BACKGROUND: Thymomas can benefit of cytoreductive surgery even if a complete resection is not feasible. The pleural cavity is the most common site of progression and the resection of pleural metastases can be performed in selected patients. We evaluated the results of stereotactic body radiation therapy for the treatment of pleural metastases in patients not eligible for surgery. METHODS: We retrospectively selected 22 patients treated with stereotactic body radiation therapy for pleural metastases between 2013 and 2019. According to RECIST criteria 1.1 modified for thymic epithelial tumors, time to local failure and progression free survival were calculated using Kaplan-Meier method. RESULTS: The median age was 40 years (range, 29–73 years). There were 1 A, 3 AB, 3 B1, 3 B2, 3 B2/B3 and 9 B3 thymomas. Pleural metastases and primary tumor were synchronous in 8 patients. Five patients had a single pleural metastatic site and 17 presented multiple localizations. Sixteen patients received stereotactic body radiation therapy on multiple sites of pleural metastases. The median dose of radiation was 30 Gy (range, 24–40 Gy). With a median follow-up of 33.2 months (95% CI: 13.1–53.3 months), ten patients experienced disease progression with a median progression free survival was 20.4 months (95% CI: 10.7–30.0 months). The disease control rate was 79% and 41% after 1 and 2 years, respectively. Local disease control rate was 92% and 78% after 1 and 2 years, respectively. There were not significant differences in progression free survival between patients diagnosed with synchronous and metachronous metastases (P=0.477), across those treated or not with chemotherapy (P=0.189) and between those who received or not a previous surgical resection of the pleural metastases (P=0.871). There were not grade 3–4 toxicities related to the treatment. CONCLUSIONS: Stereotactic body radiation therapy of pleural metastases is feasible and offers a promising local control of diseases. The impact of this treatment on patients’ survival is hardly predictable because of the heterogeneous clinical behavior of thymomas.

Extracorporeal membrane oxygenation (ECMO) is a form of cardiopulmonary life-support, and may be very useful in selected cases as traumatic tracheo-bronchial injuries, allowing an efficient gas exchange where ventilation is not feasible. Even if, nowadays, little used, ECMO is an additional arrow in the quiver of the doctors, especially in challenging cases.

Surgery is one of the steps of multimodality approach for the treatment of MPM. Due to anatomical features, microscopically radical (R0) resection is never possible and a Macroscopic Complete Resection (R1) is considered the target for mesothelioma surgeons. Recently, intracavitary therapies have been described with the aim of extending the loco-regional effect of surgery. Different agents might be administered intrapleurally: chemotherapy drugs are the most widely used, but also photodynamic therapy (PDT) showed to lead to satisfactory long-term outcomes; furthermore, immunotherapies and gene therapies have been also reported. Despite promising results, no high-quality evidences are currently available and controlled randomized trials are required to establish the exact role of intracavitary therapies and to standardize the technique.

Metastases to lymph nodes of the superior mediastinum have been found in up to 10% of patients with thyroid carcinoma. The treatment of mediastinal lymph node metastases in thyroid cancer is still far from being standardized. However, in selected cases, especially in case of radioiodine ablation and radiation therapy failure, a surgical resection may improve the patients’ survival in addition to the symptomatic benefits. We report our experience in four consecutive patients who underwent extended mediastinal surgery for large lymph node metastases from thyroid cancer during the last year. All patients were pre-operatory evaluated by a multidisciplinary team involving endocrinologist, anaesthesiologist and surgeons. Surgery was carried out in all cases in two steps, firstly through a cervicotomy in order to remove the eventual local recurrence and ensure a radical neck dissection bilaterally and then by sternotomy, in order to dissect all the lymphadenopathies from the mediastinal structures. Two patients (50%) had an extended vascular infiltration and the extracorporeal circulation (ECC) and, at least, one vascular graft, were needed. No intraoperative or perioperative mortality occurred in this series. Two patients (50%) presented a sternal dehiscence that required a second surgery and a prolonged postoperative hospital stay. In all cases the surgery was radical, and the histological report revealed a macroscopic complete resection and, up to date, no recurrences were found. When feasible, surgery is more effective than systemic therapy in the treatment of thyroid cancer recurrences, especially for medullary thyroid cancer in which radioiodine therapy is ineffective and should always be evaluated in case of vascular infiltration or symptomatic patients. Moreover, due to diversity and complexity of the cervical and mediastinal structures that may be involved, a multidisciplinary medical team should always be guaranteed during the surgery.

Small cell lung cancer (SCLC) has always been an insidious field for thoracic surgeons, due to unclear possible indications and deceiving results. The majority of studies regarding surgery for SCLC are dated and the poor outcomes obtained have discouraged their continuation. Nevertheless, since platinum-based chemotherapy protocols have been introduced and the use of new technologies, such as PET scan, allowed a more careful and precise staging, new possible indications raised. To date, surgery plays a very marginal role in the treatment of SCLC and major oncological guidelines indicate a resection only in case of a very limited disease (T1–2N0M0), which account for less than 5% of all diagnosed SCLC (1); in this highly selected group of patients, surgery associated with adjuvant therapies bring to an impressive 65% 5-year survival.
