Certain guidelines should be followed in the restoration of endodontically treated teeth (treatment of the dental pulp and surrounding tissue). The practitioner should minimize the loss of tooth structure by making the dowel diameter as small as possible, and necessary rigidity should be preserved. Posts should be as long as possible; longer posts have a greater rate of retention and capacity to resist fracture. An intact interior tooth with intact marginal ridges does not require a post or coronal coverage. However, a post or core is required if the tooth is to receive a crown. Cuspal coverage is needed for posterior teeth, with the exception of certain molars; posts are rarely needed in molars. Surface roughness of the post enhances its retention qualities, and all posts should have antirotation devices. Irregularities and vertical height should be left in the coronal preparation for the core to increase resistance form. Concentration of stress should be avoided during post insertion, and the core material must be of adequate strength. If possible, the post, core, and crown should consist of similar metals. A critical summary of a study entitled 'Dental arch space changes following premature loss of primary first molars: a systematic review,' is presented. The study found that there will be minimal space loss across six to eight months if a child prematurely loses a primary first molar and receives no intervention for space maintenance.
