the bright end of the SMC Planetary Nebulae (PNe) sample - point-like radio emission is associated with 38 of 102 optically known PNe, of which 19 are new detections. Lastly, we present the detection of three foreground radio stars amidst 11 circularly polarised sources, and a few examples of morphologically interesting background radio galaxies from which the radio ring galaxy ESO 029-G034 may represent a new type of radio object., Comment: 31 pages, 27 figures

Abstract: Purpose: The use of arterial partial pressure of carbon dioxide (PaCO2) as a target intervention to manage elevated intracranial pressure (ICP) and its effect on clinical outcomes remain unclear. We aimed to describe targets for PaCO2 in acute brain injured (ABI) patients and assess the occurrence of abnormal PaCO2 values during the first week in the intensive care unit (ICU). The secondary aim was to assess the association of PaCO2 with in-hospital mortality. Methods: We carried out a secondary analysis of a multicenter prospective observational study involving adult invasively ventilated patients with traumatic brain injury (TBI), subarachnoid hemorrhage (SAH), intracranial hemorrhage (ICH), or ischemic stroke (IS). PaCO2 was collected on day 1, 3, and 7 from ICU admission. Normocapnia was defined as PaCO2 > 35 and to 45 mmHg; mild hypocapnia as 32–35 mmHg; severe hypocapnia as 26–31 mmHg, forced hypocapnia as < 26 mmHg, and hypercapnia as > 45 mmHg. Results: 1476 patients (65.9% male, mean age 52 ± 18 years) were included. On ICU admission, 804 (54.5%) patients were normocapnic (incidence 1.37 episodes per person/day during ICU stay), and 125 (8.5%) and 334 (22.6%) were mild or severe hypocapnic (0.52 and 0.25 episodes/day). Forced hypocapnia and hypercapnia were used in 40 (2.7%) and 173 (11.7%) patients. PaCO2 had a U-shape relationship with in-hospital mortality with only severe hypocapnia and hypercapnia being associated with increased probability of in-hospital mortality (omnibus p value = 0.0009). Important differences were observed across different subgroups of ABI patients. Conclusions: Normocapnia and mild hypocapnia are common in ABI patients and do not affect patients' outcome. Extreme derangements of PaCO2 values were significantly associated with increased in-hospital mortality.

18. Radio continuum study of the Large Magellanic Cloud supernova remnant Honeycomb Nebula

Abstract: We present the first and deepest Australia Telescope Compact Array radio continuum images of the Honeycomb Nebula at 2000 and 5500MHz solely from archival data. The resolutions of these images are 3:6 x 2:8 arcsec2 and 1:3 x 1:2 arcsec2 at 2000 and 5500 MHz. We find an average radio spectral index for the remnant of -0:76±0:07. Polarisation maps at 5500MHz reveal an average fractional polarisation of 25 ± 5% with a maximum value of 95 ± 16. We estimate the equipartition field for Honeycomb Nebula of 48 ± 5 µG, with an estimated minimum energy of Emin = 3 x 1049 erg. The estimated surface brightness, Σ1 GHz, is 30 x 10-20Wm-2 Hz-1 sr-1; applying the Σ-D relation suggests this supernova remnant is expanding into a low-density environment. Finally, using Hi data, we can support the idea that the Honeycomb Nebula exploded inside a low-density wind cavity. We suggest that this remnant is likely to be between late free expansion stage and early Sedov phase of evolution and expanding into a low-density medium.

Abstract: The recent unprecedented progress in ageing research and drug discovery brings together fundamental research and clinical applications to advance the goal of promoting healthy longevity in the human population. We, from the gathering at the Aging Research and Drug Discovery Meeting in 2023, summarised the latest developments in healthspan biotechnology, with a particular emphasis on artificial intelligence (AI), biomarkers and clocks, geroscience, and clinical trials and interventions for healthy longevity. Moreover, we provide an overview of academic research and the biotech industry focused on targeting ageing as the root of age-related diseases to combat multimorbidity and extend healthspan. We propose that the integration of generative AI, cutting-edge biological technology, and longevity medicine is essential for extending the productive and healthy human lifespan.

