The extradural supraodontoid space lies anteriorly at the craniocervical junction (CCJ) between the alar ligaments and foramen magnum. It occupies the space between the tectorial and atlanto-occipital membranes. A variety of benign and traumatic lesions may result in neurological compression here with harmful effects. Decompression by the transoral surgical approach often provides relief from these effects. Knowledge of the detailed microanatomy of this space is fragmentary. The purpose of this study was to identify the boundaries and contents of this space by microdissection. Twenty-three en bloc preserved adult cadaveric specimens of the CCJ were dissected to identify the boundaries and contents of the supraodontoid space. The posterior bony elements of the CCJ were removed to enable microdissection (Zeiss DXE Microscope 4-40x) from the tectorial membrane (TM) forwards. The cave-like space faced posteriorly. It had a roof which extended into a wall (anterior atlanto-occipital membrane), a floor (superior surface of the alar ligament), and a mouth covered by the TM. The apical ligament and a thin lining membranous fatty layer divided the cave into a pair of symmetrical halves. The contents, from dorsal to ventral, lay deep to a thin subtectorial membrane. These were the superior fasciculus of the cruciate ligament, a fat-ensheathed knot of plexiform veins (which communicated with the surrounding CCJ vertebral venous plexuses), an arterial arcade between the veins, a pair of fat pads, and branches of the sinuvertebral nerves of the CCJ (lying on the floor). No synovial membrane was found. Knowledge of the anatomy of the apical cave may be of some assistance in transoral (extra- and transdural) surgical approaches to the anterior CCJ region.

To explore the many osseous irregularities that are found in the area between the basiocciput, the anterior arch of the atlas and the tip of the dens axis we studied 99 cadaver specimens using magnetic resonance tomography (MRT), computed tomography (CT), median saw-cut sections, and histological sections. Additionally, "dry" specimens of the skull (n = 110), atlas (n = 56), and axis (n = 33) were investigated. In the median plane, the dry and cadaver specimens exhibited osteoarthritis-related osseous outgrowths and osteophytes of the articular surfaces of the median atlanto-axial joint (n = 63), and the presence of congenitally developed free ossicles (n = 22) and of third occipital condyles (n = 3). The largest osteophytes (giant osteophytes) (n = 4) of the anterior arch of the atlas formed osseous contact zones with the basiocciput that were visible histologically as real joints and were designated accessory median atlanto-occipital joints. The third occipital condyles also formed osseous contact zones, visible histologically as real joints, with the anterior arch of the atlas or with the tip of the dens, and were designated accessory atlanto-occipital or occipito-odontoid joints. Frequent free ossicles, incorporated into the accessory joint, were found by histological examination to be covered with hyaline cartilage., (Copyright 2005 Wiley-Liss, Inc)

Anthropometric and statistical evaluation of measurements from digitization of 252 lateral cervical radiographs were used to investigate any correlation between radiographic measurements of cervical lordosis with sagittal plane facet angulation, articular pillar height, and inclination of the C2 odontoid with respect to the body of C2. Some researchers have hypothesized that facet and odontoid architecture variations can cause a reduction in cervical lordosis. To evaluate this hypothesis, the posterior aspect of the C2 dens, vertebral body corners, and superior and inferior facet surfaces of C2-C7 were digitized on 252 lateral cervical X-rays to calculate global angle, segmental angles, dens angle, facet angles, and facet height. No correlation between facet angle, articular pillar height, and cervical curve was found. Similarly, no correlation between the sagittal angle of the dens and any angle of cervical curvature was identified. There was correlation between the global ARA C2-C7 angle and the Cobb angles at C1-C7 (r = 0.71) and C2-C7 (r = 0.82). There was correlation between the global inclination of the atlas vertebral angle (APL) and the Cobb angle at C1-C7 (r = 0.66), Cobb angle at C2-C7 (r = 0.39), ARA C2-C7 (r = 0.42), and anterior translation of C2 compared to C7 (r = -0.46). Because no correlation between cervical facet and odontoid architecture and any segmental or global angle of cervical lordosis was found, conservative and surgical rehabilitative techniques aimed at the reduction of sagittal cervical deformities do not need to account for a patient's architecture of the cervical facets nor odontoid., (Copyright 2005 Wiley-Liss, Inc.)
