Objective: Clinician burnout in healthcare is extensive and of growing concern. In mental health and rehabilitation settings, research on interventions to improve burnout and work engagement is limited and rarely addresses organizational drivers of burnout. This study sought to elaborate on the organizational influence of burnout and work engagement in mental health. Methods: We randomly selected 40 mental health clinicians and managers who were participating in a burnout intervention and conducted semi-structured interviews to understand their views of organizational conditions impacting burnout and work engagement. Data were analyzed using a thematic analytical approach. Results: Analyses yielded three major themes where organizational contexts might reduce burnout and increase work engagement: (a) a work culture that prioritizes person-centered care over productivity and other performance metrics, (b) robust management skills and practices to overcome bureaucracy, and (c) opportunities for employee professional development and self-care. Participants also referenced three levels of the organizational context that they believed influenced burnout and work engagement: front-line supervisors and program managers, organizational executive leadership, and the larger health system. Conclusions and Implications for Practice: Findings point to several possible targets of intervention at various organizational levels that could guide the field toward more effective ways to reduce burnout and improve work engagement. Impact and Implications: Three major themes where organizational contexts might reduce burnout and increase work engagement were identified: a work culture that prioritizes person-centered care over productivity and other metrics, robust management skills and practices to overcome bureaucracy, and opportunities for employee professional development and self-care. The well-being of the psychiatric rehabilitation workforce is a growing concern, particularly as a result of the stresses of the COVID-19 pandemic on demand for mental health services. Research focusing on this aspect of psychiatric rehabilitation services remains limited but is important in supporting a resilient mental health workforce. This special section presents four papers that focus on aspects of worker well-being and burnout, including drivers of well-being and other outcomes, as well as exploring potential action steps and contexts that organizations could consider in their efforts to bolster well-being. Impact and Implications: Although research remains limited, workforce well-being is a major concern within psychiatric rehabilitation settings. Organizations can capitalize on the findings from studies in this special section to develop a better understanding of the drivers of well-being and related outcomes, as well as develop ideas for how to improve psychiatric rehabilitation workforce well-being. Objective: Patient activation refers to one's ability and willingness to manage their health and health care. Visit preparation, question formulation, and other elements of patient activation are core components of patient-centered care. However, they are inconsistently translated into clinical practice. Multiple factors have been shown to influence patient activation and associated activities, such as patients' race and ethnicity, illness, and clinical settings. Because race and ethnicity are important factors in patient activation, and we know little about ethnic minority patients with respect to patient activation, the goal of this study was to examine the contexts, barriers, and facilitators influencing African American veterans' involvement in visit preparation in mental health outpatient settings.Method: We conducted qualitative interviews with 49 African American veterans with mental illness receiving outpatient psychiatric care at a large, urban U.S. Department of Veterans Affairs (VA) Medical Center, and used a grounded theory approach to analyze the data.Results: Findings from this study identify patients' beliefs about preparing for the clinical encounter, the patient-provider relationship, and lack of information about patient activation as barriers to engaging in visit preparation activities.Conclusions and Implications For Practice: Findings indicate the need for greater awareness of the challenges of visit preparation as well as the potential consequences of lack of preparation. Results suggest the incorporation of visit preparation as part of routine mental health visits, and as a tool to increase patient activation, especially among minorities. Psychiatric rehabilitation practices are often poorly defined, hindering implementation, research, and dissemination efforts. Documentation of adherence to a specific psychiatric rehabilitation approach is particularly important in conducting randomized controlled trials. This paper outlines steps taken to define and measure the Diversified Placement Approach (DPA), a well-regarded vocational program for people with severe mental illnesses. Sixty-four people with serious mental illness participating in a vocational rehabilitation program completed a modified version of the Working Alliance Inventory (WAI), an instrument designed to assess therapeutic alliance. Individuals and their providers completed parallel forms. Reliability data show that the modified instrument demonstrates fair to moderate test re-test reliability and modest internal consistency. Marked discrepancies in perceptions of the working alliance were reported by participants and providers. This study explores three social network characteristics as they relate to demographic, clinical, and quality of life factors. The sample consists of 219 participants with serious mental illness. Results indicate that clients' social networks were small, and that network size is robustly related to symptoms and other measures of clinical and cognitive functioning, as well as with quality of life and self-esteem. The expected relation between network density and either the demographic variables or quality of life was not found. Moderate levels of network density, however, were associated with fewer psychiatric symptoms and higher IQ. Network multiplexity was most highly associated with demographic variables, such as ethnic minority status, less education, never being married, and living with family. [ABSTRACT FROM AUTHOR]