20. The effect of aging on mast cell density in human skin: a comparative analysis of photoexposed and photoprotected regions.

Abstract: Background: Mast cells are mononuclear cells originating from bone marrow. They produce various biologically active substances, which allow them to actively participate in immune and inflammatory processes associated with intrinsic and extrinsic skin aging. This research focused on distribution and density of mast cells in healthy skin in different stages of skin aging., Material and Methods: This project included samples of photoexposed and photoprotected skin, obtained from 90 cadavers aged 0-82 years. The samples were classified into five age groups: newborns, young age, middle age, senior age and the oldest age. In order to visualize the mast cells, we have employed several histochemical staining protocols., Results: The number of mast cells of the photoexposed skin significantly correlated to the individual's age. The number of mast cells of the photoprotected skin was in general statistically significantly lower in younger compared to older groups; however, the correlation of the mast cell density in photoprotected skin and the age did not reach statistical significance. In middle age, senior age and the oldest age groups, a significantly higher number of mast cells was recorded in the skin of the photoexposed compared to photoprotected region., Conclusions: The increase in mast cell density correlated with age only in photoexposed skin. Age-related higher accumulation of dermal mast cells in photoexposed skin can be an important factor in the photoaging process, as well as the contributing factor in the occurrence of skin cancer.

Abstract: Background: Cardiac risk evaluation prior to noncardiac surgery is fundamental to tailor peri-operative management to patient's estimated risk. Data on the degree of adherence to guidelines in patients at cardiovascular risk in Europe and factors influencing adherence are underexplored., Objectives: The aim of this analysis was to describe the degree of adherence to [2014 European Society of Cardiology (ESC)/European Society of Anaesthesiology (ESA) guidelines] recommendations on rest echocardiography [transthoracic echocardiography (TTE)] and to stress imaging prior to noncardiac surgery in a large European sample and to assess factors potentially affecting adherence., Design: Secondary analysis of a multicentre, international, prospective cohort study (MET-REPAIR)., Setting: Twenty-five European centres of all levels of care that enrolled patients between 2017 and 2020., Patients: With elevated cardiovascular risk undergoing in-hospital elective, noncardiac surgery., Main Outcome Measures: (Non)adherence to each pre-operative TTE and stress imaging recommendations classified as guideline-adherent, overuse and underuse. We performed descriptive analysis. To explore the impact of patients' sex, age, geographical region, and hospital teaching status, we conducted multivariate multinominal regression analysis., Results: Out of 15 983 patients, 15 529 were analysed (61% men, mean age 72 ± 8 years). Overuse (conduction in spite of class III) and underuse (nonconduction in spite of class I recommendation) for pre-operative TTE amounted to 16.6% (2542/15 344) and 6.6% (1015/15 344), respectively. Stress imaging overuse and underuse amounted to 1.7% (241/14 202) and 0.4% (52/14 202) respectively. Male sex, some age categories and some geographical regions were significantly associated with TTE overuse. Male sex and some regions were also associated with TTE underuse. Age and regions were associated with overuse of stress imaging. Male sex, age, and some regions were associated with stress imaging underuse., Conclusion: Adherence to pre-operative stress imaging recommendation was high. In contrast, adherence to TTE recommendations was moderate. 24. Multicomponent family support intervention in intensive care units: statistical analysis plan for the cluster-randomized controlled FICUS trial.

Abstract: The FICUS trial is a cluster-randomized superiority trial to determine the effectiveness of a nurse-led, interprofessional family support intervention (FSI) on the quality of care, family management and individual mental health of family members of critically ill patients, compared to usual care. This paper describes the statistical analysis plan of the FICUS trial. The primary outcome is quality of family care, assessed by the Family Satisfaction in ICU Questionnaire (FS-ICU-24R) at patient discharge from the ICU. Several secondary outcomes are additionally assessed 3, 6, and 12 months thereafter. Sixteen clusters (ICUs) were randomly assigned 1:1 to FSI or usual care using minimization (8 per treatment). The target sample size is 56 patients per cluster (896 in total). Recruitment has been completed in January 2024. The follow-up of the last participant will be completed in early 2025. The primary and secondary outcomes will be analyzed by linear mixed-effects models (LMM). The main model for the primary outcome will include a random intercept per cluster with treatment (FSI vs. usual care) as the only explanatory variable due to the relatively small number of clusters. In addition, covariate-adjusted analyses will be conducted, including two cluster-level characteristics used in the minimization as well as participant-level characteristics. 25. Transitional care clinics for patients discharged from hospital without a primary care provider: A systematic review.

