Background: Venous thromboembolism frequently complicates orthopedic surgery. The aim of the study was to evaluate the overall incidence, site, and risk factors for venous thromboembolism in patients undergoing hip fracture surgery in DVT prophylaxis according to guidelines recommendations. Methods: Standard ultrasonography (CUS) was performed in the 5–6th postoperative day in all patients who underwent hip fracture surgery between 1 January and 31 December 2019. Pharmacological prophylaxis was started within 12 h from admission. In the first half of the year, dalteparin (5000 IU/day) was available while nadroparin (38 IU/kg until 3rd postoperative day and thereafter 57 IU/kg) was available in the second part of the year. Results: A total of 505 patients, 144 males and 361 females, with a mean age of 84 years, entered in the study. Post-operative DVT was found at screening ultrasonography in 121 patients (24%). Most involved distal veins (91) while proximal DVT occurred in 30. Two patients had not fatal pulmonary embolism (0.3%). Time to surgery (p = 0.0009) and ≥2 comorbidities (p = 0.0198) were independent predictive factors of DVT. Moreover, dalteparin prophylaxis was associated with a 1.7-times higher risk of developing a DVT compared to nadroparin. Conclusions: DVT occurs in 24% of patients after hip fracture surgery despite thromboprophylaxis. Time to surgery and ≥2 comorbidities were independent risk factors. The protective effects of nadroparin should be confirmed by a randomized trial. All patients with DVT were discharged with indication to anticoagulation for at least three months. Pediatric central nervous system (CNS) tumors represent one of the most intricate challenges faced by children and their families in the context of cancer illness. In Italy, the psychosocial working group (GDL) of the AIEOP, composed by professionals (psycho-oncologists/neuropsychologists) involved in the care pathways of patients with CNS tumors, has initiated a consensus process regarding the psychological/neuropsychological assessment of CNS patients to improve accessibility to standardized and shared protocols. Standardized psychological and neuropsychological instruments are proposed in the document, which provides for a selection in a tailor-made approach that is simultaneously shared by various centers. This study examined the behavior of the semi-continuous hydrolysis process of corn stover (CS) in subcritical water, focusing on the effects of varying pH levels (1, 2, 4.5, 7, and 8) and temperatures (113, 130, 170, 210, and 226°C). The results showed that the process at 170°C and pH 1 was able to recover the highest amount of phenolic compounds (76.82 mg Gallic Acid Equivalents g⁻¹), consequently demonstrating the highest antioxidant activities by the Ferric Reducing Antioxidant Power (FRAP) (423.85 μMol TEAC g⁻¹) and 2,2-diphenyl-1-picrylhydrazyl (DPPH) (293.12 μMol TEAC g⁻¹) methods. Additionally, it was possible to obtain the highest concentration of sugars (cellobiose, glucose, and xylose) (460.92 mg g⁻¹). High temperatures (226°C and pH 4.5) produced the highest amount of organic acids (1,157.19 mg g⁻¹). The formation of inhibitors was observed only at temperatures of 170 and 210 °C at a pH of 1.0. The highest yields of sugars, organic acids, and inhibitors were 0.565, 1.123, and 0.665 mg g⁻¹ of carbohydrates, respectively. The statistical analysis identified the optimal conditions for the recovery of various compounds: phenolic compounds at 190.7 °C and pH 1, soluble proteins at 187.4 °C and pH 1, sugars at 134.9 °C and pH 1, organic acids at 223.59 °C and pH 4.1, and for minimizing inhibitors at 114.95 °C and pH 7.3. EcoScale analysis identified subcritical water as the most sustainable and efficient method for CS hydrolysis. Subcritical water treatment effectively recovers valuable compounds from CS, promoting a circular economy by valorizing waste and reducing resource dependence.

Although the concept that the blood-brain barrier (BBB) plays an important role in the etiology and pathogenesis of Alzheimer's disease (AD) has become increasingly accepted, little is known yet about how it actually contributes. We and others have recently identified a novel functionally distinct subset of BBB pericytes (PCs). In the present study, we sought to determine whether these PC subsets differentially contribute to AD-associated pathologies by immunohistochemistry and amyloid beta (Aβ) peptidomics. We demonstrated that a disease-associated PC subset (PC2) expanded in AD patients compared to age-matched, cognitively unimpaired controls. Surprisingly, we found that this increase in the percentage of PC2 (%PC2) was correlated negatively with BBB breakdown in AD patients, unlike in natural aging or other reported disease conditions. The higher %PC2 in AD patients was also correlated with a lower Aβ42 plaque load and a lower Aβ42:Aβ40 ratio in the brain as determined by immunohistochemistry. Colocalization analysis of multicolor confocal immunofluorescence microscopy images suggests that AD patient with low %PC2 have higher BBB breakdown due to internalization of Aβ42 by the physiologically normal PC subset (PC1) and their concomitant cell death leading to more vessels without PCs and increased plaque load. On the contrary, it appears that PC2 can secrete cathepsin D to cleave and degrade Aβ built up outside of PC2 into more soluble forms, ultimately contributing to less BBB breakdown and reducing Aβ plaque load. Collectively our data shows functionally distinct mechanisms for PC1 and PC2 in high Aβ conditions, demonstrating the importance of correctly identifying these populations when investigating the contribution of neurovascular dysfunction to AD pathogenesis., (© 2024 The Author(s).
