Background: Tafasitamab, an anti-CD19 immunotherapy that enhances antibody-dependent cellular cytotoxicity and phagocytosis, received accelerated approval in the USA and conditional authorization in Europe in combination with lenalidomide (LEN) for patients (pts) with relapsed or refractory (R/R) diffuse large B-cell lymphoma (DLBCL) ineligible for autologous stem cell transplant (ASCT) based on the results of the open-label, multicenter, single-arm, Phase II L-MIND study (NCT02399085; Salles G, et al. Lancet Oncol 2020, Duell J, et al. Haematologica 2021). Here, we report the final, 5-year follow-up of L-MIND. Data cut-off was Nov 14, 2022. Methods: Pts were aged ≥18 years with ASCT-ineligible R/R DLBCL, 1-3 prior systemic therapies (including a CD20-targeting regimen), and ECOG PS 0-2. Tafasitamab (12 mg/kg) was given for up to 12 cycles in combination with LEN (25 mg), then as monotherapy until disease progression (PD) or unacceptable toxicity. The primary endpoint was best objective response rate (ORR; complete response [CR] or partial response [PR], by independent radiology committee). Secondary endpoints included duration of response (DoR), progression-free survival (PFS), overall survival (OS), and incidence and severity of adverse events (AEs). Exploratory analyses evaluated efficacy endpoints by prior lines of therapy (pLoT). Results: Of 81 pts enrolled, 80 were treated (full analysis set [FAS]). The ORR (FAS) of 57.5% [95% CI: 45.9-68.5], with CR of 41.2% [30.4-51.6] (n=33) and PR of 16.2% [8.9-26.2] (n=13), was generally consistent with the primary and 3-year analyses. Median DoR was not reached (NR) with median follow up (mFU) of 44.0 months [29.9-57.0]. Median PFS was 11.6 months [5.7-45.7] (mFU 45.6 [22.9-57.6]) and median OS was 33.5 months [18.3-NR] (mFU 65.6 [59.9-70.3]). At data cut-off, OS was >60 months in 21 pts (18 with best response of CR, 1 PR, 1 stable disease and 1 PD), including 14 with 1 pLoT and 7 with ≥2 pLoT. Pts with 1 pLoT (n=40) in the FAS had higher ORR (67.5%; 52.5% CR [n=21] and 15% PR [n=6]) compared to pts with ≥2 pLoT (n=40; 47.5%; 30% CR [n=12] and 17.5% PR [n=7]). However, median DoR was not reached for both subgroups, indicating similar long-term efficacy for responders. AEs were consistent with previous reports and manageable; incidence declined after transition from combination to tafasitamab monotherapy and again with monotherapy >2 years. Conclusion: The final, 5-year analysis of L-MIND showed prolonged durable responses with tafasitamab + LEN combination therapy, followed by long-term tafasitamab monotherapy, in pts with R/R DLBCL ineligible for ASCT, with median DoR not reached after 44 months mFU. No new safety signals were identified, confirming the tolerability profile observed with earlier data cuts. These long-term data suggest that this immunotherapy may have curative potential that is being explored in further studies. Background: Richter transformation (RT) is an aggressive diffuse large B-cell lymphoma that can occur in 10% of patients with chronic lymphocytic leukemia (CLL). RT has no approved therapies, poor prognosis, and an estimated median overall survival (OS) of 3-11 months. Pirtobrutinib is a well-tolerated highly selective, non-covalent (reversible) Bruton tyrosine kinase inhibitor (BTKi) with favorable oral pharmacology and promising efficacy in patients with poor-prognosis B-cell malignancies following prior covalent BTKi therapy. We now report on RT patients from BRUIN. Methods: Patients previously treated for B-cell malignancies were eligible for pirtobrutinib monotherapy in dose escalation/expansion within the phase 1/2 BRUIN study. Key endpoints: investigator-assessed overall response rate (ORR) and duration of response (DoR) per Lugano 2014 criteria, and safety. The response-evaluable cohort consisted of patients with RT who completed either first response assessment or discontinued therapy. The safety cohort consisted of patients who received ≥1 dose of pirtobrutinib monotherapy (n=725). Data cut was 31 January 2022. Results: Of 57 RT patients (median age: 67 years), 91% (n=52) received at least 1 prior RT-directed therapy. Median lines of prior RT therapy were 2: (anti-CD20 antibody [86%, n=49], chemotherapy [79%, n=45], BCL-2 inhibitor [35%, n=20], BTKi [28%, n=16], PI3K