Abstract: Background: The transition from hospital to home is a high-risk period. Timely follow-up care is essential to reducing avoidable harms such as adverse drug events, yet may be unattainable for patients who lack attachment to a primary care provider. Transitional care clinics (TCCs) have been proposed as a measure to improve health outcomes for patients discharged from hospital without an established provider. In this systematic review, we compared outcomes for unattached patients seen in TCCs after hospital discharge relative to care as usual., Methods: We searched the following bibliographic databases for articles published on or before August 12, 2022: MEDLINE, Cumulative Index to Nursing and Allied Health Literature, Cochrane Database of Systematic Reviews, PsycINFO, and Web of Science. Five studies were identified that examined the effects of a dedicated postdischarge clinic on emergency department (ED) visits, readmissions, and/or mortality within 90 days of discharge for patients with no attachment to a primary care provider., Results: Studies were heterogeneous in design and quality; all were from urban centers within the United States. Four of the five studies reported a reduction in either the number of ED visits or readmissions in patients seen in a TCC following hospitalization., Conclusions: TCCs may be effective in reducing hospital contacts in the period following hospital discharge in patients with no established primary care provider. Further studies are required to evaluate the health benefits attributable to the implementation of TCCs across a broad range of practice contexts, as well as the cost implications of this model., (© 2024 The Authors. Abstract: Objectives: Delayed cerebral ischemia (DCI) is a major driver of morbidity after aneurysmal subarachnoid hemorrhage (aSAH). Quantitative pupillometry has been shown to be of prognostic value after acute neurological injury. However, the evidence for the use of pupillometric features for the detection of DCI has been conflicting. The aim of this study was to investigate the prognostic value of frequent pupillometric monitoring for DCI detection., Design: Observational cohort study from a prospective aSAH registry., Setting: Tertiary referral center., Patients: Adult patients with confirmed aSAH admitted to the ICU between March 2019 and December 2023., Interventions: None., Measurements and Main Results: One hundred fourteen patients were included, of which 31 (27.2%) suffered from DCI. All patients underwent frequent pupillometry (every 3 hr). We determined the absolute value of the neurological pupil index (NPi) and constriction velocity (CV), and their value normalized to the maximal recorded value between the admission and the pupillometry measure to account for personalized baselines. The association between pupillometry values and the occurrence of DCI within 6-24 hours was investigated. Normalized CV had the best discriminative performance to identify DCI within 8 hours, with an area under the receiver operating characteristic curve of 0.82 (95% CI, 0.69-0.91). NPi, as well as non-normalized metrics, were not significantly associated with DCI., Conclusions: Normalized CV has a clinically and statistically significant association with the occurrence of DCI after aSAH. Abstract: Background: This cross-sectional study aimed to assess the correlation between subjective oral health status and mental health in a group of university students in Serbia., Methods: The study included 948 students, aged between 18 and 27, from the Faculty of Medical Sciences, University of Kragujevac, Serbia, and was conducted in 2020. The World Health Organization's Oral Health Questionnaire for Adults was utilized to evaluate the respondents' self-perceived oral health and oral hygiene practices. The existence of depressive symptoms was evaluated using the Beck Depression Inventory-II (BDI-II), and the Zung self-rating anxiety scale (SAS) was used for determining anxiety symptoms., Results: It is observed that 28.9% of respondents exhibit symptoms of depression and 42.3% showed symptoms of anxiety. Results show that students with severe depression and anxiety symptoms very often reported lower self-perceived oral health and oral health problems. Although the nature of this relationship has not been thoroughly evaluated, several studies have shown a mutually dependent connection between oral health and mental health., Conclusions: The results suggest that some aspects of oral health are associated with higher risks of developing symptoms of depression or anxiety, and vice versa.