Objective: The current article describes efforts to develop and test a measure of recovery-oriented inpatient care. Method: The Recovery-oriented Acute INpatient (RAIN) scale was based on prior literature and current Veterans Health Administration (VHA) policy and resources and further revised based on data collection from 34 VHA acute inpatient units. Results: A final scale of 23, behaviorally anchored items demonstrated a four-factor structure including the following factors: inpatient treatment planning, outpatient treatment planning, group programming, and milieu. While several items require additional revision to address psychometric concerns, the scale demonstrated adequate model fit and was consistent with prior literature on recovery-oriented inpatient care. Conclusions and Implementations for Practice: The RAIN scale represents an important tool for future implementation and empirical study of recovery-oriented inpatient care. Impact and Implications: This study focused on developing a scale to measure recovery-oriented inpatient mental health care. The study found a 23-item scale matched prior literature and included four general areas of recovery-oriented inpatient care: inpatient treatment planning, outpatient treatment planning, group programming, and milieu. Objective: Funding cuts have increased job demands and threatened clinicians' ability to provide high-quality, person-centered care. One response to increased job demands is for clinicians to work more than their official scheduled work hours (i.e., overtime). We sought to examine the frequency of working overtime and its relationships with job characteristics, work-related outcomes, and quality of care in community health clinicians. Method: One hundred eighty-two clinicians completed demographic and job characteristics questions and measures of burnout, job satisfaction, turnover intention, work--life conflict, and perceived quality of care. Clinicians also reported the importance of reducing stress and their confidence in reducing their stress. Clinicians who reported working overtime were compared to clinicians that did not on demographic and job characteristics and work-related outcomes. Results: Ninety-four clinicians (52%) reported working overtime in a typical week. Controlling for exempt status and group differences in time spent supervising others, those working overtime reported significantly increased burnout and work--life conflict and significantly lower job satisfaction and quality of care than those not working overtime. Clinicians working overtime also reported significantly greater importance in reducing stress but less confidence in their ability to reduce stress than those not working overtime. There were no significant group differences for turnover intention. Conclusion and Implications for Practice: Working overtime is associated with negative consequences for clinician-related work outcomes and perceived quality of care. Objective: Prior research indicates Black employees may be particularly vulnerable to job dissatisfaction and that social support at work is a potential resource that could influence employee outcomes. This study examined racial differences in workplace social networks and support, and how these factors may contribute to perceived organizational support and, ultimately, job satisfaction among mental health workers. Method: Using data from an all-employee survey in a community mental health center (N = 128), we assessed racial differences in social network supports, hypothesizing that Black employees would report smaller and less supportive social networks, and lower levels of organizational support and job satisfaction compared to White employees. We also hypothesized that workplace network size and support would be positively associated with perceived organizational support and job satisfaction. Results: Hypotheses were partially supported. Compared to Whites, Blacks had smaller workplace networks that were less likely to include supervisors, were more likely to report workplace isolation (naming no workplace social ties), and were less likely to seek advice from their social ties at work. Regression analyses showed that Blacks and employees with smaller networks were more likely to perceive lower levels of organizational support, even after controlling for background variables. However, race and network size did not predict overall job satisfaction. Conclusions and Implications for Practice: These findings suggest that Black mental health services staff are less likely to have rich, diverse workplace networks than their White colleagues, which may put them at a disadvantage in terms of accessing support and other resources. Impact and Implications: Compared to Whites, Black mental health workers report smaller and less supportive social networks, and lower levels of organizational support. However, these differences did not translate into lower job satisfaction, an outcome that may be more strongly influenced by organizational factors and emotional exhaustion. Objective: This article examines the relationship between inpatient mental health units' adherence to recovery-oriented care and 30-day patient readmission. Method: The sample included patients admitted to one of 34 Veterans Health Administration inpatient mental health units. Recovery-oriented care was assessed using interviews and site visits. Patient characteristics and readmission data were derived from administrative data. Findings: Overall recovery orientation was not associated with readmission. Exploratory analyses found higher scores on a subsample of items pertaining to inpatient therapeutic programming were associated