inhibitor [12%, n=7], CAR-T [9%, n=5], immunomodulator [5%, n=3], stem cell transplant [4%, n=2], other systemic therapy [33%, n=19]) and median lines of prior CLL were 2: (BTKi [60%, n=34], anti-CD20 antibody [60%, n=34], chemotherapy [47%, n=27], BCL-2 inhibitor [42%, n=24], immunomodulator [9%, n=5], PI3K inhibitor [7%, n=4], stem cell transplant [7%, n=4], CAR-T [2%, n=1], other systemic therapy [11%, n=6]). ORR for patients within phase 2 (n=56, 200mg once/day starting dose) was 54% (95% CI, 39-68): 5 complete responses, 22 partial responses. Median DoR was 8.6 months (95% CI, 1.9-NE, 63% censored) and median OS was 13.1 months (95% CI, 7.1-NE) at a median follow up time of 5.5 months and 9.7 months respectively. Six patients electively discontinued pirtobrutinib to pursue curative-intent therapy (allogeneic transplant, n=6, 3.8 months median time-on-therapy). The most frequent TEAEs were fatigue (26%, n=191), diarrhea (22%, n=160), and contusion (19%, n=138). Of Grade≥3 TEAEs, neutropenia (20%, n=143) was the most frequent and hypertension (3%, n=20), hemorrhage (2%, n=16), and atrial fibrillation/flutter (1%, n=7) were of low grade. Fifteen (2%) patients discontinued due to a treatment-related AE. Conclusions: Pirtobrutinib demonstrated promising preliminary efficacy and was well tolerated for patients who received prior RT-directed chemoimmunotherapy and covalent BTKi. Presented:ASH2022. Introduction: Patients (pts) with CLL who relapse on therapies targeting B-cell receptor signaling pathways have limited treatment options. Cerala, a selective oral ATR inhibitor, has shown synergistic preclinical activity with a Bruton tyrosine kinase inhibitor (BTKi) in TP53- and ATM-defective CLL cells (Kwok et al. Blood. 2016;127:582-95). Acala is a selective, irreversible BTKi approved for CLL. This phase 1/2 proof-of-concept study investigated cerala alone and in combination with acala in high-risk R/R CLL. Methods: This multicenter, nonrandomized, open-label study (NCT03328273) enrolled adults with high-risk R/R CLL per iwCLL criteria and ECOG performance status ≤2. In Arm A, pts with TP53 mutation, del(17p), or del(11q) having exhausted available treatment options received cerala monotherapy (cohort 1: 160 mg BID continuously; cohort 2 [after ≥2 DLTs in cohort 1]: 160 mg BID 2 wk on/2 wk off). In Arm B, pts with del(11q) suitable for BTKi and cerala treatment per investigator judgment received acala 100 mg BID continuously alone for cycle 1 and acala 100 mg BID continuously + cerala 160 mg BID 1 wk on/3 wk off from cycle 2 onward. Primary objectives: safety and pharmacokinetics (PK). Secondary objective: activity—overall response rate (ORR), complete response rate, duration of response (DOR), progression-free survival (PFS), and overall survival (OS). Pharmacodynamic changes were explored using novel multiple reaction monitoring-targeted mass spectrometry. Results: Eleven pts were treated (median age 64 y; median prior therapy lines 3). Pts in Arm A (n=8 [7 received prior BTKi]: cohort 1 n=5; cohort 2 n=3) received cerala for a median of 3.5 mo (range 0.5-7.7). Grade (G) ≥3 AEs in >1 pt were anemia (75%), thrombocytopenia (63%), and neutropenia (25%); 4 DLTs of G4 thrombocytopenia were reported (3 in 2 pts in cohort 1; 1 in 1 pt in cohort 2). No responses were observed in Arm A. At a median follow-up of 15.1 mo, median PFS was 3.8 mo (95% confidence interval [CI] 0.7-4.6); median OS was 16.9 mo (95% CI 6.6-not reached [NR]). All pts had baseline telomeres within a “fusogenic” range (negative prognostic indicator).In Arm B (n=3; all BTKi-naive), 2 pts received acala + cerala and 1 received acala only. Median (range) treatment duration was 7.2 mo (4.9-9.5) for cerala and 15.9 mo (9.7-18.4) for acala. No G≥3 AEs or DLTs were reported. ORR was 100% (all partial responses); median DOR was NR. At a median follow-up of 15.9 mo, median PFS and OS were NR. PK of acala and cerala were consistent with historical data, with no drug-drug interactions when combined; cerala did not affect BTK occupancy. Conclusions: Cerala monotherapy in BTKi-exposed, high-risk CLL pts showed limited clinical benefit. Acala + cerala was tolerable with preliminary clinical activity in pts with CLL and del(11q). Introduction: Rituximab (R)-based therapies are standard for patients (pts) with relapsed advanced iNHL. Copanlisib (C) is a PI3K inhibitor approved as monotherapy for relapsed follicular lymphoma (FL) in pts who have had ≥2 prior systemic therapies. We report primary data from the Phase III CHRONOS-3 study of treatment with C+R vs placebo (P)+R in relapsed iNHL (NCT02367040). Methods: Pts with relapsed iNHL who were progression-free and treatment-free for ≥12 months (mo) after last R-based therapy or unwilling/unfit to receive chemotherapy were randomized 2:1 to receive C+R or P+R. C 60 mg/P was given i.v. on days 1, 8, and 15 (28-day cycle); R 375 mg/m2 was given i.v. on days 1, 8, 15, and 22 during cycle 1 and on day 1 of cycles 3, 5, 7, and 9. Primary endpoint: centrally assessed progression-free survival (PFS). Secondary endpoints: objective response rate (ORR), duration of response, complete response rate (CRR), overall survival (OS), and treatment-emergent adverse events (TEAEs). The data cut-off date was August 31, 2020. Results: 307 pts were randomized to C+R and 151 to P+R. FL was the most common lymphoma histology subtype (60.0%), followed by marginal zone (MZL, 20.7%), small lymphocytic (SLL, 10.9%), and lymphoplasmacytic/Waldenström macroglobulinemia (LPL/WM, 8.3%). Median age was 63 years (range 28-91). With a median follow-up of 19.2 mo, the primary study endpoint was met: C+R significantly reduced the risk of disease progression/death vs P+R (hazard ratio [HR] 0.52 [95% CI 0.39, 0.69]; 1-sided p=0.000002); median PFS was 21.5 mo (95% CI 17.8, 33.0) vs 13.8 mo (95% CI 10.2, 17.5), respectively. Reductions in risk of progression/death were seen across all histology subtypes (HR [95% CI]): FL 0.580 [0.404, 0.833]; MZL 0.475 [0.245, 0.923]; SLL 0.243 [0.111, 0.530]; LPL/WM 0.443 [0.160, 1.231]. ORRs were 80.8% (CRR 33.9%) for C+R and 47.7% (CRR 14.6%) for P+R. Higher ORRs and CRRs were seen across all iNHL subtypes with C+R treatment. Median OS was not estimable. Most common TEAEs (all grades [G]/G3+) in pts receiving C+R were hyperglycemia (69.4%/56.4%), hypertension (49.2%/39.7% [all G3]), and diarrhea (33.6%/4.9% [all G3]). For pts receiving P+R, hyperglycemia (23.3%/8.2% [all G3]), hypertension (19.2%/8.9% [all G3]), neutropenia (16.4%/12.3%), and upper respiratory tract infection (16.4%/0%) were the most common TEAEs. Serious adverse events were higher with C+R (47.2%) vs P+R (18.5%). G5 TEAEs occurred in 6 pts (2.0%) receiving C+R (1 [0.3%] deemed treatment-related; pneumonitis) and 1 (0.7%) receiving P+R. Conclusions: C+R demonstrated broad and superior efficacy vs P+R in pts with relapsed iNHL. The safety profile of C+R was manageable and consistent with C and R as monotherapy. Copanlisib is the first PI3K inhibitor to be safely combined with R in relapsed iNHL, representing a potential new therapy option for relapsed iNHL across all subtypes. Citation Format: Matthew J. A phase 1/2a study (LYM1002 [EudraCT 2014-005191-28]) of ibr (560 mg once daily) + nivo (3 mg/kg on a 14-day cycle) demonstrated acceptable safety and promising efficacy vs single-agent ibr in Richter’s transformation (RT), follicular lymphoma (FL), and diffuse large B-cell lymphoma (DLBCL). We examined potential biomarkers of treatment (tx) response using archived biopsy samples. GEP was used for DLBCL subtyping and to assess proportions of 22 distinct immune cells. Exome data were generated from 72 formalin-fixed paraffin-embedded samples, and sequencing analysis was used to identify mutations in genes of interest and assess somatic mutation burden. The correlations of immune cell proportions and gene variants were evaluated by investigator-assessed responses in each histology and by ongoing responses in DLBCL patients (pts; progression-free survival [PFS] > 24 months, n = 7 vs not, n = 20). In pts with available GEP and response data, overall response rates were 29.6% (8/27) for DLBCL, 43.5% (10/23) for FL and 81.3% (13/16) for RT. Proportions of CD8 and follicular helper T cells, M1 macrophages, and resting dendritic cells were higher in DLBCL responders vs nonresponders, while the proportion of regulatory T cells was decreased. These subsets did not differ by response in FL although an increase of CD4 memory resting T-cells was noted in responders. The trend toward increased follicular helper T cell and resting dendritic cell proportions