Background/Objectives: The most common variant of mixed urinary incontinence is stress-induced urge urinary incontinence with the correlating urodynamic findings of cough-induced detrusor overactivity (CIDO). This prospective study assessed the clinical outcomes and leakage improvement among patients with CIDO following conservative or surgical treatment. Methods: We included patients with CIDO treated at our tertiary referral center from January 2018 to July 2021 in this prospective cohort study. The detection of a detrusor contraction after a cough was diagnosed as CIDO by urodynamic multichannel testing. All the patients in our study received personalized care, with behavioral therapy and anticholinergic/betamimetic treatment as a first step. If leakage persisted, patients were given a choice between pelvic floor muscle exercises (PFMEs), periurethral bulking or a midurethral sling. The primary outcome was the mean difference in urine leakage in the pad test before and six months after treatment. Results: Thirty-five patients met the inclusion criteria for CIDO and all presented a positive pad test at baseline (mean: 27 g). All 35 patients participated in behavioral therapy and anticholinergic/betamimetic treatment. Twenty-two patients (62.9%) underwent PFME, twelve patients (34.2%) received periurethral bulking, and nine patients (25.7%) received a midurethral sling. After all the treatments, our cohort showed a significant improvement in the pad test (mean: 5.7 g, p < 0.001). The result was more favorable after periurethral bulking than the midurethral sling (p < 0.001). Conclusions: This study shows the effectiveness of conservative treatment as a first step. In cases needing further treatment, bulking agents may be superior to PFME and midurethral propylene slings, offering new perspectives in the field of urogynecology and urinary incontinence. Cinema Culture in 1930s Britain (CCINTB) is a pioneering inquiry into memories of 1930s cinemagoing conducted in the 1990s by Annette Kuhn. Its research design, incorporating questionnaire surveys and interviews involving male and female participants in various parts of Britain, has become a benchmark for methods of exploring everyday practices during a golden age of cinemagoing. CCINTB's successor project, Cinema Memory and the Digital Archive (led by Professor Richard Rushton), has facilitated the creation of an accessible online infrastructure that has opened up CCINTB's data and findings for further research possibilities. CMDA's co-Investigators Annette Kuhn and Sarah Neely spoke to Dario Llinares, host of the Cinematologists Podcast, about the scope and objectives of the two projects, reflecting on questions of methodology in historical audience research; on the uses and value of digital tools in gathering, recording, analysing and presenting the findings of this kind of investigation; on strategies for making these findings accessible for a range of users; and on the future of cinema memory. Background: Tension-free mid-urethral sling procedures are next to bulking agents, fascial slings and colposuspension a possible treatment in stress urinary incontinence with high success rates up to 90%. This is an unusual and unique case of delayed presentation of a tension-free vaginal tape (TVT) complication involving the pubic bone. It shows an unusual possible cause of bladder outlet obstruction by a reactive inflammatory myofibroblastic pseudotumor. We discuss the diagnosis and management of this unique case of bladder outlet obstruction. Case presentation: We present an exclusive case of a 66-year woman with a rare cause of bladder outlet obstruction (BOO) after midurethral sling placement for recurrent stress urinary incontinence. Nine years after successful midurethral sling (MUS) insertion with relief of incontinence symptoms, the patient complained of spraying stream and prolonged micturition. Bladder outlet obstruction caused by a large retropubic myofibroblastic pseudotumor was detected by clinical examination and MRI. After removal of the mass and part of the pubic bone a plate osteosynthesis was performed. The patient received physiotherapy and local estrogen therapy. Patients’ symptoms improved reporting non-bothering intermittent incontinence episodes. The patient showed up recently and received again physiotherapy. Conclusions: Although bladder outlet obstruction after midurethral sling placement is mainly caused by an overtightened sling other factors should be considered. [ABSTRACT FROM AUTHOR]