with lower patient readmissions. Additionally, patients with more prior service use and substance abuse or personality disorders were more likely to be readmitted. Conclusions and Implications for Practice: A growing body of literature supports the association between psychotherapeutic services in inpatient units and better patient outcomes. However, further research is needed to examine this association. More work is needed to develop appropriate psychotherapy services for the inpatient setting and support their implementation. Impact and Implications: While this study did not find an overall association between recovery-oriented inpatient mental health services and patient readmission, it did show an association between the provision of therapeutic services and readmissions. Objective: Patient-centered communication has been linked to patient satisfaction, treatment adherence and outcomes. Shared decision making (SDM) has been advocated as an important and ethically essential aspect of patient-centered care, but SDM has received relatively little attention in mental health care, despite studies indicating that consumers want to be involved in making decisions. This is particularly important in a recovery-oriented system, where consumers are active participants in their treatment and rehabilitation. Because medication management is a key component of recovery from severe mental illnesses, this study explores how consumers and providers make decisions in medication management consultations. Methods: Four providers (3 psychiatrists, 1 nurse practitioner) and 40 consumers with severe mental illness (10 consumers per provider) were recruited from a community mental health center with a recovery-oriented focus. We directly observed 40 medication management appointments. Observations were audio recorded and transcribed. We used emergent thematic analysis to characterize decision making processes. Results: Providers initiated most decisions, although they often invited consumers to participate in decision making. Decisions initiated by consumers elicited a greater degree of discussion and disagreement, but also frequently resulted in consumers' preferences prevailing. Consultations generally exhibited more characteristics of person-centeredness than SDM. Conclusions and Implications for Practice: While we observed a high degree of person-centeredness, SDM was not prevalent. Interventions helping consumers to take greater initiative when working with service providers may be helpful. For example, programs using tools such as peer instruction, Internet-based software, and individual case-manager instruction all have shown promise for enhancing SDM in mental health treatment. Further research is needed to determine the degree of SDM in other settings (e.g., with case managers) and the impact of SDM on consumers' recovery. [ABSTRACT FROM AUTHOR]

Objective: High-quality, person-centered care is a priority for mental health services. The current study conducted secondary data analysis to examine the impact of job stress (i.e., interaction with high-risk consumer cases, increased caseload, emotional exhaustion) and resources (i.e., increased organizational and supervisory support, autonomy, role clarity) on providers' perceived quality of care. Methods: Data consisted of 145 direct care providers from an urban community mental health center. Structural equation modeling was used for testing the hierarchical regression model, sequentially adding job stress and resource variables in the prediction models for the quality of care (i.e., person-centered care, discordant care [conflict with consumers and tardiness]). Results: Person-centered care was positively associated with increased role clarity, organizational support, and larger caseload size, while a lower level of discordant care was associated with lower emotional exhaustion, smaller caseload size, less interaction with high-risk consumer cases, and with increased role clarity. Conclusions and Implications for Practice: Resources on the job may be particularly important for improved person-centered care, and lowering job stress may help reduce discordant care. The current study suggests the need for the mental health organizations to attend to both job stress and resources for providers to improve the quality of care. Impact and Implications: Both lowering job stress and increasing job resources could impact different aspects of quality of care provided by mental health organizations and their workers. Lowering job stress may help reduce discordant care, and resources on the job may be important for improved person-centered care. Objective: To examine provider competence in providing Illness Management and Recovery (IMR), an evidence-based self-management program for people with severe mental illness, and the association between implementation supports and IMR competence. Method: IMR session recordings, provided by 43 providers/provider pairs, were analyzed for IMR competence using the IMR Treatment Integrity Scale. Providers also reported on receipt of commonly available implementation supports (e.g., training, consultation). Results: Average IMR competence scores were in the "needs improvement" range. Clinicians demonstrated low competence in several IMR elements: significant other involvement, weekly action planning, action plan follow-up, cognitive behavioral techniques, and behavioral tailoring for medication management. These elements were commonly absent from IMR sessions. Competence in motivational enhancement strategies and cognitive behavioral techniques differed based on the module topic covered in a session. Generally, receipt of implementation supports was not associated with increased competence; however, motivational interviewing training was associated with increased competence in action planning and review. Conclusions and Implications for Practice: IMR, as implemented in the community, may lack adequate competence and commonly available implementation supports do not appear to be adequate.