Abstract: Importance: Adequate situational awareness in patient care increases patient safety and quality of care. To improve situational awareness, an innovative, low-fidelity simulation method referred to as Room of Improvement, has proven effective in various clinical settings., Objective: To investigate the impact after 3 months of Room of Improvement training on the ability to detect patient safety hazards during an intensive care unit shift handover, based on critical incident reporting system (CIRS) cases reported in the same hospital., Methods: In this educational intervention, 130 healthcare professionals observed safety hazards in a Room of Improvement in a 2 (time 1 vs time 2)×2 (alone vs in a team) factorial design. The hazards were divided into immediately critical and non-critical., Results: The results of 130 participants were included in the analysis. At time 1, no statistically significant differences were found between individuals and teams, either overall or for non-critical errors. At time 2, there was an increase in the detection rate of all implemented errors for teams compared with time 1, but not for individuals. The detection rate for critical errors was higher than for non-critical errors at both time points, with individual and group results at time 2 not significantly different from those at time 1. An increase in the perception of safety culture was found in the pre-post test for the questions whether the handling of errors is open and professional and whether errors are discussed in the team., Discussion: Our results indicate a sustained learning effect after 12 weeks, with collaboration in teams leading to a significantly better outcome. The training improved the actual error detection rates, and participants reported improved handling and discussion of errors in their daily work. This indicates a subjectively improved safety culture among healthcare workers as a result of the situational awareness training in the Room of Improvement. Abstract: Background: In 2022, the European Society of Cardiology updated guidelines for preoperative evaluation. The aims of this study were to quantify: (1) the impact of the updated recommendations on the yield of pathological findings compared with the previous guidelines published in 2014; (2) the impact of preoperative B-type natriuretic peptide (NT-proBNP) use for risk estimation on the yield of pathological findings; and (3) the association between 2022 guideline adherence and outcomes., Methods: This was a secondary analysis of MET-REPAIR, an international, prospective observational cohort study (NCT03016936). Primary endpoints were reduced ejection fraction (EF<40%), stress-induced ischaemia, and major adverse cardiovascular events (MACE). The explanatory variables were class of recommendations for transthoracic echocardiography (TTE), stress imaging, and guideline adherence. We conducted second-order Monte Carlo simulations and multivariable regression., Results: In total, 15,529 patients (39% female, median age 72 [inter-quartile range: 67-78] yr) were included. The 2022 update changed the recommendation for preoperative TTE in 39.7% patients, and for preoperative stress imaging in 12.9% patients. The update resulted in missing 1 EF <40% every 3 fewer conducted TTE, and in 4 additional stress imaging per 1 additionally detected ischaemia events. For cardiac stress testing, four more investigations were performed for every 1 additionally detected ischaemia episodes. Use of NT-proBNP did not improve the yield of pathological findings. Multivariable regression analysis failed to find an association between adherence to the updated guidelines and MACE., Conclusions: The 2022 update for preoperative cardiac testing resulted in a relevant increase in tests receiving a stronger recommendation. Abstract: Purpose: Sodium MRI is challenging because of the low tissue concentration of the 23 Na nucleus and its extremely fast biexponential transverse relaxation rate. In this article, we present an iterative reconstruction framework using dual-echo 23 Na data and exploiting anatomical prior information (AGR) from high-resolution, low-noise, 1 H MR images. This framework enables the estimation and modeling of the spatially varying signal decay due to transverse relaxation during readout (AGRdm), which leads to images of better resolution and reduced noise resulting in improved quantification of the reconstructed 23 Na images., Methods: The proposed framework was evaluated using reconstructions of 30 noise realizations of realistic simulations of dual echo twisted projection imaging (TPI) 23 Na data. Moreover, three dual echo 23 Na TPI brain datasets of healthy controls acquired on a 3T Siemens Prisma system were reconstructed using conventional reconstruction, AGR and AGRdm., Results: Our simulations show that compared to conventional reconstructions, AGR and AGRdm show improved bias-noise characteristics in several regions of the brain. Moreover, AGR and AGRdm images show more anatomical detail and less noise in the reconstructions of the experimental data sets. Compared to AGR and the conventional reconstruction, AGRdm shows higher contrast in the sodium concentration ratio between gray and white matter and between gray matter and the brain stem., Conclusion: AGR and AGRdm generate 23 Na images with high resolution, high levels of anatomical detail, and low levels of noise, potentially enabling high-quality 23 Na MR imaging at 3T., (© 2023 International Society for Magnetic Resonance in Medicine.)