in DLBCL pts was also associated with longer survival (PFS > 24 months) vs not. Gene variant data and responder status were available for 26 pts with DLBCL, 26 with FL, and 17 with RT. Comparison between responders and nonresponders showed that DLBCL pts with RNF213 (4/10 [40.0%] vs 1/16 [6.2%]) and KLHL14 (3/10 [30.0%] vs 0/16) mutations were more likely to respond to ibr + nivo. Conversely, nonresponders were associated with variants in EBF1, ADAMTS20, AKAP9, SOCS1, TP53, and genes in BCR pathways such as TNFRSF14, MYD88, and NFKB1B. BCL2 mutation in FL (9/12 [75.0%] vs 4/14 [28.6%]) and ROS1 mutation in RT (0/13 vs 2/4 [50.0%]) were associated with response; both are involved in the NF-kB pathway. In DLBCL, the most frequent gene mutations were RNF213, NBPF1, and BCL2 in pts who had PFS > 24 months (3/7 [42.9%] each), and KMT2D (8/20 [40.0%]) and CSMD3 (8/20 [40.0%]) in pts who did not. Somatic mutation burden was lower in responders vs nonresponders, especially in germinal center B-cell-DLBCL, and in DLBCL pts with PFS > 24 months vs not. In conclusion, we report gene variations among DLBCL, FL, and RT pts associated with response or durable PFS with ibr + nivo. While ibr inhibits Bruton’s tyrosine kinase-dependent pathways, we identify alternative gene pathway variants that may impact tx outcomes. Immune cell infiltration into the microenvironment relates to differential tx response with this immune combination and is histology dependent. Background: Rituximab (RTX) alone or in combination with chemotherapy has substantially improved treatment outcomes for patients (pts) with marginal zone lymphoma (MZL), but relapse is common and not all pts are acceptable candidates for, or respond to, current salvage therapies. Umbralisib is a novel, next-generation PI3K-delta inhibitor with unique inhibition of casein kinase-1ϵ (CK1ϵ) and, compared to earlier generation PI3K-delta inhibitors, exhibits a differentiated tolerability profile with reduced rates of immune-mediated toxicity (Burris et al, 2018). This registration-directed study evaluates the efficacy and safety of umbralisib in pts with relapsed/refractory (R/R) MZL. Methods: Pts had histologically confirmed MZL, ECOG PS ≤2, and had previously received ≥1 prior therapy including at least one CD20 monoclonal antibody (mAb)-containing regimen. All pts received umbralisib 800 mg orally once daily until progression or unacceptable toxicity. The primary study endpoint was overall response rate (ORR) as assessed by an independent review committee (IRC) according to 2007 IWG criteria. ORR by investigator assessment is reported here, and ORR by IRC is forthcoming. Secondary endpoints included duration of response (DOR), progression-free survival (PFS), and safety. Results: Sixty-nine pts were enrolled; we report on the first 38 who are eligible for at least 6 months (mos) of follow-up as of the data cutoff date. Disease status for the 38 pts: extranodal (n=23), nodal (n=8), and splenic (n=7). Median age was 67 years (range, 34-81). Median number of prior systemic therapies was 2 (range, 1-5). Seven pts (18%) had received monotherapy RTX only, and 26 (68%) had received at least one CD20 mAb-containing chemoimmunotherapy. As of the cut-off date, the median follow-up was 9.6 mos. Per investigator assessment, ORR was 55% (4 CRs and 17 PRs), 29% of pts (n=11) had stable disease (SD) of which 6 of these SD pts remain on study with durations ranging from 7-12+ mos. The clinical benefit rate (CR+PR+SD) was 84%, and 91% of pts with at least 1 post-baseline assessment experienced tumor reductions. The median time to initial response was 2.7 mos, while the median DOR was not reached (95% CI: 8.4-not reached). The 12-month PFS was 71%. The most common (≥20%) adverse events (AE) of any grade included: diarrhea (45%), nausea (29%), fatigue (26%), headache (26%), cough (24%), and decreased appetite (21%). The most common Grade 3/4 events were neutropenia (8%), febrile neutropenia (5%), and diarrhea (5%). As of the cutoff date, 16 pts discontinued treatment (PD: 18%; AEs: 8%; pt decision: 8%; physician decision: 8%) and 58% continue treatment. Conclusions: PI3K-delta inhibition with single-agent umbralisib is active and well tolerated in pts with R/R MZL, achieving durable responses with chemotherapy-free therapy.
