Jiangfangbaoxin (JFBX) has been used to attenuate essential hypertension (EH) for over 20 years. Here, we aimed to examine the blood composition and toxicological effects of JFBX using 40 and 10 male age-matched spontaneously hypertensive rats (SHRs) and Wistar–Kyoto, respectively. After 12 weeks of intervention, blood pressure and left ventricular hypertrophy were measured. Subsequently, their organs were harvested for pathological examination. Ultra-high-performance liquid chromate- graphy with quadrupole time-of-flight mass spectrometry was used to explore the components of JFBX in blood. Low and high-doses of JFBX attenuated both systolic and diastolic blood pressure in the SHRs. The high- and low-dose JFBX groups showed significantly reduced endothelin-1 and increased nitric oxide levels. The myocardial tissue levels of tumor necrosis factor-α, interleukin (IL)-1β, and IL-6 in the JFBX-high-dose group were significantly decreased. Furthermore, superoxide dismutase and malonaldehyde levels significantly increased and decreased, respectively. The toxicological results showed that JFBX did not cause particularly severe side effects when administered after 12 weeks. In total, 87 precursor compounds, including alkaloids, iridoid glycosides, flavonoids, siterpenoids, volatile components, and organic phenols/acids, were detected in the blood. JFBX attenuated EH. Sepsis is a severe infectious disease with high mortality. However, the indicators used to evaluate its severity and prognosis are relatively complicated. The systemic inflammatory response index (SIRI), a new inflammatory indicator, has shown good predictive value in chronic infection, stroke, and cancer. The purpose of this study was to investigate the connection between sepsis and SIRI and evaluate its predictive usefulness. A total of 401 patients with sepsis were included in this study. Multiple linear regression and logistic regression analyses were performed to evaluate the relationship between SIRI and sepsis. The restricted cubic spline (RCS) method was employed to illustrate the dose-response relationship. The area under the curve (AUC) and decision curve analysis (DCA) were used to evaluate the prognostic value of SIRI. Multiple linear regression analysis revealed a significant positive correlation between SIRI and both blood cell count and Sequential Organ Failure Assessment (SOFA) score. Additionally, higher SIRI levels were significantly linked to a higher risk of sepsis worsening, according to logistic regression analysis. The RCS curve demonstrated that the risk of poor prognosis rose with increasing SIRI, particularly when SIRI exceeded 6.1. Furthermore, AUC and DCA results showed that SIRI had superior predictive value compared to traditional indicators. A higher SIRI is linked to a worse prognosis and more severe sepsis. Aim: To evaluate the relationship between time spent in sedentary behaviour and physical activity and sarcopenia in older adults, and to analyse the effect of reallocating time between different intensities of activities on sarcopenia., Design: Systematic review and meta-analysis., Methods: Following PRISMA guidelines, data were synthesised using a random-effects model, with heterogeneity assessed via Cochran's Q test and the I 2 statistic. Study quality was evaluated using the Newcastle-Ottawa Scale by two independent reviewers., Data Sources: A comprehensive search was conducted in PubMed, Web of Science, Embase, CINAHL and Cochrane databases for studies published up to November 5, 2024, with no language or date restrictions. Relevant reference lists were also manually screened., Results: The present review included six studies involving 9914 older adults. Three studies suggested that older adults without sarcopenia spent more time performing light physical activities (SMD: 0.35; 95% CI: 0.24-0.45) and moderate to vigorous physical activity (SMD: 0.61; 95% CI: 0.49-0.74) and had less sedentary behaviour (SMD: -0.34; 95% CI: -0.51 to -0.16) than did older adults with sarcopenia. Replacing sedentary behaviour with an equivalent amount of moderate to vigorous physical activity (10, 30, or 60 min) each day can reduce the risk of sarcopenia, with 30 min showing the best preventive effect. However, research findings on the relationship between substituting sedentary behaviour time with light physical activities and sarcopenia are inconsistent., Implications for the Profession: Encouraging older adults to engage in moderate to vigorous physical activity, even in short bouts of 10 min, can significantly reduce the risk of sarcopenia. Background: Non-invasive temporal artery thermometers (TATs) and non-contact infrared thermometers (NCITs) are increasingly used in community settings to measure body temperature. Existing research predominantly focuses on pediatric populations, yet the accuracy and precision of TATs and NCITs for fever screening across age groups remain unclear. This study aims to assess age-related differences in the diagnostic accuracy of TATs and NCITs for fever detection., Methods: A systematic review and meta-analysis were conducted, sourcing data from PubMed, MEDLINE, CINAHL, EMBASE, Cochrane Library, ProQuest, and Web of Science. Prospective studies comparing TATs and NCITs against body temperature measurement methods were included. Two independent researchers extracted data, and study quality was assessed with the QUADAS-2 tool. Pooled estimates of sensitivity, specificity, and the hierarchical summary area under the receiver operating characteristic (ROC) curves were calculated using STATA version 17., Results: This meta-analysis included 34 studies with 28,996 participants, of whom 5,358 were febrile. For TATs, 22 studies with 9,894 readings yielded a pooled sensitivity of 0.59 (95% CI: 0.40-0.76) and specificity of 0.91 (95% CI: 0.83-0.96). Sensitivity was higher at fever thresholds > 38 °C (0.71, 95% CI: 0.60-0.80), and higher in children (0.77, 95% CI: 0.66-0.85) than in adults (0.48, 95% CI: 0.30-0.67). Similar sensitivities were observed between rectal and other standards (0.70, 95% CI: 0.59-0.80 vs. 0.70, 95% CI: 0.41-0.89). For NCITs, 16 studies with 14,234 readings yielded a pooled sensitivity of 0.70 (95% CI: 0.54-0.82) and specificity of 0.94 (95% CI: 0.90-0.97). Sensitivity improved at fever thresholds > 38 °C (from 0.70 to 0.75, 95% CI: 0.55-0.88) and was higher in children compared to the overall estimate (0.79 vs. 0.70, 95% CI: 0.62-0.90). Comparable sensitivities were noted between axillary and other standards (0.73, 95% CI: 0.30-0.94 vs. 0.75, 95% CI: 0.49-0.90)., Conclusions: TATs and NCITs show variable diagnostic accuracy across age groups, with higher sensitivity in children and at elevated fever thresholds.