35. Early sevoflurane sedation in severe COVID-19-related lung injury patients. A pilot randomized controlled trial.

Abstract: Background: This study aimed to assess a potential organ protective effect of volatile sedation in a scenario of severe inflammation with an early cytokine storm (in particular IL-6 elevation) in patients suffering from COVID-19-related lung injury with invasive mechanical ventilation and sedation., Methods: This is a small-scale pilot multicenter randomized controlled trial from four tertiary hospitals in Switzerland, conducted between April 2020 and May 2021. 60 patients requiring mechanical ventilation due to severe COVID-19-related lung injury were included and randomized to 48-hour sedation with sevoflurane vs. continuous intravenous sedation (= control) within 24 h after intubation. The primary composite outcome was determined as mortality or persistent organ dysfunction (POD), defined as the need for mechanical ventilation, vasopressors, or renal replacement therapy at day 28. Secondary outcomes were the length of ICU and hospital stay, adverse events, routine laboratory parameters (creatinine, urea), and plasma inflammatory mediators., Results: 28 patients were randomized to sevoflurane, 32 to the control arm. The intention-to-treat analysis revealed no difference in the primary endpoint with 11 (39%) sevoflurane and 13 (41%) control patients (p = 0.916) reaching the primary outcome. Five patients died within 28 days in each group (16% vs. 18%, p = 0.817). Of the 28-day survivors, 6 (26%) and 8 (30%) presented with POD (p = 0.781). There was a significant difference regarding the need for vasopressors (1 (4%) patient in the sevoflurane arm, 7 (26%) in the control one (p = 0.028)). Length of ICU stay, hospital stay, and registered adverse events within 28 days were comparable, except for acute kidney injury (AKI), with 11 (39%) sevoflurane vs. 2 (6%) control patients (p = 0.001). The blood levels of IL-6 in the first few days after the onset of the lung injury were less distinctly elevated than expected., Conclusions: No evident benefits were observed with short sevoflurane sedation on mortality and POD. Unexpectedly low blood levels of IL-6 might indicate a moderate injury with therefore limited improvement options of sevoflurane. 37. Association between self-reported functional capacity measures and postoperative myocardial injury in patients undergoing noncardiac surgeries.

Abstract: Background: Self-reported functional capacity measures have an uncertain role in the pre-operative cardiovascular risk stratification., Aim: This substudy aimed to evaluate whether self-reported metabolic equivalent (MET) could improve the prediction of postoperative myocardial injury (MI) over other well-established cardiovascular risk factors., Methods: This is a post hoc analysis of an international multicenter prospective cohort study. We recruited patients ≥45 years old who had elective elevated-risk noncardiac surgery in 45 centers across 17 countries between June 2017 and April 2020. The primary outcome was MI defined according to the Fourth Universal Definition of Myocardial Infarction. We measured the proportion of new prognostic information added by self-reported MET using multivariable logistic regression., Results: In total, 860 (41.3%) patients suffered MI. In patients without systematic troponin surveillance, the odds ratio for MI with each 1-point increment in MET equaled 1.03 (95% confidence interval [CI], 0.99-1.07). The new prognostic information, according to the likelihood ratio adequacy index, accounted for 1.5%. Sensitivity analysis, including centers with >90% of patients with routine high-sensitivity troponin T monitoring, showed that MET added 21.8% of new information to the baseline model, and each additional point was associated with a lower risk of MI (odds ratio, 0.86; 95% CI, 0.81-0.91)., Conclusions: In elevated-risk noncardiac surgery, self-reported functional capacity measures do not significantly improve the prediction of MI; however, they add new prognostic information in centers with routine perioperative troponin monitoring.
